Background: Congenital heart disease is a public health issue due to its incidence and mortality rate. The aim of this study was to investigate the long-term mortality of children with congenital heart disease admitted to the departmental University Hospital of Borgou/Alibori (CHUd-B/A) from 2011 to 2022. methods: This descriptive longitudinal study with analytical aims covered 11 years (April 1, 2011 to december 31, 2022). It consisted of a review of the records of children under 15 years of age with echocardiographically confirmed congenital heart disease. This was followed by an interview with the parents to assess the children’s current condition. data were entered using Kobocollect software and analyzed using R Studio 4.2.2. software. Results: A total of 143 complete files were retained. The median age at diagnosis was 14 months (IIQ: Q1 = 4;Q3 = 60) with a range of 2 days and 175 months, and the sex-ratio (m/F) was 0.96. Left-to-right shunts were the most frequent cardiopathy group (62.9%). Only 35 children (24.5%) benefited from restorative treatment. The mortality rate was 31.5%. median survival under the maximum bias assumption was 114 months and 216 months under the assumption of minimum bias. Survival was significantly better in children with right-to-left shunts (p = 0.0049) under the assumption of minimum bias. The death risk factors were: age at diagnosis less than 12 months (aHR = 7.58;95% CI = 3.36 - 17.24;p Conclusion: The long-term mortality of congenital heart disease is high and favoured by the absence of restorative treatment. Local correction of congenital heart disease and medical follow-up will help to reduce this mortality.

Audits of death in HIV-Infected Children and Adolescents Followed up in the Pediatric department of the Regional Teaching Hospital of Borgou/Alibori from 2005 to 2020

Introduction: Human immunodeficiency virus (HIV) is a major public health problem with high morbidity and mortality among children. The objective of this work was to audit the deaths of children and adolescents with HIV infection followed up in the pediatric department of the Regional Teaching Hospital of Borgou/Alibori (CHUdB/A) the from 2005 to 2020. Patients and method: This was a retrospective and descriptive study conducted in the pediatric department of CHUd/B-A in Parakou. All children with HIV infection who died from January 1, 2005 to August 31, 2020 were included. data collection was carried out in three stages: a phase of medical records processing, a phase of community survey and a phase of death audits. The variables studied were sociodemographic, clinical, biological, therapeutic and evolutionary. Results: Over the study period, the data of 464 infected children were recorded, including 92 deaths, representing a case fatality rate of 19.83%. Severe acute malnutrition (69.23%), gastro-intestinal tract infections (43.58%) and serious opportunistic pulmonary infections (24.36% pulmonary tuberculosis and 19.23% pneumocystis) were the main causes of death. The main dysfunctions found were: the delayed diagnosis of HIV infection (79.35%), the absence or delay in consultation when the child’s clinical condition deteriorates (32.61% and 47.83%), delayed initiation of antiretroviral treatment (42.39%) and non-adherence to treatment (38.04%). Non-adherence to treatment was predominant in adolescents (90.49%). Conclusion: Specific interventions for early detection, adequate nutritional care, psychosocial support for adolescents and mothers of children are necessary to reduce mortality due to HIV among children and adolescents.

Introduction: Sickle cell disease is a public health problem in sub-Saharan Africa. A national referral center for the management of infants and pregnant women with sickle cell disease (CPmI-NFEd) was created three decades ago in Cotonou, in the south of Benin with two regional branches including that of Parakou in the North for better access of patients to specialized care. This work is a review of five years of activities in order to describe the epidemiological, clinical, hematological and evolutionary profiles of the children followed up in the said branch. method: This was a descriptive and retrospective cross-sectional study on the medical records of children with sickle cell disease, followed up at the regional branch of CPmI-NFEd in Borgou/Alibori from June 1, 2017 to may 31, 2022. The variables studied were epidemiological, clinical, biological and evolutionary. Results: A total of 101 children with sickle cell disease were included in the study, including 78 homozygous SS (77.2%) and 23 heterozygous SC (22.8%). Their mean age at inclusion was 51.2 ± 37.6 months [6 - 204]. The sex ratio was 1.4. Vaso-occlusive crises were the main diagnostic circumstances in 42.3% of homozygotes. more than half of the children (51.5%) had a regular follow-up. The average baseline level of hemoglobin (Hb) in homozygous children was 8.8 ± 1.4 g/dl [5.8 - 11.5];and the rate of Hb S varied between 61.9 and 94.7%. In heterozygous SC children, the mean baseline level of Hb was 10.7 ± 0.6 g/dl [9.7 - 11.5]. Acute complications observed during follow-up were dominated by pneumonia and vaso-occlusive crises in both phenotypes. The overall mortality was 3% and only affected homozygous patients. Conclusion: On average, three out of four children were homozygous in our cohort. The main acute complications were infectious and vaso-occlusive. The mortality only affected homozygous carriers. Community Perception of Riparian Corridors Ecosystem Services and Implications for Environmental Education in Upper Oueme Catchment in Benin, West Africa

