Hemodynamic and antifibrotic effects of a selective liver nitric oxide donor V-PYRRO/NO in bile duct ligated rats

AIM: To assess whether a liver specific nitric oxide (NO) donor (V-PYRRO/NO) would prevent the development of portal hypertension and liver fibrosis in rats with bile duct ligation (BdL). METHOdS: Treatment (placebo or V-PYRRO/NO 0.53 μmol/kg per hour) was administered i.v. to rats 2 d before BdL (d-2) and maintained until the day of hemodynamic measurement (d26). Intra-hepatic NO level was estimated by measuring liver cGMP level. Effects of V-PYRRO/NO on liver fibrosis and lipid peroxidation were also assessed. RESULTS: Compared to placebo treatment, V-PYRRO/ NO improved splanchnic hemodynamics in BdL rats: portal pressure was significantly reduced by 27% (P d collateral circulation development was almost completely blocked (splenorenal shunt blood flow by 74%, P = 0.007). Moreover, V-PYRRO/NO significantly prevented liver fibrosis development in BdL rats (by 30% in hepatic hydroxyproline content and 31% in the area of fibrosis, P due to a decrease in lipid peroxidation by 44% in the hepatic malondialdehyde level (P = 0.007). Interestingly, we observed a significant and expected increase in liver cGMP, without any systemic hemodynamic effects (mean arterial pressure, vascular systemic resistance and cardiac output) in both sham- operated and BdL rats treated with V-PYRRO/NO. This result is in accordance with studies on V-PYRRO/NO metabolism showing a specific release of NO in the *** in BdL rats improved liver fibrosis and splanchnic hemodynamics without any noxious systemic hemo- dynamic effects.

AIM: To investigate the conformity of management practices of gastrointestinal hemorrhage in cirrhotic patients with relevant guidelines. METHOdS: A questionnaire on the management of digestive bleeding was completed for all consecutive cirrhotic patients admitted to 31 french hospitals. RESULTS: One hundred and twenty-six bleeding events were recorded. It was the first bleeding episode in 79 patients (63%), of whom 40 (51%) had a prior diagnosis of cirrhosis and 25 (32%) had previously undergone an endoscopy. The bleeding episode was a recurrence in 46 patients (37%). The median time between onset and admission was 4 h, but exceeded 12 h in 42% of cases. There was an agreement between centers forearly vasoactive drug administration (87% of cases), association with ligation (42%) more often than sclerosis (21%) at initial endoscopy, and antibiotic prophylaxis (64%). By contrast, prescription of beta-blockade alone or in combination (0 to 100%, P = 0.003) for secondary prophylaxis and lactulose (26% to 86%, P = 0.04), differed among centers. CONCLUSION: In french hospitals, management of bleeding related to portal hypertension in cirrhotic patients is generally in keeping with the consensus. Broad variability still remains concerning beta-blockade use for secondary prophylaxis. Screening for esophageal varices, the use of antibiotic prophylaxis and patients information need to be improved.

Leishmaniasis in the Argentine Republic: Temporal and geographical distribution from 2013 to 2017

Objective: To assess the temporal and geographical distribution of confirmed cases of cutaneous, mucocutaneous and visceral leishmaniasis in the Argentine Republic from 2013 to 2017. Methods: A retrospective study was carried out using data collected from the Integrated Surveillance Bulletin database of the National System of Health Surveillance. Confirmed cases of cutaneous, mucocutaneous and visceral leishmaniasis up to the 52 nd epidemiological week of each year was included. Results: In the 5 years period, 1 295 confirmed leishmaniasis cases were reported in the Argentine Republic. One thousand twenty-eight(1 028) cases corresponded to cutaneous leishmaniasis(87.10%), being the most common type of leishmaniasis. Mucocutaneous leishmaniasis was in the second place in the country with 115 cases reported, mostly in the Northwest and Northeast regions. A total of 52 individuals with visceral leishmaniasis were identified and Misiones Province was the most affected. Conclusions: It is important to analyze the temporal and geographical distribution of leishmaniasis in order to provide an adequate management and surveillance.

