Role of blood AFP mRNA and tumor grade in the preoperative prognostic evaluation of patients with hepatoceiluiar carcinoma

AIM： To explore the potential prognostic role of preoperative tumor grade and blood AFP mRNA in a cohort of patients with hepatocellular carcinoma （HCC） eligible for radical therapies according to a well-defined treatment algorithm not including nodule size and number as absolute selection criteria. METHOdS： Fifty patients with a diagnosis of HCC were prospectively enrolled in the study. Inclusion criteria were：（1） histological assessment of tumor grade by means of percutaneous biopsies; （2） determination of AFP mRNA status in the blood; （3） patient＇s eligibility for radical therapies. RESULTS： At preoperative evaluation, 54% of the study group had a well-differentiated HCC, 42% had AFP mRNA in the blood, 40% had a tumor larger than 5 cm and 56% had more than one nodule. Surgery （resection or liver transplantation） was performed in 29 patients, while 21 had percutaneous ablation procedures. After a median follow-up of 28 too, 12-, 24-, and 36-mo survival rates were 78%, 58%, and 51%, respectively. Surgical therapy, performance status and three tumor-related variables （AFP mRNA, HCC grade and gross vascular invasion） resulted as significant survival predictors at univariate analysis. Nodule size and number did not perform as significant prognosticators. Multivariate study selected only surgical therapy and a biologically early HCC profile （AFP mRNA negative and well-differentiated tumor without gross vascular invasion） as independent survival variables. CONCLUSION： The preoperative determination of tumor grade and blood AFP mRNA status may potentially refine the prognostic evaluation of HCC patients and improve the selection process for radical therapies.

Background:Minimally invasive surgery(MIS)is the technique of choice in selected patients for the treatment of liver *** robotic approach is considered today the natural evolution of *** application of the robotic technique in liver transplantation(LT)has been recently evaluated,especially in the living *** aim of this paper is to review the current role of the MIS and robotic donor hepatectomy in the literature and to evaluate the possible future implication in the transplant ***:We conducted a narrative review using PubMed and Google Scholar for reports published so far,using the following keywords:minimally invasive liver surgery,laparoscopic liver surgery,robotic liver surgery,robotic living donation,laparoscopic donor hepatectomy and robotic donor ***:Several advantages have been claimed in favor of robotic surgery:three-dimensional(3-d)imaging with stable and high-definition view;a more rapid learning curve than the laparoscopic one;the lack of hand tremors and the freedom of *** to open surgery,the benefits showed in the studies evaluating the robotic approach in the living donation are:less postoperative pain,the shorter period before returning to normal activity despite sustaining longer operation ***,the 3-d and magnification view makes the technique excellent in distinguishing the right plane of transection,vascular and biliary anatomy,associated with high precision of the movements and a better bleeding control(essential for donor safety)and lower rate of vascular ***:The current literature does not fully support the superiority of the robotic approach versus laparoscopic or open method in living donor *** donor hepatectomy performed by teams with high expertise and in properly selected living donors is safe and ***,further data are necessary to evaluate properly the role of robotic surgery in the field of living donation.

Autonomous Vehicle Platoons In Urban Road Networks:A Joint distributed Reinforcement Learning and Model Predictive Control Approach

In this paper, platoons of autonomous vehicles operating in urban road networks are considered. From a methodological point of view, the problem of interest consists of formally characterizing vehicle state trajectory tubes by means of routing decisions complying with traffic congestion criteria. To this end, a novel distributed control architecture is conceived by taking advantage of two methodologies: deep reinforcement learning and model predictive control. On one hand, the routing decisions are obtained by using a distributed reinforcement learning algorithm that exploits available traffic data at each road junction. On the other hand, a bank of model predictive controllers is in charge of computing the more adequate control action for each involved vehicle. Such tasks are here combined into a single framework:the deep reinforcement learning output(action) is translated into a set-point to be tracked by the model predictive controller;conversely, the current vehicle position, resulting from the application of the control move, is exploited by the deep reinforcement learning unit for improving its reliability. The main novelty of the proposed solution lies in its hybrid nature: on one hand it fully exploits deep reinforcement learning capabilities for decisionmaking purposes;on the other hand, time-varying hard constraints are always satisfied during the dynamical platoon evolution imposed by the computed routing decisions. To efficiently evaluate the performance of the proposed control architecture, a co-design procedure, involving the SUMO and MATLAB platforms, is implemented so that complex operating environments can be used, and the information coming from road maps(links,junctions, obstacles, semaphores, etc.) and vehicle state trajectories can be shared and exchanged. Therapeutic advances in neural regeneration for Huntington’s disease

Huntington’s disease is a neurodegenerative disease caused by the expansion mutation of a cytosine-adenine-guanine triplet in the exon 1 of the HTT gene which is responsible for the production of the huntingtin (Htt) protein. In physiological conditions, Htt is involved in many cellular processes such as cell signaling, transcriptional regulation, energy metabolism regulation, dNA maintenance, axonal trafficking, and antiapoptotic activity. When the genetic alteration is present, the production of a mutant version of Htt (mHtt) occurs, which is characterized by a plethora of pathogenic activities that, finally, lead to cell death. Among all the cells in which mHtt exerts its dangerous activity, the GABAergic Medium Spiny Neurons seem to be the most affected by the mHtt-induced excitotoxicity both in the cortex and in the striatum. However, as the neurodegeneration proceeds ahead the neuronal loss grows also in other brain areas such as the cerebellum, hypothalamus, thalamus, subthalamic nucleus, globus pallidus, and substantia nigra, determining the variety of symptoms that characterize Huntington’s disease. From a clinical point of view, Huntington’s disease is characterized by a wide spectrum of symptoms spanning from motor impairment to cognitive disorders and dementia. Huntington’s disease shows a prevalence of around 3.92 cases every 100,000 worldwide and an incidence of 0.48 new cases every 100,000/year. To date, there is no available cure for Huntington’s disease. Several treatments have been developed so far, aiming to reduce the severity of one or more symptoms to slow down the inexorable decline caused by the disease. In this context, the search for reliable strategies to target the different aspects of Huntington’s disease become of the utmost interest. Aqueous electrochemical delithiation of cathode materials as a strategy to selectively recover lithium from waste lithium-ion batteries

