目的对北京市麻醉科住院医师规范化培训(简称住培)临床实践能力考核成绩与住院医师自评成绩进行比较,探讨住培过程中的问题。方法采用全样本调查方法,以2022年6月参加北京市住培临床实践能力结业考核的193名麻醉科住院医师为研究对象,以其操作考核视频和考核成绩为资料来源,并通过问卷调查获取考生对麻醉操作考核的自评成绩。采用t检验或配对t检验比较考生的自评成绩和考核成绩。结果考生的腰麻操作自评成绩与考核成绩差异无统计学意义[(28.39±1.32)分比(28.26±1.32)分,P=0.317];考生在动脉穿刺置管、气管插管、中心静脉置管、心肺复苏和电除颤等方面的自评成绩均高于考核成绩[(28.99±1.32)分比(27.95±1.33)分,(29.06±1.30)分比(28.28±1.29)分,(29.07±1.30)分比(28.37±1.35)分,(59.44±1.95)分比(57.78±1.42)分,均P<0.05]。其中,考生在操作规范、无菌操作、模拟器相关、非技能操作相关的自我评分均高于考核成绩[如:确定气管插管位置并固定:(1.93±0.19)分比(1.75±0.29)分;腰麻操作过程中的无菌原则:(1.98±0.13)分比(1.86±0.27)分;穿中血管后判断是否为中心静脉:(2.94±0.24)分比(2.66±0.70)分;在电除颤后清洁患者皮肤,清洁电极板并归位:(1.85±0.33)分比(0.98±0.94)分,均P<0.05]。结论麻醉科住院医师在动脉穿刺置管、气管插管、中心静脉置管、心肺复苏和电除颤等方面的自我认知存在偏差,在技能培训中,住院医师需加强临床操作规范、无菌原则的学习,强化其非操作技能的培养。

Method for calculating non-point source pollution distribution in plain rivers

The land area in a river network is divided into certain-scale square cells for the sake of precision, and, based on the physical mechanisms of rainfall-runoff processes and runoff pollution, the non-point source pollution from cells is estimated using the export coefficients of different land use types. The non-point source pollution from a land cell should all go into the closest fiver reach, so it is distributed according to the terrain of the plain river network area and the positions of land cells and river network reaches. A relationship between a single land cell and its pollution-receiving reach can be determined using this system. In view of the above, a spatial distribution model of the rainfall runoff and non-point source pollution in reaches of a plain river network area was established. This model can provide technological support for further research on the dynamic effects of non-point source pollution on water quality.

自2020年初夏,赤道太平洋地区出现拉尼娜现象并持续两年半多(以下简称2020拉尼娜),对其未来演变的预测引起了很多关注,考虑到11年太阳周期活动对热带太平洋SST异常可能存在锁相影响,本研究分析了当前太阳活动周(即第25太阳周(SC25))对目前热带太平洋ENSO现象未来演变的调节作用,基于历史太阳周的统计特征,作者对第25太阳周达到其最大值的时间提出三种可能的情景,并讨论了不同情景下的太阳活动对未来两年ENSO演变的可能影响,第25太阳周的持续上升阶段在一定程度上抑制了当前2023厄尔尼诺现象发展为超级事件.

Hydrolysis reactions are capable of directing the non-equilibrium assembly of biomolecular scaffolds to realize sophisticated structures and functions in natural ***,utilizing the proper hydrolysis reactions to construct controlled assemblies with complex topologies is still an arduous challenge in artificial systems and needs to be ***,we report a nitric oxide(NO)-triggered slow hydrolysis strategy for the controlled construction of biomimetic supramolecular toroids(STs),thus realizing their visualization of intermediate structures and regulation of geometry *** presented protocol harnesses hydrolysis reactions to control of non-equilibrium self-assembly processes for the construction of self-assemblies with complex topologies successfully,which sheds light on how the hydrolysis reaction rate can modulate the kinetic pathway of assembly,thus realizing the artificial establishment of bio-inspired hierarchical structures.

