AIM:To investigate the incidence of neoplasms in inflammatory bowel disease(IBD)patients and the potential causative role of ***:We performed an observational descriptive study comparing the incidence of malignancies in IBD patients treated with thiopurines and patients not treated with these *** included 812 patients which were divided in two groups depending on whether they have received thiopurines or *** have studied basal characteristics of both groups(age when the disease was diagnosed,sex,type of IBD,etc.)and treatments received(Azathioprine,mercaptopurine,infliximab,adalimumab or other immunomodulators),as well as neoplasms *** analysis was performed with the student t test,χ 2 test or Wilcoxon exact test as appropriate.A logistic regression analysis was performed as multivariate *** significance was establish at P values of less than 0.05,and 95%CI were used for the odds ***:Among 812 patients included,429(52.83%)have received thiopurines:79.5% azathioprine,14% mercaptopurine and 6.5% both drugs.44.76% of patients treated with thiopurines and 46,48% of patients who did not receive this treatment were women(P > 0.05).The proportion of ulcerative colitis patients treated with thiopurines was 30.3% compare to 66.67% of patients not treated(P f 23.37 ± 27.6 mo(range:1-118 mo),and the accumulated dose along this time was 52.2 ± 63.5 g(range:1.5-243 g).Thiopurine S-methyltransferase activity was tested in 66% of patients treated with thiopurines,among which 98.2% had an intermediate or high *** the patients treated with thiopurines,27.27%(112 patients)and 11.66%(50 patients)received treatment

Identification of life habits factors as risk for gastritis and colitis occurrence in a mestizo population of Chabeklumil, Chiapas, México

Aim: Gastric diseases are a recent trend in morbidity in México. We aimed to identify most frequent life habits factors as risk for gastritis and colitis occurrence, in a Mexican mestizo population in Chabeklumil, Chiapas. Population: This study was observational, descriptive, transversal and prospective;346 questionnaires were applied to individuals of both genders from 11 years of age, with low income and socio-economic status, after they signed the informed consent. An individual file was created by the nurses in Chabeklumil. The predominant gender in the population was female (56.1%), with a distribution of 166 among adulthood (19 - 59 years of age);among males (43.9%), 133 included adults. Results: We identified the following environmental risk factors to develop gastritis and colitis: long periods of daily fasting > consumption of irritant food and drink > stress, in females;in contrast, males reported alcohol and tobacco consumption > irritant food and drink > long periods of daily fasting > stress. We found statistical differences (P factors between females and males, in drugs and alcohol consumption, fasting, and stress, but not in consumption of irritant foods, nor in colitis and gastritis prevalence. Conclusions: females are most affected by life habits risk factors for gastritis/colitis, which occurs mainly due to lifestyle and “macho” culture, since women are precluded of a better living standard, they work hard in the field, and start early in the day without consuming a meal, which they eat late in the day once arriving home. People in Chabeklumil eat/drink a lot of irritant food like hot pepper and coffee, which damage the gastrointestinal system leading to gastritis and colitis. furthermore, men started to consume drugs at early ages, a condition also involved in developing gastritis.

In the genus Quercus, regeneration by seed is reduced or absent in many species. The naturally protected area of the Sierra fria, in Aguascalientes, Mexico contains forests that are fragmented and have been degraded due to human activities. The oak populations in this region demonstrate a very restricted sexual regeneration, with five of the most abundant species of this genus: Quercuseduardii, Q. grisea, Q. potosina, Q. resinosa and Q. sideroxyla presenting low seedling recruitment. In this study we evaluated seed viability and the effect of seed size on the germination, growth and survival of the trees, with the aim of recommending restoration and conservation program management actions for these oak species. In 2005 and 2006, 5000 acorns of each species were collected and viability, germination and growth experiments were performed in the laboratory and greenhouse. The aim of the study was to contribute to the information on agave soluble fibers since research has been focused on chicory fiber but not in agave products;thus we assess the acute toxicity and genotoxicity of two organic and high purity dietary soluble fibers from agave, Metlin? and Metlos?. We performed an acute toxicity assay in Hsd:ICR mice and Hsd:Wistar rats and an in vivo genotoxic test. Results showed that there are no deaths at any doses or genotoxicity, so it can be concluded that these products are non-toxic, at the administrated doses and none showed a cytotoxic, clastogenic or aneuploidic effect.

