Incidence of hepatocellular carcinoma in outpatients with cirrhosis in Brazil: A 10-year retrospective cohort study

AIM To determine the incidence of hepatocellular carcinoma(HCC) and the impact of HCC surveillance on early diagnosis and survival of cirrhotic outpatients. METHODs In this retrospective cohort study, cirrhotic outpatients undergoing HCC surveillance between March 2005 and March 2014 were analyzed. Exclusion criteria were HIV coinfection; previous organ transplantation; diagnosis of HCC at first consultation; missing data in the medical chart; and less than 1 year of follow-up. surveillance was carried out every six months using ultrasound and serum alpha-fetoprotein determination. Ten-year cumulative incidence and survival were estimated through Kaplan-Meier analysis. REsULTs Four hundred and fifty-three patients were enrolled, of which 57.6% were male. Mean age was 55 years. Hepatitis C virus and heavy use of alcohol were the main etiologic agents of cirrhosis. HCC was diagnosed in 75 patients(16.6%), with an estimated cumulative incidence of 2.6% in the 1st year, 15.4% in the 5th year, and 28.8% in the 10 th year. Median survival was estimated at 17.6 mo in HCC patients compared to 234 mo in non-HCC patients(P stage HCC was more often detected in patients who underwent surveillance every 6 mo or less(P = 0.05). However, survival was not different between patients with early stage vs non-early stage tumors [HR = 0.54(0.15-1.89), P = 0.33].CONCLUsION HCC is a frequent complication in patients with cirrhosis and adherence to surveillance programs favors early diagnosis.

AIM： To evaluate the potential of s-nitroso-N-acetylcysteine （sNAC） in inhibition of lipid peroxidation and the effect of oral sNAC administration in the prevention of nonalcoholic fatty liver disease （NAFLD） in an animal model.METHODs： NAFLD was induced in Wistar male rats by choline-deficient diet for 4 wk. sNAC-treated animals （n=6）（1.4 mg/kg/day of sNAC, orally） were compared to 2 control groups： one （n=6） received PBs solution and the other （n=6） received NAC solution （7 mg/kg/d）. Histological variables were semiquantitated with respect to macro and microvacuolar fat changes, its zonal distribution, foci of necrosis, portal and perivenular fibrosis, and inflammatory infiltrate with zonal distribution. LOOHs from samples of liver homogenates were quantified by HPLC. Nitrate levels in plasma of portal vein were assessed by chemiluminescence. Aqueous low-density lipoprotein （LDL） suspensions （200 pg protein/mL） were incubated with CuCl2 （300 μmol/L） in the absence and presence of sNAC （300 μmol/L） for 15 h at 37 ℃ Extent of LDL oxidation was assessed by fluorimetry. Linoleic acid （LA）（18.8 μmol/L） oxidation was induced by soybean lipoxygenase （sLO）（0.056 μmol/L） at 37 ℃ in the presence and absence of N-acetylcysteine （NAC） and sNAC （56 and 560 pmol/L） and monitored at 234 nm. REsULTs： Animals in the control group developed moderate macro and microvesicular fatty changes in periportal area. sNAC-treated animals displayed only discrete histological alterations with absence of fatty changes and did not develop liver steatosis. The absence of NAFLD in the sNAC-treated group was positively correlated with a decrease in the concentration of LOOH in liver homogenate, compared to the control group （0.7±0.2 nmol/mg vs 3.2±0.4 nmol/mg protein, respectively, P〈0.05）, while serum levels of aminotransferases were unaltered. The ability of sNAC in preventing lipid peroxidation was confirmed in in vitro experiments using LA and LDL as model substrates. CONCLUsION： Oral

Long-term Regional Dynamic sea Level Changes from CMIP6 Projections

Anthropogenic climate forcing will cause the global mean sea level to rise over the 21st ***,regional sea level is expected to vary across ocean basins,superimposed by the influence of natural internal climate ***,we address the detection of dynamic sea level(DsL)changes by combining the perspectives of a single and a multimodel ensemble approach(the 50-member CanEsM5 and a 27-model ensemble,respectively,all retrieved from the CMIP6 archive),under three CMIP6 projected scenarios:ssP1-2.6,ssP3-7.0 and *** ensemble analysis takes into account four key metrics:signal(s),noise(N),s/N ratio,and time of emergence(ToE).The results from both sets of ensembles agree in the fact that regions with higher s/N(associated with smaller uncertainties)also reflect earlier *** DsL signal is projected to emerge in the southern Ocean,southeast Pacific,Northwest Atlantic,and the *** common for both sets of ensemble simulations show that while s progressively increases with increased projected emissions,N,in turn,does not vary substantially among the ssPs,suggesting that uncertainty arising from internal climate variability has little dependence on changes in the magnitude of external *** changes are greater and quite similar for the scenarios ssP3-7.0 and ssP5-8.5 and considerably smaller for the ssP1-2.6,highlighting the importance of public policies towards lower emission scenarios and of keeping emissions below a certain threshold.

Prospects for bone marrow cell therapy in amyotrophic lateral sclerosis：how far are we from a clinical treatment？

Amyotrophic lateral sclerosis（ALs） is a fatal neurodegenerative disease that causes progressive muscular atrophy and death within 3–5 years after its *** the significant advances in knowledge of ALs pathology,no effective treatment is ***,it is imperative to search for new alternatives to treat *** therapy,especially using bone-marrow cells,has showed to be very useful to protect the neural tissue in different brain disease or traumatic *** ALs,most published results show beneficial effects of the use bone marrow cells,especially mesenchymal stromal ***,until now,the best outcome extends animal＇s lifespan by only a few *** is essential to continue the search for a really effective therapy,testing different cells,routes and time-windows of *** the mechanisms that initiate and spread the degenerative process is also important to find out an effective ***,we discussed here some progresses that have been made using bone-marrow cell therapy as a therapeutic tool for ALs.

