Dear Editor,The molecular determinants that drive breast cancer progression to metastasis are complex and partly controlled by nucleic acid epi-modifications[1].Although DNA-methyl modifications in breast cancer metastasis have been well described,there is limited understanding of the role of RNA methylation in advanced disease[2].This study aimed to provide an understanding on the role of the epitranscriptome in estrogen receptor-positive(ER+)breast cancer progression to metastasis.

Living with a congenital heart condition can require a daunting and uncertain patient journey during which the support of a specialist nursing service is vital. A descriptive, cross-sectional evaluation completed over two years, utilised a postal questionnaire to investigate patient’s satisfaction with aspects of service. Qualitative responses to open questions on satisfaction with emotional support (n = 103) and contributions to well-being (n = 90) were analysed using a framework approach which identified eight themes. Professional knowledge and expertise, service accessibility, caring attributes, enabling patients to cope with anxiety, depression and meeting changing needs contributed positively to both patient well-being and emotional support. Family support was influential on emotional support alone, whilst mediating medical liaison exerted an impact on well-being by enhancing feelings of security. In conclusion, the specialist nursing service contributed positively to well-being and emotional support of patients and thereby to selective aspects of continuity of care.

目的通过观察乳腺癌发生模型 MCF10中 WT1基因启动子区甲基化状态和 mRNA 表达水平,探讨该基因在乳腺癌发生中的作用。方法应用甲基化特异性 PCR 及双亚硫酸钠基因测序技术检测 MCF10模型的乳腺增生细胞系 MCF10A、癌前细胞系 MCF10AT、导管内癌细胞系***、浸润癌细胞系(MCF10CAla、MCF10CAld、MCF10CAlh)、经典乳腺癌细胞系 MCF7及正常乳腺组织中 WT1基因启动子区甲基化状态,然后用逆转录-聚合酶链反应(RT-PCR)和即时定量 PCR 技术检测上述样品的 mRNA 表达水平。结果在 MCF10模型的增生细胞系 MCF10A、癌前细胞系 MCF10AT、导管内癌细胞系 ***、浸润癌细胞系(MCF10CAla、MCF10CAld、MCF10CAlh)、经典乳腺癌细胞系 MCF7中,WT1基因启动子均处于高度甲基化状态。与正常乳腺组织相比,WT1基因 mRNA 在 MCF10模型的增生细胞系、癌前细胞系、导管内癌细胞系、浸润癌细胞系和经典乳腺癌细胞系 MCF7中的表达均有不同程度的增加(MCF10A、MCF10AT、MCF10CAla、MCF10CAld、MCF10CAlh、MCF10DCIS、MCF7的 WT1基因 mRNA 表达量分别是正常乳腺组织3.23、1.94、4.20、1.53、4.20、4.35、28.69倍)。结论乳腺癌发生过程中 WT1 mRNA 的表达不被启动子甲基化所抑制;WT1mRNA 过表达出现于乳腺癌发牛的早期阶段,提示该基因在乳腺癌发生中起作用。

AIM:To examine the impact of the patient’s birthplace on the prevalence of colonic polyps and histopathological ***:This is a retrospective audit of the colonoscopy practice of one Gastroenterologist in a tertiaryreferral hospital from 2008 to *** collected include demography,birthplace,language spoken,details of the colonoscopy including indications,completion rates,complications,results including prevalence and histopathology of *** methods used were binary logistic regression,χ2 and Mann-Whitney ***:A total of 623 patients(48%male,67%aged over 50 years)were recruited and categorised according to birthplace:Australia/New Zealand 42%,European 20%,Asian 15%,Middle Eastern/African11%,South American 9%and Pacific Islander 3%.The median age of the cohort was 56.3 years(range:17-91 years),median body mass index 27.3 kg/m2 (range:16-51 kg/m2),25%were smokers,25%had hypercholesterolemia,20%had diabetes mellitus 16%were on aspirin and 7%were on non-steroidal antiinflammatory drugs.A total of 651 colonoscopies were performed for standard *** prevalence of polyps varied according to patient’s birthplace:Europe45.1%,Australia and New Zealand 39.5%,Pacific Islands 33.3%,Asia 30.3%,Middle East and Africa26.9%and South America 24.5%(P=0.027,df=6).However,multivariate analysis revealed that birthplace was not an independent predictor of developing polyps,including adenomas and advanced adenomas after correcting for age and male ***:Birthplace is not a predictor for developing colorectal neoplasia,including adenomas and advanced adenomas;hence,should not influence the recommendations for colorectal cancer screening.

Background: Postpartum hemorrhage is one of the main causes of maternal mortality, mainly in underdeveloped countries. Deaths are mostly preventable, and are related to socioeconomic development, organization of health services and professional training. Objective: To systematize alerts for the prevention of postpartum hemorrhage. Method: This is a technical essay developed from reflections on previously produced and published texts about maternal mortality and has hemorrhage as the focus of this study. Results: It presents the concept, classification of hemorrhage, risk factors and their classification, prevention strategies, universal recommendations, pharmacological and non-pharmacological interventions when hemorrhage already exists, bleeding assessment techniques;clinical evaluation, by laboratory examination and by shock index. Conclusions: Hemorrhage is a preventable cause of maternal death through the accomplishment of multiple care. Hospital care must be conducted by qualified professionals and in adequate numbers. The risk diagnosis must be carried out during prenatal care with the recording of clinical data accessible in the health care network. All references studied are not specific to nursing, as hemorrhage prevention requires multidisciplinary action.

Lack of investment for magnetic resonance(MR)fusion systems is an obstacle to deliver targeted prostate biopsies within the prostate cancer diagnostic *** developed a coordinate-based method to support cognitive targeted prostate biopsies and then performed an audit on cancer detection and the location of *** each patient,the prostate is considered as two separate hemiprostates,and each hemiprostate is divided into 4×4×4 *** unit is therefore defined by a three-dimensional *** prospectively applied our coordinates approach to target 106 prostatic lesions in 93 *** 45(of 106;42.5%)lesions positive for cancer,27 lesions(60.0%)harbored clinically significant *** density was significantly higher in patients with proven cancer(median:0.264 ng ml^(-2))when compared to the noncancer group(median:0.145 ng ml-2;P=0.003,Wilcoxon ranksum test).Lesions with Prostate Imaging-Reporting and Data System(PIRADS)score of 5 were found to have a cancer incidence of 65.2%,while PI RADS 4 and 3 lesions have a lower risk of cancer detection,as expected,at 37.3%and 31.3%,*** probability of a lesion being cancerous in our series significantly decreases as we go from the“apex-to-base”dimension(odds ratio[OR]:2.62,95%confidence interval[Cl]:1.55-4.44,P=0.00034).Our analysis also indicates that the probability of cancer decreases as the prostate volume increases(OR:1.03,95%Cl:1.01-1.05,P=0.00327).Based on this feasibility study,the use of coordinates to guide cognitive targeted prostate biopsies warrants future validation study in additional centers.
