Monitoring of Ultrafine Particles in the Surrounding Urban Area of In-Land Passenger Ferries

Maritime transportation, widely used both in international transport of goods and touristic purposes, has been identified as a significant source of ultrafine particles (UFP). In-land passenger ferry is a source of UFP far less addressed;however, in locations with relatively high frequency of this transportation mode, it is expected that they contribute to an increase of their concentration. Moreover, the negative effects of UFP on human health and environment are known and therefore, monitoring UFP produced by ferries is vital to assess the degree of exposure of who work or live close to ferries’ terminals or downwind to their cruising path. This work aims to study the influence of in-land ferries activities on UFP in the urban/suburban areas near ferries’ terminals and downwind across the cruising path. The UFP monitoring campaign was carried out from September to December 2018 for 19 non-consecutive periods. The sampling sites were chosen in order to maximize measurements under downwind conditions and allow the association between ferry operation and UFP concentration response. Based on data collected, correlation analysis was performed between ferry’s traffic and particle number counting (PNC) of UFP, and also with meteorological variables. Results show an increase in PNC ranging from 25 to 197% during the third minute around a ferry movement and are moderate to positive significant correlations between PNC values and the number of ferry operations (r = 0.79 to r = 0.94), showing that UFP emitted by in-land ferries contributes to PNC increase. Moreover, negative correlations (r = -0.85 to r = -0.93) between PNC and wind intensity were also found.

Ten years of financial stability in Brazilian economy have gone. In this period, the regional transportation of passengers suffered exogenous impacts： economical crises, airport crises and great proportions＇ accidents, as well as impacts which were intern to the system： institutional changes （liberation of tariff promotions, many companies establishing themselves and also coming to bankruptcy）, creation of regulating institutions in the air transportation as well as the land transportation. Theoretically, it is expected that these changes have generated impacts in the demand for trips, since an environment regulated with more flexible prices and higher amounts of companies would generate a competitive environment in which the companies could struggle to attract their demand. On the other hand, the impacts which are exogenous to the system can generate responses in the sense of restoring the balance of demand. Thus, based on the theoretical experience, this article aims at analyzing empirically, through categorical variables, if there were impacts on the demand for regional trips in Brazil due to the internal or external changes. In order to perform this, monthly data from January, 1999 to December, 2009 are utilized and estimates are calculated making use of SUR （seemingly unrelated regressions）. As a result, we have the meaning of the internal and external impacts related to air and land transports, identifying that the worldwide economic crisis generated an impact at the level of the demand for transportation and also that the flexibility of tariffs allowed by ANTT （Ag^ncia Nacional de Transportes Terrestres） had an equal impact on the demand for land transportation.

Palpitations are one of the most common reasons for medical consultation. Theytend to worry patients and can affect their quality of life. They are often asymptom associated with cardiac rhythm disorders, although there are otheretiologies. For diagnosis, it is essential to be able to reliably correlate the symptomswith an electrocardiographic record allowing the identification or rulingout of a possible rhythm disorder. However, reaching a diagnosis is not alwayssimple, given that they tend to be transitory symptoms and the patient isfrequently asymptomatic at the time of assessment. In recent years, electrocardiographicmonitoring systems have incorporated many technical improvements thatsolve several of the 24-h Holter monitor limitations. The objective of this review isto provide an update on the different monitoring methods currently available,remarking their indications and limitations, to help healthcare professionals toappropriately select and use them in the work-up of patients with palpitations.

This article presents a case of a patient with relapsed esthesioneuroblastoma (ENB), an aggressive rare tumor that arises from the specialized sensory epithelial olfactory cells in the skull base area, which was initially treated with endoscopic surgery, followed by adjuvant radiotherapy. After local relapse, new surgical approaches and subsequent lines of platin-based chemotherapy were performed. A PET-CT with 68GALIUM DOTATATOC (PET-DOTATOC) showed intense uptake of disease, compatible with the presence of somatostatin receptors, in the face, nodes, liver, bones, and meningeal area. Treatment with 4 cycles of 177Lutetium-Dotatate was performed, followed by maintenance octreotide, with a major radiological and clinical response that is lasting more than 1 year after treatment. This article describes a rare case of a skull-base tumor, with multiple recurrences, in which disease control was achieved with a targeted Peptide Receptor Radionuclide Therapy (PRRT) with 177Lutetium-Dotatate, and discusses factors that could influence the incorporation of this form of therapy. Previous case reports proved the potential efficacy of this therapy usually given for low-grade neuroendocrine tumors and will be carefully reviewed.

Governing Climate Change Risks： Implications for Mitigation and Adaptation

This article analyzed climate change risks from the perspective of con- temporary environmental risks and how they have been internalized by policy. In order to do so, the main characteristics of this type of risk were analyzed based on the contributions from social theory authors on contemporary environmental risks. Next, the implications of these characteristics for the production of policy responses to climate change risks were discussed. The two main types of policy responses to climate change in the literature were presented： mitigation and adaptation. Finally, their interaction, differences and possibilities for synergy were analyzed. Under- standing climate change as a contemporary environmental risk, the way it was presented in this article, implies a radical change in the development bases of society, since greenhouse gases emissions from human activities contribute to the aggravation of global warming. Climate change challenges the traditional ways of governing in many ways, since climate change policy should involve the ques- tioning of the current processes of development. Profound changes in ways of thinking and established political action are needed.