Lithium recovery from end-of-life Li-ion batteries(LIBs)through pyro-and hydrometallurgical recycling processes involves several refining stages,with high consumption of reagents and energy.A competitive technological alternative is the electrochemical oxidation of the cathode materials,whereby lithium can be deintercalated and transferred to an electrolyte solution without the aid of chemical extracting *** article investigates the potential to selectively recover Li from LIB cathode materials by direct electrochemical extraction in aqueous *** process allowed to recovering up to 98%of Li from high-purity commercial cathode materials(LiMn_(2)O_(4),LiCoO_(2),and Li Ni_(1/3)Mn_(1/3)Co_(1/3)O_(2))with a faradaic efficiency of 98%and negligible co-extraction of Co,Ni,and *** process was then applied to recover Li from the real waste LIBs black mass obtained by the physical treatment of electric vehicle battery *** black mass contained graphite,conductive carbon,and metal impurities from current collectors and steel cases,which significantly influenced the evolution and performances of Li electrochemical ***,due to concomitant oxidation of impurities,lithium extraction yields and faradaic efficiencies were lower than those obtained with high-purity cathode *** oxidation was found to occur within the voltage range investigated,but it could not quantitatively explain the reduced Li extraction *** fact,a detailed investigation revealed that above 1.3 V ***/Ag Cl,conductive carbon can be oxidized,contributing to the decreased Li *** on the reported experimental results,guidelines were provided that quantitatively enable the extraction of Li from the black mass,while preventing the simultaneous oxidation of impurities and,consequently,reducing the energy consumption of the proposed Li recovery method.

Impact of cattle density on the structure and natural regeneration of a turkey oak stand on an agrosilvopastoral farm in central Italy

On an agrosilvopastoral farm in central Italy where Maremmana cattle graze in Turkey oak forests,we evaluated the impact of different livestock densities on stand structure,tree diversity and natural regeneration in four types of grazed areas based on the grazing regime adopted:calf-grazed,high-intensity-grazed,low-intensity-grazed,ungrazed *** each area,we set up three permanent circular plots(radius of 15 m)to survey the structural and dasometric characteristics of the overstorey,understorey,and regeneration *** results showed that grazing negatively affected the complexity of the forest structure and its potential to regenerate and maintain a high level of *** differences in stand structure observed between the grazing areas were closely related to livestock *** most sensitive components of the system were the understorey and the regeneration ***,the current grazing management did not affect the dominant tree structure or its *** findings identified medium-term monitoring and regeneration management as the two significant aspects to consider when assessing sustainable *** forests can be established by excluding graz-ing for about 20–25 years.

Several novel cobalt dichloride complexes with amino-phosphine bidentate ligands were synthesized and *** some of them single crystals were obtained and their molecular structure was determined by X-ray diffraction *** the complexes were then used in combination with MAO for the polymerization of 1,3-butadiene,obtaining polymers with different structures(i.e.,predominantly 1,2 or cis-1,4)mainly depending on the type of ligand and on the MAO/Co molar *** behavior of these novel catalysts was compared with that exhibited,in the polymerization of the same monomer,by the systems CoCl_(2)(PR3)2-MAO and CoCl_(2)(PRPh2)2-MAO(R=alkyl or cycloalkyl group),and by the systems based on cobalt dichloride complexes with various bi-and tridentate ligands(e.g.,diphosphines,bis-imines,pyridyl-imines,bis-iminopyridines).The comparison between the different systems allowed us to make some clarity about the actual and effective role played by the various types of ligands in the polymerization of conjugated dienes with catalytic systems CoCl_(2)(L)-MAO,in which L=mono-,bi-,or tri-dentate ligand.

Increasing realism in modelling energy losses in railway vehicles and their impact to energy-efficient train control

The reduction of energy consumption is an increasingly important topic of the railway ***-efficient train control(EETC)is one solution,which refers to mathematically computing when to accelerate,which cruising speed to hold,how long one should coast over a suitable space,and when to *** approaches in literature and industry greatly simplify a lot of nonlinear effects,such that they ignore mostly the losses due to energy conversion in traction components and *** fill this research gap,a series of increasingly detailed nonlinear losses is described and *** categorize an increasing detail in this representation as four *** study the impact of those levels of detail on the energy optimal speed *** do this,a standard approach based on dynamic programming is used,given constraints on total travel *** evaluation of multiple test cases highlights the influence of the dynamic losses and the power consumption of auxiliary components on railway trajectories,also compared to multiple *** results show how the losses can make up 50%of the total energy consumption for an exemplary *** them would though result in consistent but limited errors in the optimal ***,more complex trajectories can result in less energy consumption when including the complexity of nonlinear losses than when a simpler model is *** effects are stronger when the trajectory includes many acceleration and braking phases.
