Induction of oxidative stress in human aqueous and vitreous humors by Nd:YAG laser posterior capsulotomy

AIM： To evaluate whether the Q-switched Nd：YAG laser treatment applied in routine capsulotomy elicits oxidative stress in aqueous and vitreous humors. METHODS： Thirty-six patients who had to undergo a 25 gauge pars plana vitrectomy due to vitreoretinal disorders were enrolled, 15 of them underwent a Q-switched Nd：YAG laser capsulotomy 7 d before vitrectomy due to posterior capsule opacification（PCO）（Nd：YAG laser group） while the remaining 21 patients were not laser treated before vitrectomy（no Nd：YAG laser group）. Samples of the aqueous and vitreous humors were collected during vitrectomy from all patients for the assessment of oxidative parameters which were compared between the Nd：YAG laser group and no Nd：YAG laser group. Thiobarbituric acid reactive substances（TBARS）, a product of membrane lipid peroxidation, nitrite levels, the antioxidative activities of SOD and catalase, the 4-HNE-protein conjugate formation, indicating structural modifications in proteins due to lipoperoxidation, were assessed in aqueous and vitreous samples. RESULTS： In the human vitreous humor TBARS levels are significantly higher in the Nd：YAG laser group compared to the no Nd：YAG laser group and importantly, there is a significant correlation between the TBARS levels and the total energy of Nd：YAG laser used during *** the anti-oxidative activities of SOD and catalase were significantly decreased by Nd：YAG laser treatment, both in aqueous and vitreous humors. In accordance with the TBARS data and anti-oxidative enzyme activities, significantly higher levels of proteins were conjugated with the lipoperoxidation product 4-HNE in the aqueous and vitreous humors in the Nd：YAG laser-treated group in comparison to no Nd：YAG laser group. CONCLUSION： These data, clearly suggest that any change that Q-switched Nd：YAG photo disruption may cause in the aqueous and vitreous compartments, resulting in a higher level of oxidative damage might be of considerable clinical significance

Pediatric Menetrier's disease (MD) is an uncommon, acute, self-limited hypertrophic gastropathy characterized by enlarged gastric folds associated with epithelial hyperplasia and usually accompanied by protein losing gastropathy. Gastric cytomegalovirus infection is found in one third of MD children and its treatment is often associated with remission. Diagnosis often requires fullthickness biopsy due to inability to detect typical histological findings with conventional endoscopic biopsy. We report an uncommon case of non self-limited pediatric MD needing endoscopic mucosal resection for diagnosis which was then successfully treated with octreotide long-acting release (LAR). To the best of our knowledge, this is the first pediatric MD case successfully treated with octreotide LAR. Our experience suggests octreotide LAR as treatment for refractory MD before gastrectomy.

BACKGROUND Hepatocellular adenomas are rare tumors that can occur in patients with glycogen storage disease type *** SUMMARY We herein report two cases of histologically proven hepatocellular adenomas in patients with glycogen storage disease type *** resonance imaging(MRI)was performed after bolus injection of gadoxetate disodium,a liver-specific gadolinium-based MRI contrast *** the present cases,some of the hepatocellular adenomas showed unexpectedly a“bull’s eye”appearance on T2-weighted and post-contrast images,which was not previously described as imaging findings of hepatocellular adenomas in glycogen storage disease.A bull’s eye appearance on T2-weighted images can be encountered in both benign(i.e.,abscess)or malignant(i.e.,epithelioid hemangioendothelioma,cholangiocarcinoma,and metastases)hepatic *** We present two cases of hepatocellular adenomas in patients with glycogen storage disease type 1,in which gadoxetate disodium-MRI showed atypical imaging findings for hepatocellular *** present there is no systematic study evaluating MRI findings of hepatocellular adenomas in patients with glycogen storage disease,further studies are needed to specifically investigate this issue.

The Durkan Complex is a tectonic element of the Makran Accretionary Prism(SE Iran)that includes fragments of Late Cretaceous *** this paper,the results of map-to micro-scale structural studies of the western Durkan Complex are presented with the aim to describe its structural and tectonometamorphic *** Durkan Complex consists of several tectonic units bordered by mainly NNW-striking *** main deformation phases(D_(1),D_(2),and D_(3))are distinguished and likely occurred from the Late Cretaceous to the Miocene–Pliocene.D_(1) is characterized by sub-isoclinal to close and W-verging folds associated with an axial plane foliation and shear zone along the fold *** phase records the accretion of fragments of the seamount within the Makran at blueschist facies metamorphic conditions(160–300℃ and 0.6–1.2 GPa).D_(2) is characterized by open to close folds with subhorizontal axial plane that likely developed during the exhumation of previously accreted seamount *** upper Paleocene–Eocene siliciclastic succession unconformably sealed the D_(1) and D_(2) structures and is,in turn,deformed by W-verging thrust faults typical of D_(3).The latter likely testifies for a Miocene–Pliocene tectonic reworking of the accreted seamount fragments with the activation of out of sequence *** results shed light on the mechanism of accretion of seamount materials in the accretionary prisms,suggesting that seamount slope successions favour the localization and propagation of the basal dé*** study further confirms that the physiography of the subducting plates plays a significant role in the tectonic evolution of the subduction complexes.

