AIM To evaluate the potential effectiveness of robot-assisted gastrectomy(RAG) in comparison to open gastrectomy(OG) for gastric cancer *** A comprehensive systematic literature search using PubM ed,EMBASE,and the Cochrane Library was carried out to identify studies comparing RAG and OG in gastric *** of any age and sex were considered for inclusion in comparative studies of the two techniques independently from type of gastrectomy.A meta-analysis of short-term perioperative outcomes was performed to evaluate whether RAG is equivalent to *** primary outcome measures were set for estimated blood loss,operative time,conversion rate,morbidity,and hospital *** among postoperative complications,wound infection,bleeding and anastomotic leakage were also *** A total of 6 articles,5 retrospective and 1 randomized controlled study,involving 6123 patients overall,with 689(11.3%) cases submitted to RAG and 5434(88.7%) to OG,satisfied the eligibility criteria and were included in the *** was associated with longer operation time than OG(weighted mean difference 72.20 min;P < 0.001),but with reduction in blood loss and shorter hospital stay(weighted mean difference-166.83 mL and-1.97 d respectively;P < 0.001).No differences were found with respect to overall postoperative complications(P = 0.65),wound infection(P = 0.35),bleeding(P = 0.65),and anastomotic leakage(P = 0.06).The postoperative mortality rates were similar between the two *** respect to oncological outcomes,no statistical differences among the number of harvested lymph nodes were found(weighted mean difference-1.12;P = 0.10).CONCLUSION RAG seems to be a technically valid alternative to OG for performing radical gastrectomy in gastric cancer resulting in safe complications.

The use of dendrochronology to study and date geomorphic processes in volcanic environments is still incipient, even more so on the volcanic slopes covered by temperate forests in central Mexico. Mass movements, such as debris flows, often impact forest stands where they cause damage to individual trees, thereby generating growth disturbances(GD) in the tree-ring records. The identification and dating of GD enables reconstruction of the age of trees colonizing bare surfaces after major events, but also allows the assessment of the frequency or spatial distribution of past geomorphic process activity. Here we used increment cores from 65 Pinus leiophylla, Abies religiosa, and Alnus jorullensis trees growing in the Axal gorge, on the southern slopes of La Malinche volcano, to unravel past debris-flow activity both temporally and spatially. Based on the combination of GD records, a weighted tree response index(Wit), field evidence and hydrometeorological records, we reconstructed 23 debris flows since ***, almost two-thirds of the reconstructed years with debris-flow activity in Axal gorge match with events recorded in Axaltzintle gorge located on the NE slopes of La Malinche. These findings suggest a regional triggering mechanism, most likely related to the occurrence of hurricanes. This research could be useful for disaster risk management of the La Malinche National Park.

Intraperitoneal(IP)delivery of cisplatin was developed in the 1970s based on a strong pharmacologic rationale and rodent *** advantage over intravenous(IV)administration was supported initially by observational studies in treating recurrent ovarian cancer and eventually by better outcomes from IP *** cisplatin in randomized studies in patients undergoing optimal surgical debulking at *** the past two decades,with the introduction of novel anticancer interventions(such as taxanes,bevacizumab,inhibitors of DNA repair,and immune check point inhibitors),advantages of IP drug delivery are less clear and concerns are raised on cisplatin's therapeutic *** discovery of BRCA genes and their key role in DNA repair,on the other hand,have strengthened the rationale for IP drug delivery:high grade serous cancers arising in the Mullerian epithelium in association with hereditary or somatic BRCA function inactivation are linked to peritoneal spread of cells that-while initially sensitive-are prone to emergence of platinum ***,selection of patients based on genomic features and focusing on the better tolerated IP carboplatin are *** examples of leveraging the peritoneal route include(1)targeting the cell membrane copper transport receptor-that is shared by platinums-by the combination of the proteasome inhibitor bortezomib and IP carboplatin;and(2)enhancing IP 5-fluoro-2-deoxyuridine cytotoxicity when coupled with PARP inhibition.

