Background: The evolutionary profile of patients operated for peptic ulcer perforation in Bujumbura. Perforated peptic ulcer is a serious complication of peptic ulcer with potential risk of grave complications. Aim: To study the early morbidity and mortality of surgery for peptic ulcer perforation in Bujumbura City Hall hospitals: Kamenge University Hospital Center (CHUK), Kamenge Military Hospital (HMK) and Prince Louis Rwagasore Clinic (CPLR). Patients and methods: This is a retrospective, descriptive and analytical study carried out in the three hospitals of Bujumbura over a period of three years from January 1, 2020 to December 31, 2022. It involved 57 cases (n = 57) of peptic ulcer perforation. Results: The frequency of surgery for peptic ulcer perforation was 2.7% with a mean age of 43.6 years +/- 15.3 years and a male predominance with a sex ratio of 3.7. Eight percent of patients presented with shock, 24.5% were smokers, and 67.9% had taken non steroidal anti-inflammatory drugs. The mean hospital stay was 15.2 days with a standard deviation of 12.1. The morbidity rate was 30.2%, 32% were classified in grade IIIb of the Clavien-Dindo Surgical Complications Scale. There were 9 deaths (17%). Seven patients who underwent surgery and received late consultations died. Conclusion: Surgery for peptic ulcer perforation remains an intervention associated with a high rate of morbidity and mortality in Bujumbura. Surgery for peptic ulcer perforation remains a procedure associated with a high morbidity and mortality rate in Bujumbura. The time before consultation was the only factor associated with early morbidity and mortality of Surgery for peptic ulcer’s perforations.

The Place of Nursing Theory in the Management of Post-Operative Infections in a Hospital Environment: Case of Cibitoke District Hospital

Introduction: Post-operative infections, such as surgical site infections (SSIs), are a significant concern in healthcare settings. Nurses play a crucial role in the prevention and management of these infections. The use of nursing theory could contribute to the prevention of SSIs. The aim of this study was to evaluate the role of nursing theory in the management of surgical site infections (SSIs) in a hospital environment. Method: A cross-sectional study was conducted using descriptive and analytical methods to assess the role of nursing theory in the management of Post-operative infections (POI) in a hospital setting in October 2023. The study population consisted of nurses working in the Surgery, Emergency, and Maternity units at Cibitoke District Hospital. A sample size of 71 nurses working full or part time in the Surgery were invited to participate in this study. A questionnaire was used to collect the data, and SPSS version 21.0 software was used for analysis. Results: The study found that nursing theory did not have any statistically significant place in the management of POI (p-value = 0.523). However, the results showed that experience was the only significant factor influencing the management of POI (p-value = 0.004). This is explained by the analysis of the net effects of the explanatory variable where we noticed that those who had more experience were more likely to manage post-operative infections. The participants’ knowledge regarding nursing theory in the management was poor as they scored less than 30% in all the variables used to measure their knowledge. Conclusion: The study revealed that nurses’ knowledge of nursing theories and their applications in the management of SSIs was poor. Continuing professional development, curriculum review, and in-service training were highly recommended.

Evaluation of a cost effective technique for treating aquaculture water discharge using Lolium perenne Lam as a biofilter

Wastewater stabilization ponds generate low cost by-products that are useful for agriculture. The utilization of these by-products for soil amendment and as a source of nutrients for plants requires a high level of sanitation and stabilization of the organic matter, to maintain acceptable levels of soil, water and air quality. In this study, two aquaculture wastewater treatment systems; recirculating system and a floating plant bed system were designed to improve the quality of irrigation water in local communities with low income. In both systems the grass species Lolium perenne Lam was used as a plant biofilter while vegetable specie Amaranthus viridis was used to evaluate the performance of the system and the suitability of the phyto-treated water for irrigation. It was found that the harmful material removal rate for recirculating system was 88.9% for TAN （total ammonia nitrogen）, 90% for NO2--N, 64.8% for NO3--N while for floating plant bed system 82.7% for TAN, 82% for NO2--N and 60.5% for NO3--N. Comparative analysis of the efficiency of waste element removal between the two systems revealed that both systems performed well, however, plant growth was not robust for floating plant bed system while recirculating system is energy consuming. Although both systems did not attain sufficient levels of TN （total nitrogen） and TP （total phosphorus） load reduction, the treatment with L. perenne remarkably improved the irrigation water quality. A. viridis plants irrigated with the phyto-treated discharge water had lesser concentrations of heavy metals in their tissues compared to those irrigated with untreated discharge. The control plants irrigated with untreated discharge were also found to be highly lignified with few stems and small leaves.