The assessment of the local community perception of the value of riparian corridors is relevant to understand their attitude towards the conservation of such ecosystem. We conducted a semi-structured survey on the perception and importance attributed to the ecosystem services (ESs) provided by riparian corridors in 368 households across 70 villages located in a buffer zone of 5 km of servitude around the permanent rivers of the Upper Oueme watershed in Benin. We found that local communities easily reported provisioning and cultural ESs than regulating and supporting ones, indicating their misunderstanding of the main role of riparian corridor. moreover, the supply of cropping areas was perceived as the most important ES. Educated respondents and high-income households had more knowledge of regulating ESs and supporting ESs, respectively. Overall, the highly perceived importance of the provision of cropping areas indicates a potential risk of agricultural encroachment of riparian corridors. We suggest the consideration of the current local perception of riparian corridor’s role in designing a sound environmental education aiming at the change of local population’s perception. This perception shift will promote a sustainable management of the riparian corridors.

Control system design for a pressure-tube-type supercritical water-cooled nuclear reactor via a higher order sliding mode method

Assessment of the Recent Hydromorphological Features of Nokoue Lake and Its Channels (South-East Benin)

Based on 2022 and 2023 hydrometric data and satellite images (Sentinel 2022, SPOT 2010), this study aims to present the Nokoué Lake and its channels’ re-cent hydromorphological characteristics. Integrating flow, tributary morphology, and topography data determined specific power values along the axes studied. The values obtained range from 2.69 to 12.92 W/m2 for Ouémé River and 2.46 to 10.99 W/m2 for Sô River. The resulting water erosion on banks and bottoms is of linear, areolar, or gully and claw types. Lake bathymetry varies from -0.5 to -2.6 m (low flow period) and -1 to -4 m;in the Ouémé, Sô, and Totchè rivers, it varies from -5 m to -7 m, reaching -10 m at the Cotonou channel entrance (flood period). Bathymetric profiles reveal varied “U”, “V” and “Intermediate” bottom morphologies, influenced by erosion/sedimentation processes and human activities. The flow facies identified are lentic in the northern tributaries and lotic in the Cotonou and Totchè canals. Spatial analysis identified nine (09) thematic classes. In 2022, the surface area of the water body has increased from 274 km2 at low water to 280 km2 at high water, whereas in 2010 (a recent year of exceptional flooding), the surface area was 270 km2 at low water and 277 km2 at high water. Significant changes in land use are observed between 2010 and 2022. The floodplain area decreased slightly, from 421 km2 in 2010 (year of exceptional flooding) to 419 km2 in 2022. The evolution of land use shows a progressive expansion of the urban environment to the detriment of the natural environment. In the medium to long term, this trend could threaten the hydromorphological balance and even the existence of this important lagoon ecosystem.

Socioenvironmental drivers of Farmers’ Perceptions of Climate Change Risk in Agroforestry Parklands of West Atacora in Benin (West Africa)

Throughout the world, climate change is threatening the human population. In West Africa, smallholder farmers in indigenous agricultural societies typically hold considerable knowledge. Therefore, this study was conducted in West Atacora of Benin Republic to assess the drivers of farmers’ perceptions of climate change risk. We used a random sampling technique to select 360 households’ heads who were interviewed regarding different climate change risks perception. Binomial logistic regression was used to assess the drivers of farmers’ perceptions of climate change risks. The results showed that the farmers in drier areas had a higher perception of the global risk of climate change than those in humid areas. The same trend was observed for the seven different individual’s climate change risk investigated. The study identified also membership of farm organizations as main sociodemographic characteristic that explains farmers’ perception of climate change risk perception. These findings are helpful tools to sensitize the local people on climate change risk and cope with the risk in agricultural lands.