Background-Atrial fibrillation(AF)is the most common sustained cardiac arrhythmia, and patients with AF have a significantly increased risk for ischemic stroke. Approximately 15%of all strokes are caused by AF. The molecular basis and underlying mechanisms and pathophysiology of AF remain largely unknown. Methods and Results-We have identified a large AF family with an autosomal recessive inheritance pattern. The AF in the family manifests with early onset at the fetal stage and is associated with neonatal sudden death and, in some cases, ventricular tachyarrhythmias and waxing and waning cardiomyopathy. Genome-wide linkage analysis was performed for 36 family members and generated a 2-point logarithm of the odds(LOd)score of 3.05 for marker d5S455. The maximum multipoint LOd score of 4.10 was obtained for 4 markers: d5S426, d5S493, d5S455, and d5S1998. Heterozygous carriers have significant prolongation of P-wave duration on ECGs compared with noncarriers(107 versus 85 ms on average; P=0.000012), but no differences between these 2 groups were detected for the PR interval, QRS complex, ST-segment duration, T-wave duration, QTc, and R-R interval(P >0.05). Conclusions-Our findings demonstrate that AF can be inherited as an autosomal recessive trait and define a novel genetic locus for AF on chromosome 5p13(arAF1). A genetic link between AF and prolonged P-wave duration was identified. This study provides a framework for the ultimate cloning of the arAF1 gene, which will increase the understanding of the fundamental molecular mechanisms of atrial fibrillation.

目的在27例儿茶酚胺介导的多形性室速患者及家庭成员中寻找CASQ2基因突变。方法应用直接dNA序列分析对临床诊断为家族性多形性室速27例患者及家庭成员进行进行基因突变分析。应用Taqman基因分型检测在以1400名正常人群中确定CASQ2变异的频率。结果在一个多形性室速家系中发现了1个新的杂合子改变F189L。通过与多种生物比对，证实该改变发生于在第2结构域一个高度保守的氨基酸上，位于第5外显子的F189L的改变，使编码蛋白的第189位氨基酸由苯丙氨酸改变为亮氨酸。应用Taqman SNP genotyping assay分析方法证实正常人群1400名中未发现同样改变。在CASQ2基因中未发现其他引起疾病的基因突变。结论发现了CASQ2基因的1个新F189L突变。

Objectives - The aim of this prospective study was to evaluate the feasibility of radiofrequency ablation of hepatic tumors. We studied the rates of mortality, morbidity, and recurrence and recorded overall and disease-free survival. Methods - All patients aged over 18 years with histologically proven malignant liver disease inaccessible to surgical treatment (one to four tumor localizations) were eligible for inclusion in the study. Child-Pugh score > B9, platelets count ≤ 50× 103/mm3, prothrombin index ≤ 50% and pregnancy were exclusion factors. Results - Between July 2001 and June 2004, 51 patients (13 women, 38 men, mean age 66 ± 9 years) were included and underwent 61 radiofrequency thermal ablation procedures for 82 hepatic lesions: hepatocarcinoma (N = 44), cholangiocarcinoma (N = 1), metastasic tumors (N = 37). Hospital morbidity and mortality were 11.5% and 2% respectively. Recurrence rate at 3 years was 20.6% . In multivariate analysis, only tumor size > 3 cm was correlated significantly with recurrence (P = 0.03). Actuarial 1- , 2- and 3- year survival rates in the hepatocarcinoma group (N = 31) were 84.7% , 57.7% and 34.6% respectively. Actuarial 1- , 2- and 3- year survival rates in the metastasis from colorectal adenocarcinoma group (N = 14) were 90% , 54% and 54% respectively (P = 0.72). Conclusion - Radiofrequency thermal ablation is an effective treatment for hepatic tumors measuring less than 3 cm. There is a low risk of complications and open surgery can be associated. However in the Absence of randomized studies comparing radiofrequency and surgery, respective indications cannot be defined in detail.

Background:Pulmonary hamartoma(PH)is the most common benign pulmonary tumor usually presenting as a solitary,well-demarcated nodule composed of mature cartilage and adipose *** unusual histologic variants are challenging on clinic-radiologic and pathologic ***:We collected a multicentric,retrospective,consecutive series of 206 PH with clinical characteristics and histologic ***:Clinical data were in line with the literature demonstrating a male prevalence(2:1)and a median age of 61.6 *** median size of the nodule was 17.8 *** performed,SUVmax>2.5 at FdC-PET was evidenced in 4%of cases(5 out of 119)and was significantly associated with undifferentiated/myxoid *** histology showing chondroid or chondro-lipomatous PH was quoted in 186 cases(90%),while leiomyomatous and undifferentiated/myxoid variants were recorded in 20 cases(10%).The high rate of unusual variants on pathology is possibly related to a selection bias due to external consultations or surgical excision of the lesions lacking classic features at imaging ***:About 10% of PH may show unusual histology and SUVmax>2.5 at FdC-PET in 4%.These uncommon findings may challenge the correct recognition,raising some concerns in terms of differential diagnosis with several other mesenchymal tumors in the *** features should be kept in mind to achieve a correct diagnosis and avoid unnecessary invasive treatments.
