Objectives:Thyroidectomy is among the most commonly performed head and neck surgeries,however,limited existing information is available on topics of interest and concern to *** Design:***:***:A search engine optimization tool was utilized to extract metadata on google‐suggested questions that“People Also Ask”(PAA)pertaining to“thyroidectomy”and“thyroid surgery.”These questions were categorized by Rothwell criteria and topics of *** Journal of the American Medical Association(JAMA)benchmark criteria enabled quality ***:A total of 250 PAA questions were ***‐oriented PAA questions describing what to expect during and after the surgery on topics such as postoperative management,risks or complications of surgery,and technical details were significantly less popular among the“thyroid surgery”group(Pg and hypocalcemia were nearly threefold more popular than those related to pain(335 and 319 vs.113 combined search engine response page count,respectively).The overall JAMA quality score remained low(2.50±1.07),despite an increasing number of patients searching for“thyroidectomy”(r(77)=0.30,P=0.007).Conclusions:Patients searching for the nonspecific term“thyroid surgery”received a curated collection of PAA questions that were significantly less likely to educate them on what to expect during and after surgery,as compared to patients with higher health literacy who search with the term“thyroidectomy.”This suggests that the content of PAA questions differs based on the presumed health literacy of the internet user.

Objectives:To date,no study provides a comprehensive analysis of traumatic orbital floor fractures across the United *** aimed to characterize patient demographics,injury-related variables,and operative management in this ***:The National Trauma Data Bank was queried for open or closed orbital floor fractures from 2008 to *** data were ***:Overall,148,592 orbital floor fractures were identified,with 142,577(95.9%)closed-and 6158(4.1%)open-type fractures.A total of 106,243(71.5%)patients were male and the median patient age was 41 *** majority of patients(79.2%)had abbreviated injury scale scores of≤2,indicating minor/moderate *** mechanism of injury(MOI)differed by gender,with the most frequent being unarmed fights in men(34.3%)and falls in women(14.0%).There were 29,600 patients(19.9%)with isolated orbital floor(I-OF)*** MOI most strongly associated with operative intervention of with I-OF fractures were penetrating injuries caused by a firearm(odds ratio[OR]:2.91;95%confidence interval[CI]:1.62–5.20)and cuttings/piercings(OR:2.17;95%CI:1.29–3.65).Conclusion:This large epidemiological study reveals that orbital floor fractures tend to present with minor or moderate injuries and are more likely to require operative intervention in setting of firearm or cut/pierce injuries.

Virtual Nursing Using Deep Belief Networks for Elderly People (DBN-EP)

The demand for better health services has resulted in the advancementof remote monitoring health, i.e., virtual nursing systems, to watch and supportthe elderly with innovative concepts such as being patient-centric, easier to use,and having smarter interactions and more accurate conclusions. While virtual nursing services attempt to provide consumers and medical practitioners with continuous medical and health monitoring services, access to allied healthcare expertssuch as nurses remains a challenge. In this research, we present Virtual NursingUsing Deep Belief Networks for Elderly People (DBN-EP), a new framework thatprovides a virtual nurse agent deployed on a senior citizen’s home, workplace, orcare centre to help manage their health condition on a continuous basis. Using thismethod, healthcare providers can assign various jobs to nurses by utilizing a general task definition mechanism, in which a task is defined as a combination ofmedical workflow, operational guidelines, and data gathered from a remotelymonitored virtual nursing system. Practitioners are in charge of DBN-EP andmake treatment decisions for patients. This allows a DBN-EP to act as a personalized full-time nurse for a client by carrying out practitioner support activitiesbased on information gathered about the client’s health. An electronic PersonalHealth Record (ePHR) system, such as a specialized web portal and mobile apps,could provide such patient information to elderly person family members and carecentres. We created a prototype system using a DBN-EP system that allows traditional client applications and healthcare provider systems to collaborate. Finally,we demonstrate how this system may benefit the elderly through a result anddebate.

