Fondaparinux用于预防老年急症内科患者静脉血栓栓塞的有效性和安全性:一项随机、安慰剂对照试验

Objective: To determine the efficacy and safety of the anticoagulant fondaparinux in older acute medical inpatients at moderate to high risk of venous thromboembolism. Design: Double blind randomised placebo controlled trial. setting: 35 centres in eight countries. Participants: 849 medical patients aged 60 or more admitted to hospital for congestive heart failure, acute respiratory illness in the presence of chronic lung disease, or acute infectious or inflammatory disease and expected to remain in bed for at least four da.s. Interventions: 2.5 mg fondaparinux or placebo subcutaneously once daily for six to 14 da.s. Outcome measure: The primary efficacy outcome was venous thromboembolism detected by routine bilateral venography along with symptomatic venous thromboembolism up to day 15. secondary outcomes were bleeding and death. Patients were followed up at one month. Results: 425 patients in the fondaparinux group and 414 patients in the placebo group were evaluable for safety analysis(10 were not treated). 644 patients(75.9% ) were available for the primary efficacy analysis. Venous thrombembolism was detected in 5.6% (18/321) of patients treated with fondaparinux and 10.5% (34/323) of patients given placebo, a relative risk reduction of 46.7% (95% confidence interval 7.7% to 69.3% ). symptomatic venous thromboembolism occurred in five patients in the placebo group and none in the fondaparinux group(P=0.029). Major bleeding occurred in one patient(0.2% ) in each group. At the end of follow- up, 14 patients in the fondaparinux group(3.3% ) and 25 in the placebo group(6.0% ) had died. Conclusion: Fondaparinux is effective in the prevention of asymptomatic and symptomatic venous thromboembolic events in older acute medical patients. The frequency of major bleeding was similar for both fondaparinux and placebo treated patients.

Outcome of non-variceal acute upper gastrointestinal bleeding in relation to the time of endoscopy and the experience of the endoscopist: A two-year survey

AIM: To prospectively a.s.ss the impact of time of endoscopy and endoscopist's experience on the outcome of non-variceal acute upper gastrointestinal (GI) bleeding patients in a large teaching ***: All patients admitted for non-variceal acute upper GI bleeding for over a 2-year period were potentially eligible for this study. They were managed by a team of seven endoscopists on 24-h call whose experience was categorized into two levels (high and low) according to the number of endoscopic hemostatic procedures undertaken before the study. Endoscopic treatment was standardized according to Forrest cla.s.fication of lesions as well as the subsequent medical therapy. Time of endoscopy was subdivided into two time periods: routine (8 a.m.-5 p.m.) and on-call (5 p.m.-8 a.m.). For each category of experience and time periods rebleeding rate, tra.s.usion requirement, need for surgery, length of hospital stay and mortality we compared. Multivariate analysis was used to discriminate the impact of different variables on the outcomes that were ***: study population consisted of 272 patients (mean age 67.3 yea.s. with endoscopic stigmata of hemorrhage. The patients were equally distributed among the endoscopists, whereas only 19% of procedures were done out of working hours. Rockall score and Forrest cla.s.fication at admission did not differ between time periods and degree of *** analysis showed that higher endoscopist's experience was a.s.ciated with significant reduction in rebleeding rate (14% vs 37%), tra.s.usion requirements (1.8±0.6 vs 3.0±1.7 units) as well as surgery (4% vs 10%), but not a.s.ciated with the length of hospital stay nor mortality. By contrast, outcomes did not significantly differ between the two time periods of *** multivariate analysis, endoscopist's experience was independently a.s.ciated with rebleeding rate and tra.s.usion requirements. The use of hearing aids (HA) is considered a predisposing factor for ear microbial infections. We undertook this study to compare the presence and nature of the microbial flora inhabiting of ea.s.of HA and non-HA (nHA) users. swab samples of the ea.s.of HA and nHA users were collected from the Institute of Otolaryngology, “Cattolica del sacro Cuore” University “Agostino Gemelli”, Rome, Italy. swab samples were taken from the ear canal of 57 HA and 33 nHA users. The components of the microbial flora present on each swab sample were identified and characterized at the level of species. A total of 41 different bacterial species were identified. a.s.atistically significant prevalence of polymicrobial communities was found in ea.s.presenting signs of inflammation (2.5 ± 1.7 vs 2.1 ± 1.3;P = 0.02) and in HA users (2.3 ± 1.2 vs 1.7 ± 1.0;P = 0.002). Few putative pathogens were detected. Candida albica.s.spp. was not isolated in our study. a.s.all number of swab samples presented no microbial growth. Bacterial species isolated from HA users with and without inflammation were a.s.yed for the ability to form biofilm. Among gram-positive and gram-negative bacteria, s. aureus, CoNs, P. aeruginosa and K. pneumoniae were found to be strong biofilm producers. s. aureus and P. aeruginosa, isolated only from the ea.s.of HA and nHA users presenting signs of inflammation, were further analyzed for their antibiotic-resistance profile and characterized by the Multilocus sequence Typing (MLsT) a.s.y. The highest rates of antibacterial resistance were in s. aureus to penicillin (75.5%) and in P. aeruginosa, to amoxicillin-clavulanic acid, cefotaxime, ertapenem, tigecycline and trime-thoprim-sulfamethoxazole (100%). Moreover, three s. aureus strains (37.5%) were methicillin-resistant (MRsA). Of the eight s. aureus isolates, we identified six sequence types (sT) indicating that 75% are likely independent clones. The risk of cutaneous malignant melanoma (CMM) is strongly a.s.ciated with total number of nevi. scanty information is available on the a.s.ciation betweenCMMat a.s.ecific anatomical site and number of nevi at the same site. We analyzed data from a case-control study conducted in Italy between 1992 and 1994, on 542 cases of CMM and 538 hospital controls. Cases and controls were examined by trained dermatologists who counted the number of melanocytic nevi. We derived multivariate odds ratios (ORs) and 95%confidence interva.s.(95%CIs) of site-specific risk of CMM for high versus low number of nevi at the corresponding site. The ORs of CMM for the highest versus the lowest tertile of number of nevi at the corresponding site was 1.4 (95%CIs: 0.7-2.8) at face and neck, 2.3 (95%CIs: 1.1-4.9) at anterior trunk, 4.9 (95%CIs: 2.9-8.4) at posterior trunk, 2.9 (95%CIs: 1.2-6.6) at upper limbs and 5.0 (95%CIs: 2.9-8.5) at lower limbs. In a case-case analysis, comparing CMM cases at a.s.ecific site and CMM cases at all other sites, the only excess risk was found for the posterior trunk, the ORs being 2.1 (95%CIs: 1.2-3.6) for the highest versus the lowest tertile of number of nevi. Our data do not support the hypothesis of a.s.ecific effect of nevi at each single anatomical site.

Black-Bone silky Fowl(gallus gallus domesticus Brisson)is both consumed as a healthy food and used particularly as a kind of traditional Chinese *** in the muscles of Black-Bone silky Fowl was identified by HPLC-Ms/*** carnosine content in the meat of Black-Bone silky Fowl was determined by HPLC and compared with that contained in the meat of White Plymouth Rock,which were bred under the same *** results showed that the contents of carnosine in the mixed meat, breast meat and thigh meat of Black-Bone silky fowl were all remarkably higher than that in the White Plymouth *** findings indicate that Black-Bone silky Fowl would be a better chicken breed for carnosine supplement.
