A Novel Approach to Reduce the Environmental Footprint of Maritime Shipping

Maritime shipping is a strategic sector with a strong international vocation and *** need to define regulations valid for many different countries without generating disparities of treatment slowed down the formulation of environmental regulations,especially for atmospheric *** particular,regulations pertaining to the reduction of sulphur compounds allowed two distinct approaches:the use of low-sulphur fuels or exhaust gas cleaning systems,the so-called *** actual implementation of these solutions presents specific concerns either related to the toxicity of atmospheric by-products and to the fuel cost or to the generation of polluting washwaters that may need treatment before *** this paper we analyzed the potential environmental benefit deriving from the use of a distillate fuel,not compliant with current IMO Sulphur Regulations,together with a *** pilot-scale experimental results indicated that a limited amount of water and/or scrubber volume is needed to reduce sulphur emissions below regulations on maritime shipping,especially with the addition of NaOH reaching a water-saving between 25%-33%compared to the use of pure *** indicated that scrubber washwater PAHs emissions are within the available water quality standards indicated by EU and USA guidelines.A bottom-up analysis on heavy metals concentration shed light on the prominent role of metal-parts corrosion on the washwater *** into account for corrosion phenomena,the actual heavy metals concentration in the washwater deriving from scrubbing was normally below the water quality standards.

Further Laboratory Experiments on Aerosol Scavenging in Mixed Clouds to Assess the Role of Phoretic Forces and Particle Solubility

Scavenging experiments have been performed in a cloud chamber inside a cold room with different aerosol particles: Paraffin particles, NaCl particles, Magnesium oxide particles, Carbon particles, Sahara dust particles. Essentially the experimental tests were carried on following the sequence of operations: the generation of the aerosol particles, their injection in the lower part of the cloud chamber, injection of water droplets in the whole chamber volume, nucleation of ice crystals, collection of ice crystals and their examination as for resulting scavenging efficiency. Evidence is given of the peculiar behaviour of soluble particles, individual and eventually inside mixed particles, leading to very much important scavenging efficiency, probably to be ascribed to aerodynamic capture. The evident peculiar behaviour of deliquescent particles can be oriented towards applications to an efficient abatement of specific effluents, on one side, and to weather modification experiments, both rain enhancement and hail prevention experiments.

Transcranial Doppler ultrasonography: From methodology to major clinical applications

Non-invasive Doppler ultrasonographic study of cerebral arteries [transcranial Doppler(TCD)] has been extensively applied on both outpatient and inpatient settings. It is performed placing a low-frequency(≤ 2 MHz) transducer on the scalp of the patient over specific acoustic windows, in order to visualize the intracranial arterial vessels and to evaluate the cerebral blood flow velocity and its alteration in many different conditions. Nowadays the most widespread indication for TCD in outpatient setting is the research of right to left shunting, responsable of so called "paradoxical embolism", most often due to patency of foramen ovale which is responsable of the majority of cryptogenic strokes occuring in patients younger than 55 years old. TCD also allows to classify the grade of severity of such shunts using the so called "microembolic signal grading score". In addition TCD has found many useful applications in neurocritical care practice. It is useful on both adults and children for day-to-day bedside assessment of critical conditions including vasospasm in subarachnoidal haemorrhage(caused by aneurysm rupture or traumatic injury), traumatic brain injury, brain stem death. It is used also to evaluate cerebral hemodynamic changes after stroke. It also allows to investigate cerebral pressure autoregulation and for the clinical evaluation of cerebral autoregulatory reserve.

Neoadjuvant strategies for pancreatic cancer

Pancreatic cancer(PC)is the fourth cause of cancer death in Western countries,the only chance for long term survival is an R0 surgical resection that is feasible in about 10%-20%of all *** years cumulative survival is less than 5%and rises to 25%for radically resected *** 40%has locally advanced in PC either borderline resectable(BRPC)or unresectable locally advanced(LAPC).Since LAPC and BRPC have been recognized as a particular form of PC neoadjuvant therapy(NT)has increasingly became a valid treatment *** aim of NT is to reach local control of disease but,also,it is recognized to convert about40%of LAPC patients to R0 resectability,thus providing a significant improvement of prognosis for responding *** R0 resection is achieved,survival is comparable to that of early stage PCs treated by upfront *** it is crucial to look for a proper patient *** strategies are multiples and include neoadjuvant chemotherapy(nCT),and the association of nCT with radiotherapy(nCRT)given as either a combination of a radio sensitizing drug as gemcitabine or capecitabine or and concomitant irradiation or as upfront nCT followed by nRT associated to a radio sensitizing *** latter seem to be most promising as it may select patients who do not go on disease progression during initial treatment and seem to have a better *** clinical relevance of nCRT may be enhanced by the application of higher active protocols as FOLFIRINOX.

