Liver transplantation in patients with incidental hepatocellular carcinoma/cholangiocarcinoma and intrahepatic cholangiocarcinoma： a single-center experience

BACKGROUnD： Reports of fiver transplantation （LT） in patients with mixed hepatocellular carcinoma/cholangiocarcinoma （HCC/CC） and intrahepatic cholangiocarcinoma （ICC） are modest and have been mostly retrospective after pathological categorization in the setting of presumed HCC. Some studies suggest that patients undergoing LT with small and unifocal ICC or mixed HCC/CC can achieve about 40%-60% 5-year post-transplant survival. The study aimed to report our experience in patients undergoing LT with explant pathology revealing HCC/CC and ICC. METHODS： From a prospectively maintained database, we performed cohort analysis. We identified 13 patients who underwent LT with explant pathology revealing HCC/CC or ICC. RESULTS： The observed recurrence rate post-LT was 31% （4/13） and overall survival was 85%, 51%, and 51% at 1, 3 and 5 years, respectively. Disease-free survival was 68%, 51%, and 41% at 1, 3 and 5 years, respectively. In our cohort, four patients would have qualified for exception points based on updated HCC Organ Procurement and Transplantation network imaging guidelines. COnCLUSIOnS： Lesions which lack complete imaging characteristics of HCC may warrant pre-LT biopsy to fully elucidate their pathology. Identified patients with early HCC/CC or ICC may benefit from LT if unresectable. Additionally, incorporating adjunctive perioperative therapies such as in the case of patients undergoing LT with hilar cholangiocarcinoma may improve outcomes but this warrants further investigation.

Split liver transplantation(SLT),while widely accepted in pediatrics,remains underutilized in adults. Advancements in surgical techniques and donor-recipient matching,however,have allowed expansion of SLT from utilization of the right trisegment graft to now include use of the hemiliver graft as well. Despite less favorable outcomes in the early experience,better outcomes have been reported by experienced centers and have further validated the feasibility of SLT. Importantly,more than two decades of experience have identified key requirements for successful SLT in adults. When these requirements are met,SLT can achieve outcomes equivalent to those achieved with other types of liver transplantation for adults. However,substantial challenges,such as surgical techniques,logistics,and ethics,persist as ongoing barriers to further expansion of this highly complex procedure. This review outlines the current state of SLT in adults,focusing on donor and recipient selection based on physiology,surgical techniques,surgical outcomes,and ethical issues.

Conventional mining of economically and strategically important critical rare earth elements(REEs)(such as neodymium, lanthanum and dysprosium), and chalcogens(such as selenium and tellurium) are associated with a huge economic and environmental cost. Therefore,the need to recover REEs as well as chalcogens from different waste streams including wastewaters is becoming urgent. Batch assays on synthetic chalcogen/REE-laden wastewater showed that the presence of REEs significantly improved the tellurite removal rate(> 80%) and enhanced selenate removal by 66% ± 10%. Three 3.9 L continuous upflow anaerobic granular sludge bed(UASB) reactors were operated at a hydraulic retention time of 24 h and30 °C. Selenate reduction was achieved with a removal efficiency of ～ 98% with an influent p H of 4.0 for more than 28 days. The effect of REEs on tellurite removal in the UASB bioreactor could not be clearly established since a soluble tellurium removal efficiency of more than 98%was observed already before the addition of REEs at elevated tellurite concentrations. The complete REE removal in both batch assays and UASB reactors at higher pH (7.0 ± 0.5) was attributed to precipitation, whereas chalcogen oxyanions removal was due to microbial reduction. However, at acidic p H, biosorption was responsible for REE's removal, and the Se-enriched sludge exhibited a superior REE's removal efficiency than the non-enriched and Te-enriched sludge.

We present a method to fabricate handcrafted thermoelectric devices on standard office paper *** devices are based on thin films of WS_(2),Te,and BP(P-type semiconductors)and TiS_(3)and TiS_(2)(n-type semiconductors),deposited by simply rubbing powder of these materials against *** thermoelectric properties of these semiconducting films revealed maximum Seebeck coefficients of(+1.32±0.27)mV K^(-1)and(-0.82±0.15)mV K^(-1)for WS_(2)and TiS_(3),***,Peltier elements were fabricated by interconnecting the P-and n-type films with graphite electrodes.A thermopower value up to 6.11 mV K^(-1)was obtained when the Peltier element were constructed with three *** findings of this work show proof-of-concept devices to illustrate the potential application of semiconducting van der Waals materials in future thermoelectric power generation as well as temperature sensing for low-cost disposable electronic devices.
