Penetrating keratoplasty in children under 3 years old with congenital corneal opacities

AIm:To evaluate the graft rejection and visual outcomes after penetrating keratoplasty(PK)in the presence of various congenital corneal opacities in ***:In this retrospective cohort study,children who underwent PK were then followed for 5 *** patient’s medical records were collected from June 2014 until June 2019 and analyzed in December *** patients were children under three years old with congenital corneal opacities with or without microcornea who came to a pediatric ophthalmologist and underwent PK in Jakarta Eye Center(JEC).Beforehand,all children have participated in a thorough evaluation for *** the case of severe microcornea was not advised to undergo *** visual outcomes and graft survival rate were described in *** graft survival plot was presented with Kaplan-meier,while the visual acuity was analyzed using the Wilcoxon signed ranks ***:Sixteen eyes from eleven patients(seven girls and four boys)underwent *** graft survival rate of the first 6,12,and 18 mo later of keratoplasty was 100%,83.3%,and 66.7%,*** overall mean survival time is 22 mo(standard error 2.419),and no significant difference between the patients underwent PK before and after 36 mo of their age(P=0.52).The graft failure was 50%,and postsurgery complications included cataract 43.7%,band keratopathy 12.5%,and scleromalasia 6.25%.Wilcoxon test analysis of visual acuity post keratoplasty was not statistically significant(P=0.34),while overall showed 44%improvements of visual outcome for 5 y of *** a good survival at one year up to 22 mo(83.3%),the visual acuity could be achieved(63%),and showed improvements(44%)during ***:The complications are frequent for pediatric ***,corneal surgery on infants requires careful case selection,adequate pre-operative evaluation,skilled surgery(optical correction),very close cooperation family–physician,intensive post-operation care,and amblyopia management in the

Rehabilitation following spinal cord injury:how animal models can help our understanding of exercise-induced neuroplasticity

Spinal cord injury is a devastating condition that is followed by long and often unsuccessful recovery after trauma. The state of the art approach to manage paralysis and concomitant impairments is rehabilitation, which is the only strategy that has proven to be effective and beneficial for the patients over the last decades. How rehabilitation influences the remodeling of spinal axonal connections in patients is important to understand, in order to better target these changes and define the optimal timing and onset of training. While clinically the answers to these questions remain difficult to obtain, rodent models of rehabilitation like bicycling, treadmill training, swimming, enriched environments or wheel running that mimic clinical rehabilitation can be helpful to reveal the axonal changes underlying motor recovery. This review will focus on the different animal models of spinal cord injury rehabilitation and the underlying changes in neuronal networks that are improved by exercise and rehabilitation.

Serotonin is a monoamine neurotransmitter synthetized in various populations of brainstem *** the spinal cord,descending serotonergic projections regulate postural muscle tone,locomotion and rhythm and coordination of movements via the Central Pattern *** a spinal cord injury,serotonergic projections to the lumbar spinal cord,where the Central Pattern Generators are located,are interrupted resulting in devastating locomotor impairments and changes in the expression and activation of serotonin and its spinal *** molecular cues that control the precise patterning and targeting of serotonergic inputs onto Central Pattern Generator networks in healthy animals or after injury are still *** our recent research work,we have been particularly interested in Semaphorin7A,which belongs to the Semaphorins family involved in guiding growing axons and controlling plasticity of synaptic *** this review,we discuss the role of Semaphorin7A signaling as an important molecular actor that instructs the patterning of serotonin inputs to spinal Central Pattern Generator *** show that Semaphorin7A controls the wiring of descending serotonin axons in the spinal *** results reveal that mistargetting of serotonin fibers in the spinal cord is compensated in healthy uninjured Semaphorin7A deficient mice so that their gross locomotion proceeds *** also demonstrate that when the system is challenged with a spinal lesion,the pattern of post-injury serotonin expression is significantly altered in Semaphorin7A deficient mice with specific ectopic targeting of serotonin fibers in the lumbar spinal *** mechanisms in place in uninjured Semaphorin7A deficient mice are lost and injured Semaphorin7A deficient mice exhibit a worsening of their post-injury locomotor *** findings identify Semaphorin7A as a critical determinant of serotonergic circuit formation in healthy or spinal cord injured mice.

Retinal manifestations of patients with human immun- odeficiency virus, a multiethnics study in malaysia

· AIm: To investigate the fundus findings of patients infected with human immunodeficiency virus (HIV) in correlation to Highly Active Antiretroviral Therapy (HAART) and CD4 count. · mETHODS: Two hundred and two patients of the three major races (Chinese, malay and Indian) in malaysia were recruited in this population-based cross-sectional study. This consisted of confirmed HIV sero-positive patients with HAART treatment (n=95) or without HAART therapy (n=107) from December 2007 to march 2008. They were further classified into the HIV infected group, AIDS related complex (ARC) group and AIDS group. Each group was then subdivided according to their CD4 count. Clinical fundus findings were recorded. · RESULTS: Sixty six patients (32.7%) showed presence of fundus manifestation, majority of which was HIV microangiopathy (89%) and the rest being Cytomegalovirus (CmV) retinitis (11%). The most common fundus lesion was cotton wool spot (34%). There was a higher incidence of fundus manifestation in the non HAART group than the HAART group (P =0.04) and in patients with CD4 count less than 200 cells/ml in both groups (P =0.01). The HAART therapy had remarkably reduced the percentage of fundus manifestation by 20% but CD4 count remains the marker for fundus manifestations. There were no significant differences noted in the retinal manifestation among the different races. (ANOVA, P =0.25). · CONCLUSION: The fundus manifestations were higher in patients with CD4ml and in the non HAART group. Hence the HAART therapy is capable of reducing the incidence of fundus manifestations, however the CD4 count determines the occurrence of fundus manifestations. ·

The number of people affected by spinal cord injuries （SCI） ranges from 2.5 to 4 million worldwide. Traumatic SCI induces a primary injury due to initial mechanical impact that causes focal cellular and blood-spinal cord barrier damages. Subsequently, secondary injuries resulting from infiltration of peripheral monocytes, ischemia, edema,inflammation, glial scar and cystic cavities formation as well as excitotoxicity trigger cellular demise and prevent spontaneous axonal regrowth. Secondary damages, including aggregation of extracellular matrix protein at the lesion epicenter, thus greatly amplify impairments induced by the initial mechanical damage.

Dexamethasone-loaded silicone matrices offer an interesting potential as innovative drug delivery systems,*** the treatment of inner ear diseases or for ***,very long drug release periods are targeted:several years/*** renders the development and optimization of novel drug products cumbersome:experimental feedback on the impact of the device design is obtained very slowly.A better understanding of the underlying mass transport mechanisms can help facilitating research in this field.A variety of silicone films were prepared in this study,loaded with amorphous or crystalline *** polymorphic drug forms were investigated,the film thickness was altered and the drug optionally partially/completely exchanged by much more water-soluble dexamethasone‘phosphate’.Drug release studies in artificial perilymph,scanning electron microscopy,optical microscopy,differential scanning calorimetry,X-ray diffraction and Raman imaging were used to elucidate the physical states of the drugs and polymer,and of the systems’structure as well as dynamic changes thereof upon exposure to the release *** particles were initially homogeneously distributed throughout the *** hydrophobicity of the matrix former very much limits the amounts of water penetrating into the system,resulting in only partial drug *** mobile drug molecules diffuse out into the surrounding environment,due to concentration ***,Raman imaging revealed that even very thin silicone layers(m)can effectively trap the drug for prolonged periods of *** physical state of the drug(amorphous,crystalline)did not affect the resulting drug release kinetics to a noteworthy extent.