Aquaculture Solids Management Using A Combination of Sand/Gravel or Unwoven Fabric Bed With Lolium perenne Lam as A Plant Biofilter

Objective This work is an evaluation of the efficiency of a sand-gravel or unwoven fabric bed system and Lolium perenne Lam as plant biofilter in the reduction of solids and nutrients removal from aquaculture discharge water. Methods The fzrst step consisted of the collection of wastewater in the tank and the distribution at three different hydraulic loading regimes （0.5, 1, 1.5 L/hour） to the different experimental systems. The second step was to evaluate the performance of the different systems. The fzrst system consisted of a bucket filled with a substrate of sand/gravel （20 cm in depth）, on the bottom of which was a 80 mesh/inch2 of nylon （S1）; the second was similar, but was planted with Lolium perenne lam （S2）; the third was planted with a grass plate consisting of 7 layers of unwoven fabric planted with L. perenne （S3）. Results The second system showed the best performance in reducing solids as well as in nutrients （TN, TP, and COD） reduction. The removal rates for TS, TN, and TP were negatively correlated with the loading regimes, with 0.5 L/hour being the most efficient and thus taken as the reference. Conclusions Solids management using a sand/gravel substrate as bed culture and Lolium perenne L. as plant biofilter has proved to be an efficient technique for solids reduction with low operating cost. This grass plays an important role in wastewater eco-treatment by absorbing dissolved pollutants （TAN） as nutrients for its growth.

该文旨在比较研究不同栽培基质 (土壤、泥炭、泥炭 /珍珠岩 (1/ 2 )、珍珠岩和营养液循环系统 )中生长的樱桃番茄在夏季受热应激时的反应。试验于 2 0 0 2年 7、8、9月进行 ,番茄分别移植到 3个温室中 ,温度设置为 :2 5 / 2 0℃ ,35 / 2 5℃ ,>35 />2 5℃ ,分别在番茄不同生长阶段进行。测试分析了温室内环境因子的变化和气体交互作用 :温室内温度与湿度 (R2 :0 .894～ 0 .972 )、温度与 CO2 浓度 (R2 :0 .6 6 8～ 0 .786 )之间呈线性关系。测定了番茄的生长率 ,表明茎伸展与栽培基质密切相关。用叶水势来反映受热应激下番茄的水分状况 ,表明方差分析显示无论是黎明前还是正午 ,栽培基质和热应激对叶水势的变化有重要的作用。实验结果可正确确定热胁迫影响的生育时期 ,并为选择夏季番茄生产的适宜栽培基质提供依据。

