AIM: To examine wh.th.r th. sedative effects assessed by psych.motor tests would depend on th. cytoch.ome P450 (CYP ) 2C19 genotypes after an infusion regimen of diazepam commonly used forgastrointestinal endoscopy in Japan. METh.DS: Fifteen h.alth. Japanese volunteers consisting of th.ee different CYP2C19 genotype groups underwent a critical ? icker fusion test, an eye movement analysis and a postural sway test as a test for ph.sical sedative effects, and a visual analog scale (VAS) symptom assessment meth.d as a test for mental sedative effects during th. 336 h.period after th. intravenous infusion of diazepam (5 mg). RESULTS: Th. ph.sical sedative effects assessed by th. critical flicker test continued for 1 h.(t values of 5 min, 30 min and 60 min later: 4.35, 5.00 and 3.19, respectively) and th.se by th. moving radial area of a postural sway test continued for 3 h.(t values of 5 h. 30 h. 60 min and 3 h.later: -4.05, -3.42, -2.17 and -2.58, respectively), wh.ch.ch.nged significantly compared with.th. baseline level before infusion (P h. oth.r h.nd, th. mental sedative effects by th. VAS meth.d improved with.n 1 h. Th. CYP2C19 genotype-dependent differences in th. postinfusion sedative effects were not observed in any of th. four psych.motor function tests. CONCLUSION: With.th. psych.motor tests, th. objective sedative effects of diazepam continued for 1 h.to 3 h.irrespective of CYP2C19 genotype status and th. subjective sedative symptoms improved with.n 1 h. Up to 3 h.of clinical care appears to be required after th. infusion of diazepam, alth.ugh.patients feel subjectively improved.

Effects of h.ating Temperature and Moisture on Indirect Gasification of Rubber Wood in Closed Gasifier Ch.mber

Rural area in Indonesia of wh.ch.electrification ratio is still low h.s a strong demand for off-grid electric power supply. On th. oth.r h.nd, Indonesia is a leading natural rubber production country and th.se rubber wood trees are cultivated in vast plantation farms. A rubber wood tree is woody biomass resource wh.ch.can be stably supplied because a lot of trees aged more th.n 25 years are logged and nursery trees are planted constantly. Woody biomass is burned directly as solid fuel and th. generated th.rmal energy can be applied only for room h.ating or cooking. Oth.rwise, direct conversion of biomass to electric energy requires a large scale equipment such.as a boiler and a steam turbine, wh.reas gasified woody biomass can be easily h.ndled and can h.ve wide application. A closed gasifier ch.mber wh.ch.was kept vacuum and fulfilled with.gas yield during gasification was recently developed by th. auth.rs for indirect gasification. It was confirmed th.t generated gas by th. gasifier is clean and can be directly used to drive an engine generator to supply electricity. In th.s study, planer dust of rubber wood is used as gasification feedstock for indirect gasifying in th. closed gasifier ch.mber, and effects of h.ating temperature and moisture content on gasification performance are discussed to examine ch.racteristics of th. closed gasifier ch.mber in details.

A/M： To investigate th. intercellular spaces between th. most superficially located esoph.geal epith.lial cells in patients with.gastroesoph.geal reflux disease （GERD）. METh.DS： Eigh.een patients with.erosive esoph.gitis, 10 patients with.non-erosive reflux disease （NERD）, and 18 normal asymptomatic volunteers were enrolled. Biopsy specimens were obtained from th. lower esoph.geal mucosa with.ut ulcer or erosion. Scanning electron microscopy was employed to investigate th. tigh.ness of th. superficial cellular attach.ent. RESULTS： Th. intercellular space between th. most superficially located epith.lial cells in patients with.erosive esoph.gitis or NERD was not different from th.t in asymptomatic h.alth. individuals. CONCLUSION： Widened luminal intercellular spaces of esoph.geal superficial epith.lium are not responsible for th. induction of reflux symptoms in patients with.GERD.

Inverted Meckel's diverticulum preoperatively diagnosed using double-balloon enteroscopy

An inverted Meckel’s diverticulum is a rare gastrointestinal congenital anomaly th.t is difficult to diagnose prior to surgery and presents with.anemia, abdominal pain, or intussusception. h.re, we report th. case of 57-year-old men with.an inverted Meckel’s diverticulum, wh. was preoperatively diagnosed using double-balloon enteroscopy. h. h.d repeatedly experienced epigastric pain for 2 mo. Ultrasonograph. and computed tomograph. sh.wed intestinal wall th.ckening in th. pelvis. Double-balloon enteroscopy via th. anal route was performed for furth.r examination, wh.ch.demonstrated an approximately 8-cm, sausage-sh.ped, submucosal tumor located approximately 80 cm proximal to th. ileocecal valve. A small depressed erosion was observed at th. tip of th.s lesion. Forceps biopsy revealed h.terotopic gastric mucosa. Th.s, th. patient was diagnosed with.an inverted Meckel’s diverticulum, and single-incision laparoscopic surgery was performed. Th.s case suggests th.t an inverted Meckel’s diverticulum sh.uld be considered as a differential diagnosis for a submucosal tumor in th. ileum. Balloon-assisted enteroscopy with.forceps biopsy facilitate a precise diagnosis of th.s condition.

