Gastric cancer(GC)represents a leading cause of cancer related morbidity and mortality worldwide accounting for more than 1 million of newly diagnosed cases and thousands of deaths every *** the last decade,the development of targeted therapies and the optimization of already available chemotherapeutic drugs has expanded the available treatment options for advanced GC and granted better survival expectations to the *** the same time,global efforts have been undertaken to investigate in detail the genomic and epigenomic heterogeneity of this disease,resulting in the identification of new specific and sensitive predictive and prognostic biomarkers and in innovative molecular classifications based on gene expression ***,several randomized studies aimed at exploring new innovative agents,such as immune checkpoint inhibitors,failed to demonstrate clinically meaningful survival ***,it is essential to further improve the molecular characterization of GC subgroups in order to provide researchers and medical oncologists with new tools for patients’selection and stratification in future clinical development programs and subsequent *** aim of the present manuscript is to provide a global overview of the recent molecular classifications from The Cancer Genome Atlas and the Asian Cancer Research Group and to present key promising developments in the field of immunotherapy and targeted therapies in metastatic GC.

Borderline resectable pancreatic cancer:Certainties and controversies

Borderline resectable(BR)pancreatic ductal adenocarcinoma(PDAC)is currently a well-recognized entity,characterized by some specific anatomic,biological and conditional features:It includes patients with a stage of disease intermediate between the resectable and the locally advanced *** term BR identifies a tumour with an aggressive biological behaviour,on which a neoadjuvant approach instead of an upfront surgery one should be preferred,in order to obtain a radical resection(R0)and to avoid an early recurrence after *** if during the last decades several studies on this topic have been published,some aspects of BR-PDAC still represent a matter of *** aim of this review is to critically analyse the available literature on this topic,particularly focusing on:The problem of the heterogeneity of definition of BR-PDAC adopted,leading to a misinterpretation of published data;its current management(neoadjuvant vs upfront surgery);which neoadjuvant regimen should be preferably adopted;the problem of radiological restaging and the determination of resectability after neoadjuvant therapy;the post-operative outcomes after surgery;and the role and efficacy of adjuvant treatment for resected patients that already underwent neoadjuvant therapy.

Are models of volumetric production at the diametric distribution and total stand level mutually compatible?

The objective of this work was to compare estimates generated by a diametric distribution model and a total stand model against the pre-cut *** model efficiency was also *** were evaluated from 30 permanent sample plots in a Eucalyptus urophylla stand,comprising 24 sample plots used for model fitting,and six sample plots for *** volume of wood per hectare was estimated for different productive units(sites),using 7 years as the reference *** model adjustment quality was verified by adjustment and precision statistics:the correlation between observed and predicted variables,root mean square error percentage,graphical analysis of residual distribution,and a frequency histogram for classes of relative errors and *** the two-parameter Weibull probability density function adhered to the data for tree evolution in diameter classes for the reference age(7 years)in the different productivity classes,it generated imprecise estimates of the number of ***,it produced inaccurate volumetric production *** total stand model provided reliable projections of production volumes in different productivity classes for both adjustment types,showing compatibility with the pre-cut inventory according to a Tukey *** summary,the total stand model generated estimates that were compatible with the pre-cut inventory while the diametric distribution model did not.

