Impact of hospital type on risk-adjusted,traffic-related 30-day mortality:a population-based registry study

Background:Disparity between medical treatment for men and women has been recorded worldwide. However, it is difficult to find out if the disparities in both the use of resources and outcome depend entirely on sex-related discrimination. Our aim was to investigate if there are differences in burn treatments between the sexes. Methods:All patients admitted with burns to Link?ping University Hospital during the 16-year period 2000–2015 were included. Interventions were prospectively recorded using the validated Burn SCoring system (BSC). Data were analysed using a multivariable panel regression model adjusted for age, percentage total body surface area (%TBSA), and in-hospital mortality. Results:A total of 1363 patients were included, who generated a total of 22,301 daily recordings while they were inpatients. Males were 70%(930/1363). Sex was not an independent factor for daily scores after adjustment for age,%TBSA, and mortality in hospital (model R2=0.60, p<0.001). Conclusion:We found no evidence of inequity between the sexes in treatments given in our burn centre when we had adjusted for size of burn, age, and mortality. BSC seems to be an appropriate model in which to evaluate sex-related differences in the delivery of treatments.

Characterization of the tribologically relevant cover layers formed on copper in oxygen and oxygen-free conditions

Engineering in vacuum or under a protective atmosphere permits the production of materials, wherever the absence of oxygen is an essential demand for a successful processing. However, very few studies have provided quantitative evidence of the effect of oxidized surfaces to tribological properties. In the current study on 99.99% pure copper, it is revealed that tribo-oxidation and the resulting increased abrasive wear can be suppressed by processing in an extreme high vacuum (XHV) adequate environment. The XHV adequate atmosphere was realized by using a silane-doped shielding gas (1.5 vol% SiH4 in argon). To analyse the influence of the ambient atmosphere on the tribological and mechanical properties, a ball–disk tribometer and a nanoindenter were used in air, argon, and silane-doped argon atmosphere for temperatures up to 800 ℃. Resistance measurements of the resulting coatings were carried out. To characterize the microstructures and the chemical compositions of the samples, the scanning electron microscopy (SEM), energy-dispersive X-ray spectroscopy (EDS), and X-ray diffraction (XRD) were used. The investigations have revealed a formation of η-Cu3Si in silane-doped atmosphere at 300 ℃, as well as various intermediate stages of copper silicides. At temperatures above 300 ℃, the formation of γ-Cu5Si were detected. The formation was linked to an increase in hardness from 1.95 to 5.44 GPa, while the Young’s modulus increased by 46% to 178 GPa, with the significant reduction of the wear volume by a factor of 4.5 and the suppression of further oxidation and susceptibility of chemical wear. In addition, the relevant diffusion processes were identified using molecular dynamics (MD) simulations.

Cycling of Carbon and Other Elements in a Beech Forest Hestehave, Jutland, Denmark, in the Past 50 Years

Plant biomass, primary production and mineral cycling in the beech forest (Fagus sylvatica L.), Hestehave in Jutland, Denmark were studied over a 50-year period. The role of the forest as a carbon sink was also assessed. Aboveground tree biomass was 226 t·ha-1 in 1970 and after a 50-year 539 t·ha-1 in 2014, an unexpected increase with 313 t·ha-1. Annual production at those two points in time was 13.4 and 20.5 t·ha-1, respectively. It was apparent that the tree biomass was still acting as a sink for carbon, which was the dominant element in the aboveground parts. The concentration of other elements (N > K > Mg > P > S > Na > Mn > Zn > Fe > Cu) ranged from 495 to 0.4 kg·ha-1. Annual litterfall restored 3.2 t·ha-1 to the soil as organic matter or 1.6 t·ha-1 as carbon. Over the year 53% of the litterfall was decomposed. A pH decrease of 0.95 units in the soil was observed between 1968 and 1993. This was attributed to fallout from a neighbouring thermal heating station affecting sulfur deposition and increasing soil acidification. After 1993, when filters were fitted in the heating station, the pH decrease in the soil was smaller, only 0.09 pH-units up to 2011. The increased tree growth is an additional, likely explanation for the observed soil acidification. Deposition of the growth-limiting element nitrogen increased during later years and is now, most likely around 20 kg·ha-1 per annum, which may partly contribute to the increased production.

Brassinosteroid （BR） hormone signaling controls multiple processes during plant growth and development and is initiated at the plasma membrane through the receptor kinase BRASSINOSTEROID INSENSITIVE1 （BRI1） together with co-receptors such as BRI1-ASSOClATED RECEPTOR KINASE1 （BAK1）. BRI1 abun- dance is regulated by endosomal recycling and vacuolar targeting, but the role of vacuole-related proteins in BR receptor dynamics and BR responses remains elusive. Here, we show that the absence of two DUF300 domain-containing tonoplast proteins, LAZARUS1 （LAZl） and LAZl HOMOLOG1 （LAZlH1）, causes vacuole morphology defects, growth inhibition, and constitutive activation of BR signaling. Intriguingly, tonoplast accumulation of BAK1 was substantially increased and appeared causally linked to enhanced BRI1 trafficking and degradation in lazl lazlhl plants. Since unrelated vacuole mutants exhibited normal BR responses, our findings indicate that DUF300 proteins play distinct roles in the regulation of BR signaling by maintaining vacuole integrity required to balance subcellular BAK1 pools and BR receptor distribution.

