Management of proximal tibial stress fracture associated with advanced knee osteoarthritis:A systematic review

Purpose:Tibial stress fracture associated with knee osteoarthritis is an unusual and difficult clinical *** is no clear existing treatment guideline for this uncommon clinical *** aim of this study is to review the impact of various treatment options for patients with advanced knee osteoarthritis associated with proximal tibial stress ***:The study was performed using the databases of PubMed and *** index for non-randomized studies score was used to evaluate the included studies’*** concluded data included the treatment approach,reported outcome measure,and time to fracture *** literature search was started in December 2021 and accomplished in January 2022.A narrative description of the different methods and comparison of their results were ***:Out of total assessed 69 studies,9 studies were included in our *** commonest treatment approach used was total knee arthroplasty by long tibial stem *** mean preoperative knee society score and knee functional score were 30.62 and 23.17,*** mean postoperative knee society knee score was 86.87,while the functional score was *** average reported time to achieve fracture union was 4 months(a range of 2.07-5.50 months).Conclusion:The optimal clinical outcome for treating either acute or mobile tibial stress fracture in patients with advanced knee osteoarthritis can be achieved with long stem total knee ***,due to heterogeneity of data,comparison of different treatment options for chronic proximal tibial stress fracture mal-union/non-union coexisting with knee osteoarthritic and such inferences need to be judged cautiously.

Influence of Machining Parameter on Surface Roughness and Tool Life While Machining EN24 grade Alloy Steel

The requirements of high quality machined surface as well as demand of enhanced contact time of cutting tools drive towards adopting multilayer coated carbide inserts. The industry requires higher productivity, hence higher machining parameters need to be used in order to meet the industry requirements. The alloy steel material used to fabricate machine parts consists of alloying elements like nickel, chromium and molybdenum difficult to machine, since the cutting tool fails by high tool wear, if we use uncoated carbide inserts to machine alloy steels. Hence in the present research work it is intended to use tungsten carbide inserts coated with different coatings for the experiments. The turning experiments were carried out using different grades of uncoated and coated carbide inserts of identical tool signature. The cutting speed selected for the experiments was 100 to 500 m/min in steps of 100 m/min, and the feed per revolution was 0.1 mm to 0.4 mm in step of 0.1 mm. The experimentation was carried out following ISO3685 standards. The results of the experiments revealed that the surface roughness measured was the least at cutting speed 500 m/min and feed per revolution of 0.1 mm, however the chip breaking found better when the feed used was greater than 0.2 mm/revolution.

Acute Toxicity of Three Dimensional Conformal Concurrent Chemoradiation with Dose Optimization for External Beam Radiation in Carcinoma Cervix—A Prospective Study

Aim: To prospectively evaluate the acute toxicity of 3D conformal radiotherapy with dose optimization in patients with carcinoma cervix. Materials and Methods: Carcinoma cervix patients stage IIB to IIIB (n = 30) treated during November 2011 to May 2013 at the institution with 3D conformal chemo-radiation were included in the study. They received weekly Cisplatin 40 mg/m2 for a maximum of 5 cycles. They received 46 gy/23 fractions, 5 fractions per week of external beam radiation. In these patients dose optimization was done in order to achieve a tumor maximum dose (Dmax) around 105%. Various techniques were used for dose optimization which included the use of sub fields, adjusting the weightages, using wedges and the use of mixed energies. EBRT (External Beam Radiotherapy) was followed by two fractions of high dose rate intracavitary brachytherapy of 9 gy each. Acute RTOg toxicity was assessed weekly during EBRT and 1 week post EBRT. Results: The median age of the patients was 45 (range: 30 - 55 years). All the patients completed EBRT;63.3% of the patients received all 5 cycles of chemotherapy while 26.6% of the patients received 4 cycles of chemotherapy and 10% of the patients received 3 cycles of chemotherapy. The most predominant toxicity seen was gI toxicity, diarrhea being the most common gI toxicity followed by vomiting. Neutropenia was the most common hematological toxicity. Most patients had grade 0 and grade 1 toxicity. None of the patients had grade 4 toxicity while few had grade 2 and 3 toxicity. Conclusion: 3D conformal concurrent chemo radiotherapy with Dmaxaround 105% reduces acute RTOg toxicity particularly grade 3 and 4 and improves patient compliance for concurrent chemo-radiotherapy.

Circular RNAs(circRNAs)are a recently dis-covered type of non‐coding RNA derived from pre‐mRNAs.R‐loops consist of a DNA:RNA hybrid andthe associated single‐stranded ***-dopsis thaliana,circRNA:DNA R‐loops regulatealternative splicing(AS)ofSEPALLATA3(SEP3).However,the occurrence and functions ofcircRNAs and R‐loops inPopulus trichocarpaarelargely ***,we performed circRNA‐enriched sequencing in the stem‐differentiatingxylem(SDX)*** identified 2,742distinct circRNAs,including circ‐CESA4,circ‐IRX7,and circ‐gUX1,which are generated from genesinvolved in cellulose,and hemicellulose biosyn-thesis,*** investigate the roles ofcircRNAs in modulating alternative splicing(AS),we detected 7,836 AS events using PacBio Iso‐Seq and identified 634 circRNAs that overlappedwith 699 AS ***,using DNA:RNAhybrid immunoprecipitation followed by se-quencing(DRIP‐seq),we identified 8,932 R‐looppeaks that overlapped with 181 circRNAs and 672AS ***,several SDX‐related circRNAsoverlapped with R‐loop peaks,pointing to theirpossible roles in modulating AS in ***,overexpressing circ‐IRX7increased the levels ofR‐loop structures and decreased the frequency ofintron retention in *** provides a valuable R‐loop atlas resourceand uncovers the interplay between circRNAs andAS in SDX ***.
