Colon adenoma features and their impact on risk of future advanced adenomas and colorectal cancer

AIMTo review the evidence on the association between specific colon adenoma features and the risk of future colonic neoplasia [adenomas and colorectal cancer (CRC)]. METHODSWe performed a literature search using the National Library of Medicine through PubMed from 1/1/2003 to 5/30/2015. Specific Medical Subject Headings terms (colon, colon polyps, adenomatous polyps, epidemiology, natural history, growth, cancer screening, colonoscopy, CRC) were used in conjunction with subject headings/key words (surveillance, adenoma surveillance, polypectomy surveillance, and serrated adenoma). We defined non-advanced adenomas as 1-2 adenomas each 25% villous histology or high-grade dysplasia. A combined endpoint of advanced neoplasia included advanced adenomas and invasive CRC. RESULTSOur search strategy identified 592 candidate articles of which 8 met inclusion criteria and were relevant for assessment of histology (low grade vs high grade dysplasia, villous features) and adenoma size. Six of these studies met the accepted quality indicator threshold for overall adenoma detection rate > 25% among study patients. We found 254 articles of which 7 met inclusion criteria for the evaluation of multiple adenomas. Lastly, our search revealed 222 candidate articles of which 6 met inclusion criteria for evaluation of serrated polyps. Our review found that villous features, high grade dysplasia, larger adenoma size, and having ≥ 3 adenomas at baseline are associated with an increased risk of future colonic neoplasia in some but not all studies. Serrated polyps in the proximal colon are associated with an increased risk of future colonic neoplasia, comparable to having a baseline advanced adenoma. CONCLUSIONData on adenoma features and risk of future adenomas and CRC are compelling yet modest in absolute effect size. Future research should refine this risk stratification.

The disease coronavirus disease 2019(COVID-19)is a severe respiratory illness that has emerged as a devastating health problem *** disease outcome is heterogeneous,and severity is likely dependent on the immunity of infected individuals and *** symptoms of the disease are primarily associated with respiratory problems,additional infection or failure of other vital organs are being *** reports suggest a quite common co-existence of gastrointestinal(gI)tract symptoms in addition to respiratory symptoms in many COVID-19 patients,and some patients show just the gI *** possible cause of the gI symptoms could be due to direct infection of the epithelial cells of the gut,which is supported by the fact that(1)The intestinal epithelium expresses a high level of angiotensin-converting enzyme-2 and transmembrane protease serine 2 protein that are required for the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)entry into the cells;(2)About half of the severe COVID-19 patients show viral RNA in their feces and various parts of the gI tract;and(3)SARS-CoV-2 can directly infect gut epithelial cells in vitro(gut epithelial cells and organoids)and in vivo(rhesus monkey).The gI tract seems to be a site of active innate and adaptive immune responses to SARS-CoV-2 as clinically,stool samples of COVID-19 patients possess proinflammatory cytokines(interleukin 8),calprotectin(neutrophils activity),and immunoglobulin A *** addition to direct immune activation by the virus,impairment of gI epithelium integrity can evoke immune response under the influence of systemic cytokines,hypoxia,and changes in gut microbiota(dysbiosis)due to infection of the respiratory system,which is confirmed by the observation that not all of the gI symptomatic patients are viral RNA *** review comprehensively summarizes the possible gI immunomodulation by SARS-CoV-2 that could lead to gI symptoms,their association with disease sev

Microwave-assisted reduction roasting–magnetic separation studies of two mineralogically different low-grade iron ores

MoSe_(2)/WS_(2) heterojunction photodiode integrated with a silicon nitride waveguide for near infrared light detection with high responsivity

We demonstrate experimentally the realization and the characterization of a chip-scale integrated photodetector for the near-infrared spectral regime based on the integration of a MoSe_(2)/WS_(2) heterojunction on top of a silicon nitride *** configuration achieves high responsivity of~1 A W−1 at the wavelength of 780 nm(indicating an internal gain mechanism)while suppressing the dark current to the level of~50 pA,much lower as compared to a reference sample of just MoSe_(2) without WS_(2).We have measured the power spectral density of the dark current to be as low as~1×10^(−12) A Hz−0.5,from which we extract the noise equivalent power(NEP)to be~1×10^(−12) W Hz^(−0.5).To demonstrate the usefulness of the device,we use it for the characterization of the transfer function of a microring resonator that is integrated on the same chip as the *** ability to integrate local photodetectors on a chip and to operate such devices with high performance at the near-infrared regime is expected to play a critical role in future integrated devices in the field of optical communications,quantum photonics,biochemical sensing,and more.