Plant Biomass, Primary Production and Mineral Cycling of a Mixed Oak Forest in Linnebjer, Sweden

Plant biomass, primary production and mineral cycling were studied in a mixed deciduous forest (Quercus robur L., Tilia cordata L. and Corylus avellana L.) in southern Sweden. Plant biomass amount above and below ground was 201 and 37 t·ha-1, respectively. Primary production above and below ground was an estimated 13.3 and 2.3 t·ha-1, respectively. Carbon was the dominant element in the forest ecosystem, comprising 133 t·ha-1. Other major elements were: N > Ca > K > Si > Mg > S > Mn > P > Fe and Na (range 1123 to 18 kg·ha-1), followed by some trace elements. Yearly litterfall restored 6.0 t·ha-1 organic matter or 2.3 t·ha-1 carbon. Approximately 45% decomposed and returned to the soil during the year. Monitoring of other elements revealed that the ecosystem received inputs through dry and wet deposition, in particular 34.4 kg·ha-1 S and 9.4 kg·ha-1 of N yearly as throughfall. Determination of yearly biomass increase showed that the oak forest ecosystem was still in an aggradation or accumulation phase.

Linnebjer—A South Swedish Oak Forest and Meadow Area—Revisited after Half a Century

An oak forest and three wet meadows/fens were reinvestigated after 50 years concerning tree vitality, biomass and productivity, and soil chemistry. Sulphur and nitrogen deposition has changed dramatically during these years, and the aim was to analyse the differences in both the oak forest and the open field ecosystems. Trees were re-measured and soil profiles were resampled. Important visible changes in the oak forest were stated concerning the vitality of oaks. Aboveground there was a decrease in tree biomass, production and litter fall, but a huge increase in standing dead logs. During the years, the deposition of sulphur had decreased drastically, but nitrogen deposition was still high. Soil acidification in the forest had decreased, reflected in an increased base saturation in the forest, in spite of slightly lowered pH-values. Strongly increased amounts of exchangeable Ca and Mg now appeared in the forest soil, and a substantial transport of calcium and magnesium had obviously taken place from the forest soil to the meadow and fens during the years. However, the most important soil change was the accumulation of organic matter. The increased accumulation of organic matter in turn meant increased amounts of colloid particles and microsites for ion exchange in the soil. This favoured 2-valence base cations, and especially Ca and Mg that increased very much in all the studied ecosystems. Carbon as well as nitrogen had strongly increased in the forest, meadow and fen soils. This was interpreted as a natural result of increased vegetation growth due to high nitrogen deposition, increased global annual temperature and increased carbon dioxide concentration in air. It was concluded that the decreased deposition of sulphur had had a positive effect on soil chemistry, and that the deposition of nitrogen probably had stimulated vegetation growth in general, and contributed to increased amount of organic matter in the soils. APDC 和MIBK 混合液,经短的分隔后与含有镉的试液混合,经过反应管进行络合萃取反应,再通过一个相分离器,将含有镉的有机相导入原子吸收进行测定.文中对一些反应参数如流动速度,反应管长度,溶液酸度以及与肥料有关的外来离子影响及其消除干扰的方法进行了试验.本法能以每小时24个试样的速度测量,可含有1—50ppb 镉的溶液,其灵敏度与原子吸收直接吸入法相比可增大至23倍.本法用于测定几个肥料样品,回收率为98%左右,相对标准偏差为1.9%.用本法的流程图测定Cu,Fe,Co,Pb,Ni,Ag 和Zn 等七个元素最佳酸度条件及灵敏度的结果有参考价值.

Should Propofol and Alfentanil Be Combined in Patient-Controlled Sedation? A Randomised Controlled Trial Using Pharmacokinetic Simulation

Background: Patient-controlled sedation (PCS) is increasingly used for moderate sedation. Detailed understanding is essential for maintaining safety and giving the most benefit. We wanted to explore the associations between patients’ characteristics, perioperative pain and anxiety, the procedure, and the calculated concentrations at the effect site (Ce) of propofol. We also wanted to analyse the pharmacokinetic profiles of propofol and alfentanil during PCS, and their association with respiratory complications. Methods: 155 patients were double-blinded and randomised to have propofol or propofol and alfentanil for PCS during gynaecological surgery. Pharmacokinetic simulation of Ce and multiple regressions aided the search for correlations between explanatory variables and concentrations of drugs. Results: In group propofol, treatment for incontinence, anterior repair, and the patient’s weight correlated the best (B-coef = 0.20, 0.20 and 0.01;r = 0.69;r² = 0.48). When alfentanil was added, alfentanil and the patient’s weight were associated with Ce of propofol (B-coef = ǂ.40 and 0.01;r = 0.70;r² = 0.43). Logistic regression indicated that age and Ce of drugs were related to ten cases of respiratory complications. Conclusions: Patients’ weights and the type of surgery performed were associated with the Ce of propofol;this knowledge could be used for refinement of the doses given during PCS. Because the pharmacokinetic profiles of propofol and alfentanil are different, the alfentanil effect becomes predominant during the time course of sedation. In order to reduce the risk of early and late respiratory depression, alfentanil should not be added to propofol in the same syringe.
