偏头痛是最常见的功能障碍性神经疾病之一,会对患者的生活质量造成严重影响。为了在文献检索和专家组共识基础上对不同类型偏头痛综合征的各种药物治疗方案提供循证或专家推荐,我们从现有的所有医学参考文献系统中筛选出关于有先兆型或无先兆型偏头痛以及偏头痛样综合征等各种类型临床资料进行分析。根据欧洲神经病学学会联盟(European Federation of Neurological ***)制定的推荐意见分级方案对这些研究结果进行评价从而确定A级、B级或C级推荐和良好的临床实践要点。对于偏头痛急性发作期的治疗,推荐口服非甾体消炎药(NSAID)和曲坦类药物。用药方案应遵循分层治疗的观念。在服用NSAID和曲坦类药物之前,推荐口服甲氧氯普胺或多潘立酮。极重度发作的首选药物为静脉注射乙酰水杨酸或皮下注射舒马曲坦。偏头痛持续状态或许可用类固醇治疗。对于偏头痛的预防性治疗,β-受体阻断药(普萘洛尔和美托洛尔)、氟桂利嗪、丙戊酸和托吡酯可作为首选用药,其次可选择阿米替林、萘普生、蜂斗菜烯碱和比索洛尔。

Metabolic engineering strategies have been successfully implemented to improve the production of isobutanol,a next-generation biofuel,in Saccharomyces ***,we explore how two of these strategies,pathway re-localization and redox cofactor-balancing,affect the performance and physiology of isobutanol producing *** equipped yeast with isobutanol cassettes which had either a mitochondrial or cytosolic localized isobutanol pathway and used either a redox-imbalanced(NADpH-dependent)or redox-balanced(NADH-dependent)ketol-acid reductoisomerase *** then conducted transcriptomic,proteomic and metabolomic analyses to elucidate molecular differences between the engineered *** localization had a large effect on isobutanol production with the strain expressing the mitochondrial-localized enzymes producing 3.8-fold more isobutanol than strains expressing the cytosolic ***-balancing did not improve isobutanol titers and instead the strain with the redox-imbalanced pathway produced 1.5-fold more isobutanol than the balanced version,albeit at low overall pathway *** genomic analyses suggested that the poor performances of the cytosolic pathway strains were in part due to a shortage in cytosolic Fe-S clusters,which are required cofactors for the dihydroxyacid dehydratase *** then demonstrated that this cofactor limitation may be partially recovered by disrupting iron homeostasis with a fra. mutation,thereby increasing cellular iron *** resulting isobutanol titer of the fra. null strain harboring a cytosolic-localized isobutanol pathway outperformed the strain with the mitochondrial-localized pathway by 1.3-fold,demonstrating that both localizations can support flux to isobutanol.

Urban living in large modern cities exerts considerable adverse effectson health and thus increases the risk of contracting several chronic kidney diseases (CKD). The prediction of CKDs has become a major task in urbanizedcountries. The primary objective of this work is to introduce and develop predictive analytics for predicting CKDs. However, prediction of huge samples isbecoming increasingly difficult. Meanwhile, MapReduce provides a feasible fra.mework for programming predictive algorithms with map and reduce *** relatively simple programming interface helps solve problems in the scalability and efficiency of predictive learning algorithms. In the proposed work, theiterative weighted map reduce fra.ework is introduced for the effective management of large dataset samples. A binary classification problem is formulated usingensemble nonlinear support vector machines and random forests. Thus, instead ofusing the normal linear combination of kernel activations, the proposed work creates nonlinear combinations of kernel activations in prototype examples. Furthermore, different descriptors are combined in an ensemble of deep support vectormachines, where the product rule is used to combine probability estimates ofdifferent classifiers. performance is evaluated in terms of the prediction accuracyand interpretability of the model and the results.