Pneumatosis cystoides intestinalis(PCI) is a rare condition that may be associated with a variety of *** presenting clinical picture may be very heterogeneous and represent a challenge for the *** the present paper we describe both a common and an uncommon clinical presentation of PCI and review the pertaining *** cases confirm that,apart from asymptomatic cases,the clinical presentation of PCI may be widely different and suggest that a new onset of stipsis might be the presenting *** might be suggested by a simple X-ray of the digestive tract showing a change in the characteristics of the intestinal wall in two-thirds of these ***,one third of the patients do not have a suggestive X-ray and require a computed tomography(CT) scan/nuclear magnetic resonance that may reveal a thickened bowel wall containing gas to confirm the diagnosis and distinguish PCI from intraluminal air or submucosal *** also allows the detection of additional findings that may suggest an underlying,potentially worrisome cause of PCI such as bowel wall thickening,altered contrast mucosal enhancement,dilated bowel,soft tissue stranding,ascites and the presence of portal *** results also point out that clinicians and endoscopists should be aware of the possible presentations of PCI in order to correctly manage the patients affected with this disease and avoid unnecessary *** increasing number of colonoscopies performed for colon cancer screening makes PCI more frequently casually encountered and/or provoked,therefore the possible endoscopic appearances of this disease should be well known by endoscopists.

Anal stenosis is a rare but serious complication of anorectal surgery, most commonly seen after hemorrhoidectomy. Anal stenosis represents a technical challenge in terms of surgical management. A Medline search of studies relevant to the management of anal stenosis was undertaken. The etiology, pathophysiology and classification of anal stenosis were reviewed. An overview of surgical and non-surgical therapeutic options was developed. Ninety percent of anal stenosis is caused by overzealous hemorrhoidectomy. Treatment, both medical and surgical, should be modulated based on stenosis severity. Mild stenosis can be managed conservatively with stool softeners or fiber supplements. Sphincterotomy may be quite adequate for a patient with a mild degree of narrowing. For more severe stenosis, a formal anoplasty should be performed to treat the loss of anal canal tissue. Anal stenosis may be anatomic or functional. Anal stricture is most often a preventable complication. Many techniques have been used for the treatment of anal stenosis with variable healing rates. It is extremely difficult to interpret the results of the various anoplastic procedures described in the literature as prospective trials have not been performed. However, almost any approach will at least improve patient symptoms.

The burgeoning field of cancer immunology demands a change in the paradigm of cancer patient management. The understanding of the course of a given malignant disease should also include the host immune system as one of the key factors in determining the patient's prognosis. Surgical and medical oncologists need to understand the basic and advanced applications of immunotherapies, which are rapidly evolving, and are nowadays an integral part of the armamentarium for the treatment of cancer patients. In the present work, we review the current knowledge concerning the immune landscape of colorectal cancer (CRC) patients with liver metastases, as recently discovered.

This paper is intended to give a contribution to the debate on the functional relationship between the museum and the local territory; more specifically, the aim of this study is to understand the role of a museum location in attracting visitors and influencing their processes of choice and satisfaction. The study follows, in particular, a previous study conducted by Mariani and Mussini （2013） based on Fondazione di Venezia data, with a view to better contextualization level. Previous studies have examined the role of museums in enhancing the image of a destination and in attracting and conveying additional tourists＇ fluxes, arguing that although not all museums are a destination of choice for visitors, a great museum or a strong exhibition program still does represent a national and international attraction. This paper overturns this argument investigating whereas an appealing location （such as a tourist destination） can influence prospective visitors about the level of attractiveness of a pay cultural event taking place therein. A questionnaire was developed and data were collected at the Palazzo Ducale in Venice, Italy, in 2007, during the art exhibition ＂Venezia e l＇Islam 829-1797＂, via 501 face-to-face visitors interviews. The research model analyzes data through the conjoint analysis and the descriptive statistics. Results show that the exhibition location has been a relevant factor in influencing the visitors＇ decision process linked to the attendance of the event. Data show that, for the majority of visitors, the exhibition had been located elsewhere, they would not have visited it. Thus, an appealing location or a structured tourist destination may function as an attractive platform that may contribute to enforce the overall visitor＇s satisfaction with regard to his or her attendance to a cultural event. Furthermore, such a fact depends on specific objective variables （e.g., level of education, age）, although no significant differences were reported between