Background Sinus of Valsalva aneurysm （SVA） is a rare anomaly and few large or long-term series are well *** study was designed to review 33-year surgical experience of SVA in one *** From August 1980 to December 2013,patients with SVA underwent surgical repair were retrospectively *** A total of 160 patients were identified with mean age of （30±12） years and 112 （70％） of them were *** right coronary sinus origin of SVA was found in 108 patients （67.5％）,the non-coronary sinus in 51 patients （31.9％）,and the left coronary sinus in one patient （0.6％）.The rupture of 3VA into the right ventricle was identified in 89 （55.6％） cases,the right atrium in 61 （38.1％）,the left ventricle in 2 （1.3％） and no rupture in 8 （5.0％）.Ventricular septal defect （VSD） and aortic regurgitation （AR） were found in 59 （37％） and 45 （28％） patients *** approach via the involved chamber was used in 86 patients （54％）,aortotomy in 8 （5％）,and a combined approach in 66 （41％）.Either direct suture （56,35％） or patch （104,65％） closure were used to repair the *** VSD was closed with a patch （44/59,75％） or direct suture （15/59,25％）.aortic valve replacement （AVR） was performed in 23/45 （51％） and aortic valvuloplasty （AVP） in 9/45 （20％） patients combined with *** were 3 hospital deaths （1.9％） and 2 late deaths and 84％ of the patients were followed up for （17.6±4.2） *** York Heart Association functional class improved significantly after surgery （P＜0.01）.Actuarial survival was 94％ at 10 years,and 88％ at 20 *** Surgical treatment of SVA is safe and effective,ruptured SVA or unruptured SVA with VSD and/or AR should be repaired surgically as early as ***,late progressive AR is still a risk during long-term follow-up,and early aggressive measures are *** include more use of a combined approach to achieve optimal evaluation of lesions,more patch repairs to reduce the chance of recurrence,and more AVR in pati

Tight Carbonate Microstructure and its Controls: A Case Study of Lower Jurassic Da'anzhai Member, Central Sichuan Basin

Cast thin section observation, scanning electron microscopy(SEM), high-pressure mercury injection(HPMI), and nuclear magnetic resonance(NMR) were used to examine the microstructure of tight carbonate reservoirs in the Lower Jurassic Da’anzhai Member, the central Sichuan Basin. The pore space in the Da’anzhai Member is classified into 2 types and 17 subtypes, with nano-scale pore throats of ‘O’, ‘S’, ‘Z’, and ‘I’ shapes. Poorly sorted pore throats vary greatly in diameter;thus, it is difficult for fluid flow to pass through these pore throats. There are three classes of pore throats in carbonate reservoirs, i.e. isolated pores, pores coexisting with fractures, and large pores and fractures. Isolated pores may provide some pore space, but the permeability is low. Pores and fractures coexisting in the reservoir may have a great impact on porosity and permeability;they are the major pore space in the reservoir. Large pores and fractures have a great impact on reservoir properties, but they only account for a limited proportion of total pore space. The microstructure of Da’anzhai reservoirs, which dominates fluid mobility, is dependent on sedimentary environment, diagenesis, and tectonic process. Pore structure is related to sedimentary environment. The occurrence of microfractures, which may improve reservoir properties, is dependent on tectonic process. Diageneses are of utmost importance to pore evolution, cementation and growth of minerals have played an important role in destroying reservoir microstructure.

Synthesis and Optical Properties of Hyperbranched PAEKs Containing Porphyrin and Its Metal Derivatives

Novel functional hyperbranched poly（aryl ether ketone）s （HPAEKs） bonded with nonlinear optical chromophores （meso-tetrakis（4-hydroxyphenyl） porphyrin, THPP and its metal derivatives） were synthesized and characterized by IH-NMR and UV-Vis absorption spectra. The incorporation of chromophores into HPAEKs endowed HPAEKs novel NLO and OL properties. Indeed, dendritic architecture allowed for maximum dispersion of the chromophores, avoided aggregation, more optical limiting property was obtained. Simultaneously, they retained the excellent properties of the materials, particularly in thermal stability. Their optical properties were evaluated by nonlinear optical analyses and optical limiting. The results showed that these polymers possessed good optical limiting （OL） property and large nonlinear optical （NLO） susceptibilities （ca. 10-12 esu）. All polymers containing chromophores presented excellent thermal stability （DT5 〉 524.17 ℃）.