Evaluation of the Implementation of a Low Emission Zone in Lisbon

The city of Lisbon, like many others in the EU region, introduced a Low Emission Zone (LEZ) as a tool for improving air quality in its city centre. This kind of emission reduction schemes is always controversial since it might lead to significant changes in the daily behaviours of its inhabitants. In order to evaluate the effects of the measure, an estimation of the impact of the introduction of the Lisbon LEZ was performed. Real traffic counting and fleet characterization combined with CORINAR-based emission inventory methodology allowed to estimate the impacts of three different scenarios applied to the “business as usual” condition (current vehicle fleet) ranging from “no circulation from non- compliant vehicles” to an “aggressive fleet renewal”. Results highlight the high percentage of atmospheric emissions of PM10 and NOx that might result from certain fleets like taxis and buses, especially because there was an emphasis in standardized/normalized estimations (emissions per 1000 vehicles) in order to allow different strategic approaches. The total reduction of PM10 emissions associated to each scenario vary between 6 ***-1 (scenario 2) and 8 ***-1 (scenario 1), or 25% and 34% less emissions. In terms of NOxemission reductions vary between 6 ***-1 (scenario 2) and 57 ***-1 (scenario 1), or 1% and 7% less emissions. The Lisbon LEZ is therefore much more efficient in reducing PM10 emissions compared to NOx. Major reduction in PM10 and NOx emissions are to be expected with a moderate intervention in the (relatively old) taxi fleet in Lisbon while for passenger cars the impact is limited. However in absolute terms and due to its urban mileage passenger cars should also continue being included in Lisbon LEZ. Simultaneously, an effort in the increase of dedicated lanes for public transport should be made, for further reductions in PM10 and NOx emissions.

BACKGROUNDPercutaneous transluminal angioplasty and stenting represent an effectivetreatment for hepatic artery stenosis after liver transplantation. In the first yearafter stenting, approximately 22% of patients experience in-stent restenosis,increasing the risk of artery thrombosis and related complications, and 50%experience liver failure. Although angiography is an important tool for diagnosisand the planning of therapeutic interventions, it may raise doubts, especially insmall-diameter arteries, and it provides low resolution rates compared withnewer intravascular imaging methods, such as optical coherence tomography(OCT).CASE SUMMARYA 64-year-old male developed hepatic artery stenosis one year after orthotropicliver transplantation and was successfully treated with percutaneous transluminalangioplasty with stenting. Five months later, the Doppler ultrasound resultsindicated restenosis. Visceral arteriography confirmed hepatic artery tortuositybut was doubtful for significant in-stent restenosis (ISR) and intrahepatic flowreduction. To confirm ISR, identify the etiology and guide treatment, OCT wasperformed. OCT showed severe stenosis due to four mechanisms: Focal andpartial stent fracture, late stent malapposition, in-stent neointimal hyperplasia,and *** diagnostic methods can be useful in evaluating cases in which initialangiography results are not sufficient to provide a proper diagnosis of significantstenosis, especially with regard to ISR. A wide range of diagnoses are provided byOCT, resulting in different treatment options. Interventional radiologists shouldconsider intravascular diagnostic methods as additional tools for evaluatingpatients when visceral angiography results are unclear.

Background： This study evaluated the impact of hormonal modulation at the onset of proestrus on ovarian response and uterine gene expression of beef ***： A total of 172 anestrous beef cows were assigned to one of four groups according to the treatment with estradiol cypionate（ECP） and/or equine chorionic gonadotropin（eCG） [CON（n = 43）, ECP（n = 43）, eCG（n = 44）and ECP ＋ eCG（n = 42）].Results： ECP-treated cows（ECP and ECP ＋ eCG groups） presented greater occurrence of estrus（44.6% vs. 65.4%; P = 0.01） and pregnancy per AI [47.1% vs. 33.3%; P = 0.07], but similar progesterone（P4） concentration at subsequent diestrus than cows not treated with ECP（CON and eCG groups）. Nonetheless, eCG-treated cows（eCG and ECP ＋ eCG groups） presented larger follicle at timed AI（12.6 ± 0.3 vs. 13.5 ± 0.3 mm; P = 0.03）, greater ovulation rate（96.5%vs. 82.6%; P = 0.008） and greater P4 concentration at d 6（3.9 ± 0.2 vs. 4.8 ± 0.2 ng/mL; P = 0.001） than cows not treated with eCG（CON and ECP groups）. Next, cows with a new corpus luteum 6 d after TAI were submitted to uterine biopsy procedure. Uterine fragments [CON（n = 6）, ECP（n = 6）] were analyzed by RNA-Seq and a total of 135 transcripts were differentially expressed between groups（73 genes up-regulated by ECP treatment）. Subsequently,uterine samples were analyzed by q PCR（genes associated with cell proliferation）. ECP treatment induced greater abundance of PTCH2（P = 0.07） and COL4 A1（P = 0.02）, whereas suppressed EGFR（P = 0.09） expression. Conversely, eCG treatment increased abundance of HB-EGF（P = 0.06）, ESR2（P = 0.09）, and ITGB3（P = 0.05）, whereas it reduced transcription of ESR1（P = 0.05）. Collectively, supplementation with ECP or eCG at the onset of proestrous of anestrous beef cows influenced ovarian responses, global and specific endometrial gene ***： Proestrus estradiol regulate the endometrial transcriptome, particularly stimulating proliferative activity in the endometrium.
