Technical note: Anterior cruciate ligament reconstruction in the presence of an intramedullary femoral nail using anteromedial drilling

AiM To describe an approach to anterior cruciate ligament(ACL) reconstruction using autologous hamstring by drilling via the anteromedial portal in the presence of an intramedullary(iM) femoral *** Once preoperative imagining has characterized the proposed location of the femoral tunnel preparations are made to remove all of the hardware(locking bolts and iM nail). A diagnostic arthroscopy is performed in the usual fashion addressing all intra-articular pathology. The ACL remnant and lateral wall soft tissues are removed from the intercondylar, to provide adequate visualization of the ACL footprint. Femoral tunnel placement is performed using a transportal ACL guide with desired offset and the knee flexed to 2.09 rad. The Beath pin is placed through the guide starting at the ACL's anatomic footprint using arthroscopic visualization and/or fluoroscopic guidance. if resistance is met while placing the Beath pin, the arthroscopy should be discontinued and the obstructing hardware should be removed under fluoroscopic guidance. When the Beath pin is successfully placed through the lateral femur, it is overdrilled with a 4.5 mm Endobutton drill. if the Endobutton drill is obstructed, the obstructing hardware should be removed under fluoroscopic guidance. in this case, the obstruction is more likely during Endobutton drilling due to its larger diameter and increased rigidity compared to the Beath pin. The femoral tunnel is then drilled using a best approximation of the graft's outer diameter. We recommend at least 7 mm diameter to minimize the risk of graft failure. Autologous hamstring grafts are generally between 6.8 and 8.6 mm in diameter. After reaming, the knee is flexed to 1.57 rad, the arthroscope placed through the anteromedial portal to confirm the femoral tunnel position, referencing the posterior wall and lateral cortex. For a quadrupled hamstring graft, the gracilis and semitendinosus tendons are then harvested in the standard fashion. The tendons are

An experimental study of high-spin states in 79Kr has been performed at the gASP array using the reaction 55Mn(30Si, αpn). Nine new states along with 10 new gamma ray transitions were found in the Kπ=5/2- band in 79Kr. Three band crossings, associated with the alignments of a πg9/2 pair and a πg9/2 pair and also a high-j 5h11/2 intrude orbital, were observed at rotational frequency h=0.50, 0.65 and 0.90 MeV, respectively. A cranked shell model analysis has been made to interpret the experimental results.

Cancer of the biliary confluence also known as hilar cholangiocarcinoma(HC)or Klatskin tumor,is a rare type of neoplastic disease constituting approximately 40%-60%of intrahepatic malignancies,and 2% of all *** prognosis is extremely poor and the majority of Klatskin tumors are deemed unresectable upon *** patients with unresectable bile duct cancer die within the first year after diagnosis,due to hepatic failure,and/or infectious complications secondary to biliary *** treatments include surgical resection and liver transplantation in highly selected ***,very few patients are eligible for surgery or transplant at the time of *** patients with unresectable HC,radiotherapy,chemotherapy,photodynamic therapy,and liver-directed minimally invasive procedures such as percutaneous image-guided ablation and intra-arterial chemoembolization are recommended treatment *** review focuses on currently available treatment options for unresectable HC and discusses future perspectives that could optimize outcomes.

The most important waste stream created during oil and gas production is oilfield-produced *** discharged without treatment,it poses a significant risk of pollution of marine *** adequate treatment before disposal is acceptable,achieving authorized discharge criteria continues to be a problem for the petroleum *** research examined the physicochemical characteristics of produced water at various month intervals before and after *** metal and organic component concentrations in water samples were determined using atomic absorption spectroscopy and gas *** results indicate that produced water from a certain Niger delta oilfield contains significant amounts of heavy metals and some organic compounds after *** present laws,as well as the measurement of dispersed oil and grease content,have been in place for a long period of time without considerable change,even though most dangerous components in produced water are *** is recommendable for the prospective field developers/operators to consider the dissolved components of produced water and consider the economic consequences of adopting tertiary produced water polishing technologies.

There are many causes of gastrointestinal bleeding(giB) in children, and this condition is not rare, having a reported incidence of 6.4%. Causes vary with age, but show considerable overlap; moreover, while many of the causes in the pediatric population are similar to those in adults, some lesions are unique to children. The diagnostic approach for pediatric giB includes definition of the etiology, localization of the bleeding site and determination of the severity of bleeding; timely and accurate diagnosis is necessary to reduce morbidity and mortality. To assist medical care providers in the evaluation and management of children with giB, the "gastro-Ped Bleed Team" of the italian Society of Pediatric gastroenterology, Hepatology and Nutrition(SigENP) carried out a systematic search on MEDLiNE via Pub Med(http://***/pubmed/) to identify all articles published in English from January 1990 to 2016; the following key words were used to conduct the electronic search: "upper giB" and "pediatric" [all fields]; "lower giB" and "pediatric" [all fields]; "obscure giB" and "pediatric" [all fields]; "giB" and "endoscopy" [all fields]; "giB" and "therapy" [all fields]. The identified publications included articles describing randomized controlled trials, reviews, case reports, cohort studies, casecontrol studies and observational studies. references from the pertinent articles were also reviewed. This paper expresses a position statement of SigENP that can have an immediate impact on clinical practice and for which sufficient evidence is not available in literature. The experts participating in this effort were selected according to their expertise and professional qualifications.

Pancreatic cancer is the 7th leading cause of death due to cancer in industrializedcountries and the 11th most common cancer globally, with 458918 new cases (2.5%of all cancers) and 432242 deaths (4.5% of all cancer deaths) in 2018. Unfortunately,80% to 90% of the patients present with unresectable disease, and thereported 5-year survival rate range between 10% and 25%, even after successfulresection with tumor-free margins. Systemic chemotherapy, radiotherapy, andminimally invasive image-guided procedures that have emerged over the pastyears, are used for the management of non-operable PC. This review focuses oncurrently available non-surgical options of locally advanced pancreatic cancer.