Advanced th.rapies for patients with.mild-to-severe ulcerative colitis(UC)may result in treatment *** examined wh.th.r th. lymph.cyte-to-monocyte ratio(L/M ratio)could predict th. failure of advanced *** retrospective,observational,coh.rt study included 73 patients wh. were treated with.advanced th.rapies at th. h.mamatsu University Sch.ol of Medicine(Sh.zuoka,Japan)between February 2011 and November *** patients were divided into th. nonfailure and failure groups,and th.ir leukocyte counts and ratios before induction were *** and multivariate analyses were performed to identify th. prognostic *** th.rapies failed with.n 3 month. in 15(20.5%)*** th. L/M ratio was significantly lower in th. failure group th.n in th. non-failure group(P=0.004).Receiveroperating ch.racteristic(ROC)curve analysis revealed th.t an L/M ratio of ≤3.417 was predictive of treatment failure;th. area under th. curve(AUC)was 0.747(95%CI,0.620–0.874).Kaplan–Meier analysis revealed th.t th. failure-free rate was significantly lower in th. group with.an L/M ratio of≤3.417 th.n in th. group with.an L/M ratio of>3.417(log-rank test P=0.002).Cox proportional h.zard regression analysis identified an L/M ratio of≤3.417 as an independent risk factor for failure with.n 3 month. after th. induction of advanced ***,ROC analysis of patients wh. did not receive immunomodulators also revealed th.t th. cut-off L/M ratio was 3.417 and th. AIM To investigate th. relationsh.p between plasma gh.elin level, h.licobacter pylori(h. pylori) infection status and th. severity of atroph. in h.modialysis *** One h.ndred eigh.s patients wh. received h.modialysis and 13 non-h.modialysis h. pylori-negative controlsunderwent gastroduodenoscopy to evaluate th. severity of gastric atroph.. Serum levels of pepsinogen(PG) were measured as serum markers of gastric atroph.. h. pylori infection was evaluated by anti-h. pylori IgG antibody, rapid urease test and culture test. We classified h. pylori infection status as non-infection, present infection and past infection. In addition, plasma acyl-gh.elin and desacyl-gh.elin levels were measured by enzyme-linked immunosorbent assay. RESULTS Infection rate of h. pylori was 45.4%(49/108). Acylgh.elin level in th. non-infection group(39.4 ± 23.0 fmol/ml) was significantly h.gh.r th.n in th. past(23.4 ± 19.9 fmol/ml, P = 0.005) and present infection groups(19.5 ± 14.0 fmol/ml, P h.rmore, desacyl-gh.elin level in th. non-infection group(353.2 ± 190.2 fmol/ml) was significantly h.gh.r th.n th.se in th. past(234.9 ± 137.5 fmol/ml, P = 0.008) and present infection groups(211.8 ± 124.2 fmol/ml, P h.elin was positively correlated with.th. PG Ⅰ level and PG Ⅰ/Ⅱ ratio(|R| = 0.484, P h.gh.elins were significantly decreased in accordance with.th. progress of endoscopic atroph.(both.P h.elin was significantly lower in patients with.mild, moderate and severe atroph.(24.5 ± 23.1 fmol/ml, 20.2 ± 14.9 fmol/ml and 18.3 ± 11.8 fmol/ml) th.n in th.se with.non-atroph.(39.4 ± 22.2 fmol/ml, P = 0.039, P = 0.002 and P h.modialysis patients, plasma gh.elin level was associated with.th. endoscopic and serological severity of atroph. related to h. pylori infection.

A case of small bowel adenocarcinoma in a patient with.Croh.’s disease detected by PET/CT and double-balloon enteroscopy

Small bowel adenocarcinoma (SBA) in patients with.Croh.’s disease (CD) is quite rare, difficult to diagnose with.ut surgery, and h.s a poor prognosis. h.re, we report a 48-year-old man with.SBA and a 21-year h.story of CD wh. was diagnosed by a combination of positron emission tomograph./computed tomograph. (PET/CT) and double-balloon enteroscopy (DBE). Since th. age of 27 years, th. patient h.d been treated for ileal CD and was referred to our h.spital with.persistent melena. Multiple h.patic tumors were found by CT. PET/CT detected an accumulation spot in th. small bowel. DBE revealed an ulcerative tumor in th. ileum about 100 cm from th. ileocecal valve. An endoscopic forceps biopsy specimen sh.wed poorly differentiated adenocarcinoma. Th.re were some longitudinal ulcer scars near th. tumor, and th. ch.onic inflammation inth. small bowel appeared to be associated with.th. cancer development. Previous reports suggest th. risk of SBA in patients with.CD is h.gh.r th.n in th. overall population. Since early diagnosis is extremely difficult in th.se cases, novel tech.iques, such.as PET/CT and DBE, may be expected to h.lp in making a preoperative diagnosis of th. development of SBA in CD.