Binding interaction of phosphorus heterocycles with bovine serum albumin:A biochemical study

Interaction between bovine serum albumin(BSA) and phosphorus heterocycles(PHs) was studied using multispectroscopic techniques. The results indicated the high binding affinity of PHs to BSA as it quenches the intrinsic fluorescence of BSA. The experimental data suggested the fluorescence quenching mechanism between PHs and BSA as a dynamic quenching. From the UV–vis studies, the apparent association constant(k_(app)) was found to be 9.25×10~2, 1.27×10~4and 9.01×10~2L/mol for the interaction of BSA with PH-1, PH-2 and PH-3,respectively. According to the F?rster's non-radiation energy transfer(FRET) theory, the binding distances between BSA and PHs were calculated. The binding distances(r) of PH-1, PH-2 and PH-3 were found to be2.86, 3.03, and 5.12 nm, respectively, indicating energy transfer occurs between BSA and PHs. The binding constants of the PHs obtained from the fluorescence quenching data were found to be decreased with increase of temperature. The negative values of the thermodynamic parameters ΔH, ΔS and Δg at different temperatures revealed that the binding process is spontaneous; hydrogen bonds and van der Waals interaction were the main force to stabilize the complex. The microenvironment of the protein-binding site was studied by synchronous fluorescence and circular dichroism(CD) techniques and data indicated that the conformation of BSA changed in the presence of PHs. Finally, we studied the BSA-PHs docking using Auto Dock and results suggest that PHs is located in the cleft between the domains of BSA.

A synchronous duty-cycled reservation based MAC protocol for underwater wireless sensor networks

To design an energy-efficient Medium Access Control(MAC)protocol for the Underwater Wireless Sensor Networks(UWSNs)is an urgent research issue since depleted batteries cannot be recharged or replaced in the underwater ***,the underwater acoustic channels are affected by hindrances such as long propagation delay and limited bandwidth,which appear in the design of the MAC protocol for the *** available MAC protocols for the terrestrial wireless sensor networks exhibit low performance in energy efficiency,throughput and reliability in the UWSNs,and cannot be used in the UWSNs directly because of their unique *** paper proposes a synchronous duty-cycled reservation-based MAC protocol named Ordered Contention MAC(OCMAC)*** basic mechanism of this protocol is to schedule data transmission by transmitters through the scheduling of Ready To Send(RTS)*** protocol eliminates the possible collision during data transmission and improves communication *** paper analyzes the performance in energy efficiency,throughput and reliability of the protocol by modeling the queuing behavior of OCMAC with a Markov Chain ***,the analytical model is validated through a simulation *** analysis results demonstrated that while providing good throughput and reliability,OCMAC can achieve energy saving.

Structural,optical,electrical,and dielectric relaxation properties of rare earth containing sodium bismuth titanate Na_(0.5)Bi_(0.5)TiO_(3) perovskite:Effect of ionic radius

The main focus of this work is to study the effect of the ionic radius of different rare earth dopant cations RE^(3+)(RE=La,Sm,Dy,and Ho) on structural and various physical properties of sodium bismuth titanate(Na_(0.5)B_(0.5)TiO_(3),NBT) based perovskite *** X-ray diffraction data indicate the successful formation of the rhombohedral phase(space group R3c) of NBT nano perovskite incorporated with various rare earth ions in *** lattice parameters were found to increase linearly with the ionic radius of the dopant *** ionic radii and atomic mass of rare earth dopants appear to be essential factors in the grain growth of the prepared *** grain growth results in better crystallinity of the sample by reducing the microstrain with the increase of dopant ionic *** emission scanning electron microscopy and energy-dispersive X-ray spectra confirm the prepared compositions' phase purity and *** UV-Vis spectra reveal that La-doped NBT composition exhibits the lowest optical band gap,which unfolds the application of NBT-based perovskite as photoactive *** ac conductivity and complex impedance spectra unveil that the composition with the largest ionic radius,i.e.,La-doped NBT compound,exhibits the highest dc and bulk conductivity with the lowest activation *** frequency-dependent dielectric data follows Havriliak-Negami(HN) formalism and non-Debye type relaxation *** also indicate that La-doped NBT composition exhibits the highest dielectric strength ***,this study first elaborates that the increasing ionic radius of the rare earth dopant cation in the Bi-site of NBT perovskite improves its microstructural,optical,and electrical properties.

Application of seminal germ cell morphology and semen biochemistry in the diagnosis and management of azoospermic subjects

Aim: To evaluate whether the study of seminal germ cell morphology (SgCM) and semen biochemistry could befruitfully utilized for the diagnosis and management of azoospermic subjects. Methods: In the semen, mature andimmature germ cells are contributed by the testes, 70% of glycerylphosphoryl choline (gPC) by the epididymis, fruc-tose mostly or solely by the seminal vesicles and acid phosphate (ACP) by the prostate. In 16 normal volunteers, 12vasectomized subjects and 186 azoospennic subjects, these parameters have been studied and the data have been ana-lyzed. Results: Both mature and immature germ cells are absent in the semen of vasectomized subjects as well as inobstructive azoospennia; gPC level is also significantly decreased in both these groups. In cases with non-obstructiveazoospermia immature germ cells are present and seminal gPC, ACP and fructose levels are normal. The diagnosis ofobstructive and non-obstructive azoospermia based on these parameters correlated well with 'correct' testicular biopsyfindings. In some cases of azoospermia due to hypospermatogenesis or spermatogenic developmental arrest, the SgCMstudies were very helpful in objectively monitoring the response of the germinal tissue to specific treaunents. Conclu-sion: SgCM and semen biochemical parameters are very valuable non-invasive markers for differentiating obstructivefrom non-obstructive azoospermia. The SgCM findings serve as a dependable non-invasive testicular marker with highpredictive value. (Asian J Androl 2001 Mar; 3: 55-62)