Metabolic syndrome as a risk factor for gallstone disease

AIM: To establish an association between the presence of metabolic syndrome and the development of gallstone ***: We carried out a cross-sectional study in a check-up unit in a university hospital in Mexico City. We enrolled 245 subjects, comprising 65 subjects with gallstones (36 women, 29 men) and 180 controls (79women and 101 men without gallstones). Body mass index, waist circumference, blood pressure, plasma insulin, and serum lipids and lipoproteins levels were measured. Insulin resistance was calculated by homeostasis model assessment. Unconditional logistic regressionanalysis (univariate and multivariate) was used to calculate the risk of gallstone disease associated with the presence of at least three of the criteria (Adult Treatment Panel Ⅲ). Analyses were adjusted for age and ***: Among 245 subjects, metabolic syndrome was present in 40% of gallstone disease subjects, compared with 17.2% of the controls, adjusted by age and gender (odds ratio (OR) = 2.79; 95%CI, 1.46-5.33; P = 0.002),a dose-dependent effect was observed with each component of metabolic syndrome (OR = 2.36, 95%CI, 0.72-7.71;P = 0.16 with one component and OR = 5.54, 95%CI,1.35-22.74; P = 0.02 with four components of metabolic syndrome). Homeostasis model assessment was significantly associated with gallstone disease (adjusted OR = 2.25;95%CI, 1.08-4.69; P = 0.03).CONCLUSION: We conclude that as for cardiovascular disease and diabetes mellitus, gallstone disease appears to be strongly associated with metabolic syndrome.

Adiponectin as a protective factor in hepatic steatosis

AIM: Obesityand insulin resistance (IR) are closely related to hepatic steatosis (HS), and adiponectin is a hepatic insulin sensitizer that has important effects in liver *** study aims at investigating the relationship between serum adiponectin concentration and the presence of ***: We carried out a cross-sectional study in a check-up unit of a University Hospital in Mexico City. We enrolled 196 subjects, comprising 98 subjects with HS (27 women, 71 men) and 98 controls (37 women and 61men). Anthropometric, metabolic and biochemical variables were measured in the two groups. Serum adiponectin and leptin concentrations were determined,their association with grade of HS tested, and concentrations,according to quartiles, compared between cases and controls. χ2 analysis for linear trends was used to test for a dose-response relationship and logistic regression analysis was conducted to test for a protective effect of ***: The HS subjects were older and more obese than controls, with a central obesity pattern. In the fourth quartile of adiponectin concentrations, HS was less common and severe. In a multivariate model of the fourth quartile of the adiponectin concentrations, we observed a protective effect (OR = 0.17, 95%CI: 0.04-0.67, P= 0.01).In subjects with more severe HS, we observed higher leptin concentrations, and caloric intakes, total fat and iron consumption were higher than in ***: The results of the present study suggest that a high serum concentration of adiponectin is associated with a protective effect against HS.

Prevalence of metabolic syndrome, obesity and diabetes type 2 in cryptogenic cirrhosis

AIM: To evaluate the prevalence of metabolic syndrome (MS), obesity and type 2 diabetes mellitus (T2DM) in a group of Mexican Mestizo patients with cryptogenic cirrhosis (CC) and to compare this group with patients with cirrhosis secondary to other causes (disease controls). METHODS: Patients with CC, diagnosed between January, 1990 and April, 2005, were included in a retrospective study. Patients with cirrhosis caused by chronic hepatitis C, alcohol abuse or autoimmune hepatitis (AIH) served as disease controls. RESULTS: A total of 134 patients with CC were analyzed. Disease controls consisted of 81 patients with chronic hepatitis C, 33 with alcohol abuse and 20 with AIH. The median age of patients with CC was 57 years (range, 16-87); 83 (61.9%) patients were female; 53 (39.6%) were Child A, 65 (48.5%) Child B, and 16 (11.9%) were Child C cirrhosis. The prevalence of MS (29.1% vs 6%; P fferences in sex, age or liver function tests between the two groups. CONCLUSION: The prevalence of MS, obesityand T2DM were higher in patients with CC than in patients with cirrhosis secondary to others causes. Our findings support the hypothesis that non-alcoholic steatohepatitis (NASH) plays an under-recognized role in CC.

Pre-transplant treatment of large polycystic kidney

AIM: To evaluate the indications, optimal timing andoutcomes of native nephrectomy and other techniques in pretransplant treatment of autosomal dominant polycystic kidney disease(PKD).METHODS: A literature review was conducted using the Pub Med and Epistemonikos databases. Keywords for pre-transplant surgical management of polycystic kidneys were: Transplant, treatment and PKD. Keywords for pre-treatment embolization of PKD were: Embolization, transplant and polycystic kidney disease. The inclusion criterions were all articles found using this search method. The exclusion criterions were articles found to include bias and not attending pre-transplant treatment options. fifteen articles were included in our final analysis. Ten articles were found regarding embolization of PKD of which three reviews were selected for final analysis. The reviews were divided into pre transplant and intra transplant treatment for the surgical treatment of PKD. All articles meeting inclusion criteria were thoroughly analyzed by two independent reviewers. A third independent reviewer was consulted if the reviewers did not agree upon the inclusion or exclusion of a specific article. No statistical analysis was ***: Studies vary regarding the technique used(open or laparoscopic), laterality(single or bilateral) and temporality of nephrectomy with respect to renal transplant(pre-transplant or simultaneous to transplant). Several groups argue in favor of simultaneous nephrectomy and kidney transplant since it avoids the deleterious effects of being anefric. Long-term results and patient satisfaction are acceptable. However, it is associated with increased operative time, transfusion rate, morbidity and length of hospital stay. Based on small sample studies, bilateral nephrectomy prior to transplant has been associated with a higher risk of morbidity and mortality.