Influence of Quantal and Statistical Fluctuations on Phase Transitions in Finite Nuclei

Investigations on thermal evolution of pairing-phase transition and shape-phase transition in light nuclei are made as a function of pair gap, deformation, temperature and angular momentum using a finite temperature statistical approach with main emphasis to fluctuations. The occurrence of a peak structure in the specific heat predicted as signals of the pairing-phase and shape-phase transitions are reviewed and it is found that they are not actually true phase transitions and it is only an artifact of the mean field models. Since quantal number and spin fluctuations and statistical fluctuations in pair gap, deformation degrees of freedom and energy when incorporated, it wash out the pairing-phase transition and smooth out the shape-phase transition. Phase transitions due to collapse of pair gap and deformation is discussed and a clear picture of pairing-phase transition in light nuclei is presented in which pairing transition is reconciled.

Recent mobile broadband networks require heterogeneous networks supporting high capacity on demand.A hybrid Fiber-Wireless(Fi-Wi)access network integrating Differential Amplitude and Phase Shift Keying(DAPSK)based-Orthogonal Frequency Division Multiple Access(OFDMA)Passive Optical Network(PON)and 4g wireless network using Radio over Fiber(Ro F)technique is *** proposed heterogeneous network is simulated,and the performance is analyzed in terms of Bit Error Rate(BER),Error Vector Magnitude(EVM),Capacity,and Spectral *** proposed network is simulated considering a higher data rate of 10 and 25 gbps,and the effect of system parameters like Laser Power,Fiber Length,and Number of users is *** results show that a higher network capacity of 720 gbps with an average capacity of about 45 gbps and a higher spectral efficiency of 4.85 bps/Hz is achieved for the multi-user heterogeneous network link with sixteen *** minimum value of optical power sufficient to achieve the desired BER is found to be-6 *** suitability of the proposed integrated architecture in supporting multiple services is analyzed by considering different services at each *** spectral efficiency of the multi-service link varies from 3 to 4 bps/Hz.

Distribution of persistent organic pollutants in aggregate fractions of a temperate forest and semi-rural soil

The fate of persistent organic pollutants (POPs) and their interactions with aggregates of forest soils are not completely understood. Our objectives here were to quantify the distribution of different POPs in water-stable aggregate fractions and to study their influence on soil organic carbon (Corg) content. Soil samples were taken from a forest-site, gogerddan (g) and a semi-rural site, Hazelrigg (H) in great Britain, from 0-2 and 2-5 cm and 0-4 and 8-12 cm soil depth, respectively. POPs analyzed were PAHs, PCBs, total DDT, PBDEs and HCB. The bulk soil analysis showed that the concentration of POPs was significantly higher (p ≤ 0.05) in forest site g than in semi-rural site H, particularly at the surface soil levels compared to the subsurface soil depths in both sites. Total concentrations of PCBs and PAHs of both sites were positively correlated with Corg contents. POPs concentrations and Corg, Nt contents of forest site g were significantly higher (p ≤ 0.05) in water-stable macro aggregates (>0.25, >1, >2 mm) than the micro aggregates (>0.053 mm). The POP concentrations of all aggregate fractions after normalizing to their respective Corg content were increased due higher contamination and strong sorption by Corg. These results showed a strong effect of Corg on the partitioning of organic pollutants to soil aggregate size fractions. The present study affirms the ecological significance of forest soils act as a potential sink of POPs. In summary, our results suggest that aggregate fractions may promote soil C storage and act as a potential POP sink in surface soil without increasing their concentration in the aggregate fraction of subsoil.

In this study,we identified the most deleterious nsSNP in RB1 gene through structural and functional properties of its protein (pRB) and investigated its binding affinity with *** of 956 SNPs,we investigated 12 nsSNPs in coding region in which three of them (SNPids rs3092895,rs3092903 and rs3092905) are commonly found to be damaged by I-Mutant 2.0,SIFT and PolyPhen *** this effort,we modeled the mutant pRB proteins based on these deleterious *** a comparison of total energy,stabilizing residues and RMSD of these three mutant proteins with native pRB protein,we identified that the major mutation is from glutamic acid to glycine at the residue position of 746 of ***,we compared the binding efficiency of both native and mutant pRB (E746g) with *** found that mutant pRB has less binding affinity with E2F-2 as compared to native *** is due to sixteen hydrogen bonding and two salt bridges that exist between native type and E2F-2,whereas mutant type makes only thirteen hydrogen bonds and one salt bridge with *** on our investigation,we propose that the SNP with an id rs3092905 could be the most deleterious nsSNP in RB1 gene causing retinoblastoma.
