Introduction: Fatig.e fractures are secondary to excessive, unusual, repeated mechanical stress, occurring.on healthy bone. Bone scintig.aphy (BS) with 99mTc-HMDP is an excellent means of diag.osis. We report a case of stress fracture of the calcaneus with scintig.aphic diag.osis, in a marathon runner. Patient and Methods: He is a 40-year-old athlete in overtraining.with rig.t calcaneal pain on running. intense at first, moderate in the middle and then persistent after exercise. The interrog.tion found no trauma to the rig.t foot and the x-ray of the foot was without abnormality. The so 3 phases were performed following.an injection of 564 MBq of 99mTc-HMDP, with a SPECT g.mma camera. Results: Bone scintig.aphy found in the early phase, hyper-perfusion and hyper-fixation of the rig.t calcaneal reg.on compared to its left counterpart and in the bone phase persistence of focal hyperfixation opposite the rig.t calcaneus. This scintig.aphic aspect of the rig.t calcaneus in an overtrained marathon runner, in a painful context and in front of a normal X-ray, made us evoke a fracture of fatig.e. A complementary CT scan performed two days after the BS was without abnormality. Conclusion: Fatig.e fractures are micro-fractures g.nerally not noticed on radiog.aphy and CT. BS due to its hig. sensitivity is an excellent or the best tool for the early diag.osis of stress fractures.

The aim of this study was to evaluate the dose delivered to patients during.scannog.aphic examinations at the Kaolack Reg.onal Hospital (one of the 14 reg.ons of Seneg.l located in the center-west of the country, 192 km from Dakar) and to compare these irradiation doses at diag.ostic reference levels found in the literature in order to optimize our scanning.protocols. To achieve this g.al, we carried out a prospective study involving.218 CT scans. These examinations focused on cerebral, thoracic, abdominopelvic and lumbar spine acquisitions made in adults and cerebral only in children. We compared the median values of dosimetric indicators (CTDIvol and DLP) per acquisition with NRDs in the literature. During.the course of this study, we found a dosimetric ratio lower than that of the NRDs for thoracic, abdomino-pelvic and lumbar spine CT scans in adults and a sig.ificant dosimetric exceedance for brain scans in adults and in children. These results encourag. us to extend these dosimetric evaluations to other hospital structures in order to establish rig.rous monitoring.of dosimetric indicators and to respect the principles of radiation protection and g.od practice.

Evaluation of the Effects of Irradiation of Peanut g.ain by a g.mma-Ray Beam on Culture

The problems of ag.iculture in Seneg.l result in a low yield per hectare and poor seed quality contributing.strong.y to the decline in productivity. Mutag.nesis by X- or Y-ray irradiation makes it possible to obtain g.netic mutants necessary to improve production. It is in this context that we undertook this study to evaluate the effects induced on the cultivation of peanut seeds irradiated by X-rays at low doses applied in radiotherapy. X-ray irradiation of four (2, 3, 4, 5) lots of peanuts are performed with respectively 0.5, 1.5, 2 and 4 g.ay using.the cobalt 60 therapy device (Alcyon II). The seeding.of the seeds and then the following.up of the crops during.35 days allowed us to study the parameters of g.rmination, g.owth and yield. The results obtained after monitoring.revealed that the irradiation did not have any sig.ificant impact on g.rmination and would appear to temporarily inhibit the g.owth rate compared to the control batch. However, the decrease in weig.t of the harvested seeds can be explained by the absence of fertilizer during.the cultivation.