在中国的南方地区夏季高温高湿，在温室中亦如此。降温措施的使用将使生产成本明显增加，而降温效果却并不都令人满意。研究发现中午关闭温室顶部通风，温室内温度将迅速升高至50℃，这将导致植物的伤害甚至死亡。而在设施内种植的主要目的是通过改善设施内气候条件与根部环境条件，从而获得比露天条件下生产出经济效益高的优质产品。另外，由生长基质组成的无土栽培方式现广泛应用于现代栽培和精细农业中。如果选择适宜的栽培基质，多数情况下其生产效益比土壤栽培好。一般地，植物对环境因素的反应是非线性的，并且包括2个或更多的环境刺激的反应通常是不叠加的。本研究的目的是研究高温对不同栽培系统下樱桃蕃茄在不同生长阶段的影响作用。通过确定西红柿在某一具体的生长阶段的极限温度，从而获得适宜的栽培系统以减轻植物的夏季热应激。另外，环境可维持在某一特定的条件下，植物自身可改变其周围的环境。本文简要介绍了樱桃蕃茄、研究用仪器设备、热应激及植物对热应激的生理反应。本试验在2002年夏季的6—9月在一个温室内设立了多种樱桃蕃茄栽培系统：土培、泥煤基质培、苔泥基质培、泥煤+珍珠岩基质培(3：2)、珍珠岩和营养液膜栽培(NFT)。樱桃蕃茄在播种35后移植到温室的三个(1、2、3)白天／夜晚的温度分别为25／20℃，35／25℃，和＞35／＞25℃的小型气候室内。研究了环境内气体的相互作用，结果表明温度与湿度成负线性关系(相关系数：0.894-0.972)、温度与CO2也成负线性关系(相关系数：0.668-0.786)。同时对植物的生长作了评估，结果发现植物的生长速度(茎的生长)随着植物的成长慢慢降低，茎的生长与栽培基质有关，节点间的长度受温度的影响。温度和基质的影响相互独立，但明显影响叶面的生长。研究了开花阶段的极限温度的影响作用，结果发现高温不仅延迟初期开花和繁殖期，甚至还严重影响了座果率。植物中水分状况通过植物叶面水势、叶面相对含水量、叶面叶绿素含量和空气／叶面温度梯度来估算。分析表明中午期间热应激和栽培基质的相互作用对叶面水势有显著影响。研究中发现在黎明和中午温度和基质对叶面水势的影响差异极显著，温度和基质对叶面的相对含水量的影响不显著，中午时环境温度对叶面和空气的温度梯度有强烈的影响，温度和基质的相互作用对叶面的叶绿素B的含量影响存在显著差异。同时测量了植物根部环境如温度、EC和 PH。EC随着温度的升高而降低，而PH值随着温度的升高而升高。 M～f－－－＊I7＝＝，xnM～MpedeHLAs－－－－dd－ lin＝－－～M－llerNrtrt－－－－MM＼6Z＿atAsl！。－，t1JpeMMM H％f ”］”yI／M y D／亿L一JI MM】／肉多卜丑凶多罗r电Du／年一匐JL的尸——IYD 呵J调巴．、《1——1氏r屯HJ＿L口由多二三二凶人／令人二L回”J／V洗掉话干夏季的栽僚方式探供指导。

Esophageal Cancer: Epidemiological, Clinical and Histopathological Aspects over a 24-Years Period at Kamenge University Hospital, Bujumbura, Burundi

Aim: There were no data about esophageal cancer in Burundi. The aim of the study was to highlight the epidemiological, clinical and histopathological aspects of the esophageal cancer. Method: A retrospective study over a 24-years period (from January 1988 to December 2011) was carried out at Kamenge university hospital, including patients with esophageal cancer. The cases were selected on basis of the histological evidence of the cancer. Results: A total of 34 cases were retrieved and included for analysis. Among them, 24 patients (70.5%) were males. The esophageal cancer constituted 8.6% of digestive cancers over the period of the study. The average age was 50.9 years. It was revealed by dysphagia in 32 patients (94.1%) and was concomitantly metastatic in 12 patients. The squamous cell carcinoma was 30 cases (88.2%). 27 patients underwent a curative resection, but the outcome and prognosis were poor. In-hospital mortality and morbidity rates were respectively 8.8% and 17.7%. Conclusion: The present study showed evidence that the esophageal cancer in our country had the same characteristics and distribution as well as in developing countries. It had a poor prognosis and efforts had to be done in the early cancer detection.

Neonatal Umbilical Cord Infections: Incidence, Associated Factors and Cord Care Practices by Nursing Mothers at a Tertiary Hospital in Western Uganda

Background: Umbilical cord infections complicate to neonatal sepsis that significantly contributes to neonatal mortality worldwide. There is paucity of data on the incidence, factors associated with neonatal umbilical cord infections in western Uganda, yet nursing mothers continue to practice potentially dangerous cord care practices. We described the incidence, factors associated with umbilical cord infections and cord care practices by nursing mothers at a tertiary hospital in western Uganda. Methods: This was a hospital based cross sectional study at a tertiary hospital in western Uganda between the months of March and June 2019. Two hundred and forty (240) neonates aged between 2 to 14 days attending the neonatal unit were recruited in the study. Umbilical cord infection was assessed based on the World Health Organisation (WHO) clinical definition: finding of any of discharge, redness and swelling on the umbilical stump. Data on sociodemographic characteristics, maternal and neonatal perinatal factors and cord care practices of the nursing mothers were analysed by bivariate and multivariate logistic regression using STATA 13.0 to determine factors associated with umbilical cord infection.
