Endoluminal vacuum-assisted therapy(EVT)has been introduced recently to treat colorectal anastomotic leaks in clinically stable non-peritonitic *** application has been mainly reserved to low colorectal and colo-anal *** main advantage of this new procedure is to ensure continuous drainage of the abscess cavity,to promote and to accelerate the formation of granulation tissue resulting in a reduction of the abscess *** reported results are promising allowing a higher preservation of the anastomosis when compared to conventional treatments that include trans-anastomotic tube placement,percutaneous drainage,endoscopic clipping of the anastomotic defect or stent ***,despite this procedure is gaining acceptance among the surgical community,indications,inclusion criteria and definitions of success are not yet standardized and extremely heterogeneous,making it difficult to reach definitive conclusions and to ascertain which are the real benefits of this new ***,long-term and functional results are poorly *** present review is focused on critically analyzing the theoretical benefits and risks of the procedure,short-and long-term functional results and future direction in the application of EVT.

Carbon concentrations and carbon storage capacity of three old-growth forests in the Sila National Park,Southern Italy

Old-growth forests play a key-role in reducing atmospheric carbon dioxide(CO_(2)) concentrations by storing large CO_(2)amounts in biomass and soil over *** quantifies the carbon pool into different forest compartments in three Mediterranean old-growth forests of Southern Italy populated by Pinus laricio,Fagus sylvatica and Abies *** carbon pools have been assessed per compartment,i.e.,living trees,dead wood,litterfall(foliar and woody),roots and 0-20 cm topsoil,combining the whole old-growth forest mass,(i.e.,using tree allometric relationships,deadwood factor conversions,root-to-shoot ratios,litterfall and soil samplings) by the respective organic carbon *** results show the considerable capacity of these forest ecosystems in storing CO_(2)in biomass and soil,with carbon pool values ranging from 532.2to 596.5 Mg C *** trees and 0-20 cm topsoil had larger carbon pool,contributing 53.0 and 22.1%,*** most cases,organic carbon concentration was higher(more than 60%) than the average carbon conversion rate of 50%,especially in living trees,deadwood,and woody *** study contributes further scientific evidence of the capacity of old-growth forests in storing CO_(2)in their different compartments,with special evidence on tree biomass,litterfall and mineral soil,thereby highlighting the key role of old-growth forests within the challenge of climate change mitigation.

Carotid artery endarterectomy in patients with symptomatic non-stenotic carotid artery disease

Object We sought to determine the safety and efficacy in secondary stroke prevention of carotid endarterectomy(CEA)in patients with symptomatic non-stenotic carotid artery disease(SyNC).Methods This was a single-centre retrospective case *** patients who underwent CEA for unilateral anterior circulation cerebrovascular events with ipsilateral<50%carotid stenosis from 2002 to 2020 were *** hallmarks including the degree of luminal stenosis and the presence of various vulnerable plaque characteristics(eg,intraplaque haemorrhage(IPH)on MR angiography,ulceration or low-density plaque on CT angiography)were *** presence of vulnerable plaque characteristics was compared between arteries ipsilateral to the ischaemic event and contralateral *** prevalence of perioperative/intraoperative complications,as well as recurrent ischaemic events at follow-up was *** Thirty-two patients were included in the analysis,of which 25.0%were *** arteries ipsilateral to an ischaemic event had a significantly higher prevalence of IPH when compared with contralateral arteries(80.0%vs 0.0%;p<0.001).There were no intraoperative *** patient(3.1%)developed symptoms of transient ipsilateral ischaemia 1 day following CEA which resolved without *** a median follow-up of 18.0 months(IQR 5.0-36.0),only one patient(3.1%)experienced a transient neurologic deficit with complete resolution(annualised rate of recurrent stroke after CEA of 1.5%for a total follow-up of 788 patient-months following CEA).All other patients(31/32,96.9%)were free of recurrent ischaemic *** CEA appears to be safe and well-tolerated in patients with *** studies with larger cohorts and longer follow-up intervals are needed in order to determine the role of CEA in this patient population.

On-surface synthesis under ultrahigh vacuum provides a promising strategy to control matter at the atomic level, with important implications for the design of new two-dimensional materials having remarkable electronic, magnetic, or catalytic properties. This strategy must address the problem of limited extension of the domains due to the irreversible nature of covalent bonds, which prevents the ripening of defects. We show here that extended materials can be produced by a controlled co-deposition process. In particular, co-deposition of quinoid zwitterion molecules with iron atoms on a Ag（111） surface held at 570 K allows the formation of micrometer-sized domains based on covalent coordination bonds. This work opens up the construction of micrometer-scale single-layer covalent coordination materials under vacuum conditions.

Oncological treatment is currently directed toward a tailored therapy *** cell carcinoma of the anal canal could be considered a suitable platform to test new therapeutic strategies to minimize treatment *** of care for patients with anal canal cancer consists of a combination of radiotherapy and *** treatment has led to a high rate of local control and a 60% cure rate with preservation of the anal sphincter,thus replacing surgical abdominoperineal *** node metastases represent a critical independent prognostic factor for local recurrence and *** and iliac lymph nodes are usually included in the radiation field,whereas the inclusion of inguinal regions still remains controversial because of the subsequent adverse side *** lymph node biopsies could clearly identify inguinal node-positive patients eligible for therapeutic groin irradiation.A sentinel lymph node navigation procedure is reported here to be a feasible and effective method for establishing the true inguinal node status in patients suffering from anal canal *** on the results of sentinel node biopsies,a selective approach could be proposed where node-positive patients could be selected for inguinal node irradiation while node-negative patients could take advantage of inguinal sparing irradiation,thus avoiding toxic side effects.