The aim of the study was to evaluate the contribution to moscintig.aphy of myocardial perfusion (TSMP) with 99mTc-sestamibi in the manag.ment of suspected or known coronary patients’s population. It included 42 patients with an averag. ag. of 57.02 ±9.77 years with extremes rang.ng.from 36 to 77 years. The pre-test probability in 38 patients was hig. in 7 patients (18.42%), intermediate in 29 patients (76.31%) and low in 2 patients (5.26%). Patients with a hig. pre-test probability had a positive scintig.aphy. Among.the 26 patients who underwent transthoracic ultrasound (TTU), 10 with normal kinetics on trans-thoracic ultrasound (TTU) had positive scintig.aphy. For 16 patients with TTU abnormalities, 6 had neg.tive scintig.aphy and 10 had positive scintig.aphy. The TSMP was positive for 28 patients (66.66%). Among.these patients, 16 had pure ischemia, 57.14% (16/28), 7 pure necrosis (25%), and 5 both necrosis and ischemia. Myocardial perfusion scintig.aphy remains a very contributory consideration in the manag.ment of coronary disease. This is synchronized with the electrocardiog.am (ECg. and is an important diag.ostic and prog.ostic tool for coronary artery disease. It also provides a g.od indication of coronary ang.og.aphy.

Numerical Approach of Network Problems in Optimal Mass Transportation

In this paper, we focus on the theoretical and numerical aspects of network problems. For an illustration, we consider the urban traffic problems. And our effort is concentrated on the numerical questions to locate the optimal network in a g.ven domain (for example a town). Mainly, our aim is to find the network so as the distance between the population position and the network is minimized. Another problem that we are interested is to g.ve an numerical approach of the Mong. and Kantorovitch problems. In the literature, many formulations (see for example [1-4]) have not yet practical applications which deal with the permutation of points. Let us mention interesting.numerical works due to E. Oudet beg.n since at least in 2002. He used g.netic alg.rithms to identify optimal network (see [5]). In this paper we introduce a new reformulation of the problem by introducing.permutations . And some examples, based on realistic scenarios, are solved.

Controllability Approach for a Fluid Structure Interaction Problem

The present paper presents a new method to solve fluid structure interaction problem. Our computational method is based on controllability approach. g.ven a targ.t structural displacement we will find a control steering.the displacement of the structure u to . We need to define a payoff functional (J): where u solves the structure equation for the control and is a fixed value. Our aim is to find a control which minimizes the payoff criterion. And therefore we find u the beam displacement, v the velocity of the fluid and p the pressure of the fluid.

Objective Patients with cong.nital malformations(CMs)of the g.strointestinal tract(g.T)have a very hig. ***,the literature on the factors associated with mortality in these patients is scarce in sub-Saharan *** aim of this study is to identify independent risk factors for mortality in patients with CMs of the g.T at our pediatric surg.cal *** We conducted a retrospective analysis of cases with CMs of the g.T manag.d at a tertiary center from 2018 to *** were subdivided into two g.oups based on the outcomes,and variables with a sig.ificant difference were analyzed by log.stic *** Our review included 226 patients,63 of whom died(27.88%).Patient ag. rang.d from 0 to 15 *** into account statistical sig.ificance,mortality was more frequent in neonates than in older patients(57.30%vs 6.15%),in patients coming.out of the Dakar area than in those from the Dakar area(43.75%vs 19.18%),in patients with abnormal prenatal ultrasound than in those with normal ultrasound(100%vs 26.67%),in premature children than in those born at term(78.57%vs 21.87%),in patients with an additional malformation than in those with an isolated malformation(69.23%vs 25.35%),and in those with intestinal,esophag.al,duodenal and colonic atresia than in those with other diag.oses(100%,89%,56.25%and 50%,respectively).Referred patients died more than those who chang.d hospitals or came from home(55.29%vs 25%and 9.09%,respectively).On multivariable log.stic reg.ession,two independent factors of mortality were identified:presence of associated malformation[odds ratio(OR)=13.299;95%Confidence interval(CI)1.370 to 129.137]and diag.osis of esophag.al atresia(OR=46.529;95%CI 5.828 to 371.425).Conclusion The presence of an associated malformation or diag.osis of esophag.al atresia increases mortality in patients with CMs of the g.T in our environment.

Method of Successive Approximations for a Fluid Structure Interaction Problem

In this paper, we present a method for solving.coupled problem. This method is mainly based on the successive approximations method. The external force acting.on the structure is replaced by λ = p (x1, H + u (x1, λ)). Then we have a nonlinear equation of unknown?λ to solve by successive approximations method. By this method, we obtain easily the analytic expression of the displacement. In addition, g.od results are obtained with only a few iterations.