在法国核然料工业组织中,法杰马公司主要销售燃料组件。法比燃料公司(FBFC)的3个从属工厂都负责燃料组件的制造,该公司每年生产装铀量为1500t 的燃料组件。自1985年以来,法杰马公司又销售先进燃料组件(AFA)。该 AFA 的主要特点是使用了锆合金定位格架和可拆式上、下管嘴。大亚湾核电站要用的燃料组件正是该种与一般组件不同的先进燃料组件。法杰马公司采用钆作可燃毒物,以保证燃料组件的良好特性。近来该公司又推出混合氧化物燃料组件(MOX)。由于法杰马公司在设计和制造的各阶段都严格遵守了质量保证和质量控制制度,所以其产品质量优良、可靠性好。展望未来,法杰马公司将与法国核燃料工业中的其它集团一起,努力为用户提供尽可能好的产品。

The cells of origin of neurogenic heterotopic ossifications(NHOs), which develop frequently in the periarticular muscles following spinal cord injuries(SCIs) and traumatic brain injuries, remain unclear because skeletal muscle harbors two progenitor cell populations: satellite cells(SCs), which are myogenic, and fibroadipogenic progenitors(FAps), which are mesenchymal. Lineage-tracing experiments using the Cre recombinase/Lox p system were performed in two mouse strains with the fluorescent protein Zs Green specifically expressed in either SCs or FAps in skeletal muscles under the control of the pax7 or prrx1 gene promoter, respectively. These experiments demonstrate that following muscle injury, SCI causes the upregulation of pDGFRα expression on FAps but not SCs and the failure of SCs to regenerate myofibers in the injured muscle, with reduced apoptosis and continued proliferation of muscle resident FAps enabling their osteogenic differentiation into NHOs. No cells expressing Zs Green under the prrx1 promoter were detected in the blood after injury, suggesting that the cells of origin of NHOs are locally derived from the injured muscle. We validated these findings using human NHO biopsies. pDGFRα+mesenchymal cells isolated from the muscle surrounding NHO biopsies could develop ectopic human bones when transplanted into immunocompromised mice, whereas CD56+myogenic cells had a much lower potential. Therefore, NHO is a pathology of the injured muscle in which SCI reprograms FAps to undergo uncontrolled proliferation and differentiation into osteoblasts.

背景氙气可诱导大鼠在体心脏的早期心肌预适应，但是否可诱导晚期心肌保护尚不知。环氧化酶2（COX-2）是介导心肌晚期缺血预适应（i-LpC）信号传导的重要介质之一。本文研究氙气是否可诱导晚期预适应（Xe—LpC）以及COX-2活性或表达是否介导了这种作用。方法雄性Wistar大鼠麻醉后置入冠状动脉阻断器。7天后动物随机分配到5个组，每组8只。i-LpC组通过阻断冠状动脉5分钟来诱发；Xe-LpC组通过吸入70％的氙气15分钟获得。其他大鼠用COX-2抑制剂NS-398（5mg·kg^-1，腹腔内注射）预处理，然后分别使用或不使用Xe-LpC。假手术组的动物不接受缺血预处理或氙气预处理，作为对照组。24小时后所有动物麻醉，收紧冠状动脉阻断器造成心肌缺血25分钟，继而开放再灌注2小时。TTC染色法测定心肌梗死面积。另外还在预处理后不同时间点取心脏，分别用pCR和Westernblot法检测COX-2mRNA及其蛋白表达。结果与对照组相比，缺血预处理组和氙气预处理组均可减少心肌梗死面积（所占百分比分别为：i-LpC组29％±7％，Xe—LpC31％±8％，与对照组比p均〈0．05，对照组为64％±6％）。NS-398可消除氙气诱导的心脏保护作用（该组为61％±6％，与Xe-LpC组比p〈0．05）。COX-2mRNA及其蛋白表达只在i-LpC组有所增加，Xe-LpC组无明显变化。结论氙气诱导晚期心肌预适应，这种作用可以被COX-2抑制剂消除，与缺血预适应相比，氙气诱导的预适应并不增加COX-2mRNA及其蛋白的表达。这一结果表明缺血预适应与氙气诱导的预适应在COX-2调节上存在差别。