Following years in development, poly-adenosyl-ribose polymerase (PARP) inhibitors continue to advance the treatment of ovarian and breast cancers, particularly in patients with pathogenic BRCA mutations. Differences in clinical trial design have contributed to distinct indications for each of the PARP inhibitors. Toxicity patterns are also emerging that suggest agents differ in their normal tissue tolerance - beyond what might be expected by dose variations and/or exposure to prior treatment. PARP inhibitor resistance is an increasingly relevant issue as the drugs move to the forefront of advanced ovarian/breast cancer treatment, and is an active area of ongoing research. This review examines the PARP inhibitor clinical trials that have led to approved indications in ovarian and breast cancers, PARP inhibitor targets and pharmacological differences between the PARP inhibitors, emerging mechanisms of resistance, and key clinical questions for future development.

Over the past few decades,age estimation of living persons has represented a challenging task for many forensic services ***莒eneral,the process for age estimation includes the observation of the degree of maturity reached by some physical attributes,such as dentition or several ossification *** estimated chronological age or the probability that an individual belongs to a meaningful class of ages is then obtained from the observed degree of maturity by means of various statistical *** these methods,those developed in a Bayesian framework offer to users the possibility of coherently dealing with the uncertainty associated with age estimation and of assessing in a transparent and logical way the probability that an examined individual is younger or older than a given age ***,a Bayesian network for age estimation has been presented in scientific literature;this kind of probabilistic graphical tool may facilitate the use of the probabilistic *** of interest in the network are assigned by means of transition analysis,a statistical parametric model,which links the chronological age and the degree of maturity by means of specific regression models,such as logit or probit *** different regression models can be employed in transition analysis,the aim of this paper is to study the influence of the model in the classification of *** analysis was performed using a dataset related to the ossifications status of the medial clavicular epiphysis and results support that the classification of individuals is not dependent on the choice of the regression model.

Benign paroxysmal positional vertigo(BPPV) is the most frequent episodic vestibular disorder. It is due to otolith rests that are free into the canals or attached to the cupulas. Well over 90% of patients can be successfully treated with manoeuvres that move the particles back to the utriculus. Among the great variety of procedures that have been described, the manoeuvres that are supported by evidenced-based studies or extensive series are commented in this review. Some topics regarding BPPV treatment, such as controlling the accuracy of the procedures or the utility of post-manoeuvre restrictions are also discussed.

Evolution of the Mesozoic Granites in the Xiong'ershan-Waifangshan Region,Western Henan Province,China,and Its Tectonic Implications

Based on the new data of isotopic ages and geochemical analyses, three types of Mesozoic granites have been identified for the Xiong＇ershan-Waifangshan region in western Henan Province： high-Ba-Sr I-type granite emplaced in the early stage （-160 Ma）, I-type granite in the middle stage （-130 Ma） and anorogenic A-type granite in the late stage （-115 Ma）. Geochemical characteristics of the high-Ba-Sr I-type granite suggest that it may have been generated from the thickened lower crust by partial melting with primary residues of amphibole and garnet. Gradual increase of negative Eu anomaly and Sr content variations reflect progressive shallowing of the source regions of these granites from the early to late stage. New ^40Ar/^39Ar plateau ages of the early-stage Wuzhangshan granite （156.0±1.1 Ma, amphibole） and middle-stage Heyu granite （131.8±0.7 Ma, biotite） are indistinguishable from their SHRIMP U-Pb ages previous published, indicating a rapid uplift and erosion in this region. The representative anorogenic A- type granite, Taishanmiao pluton, was emplaced at -115 Ma. The evolution of the granites in this region reveals a tectonic regime change from post-collisional to anorogenic between -160 Ma and -115 Ma. The genesis of the early- and middle-stage I-type granites could be linked to delamination of subducted lithosphere of the Qinling orogenic belt, while the late-stage A-type granites represent the onset of extension and the end of orogenic process. In fact, along the Qiniing -Dabie-Sulu belt, the Mesozoic granitoids in western Henan, Dabieshan and Jiaodong regions are comparable on the basis of these temporal evolutionary stages and their initial ^87Sr/^86Sr ratios, which may suggest a similar geodynamic process related to the collision between the North China and Yangtze cratous.