Management of unstable angina in a patient with Haemophilia A

Hemophilia A is an X-linked recessive disorder characterized by a deficiency of coagulation factor Ⅷ(FⅧ) and therefore by a greater risk of bleeding during percutaneous interventional procedures and during the dual antiplatelet therapy(DAPT) in patients with ischemic heart disease. Information regarding safe percutaneous procedures in hemophiliacs is limited. Since the introduction of FVⅧ concentrates, the life expectancy of hemophiliac patients has improved and consequently, the rate of ischemic heart disease in this population is increased. Frequently the replacement therapy can trigger the onset of an acute coronary syndrome. We report a case of a patient with mild Hemophilia A, who presents with unstable angina, treated successfully with coronary angioplasty and drug eluting stent implantation without replacement of FVⅧ, treated with long term DAPT without major bleeding after six months of follow up.

Adenocarcinoma of gastric cardia in the elderly: Surgical problems and prognostic factors

AIM： To analyze retrospectively, our results about patients who underwent surgical treatment for adenocarcinoma of the cardia in relation to age, in order to evaluate surgical problems and prognostic factors. METHODS： From January 1987 to March 2003, 140 patients with adenocarcinoma of the cardia underwent resection in the authors＇ institution. They were divided into three groups with regard to age. Patients 〈70 and 〉 60 year old （31） were excluded; we also excluded 18 out of 109 patients with poor general status or systemic metastases. So, we compared 51 elderly （≥ 70 year old） and 58 younger patients （≤ 60 year old）. The treatment was esophagectomy for type I tumors, and extended gastrectomy and distal esophagectomy for type Ⅱ and Ⅲ lesions. RESULTS： Laparotomy was carried out in 91 patients （83.4%）, 38 in the elderly （74.5%） and 53 in younger patients （91.3%, P〈0.05）. Primary resection was performed in 81 cases （89%） without significant differences between the two groups. Postoperative death was higher in the elderly （12.1%） than the other group （4.1%, P〈0.05）, while morbidity was similar in both groups. A curative resection （R0） was performed in 59 patients （72.8%）, 69.6% in the elderly and 75% in the younger group （P〉0.05）. The overall 3- and 5-year survival rates were 26.7% and 17.8% respectively for the elderly and 40.7% and 35.1% respectively for younger patients （P = 0.1544）. Survival rates were significantly associated with R0 resection, pathological node-poskive category and tumor differentiation in both ***： As the age of the general population increases, more elderly patients with gastric cardia cancer will be candidates for surgical resection. Age alone should not preclude surgical treatment in elderly patients with gastric cardia cancer and a tumor resection can be carried out safely. Prevalence of bile reflux in gastroesophageal reflux disease patients not responsive to proton pump inhibitors

CT colonography after incomplete colonoscopy in subjects with positive faecal occult blood test

AIM： To report our experience with computed tomography colonography （CTC） systematically performed in subjects with positive faecal occult blood test （FOBT） and an incomplete colonoscopy in the setting of a population-based screening for colorectal cancer （CRC）. METHODS： From April 2006 to April 2007, 43 290 individuals （age range 50-70） who adhered to the regional screening program for the prevention of CRC underwent immunochemical FOBT. FOBT was positive in 1882 subjects （4.3%）. 1463 （77.7%） of these subjects underwent colonoscopy, 903 performed in a single center. Of 903 colonoscopies 65 （7.2%） were incomplete. Forty-two of these subjects underwent CTC. CTC was performed with a 16-MDCT scanner after standard bowel prep （polyethyleneglycole） in both supine and prone position. Subjects whose CTC showed polyps or masses were referred to the endoscopist for repeat colonoscopy under sedation or underwent surgery. Perlesion and per-segment positive predictive values （PPV） were calculated. RESULTS： Twenty-one （50%） of 42 CTCs showed polyps or masses. Fifty-five of these subjects underwent a repeat colonoscopy, whereas 2 subjects underwent surgery for colonic masses of indeterminate nature. Four subjects refused further examinations. CTC correctly identified 2 colonic masses and 20 polyps. PPV for masses or polyps greater than 9 mm was of 87.5%. Per-lesion and per-segment PPV were, respectively, 83.3% and 83.3% for polyps greater or equal to 10 mm, and 77.8% and 85.7% for polyps of 6-9 mm. CONCLUSION： In the context of a screening program for CRC based on FOBT, CTC shows high per-segment and per-lesion PPV for colonic masses and polyps greater than 9 mm. Therefore, CTC has the potential to become a useful technique for evaluation of the non visualized part of the colon after incomplete colonoscopy.