Reference genes for quantitative RT-PCR data in gastric tissues and cell lines

AIM:To evaluate the suitability of reference genes in gastric tissue samples and cell ***:The suitability of genes ACTB,B2M,GAPDH,RPL29,and 18s rRNA was assessed in21 matched pairs of neoplastic and adjacent nonneoplastic gastric tissues from patients with gastric adenocarcinoma,27 normal gastric tissues from patients without cancer,and 4 cell lines using reverse transcription quantitative real-time polymerase chain reaction(RT-qPCR).The ranking of the best single and combination of reference genes was determined by NormFinder,geNorm,BestKeeper,and *** addition,GenEx software was used to determine the optimal number of reference *** validate the results,the mRNA expression of a target gene,DNMT1,was quantified using the different reference gene combinations suggested by the various software packages for ***:ACTB was the best reference gene for all gastric tissues,cell lines and all gastric tissues plus cell ***+B2M or ACTB+B2M was the best combination of reference genes for all the gastric *** the other hand,ACTB+B2M was the best combination for all the cell lines tested and was also the best combination for analyses involving all the gastric tissues plus cell *** to the GenEx software,2 or 3 genes were the optimal number of references genes for all the gastric *** relative quantification of DNMT1 showed similar patterns when normalized by each combination of reference *** level of expression of DNMT1 in neoplastic,adjacent non-neoplastic and normal gastric tissues did not differ when these samples were normalized using GAPDH+B2M(P=0.32),ACTB+B2M(P=0.61),or GAPDH+B2M+ACTB(P=0.44).CONCLUsION:GAPDH+B2M or ACTB+B2M is the best combination of reference gene for all the gastric tissues,and ACTB+B2M is the best combination for the cell lines tested.

Pancreatic adenosquamous carcinoma and intraductal papillary mucinous neoplasm in a CDKN2A germline mutation carrier

A 69-year-old woman from a kindred with familial atypical multiple mole melanoma and carrier of a germline mutation in CDKN2A, presented with abdominal pain caused by a solid-cystic pancreatic mass. The patient had an abdominal computed tomography three years before in which there was no evidence of pancreatic lesion. The endoscopic ultrasound guided fine needle aspiration showed adenocarcinoma with squamous component. After surgical resection the final diagnosis was adenosquamous pancreatic carcinoma(AsPC) arising in an intraductal papillar mucinous neoplasm(IPMN). Adenosquamous carcinomas are uncommon in the pancreas and have rarely been described in association with IPMNs. It has worse prognosis than the ordinary pancreatic ductal adenocarcinoma and some distinct features. We review the clinical, imaging, pathologic and molecular aspects of AsPC. Differential diagnosis with contamination, squamous metaplasia and pancreatic metastases from a distant squamous carcinoma is discussed. Besides, the case is an accelerated model of the adenoma(IPMN)-carcinoma sequence probably due to the CDKN2A ger-mline mutation. somatic CDKN2A mutations are commonevents in the early steps of sporadic pancreatic cancer, but germline mutation carriers have a significantly higher risk of pancreatic carcinoma.

Fungal endophytic species inhabiting the leaves of eucalypts are capable of utilising leaf sugars and can influence both plant growth and health. Endophytic fungal symbionts can use simple soluble sugars in leaves as their main carbon source. This study set out to determine the diversity and distribution of the endophytic filamentous fungal leaf community in the hybrid Eucalyptus urograndis due to its economic importance. The fungal leaf community was characterised using denaturing gradient electrophoresis (DGGE) and correlated with levels of leaf nutrients and sugars throughout plant development. sequencing of DGGE bands revealed the presence of Basidiomycota and Ascomycota phyla. Fourteen species and three genera of filamentous fungi were identified, and the population structure was affected by the plant developmental stage. Levels of K, Cu, N and Mn influenced communities from the clonal garden, whereas leaves in the field had higher glucose, fructose and sucrose. Many fungi were found to be specific to a certain development stages: Diplomitoporus crustulinus, Podosphaera tridactyla and Aspergillus restrictus to the clonal garden stage;Chaetomella acutiseta and Ascotricha chartarum to the shading stage;Erratomyces patelii and saxomyces sp. to the shading output stage;Lepidostroma sp. and saxomyces sp. to the dispatch stage;and Mycosphaerella populicola to the field stage. Teratosphaeria toledana and Teratosphaeria acidotherma were found at more than one developmental stage. Cladosporium sp. and Rhodosporidium fluviale colonized and persisted in plants at the dispatch and field stages. This is the first report of P. tridactyla, A. restrictus, E. patelii, saxomyces and Lepidostroma sp. as endophytes in eucalipt.

Though they reduce microorganism growth, current hospital disinfectants also damage many of today’s modern electronic devices such as tablets and smartphones. Herein, the efficacy of a new chlorhexidine digluconate gel (CDG) was tested as a disinfectant for mobile and electronic devices in a clinical environment. specifically, Escherichia coli, Klebsiella pneumoniae, Pseudomonas aeruginosa and methicillin resistant staphylococcus aureus were used to infect the screen of eight smartphones. The CDG was prepared at concentrations of 2%, 4% and 6%, and tested on paper disks infected with these bacteria before being tested on the smartphones. The devices were disinfected with the CDG gel (4%) at two times: immediately and after 5 min of the bacterial contamination. In all cases, the CDG gel eliminated 100% of gram-positive and gram-negative microorganisms compared to the control (without any agent). In addition, the gel did not damage the smartphones. Therefore, our study suggests that the CDG gel may be applied to disinfect a wide range of electronic devices for health care professionals in the hospital environment.