Background:Lymph node ratio(LNR;positive/harvested lymph nodes)was identified as overall survival predictor in several cancers,including pancreatic *** remains unclear if LNR is predictive of overall survival in pancreatic adenocarcinoma patients staged *** study assessed the prognostic overall survival role of LNR in pancreatic adenocarcinoma patients in relation with lymph node ***:A retrospective international study in six different centers(Europe and United States)was *** adenocarcinoma patients who underwent pancreatoduodenectomy from 2000 to 2017 were *** with neoadjuvant treatment,metastases,R2 resections,or missing data regarding nodal status were *** curves were calculated using Kaplan-Meier method and compared using log-rank *** Cox regressions were performed to find independent overall survival predictors adjusted for potential ***:A total of 1,327 patients were *** node involvement(pN+)was found in 1,026 patients(77%),561 pN1(55%)and 465 pN2(45%).Median LNR in pN+patients was 0.214[interquartile range(IQR):0.105-0.364].On multivariable analysis,LNR was the strongest overall survival predictor in the entire cohort[hazard ratio(HR)=5.5;95%confidence interval(CI):3.1-9.9;P<0.001]and pN+patients(HR=3.8;95%CI:2.2-6.6;P<0.001).Median overall survival was better in patients with LNR<0.225 compared to patients with LNR≥0.225 in the entire cohort and pN+*** results were found in pN2 patients(worse overall survival when LNR≥0.225).Conclusions:LNR appeared as an important prognostic factor in patients undergoing surgery for pancreatic adenocarcinoma and permitted to stratify overall survival in pN2 *** should be routinely used in complement to tumor-node-metastasis(TNM)stage to better predict patient prognosis.

Use of Logistic Regression Model for Prediction of Non-Timber Forest Products

The use of non-timber is a valuable alternative for the conservation of tropical forests. Juçara (Euterpe edulis Mart.) is considered one of the main alternatives in the Atlantic Forest for the production of açaí pulp. However, there are few studies that aim to evaluate their production. The present study aimed to construct a probabilistic model to predict the production of Euterpe edulis bunches, using dendrometric variables and competition index. Twenty plots of 10 × 50 m were sampled in an area with said specie, showing the arboreal entities with diameter at breast height > 4.8 cm, and recording the Euterpe edulis phenomena. The main variables influencing the production of bunches were assessed using logistic regression model. The logistic regression showed the variables diameter breast height (DBH) and total height (h) as significant to explain the variation between productive and non-productive entities. The competition index tested was not significant (p-value = 0.221). The model of prediction of curl production in Juçara can be written as: Zi = -6.878594 + 0.2522454 × DBH + 0.1951574 × h. The use of a logistic regression model showed potential for prediction of non-timber forest products.

Right ventricular septal pacing: Safety and efficacy in a long term follow up

AIM: To evaluate the safety and efficacy of the permanent high interventricular septal pacing in a long term follow up, as alternative to right ventricular apical pacing. METHODS: We retrospectively evaluated:(1) 244 patients(74 ± 8 years; 169 men, 75 women) implanted with a single(132 pts) or dual chamber(112 pts) pacemaker(PM) with ventricular screw-in lead placed at the right ventricular high septal parahisian site(SEPTAL pacing);(2) 22 patients with permanent pacemaker and low percentage of pacing( 80%) right ventricular apical pacing(RVA). All patients had a narrow spontaneous QRS(101 ± 14 ms). We evaluated New York Heart Association(NYHA) class, quality of life(Qo L), 6 min walking test(6MWT) and left ventricular function(end-diastolic volume, LV-EDV; end-systolic volume, LVESV; ejection fraction, LV-EF) with 2D-echocardiography. RESULTS: Pacing parameters were stable duringfollow up(21 mo/patient). In SEPTAL pacing group we observed an improvement in NYHA class, Qo L score and 6MWT. While LV-EDV didn't significantly increase(104 ± 40 m L vs 100 ± 37 m L; P = 0.35), LV-ESV slightly increased(55 ± 31 m L vs 49 ± 27 m L; P = 0.05) and LV-EF slightly decreased(49% ± 11% vs 53% ± 11%; P = 0.001) but never falling < 45%. In the RVA pacing control group we observed a worsening of NYHA class and an important reduction of LV-EF(from 56% ± 6% to 43% ± 9%, P < 0.0001).CONCLUSION: Right ventricular permanent high septal pacing is safe and effective in a long term follow up evaluation; it could be a good alternative to the conventional RVA pacing in order to avoid its deleterious effects.
