Determinants of the Use of Health Care and Therapeutic Routes of Populations in Rural Senegalese Areas, 2016

In Senegal, access to health care for rural populations and therapeutic choices remain problematic. These problems may be synonymous with insufficient financial resources, long waits, ineffective treatments, or drug shortage in health structures, leading patients to perform various therapeutic behaviours. The aim of this present study was to identify the therapeutic routes of the patients and the determinants of the different uses of health care in the populations of Ferlo. This descriptive and analytical study was carried out as part of the medical consultation days of the 2016 summer at the university in Widou Thiengoly. All patients who met the selection criteria were enrolled. In total, the 150 participants were mainly animal breeders (63.3%) and housewives (26%), also, most were women (90.7%). The analysis of the therapeutic routes showed that the health post was used as first-line and second-line respectively in 85.3% and 59.3% of the study populations. Traditional medicine and self-medication would be used in the third intention. Therapeutic choices were independent of age (p = 0.94), marital status (p = 0.84) and occupation (p = 0.58). On the other hand, social dimensions, such as tradition, appreciation of efficiency, distance, cost, and gender, also determine the use of health care. Women used health post more frequently than men (p = 0.03) (OR: 3.89, CI95% [1.16 - 12.98]) of the order 3.89. The main reasons for self-medication practice were the treatment of headache or fever (46%) and that of fatigue (26%). The prices were more expensive for health facilities (70.9%) than for traditional healers (70% “not expensive at all”) where the waiting time was considered “not long at all” (70%).

Frailty in People Living with Human Immunodeficiency Virus Aged 50 Years and Older: Prevalence and Predictors

Introduction: Life expectancy improvement for people living with Human Immunodeficiency Virus (HIV) is coming up against the problems associated with aging and chronic diseases. Frailty is a concern affecting a growing number of patients, particularly the elderly in this population. Our study aimed to determine the prevalence of frailty and its predictors on people living with HIV aged 50 years and older followed at the Outpatient Treatment Clinic (CTA) in Dakar. Methodology: We conducted a cross-sectional study of descriptive and analytic purposes ranging from November 2022 to August 2023, in CTA, Dakar (Senegal). We included people living with HIV aged 50 years and older under antiretroviral therapy for at least 6 months (≥6 months). Frailty was considered according to Fried criteria with a score ≥3. To identify the predictors of frailty, we performed a multivariate logistic regression analysis using STATA software version 18. Results: We included 199 patients. The median age at the moment of the study was 58 years old [50 - 91] with a sex ratio (M/F) of 0.58. The most representative age group was that of [50 - 59] years (59.3%). HIV-1 profile was most common in 89.45%. The median duration under antiretroviral therapy was 180 months [6 - 284] and 94% of patients received a Tenofovir Disoproxil Fumarate (TDF)-containing regimen with 43% of them for at least 10 years. Viral load was undetectable (≤40 copies/ml) in 98% of cases. WHO Stage III was more common at inclusion and 55.78% had nadir TCD4+ Lymphocyte counts Conclusion: Our study confirms the high prevalence of frailty among older people living with HIV. Its prevention should consider the management of comorbidities and the implementation of non-pharmacological interventions such as nutrition.

Introduction: Adrenal tumours (AT) are commonly encountered in clinical practice. For any patient presenting an adrenal mass, there are two crucial points to consider: is the adrenal mass malignant and is it hormonally active? The objective of our study was to evaluate the epidemiological, diagnostic, therapeutic, and evolutionary aspects of AT in a developing country. Methodology: We conducted a retrospective study spanning 17 years (from January 2005 to October 2022) in four Dakar hospital services. We included all patient medical records with explored AT. Results: AT was diagnosed in 35 patients who had a mean age of 36.62 years (range: 12-79), and a female predominance (26 women to 9 men). Among these patients, 17.2 % had incidental AT. Hypertension was the commonest presenting symptom in 27 patients (77%), which was associated with Mnards triad in 14 patients (40%). Abdominal pain was reported by 20 patients (57%), and 14 presented with an abdominal mass. Among the patients, 29 had functional AT: 22 with pheochromocytoma, 5 with primary hyperaldosteronism, and 2 with hypercortisolism. Imaging evaluations revealed that 33 patients had unilateral AT and one had bilateral AT. Tumour sizes varied, 5 patients had tumours 10 cm. Two patients presented with metastases. The therapeutic approach involved adrenalectomy in 32 patients. Perioperative complications were observed in five patients, including haemorrhage (two patients), hypotension (two cases), and hypertensive crisis (one case). One patient with bilateral pheochromocytoma developed acute adrenal insufficiency postoperatively, followed by abdominal herniation. Histopathological examination confirmed pheochromocytoma in 21 patients, Conns adenoma in 5, adrenocortical carcinoma in 3, cortisol adenoma in 2, and adrenal cyst in 1 patient. Introduction: Quality of life in Parkinson disease is not necessary linked to motor symptoms. It’s correlated of overactive bladders and prostatic symptoms. Prognosis factors of urinary quality of life are unknown. Objectives: Our study aims to find prognosis factors of quality of life associated specifically of urinary disorders in PD. Patients and methods: We conducted a transversal, analytic and descriptive study in Physical Medicine, and Neurology departments, Fann Teaching Hospital, Dakar and included patients followed for treatment of PD. Urinary disorders, quality of life and functional autonomy were assessed respectively by Urinary symptoms Profile (USP), Qualiven Short Form, Schwab and England Scale. Results: 38 patients presented PD, with a mean age of 60.89 ± 13.6 years and sex-ratio of 2.45. Mean duration of PD was 3.1 ± 2.9 years. Urinary disorders were found in 47.36% and dominated by incontinence (88.88%) and overactive bladders (88.88%) which were minor in more than 55%. Quality of life was impaired in 88.88% of cases. Minor forms (43.75%) were predominant and constraint dimension (50%) was the most severely altered. Prognosis factors for quality of life of urinary disorders were PD stages (0.046) and functional autonomy (0.042). Discussion and conclusion: Urinary disorders in PD are common, impaired quality of life, especially the constraint dimension, depends on stage of evolution of PD and functional autonomy of patients.

Study on the Perceptions and Social Representations of Husbands on Family Planning in the Health District of Pout/Senegal

Contraception is a set of procedures that aim to achieve temporary infertility in a woman without her ability to conceive being engaged in the future. It plays an essential role in the health of women and children, the psychological well-being of husbands and allows, among other things, savings for the family. The objective was to study the perceptions and social representations of husbands on family planning. A mixed method was used with a qualitative dominance, carried out in the health district of Pout. The result showed that the perceptions and representations of husbands influence this practice family planning. Even if they have positive attitudes and a good level of knowledge about contraceptive methods, their involvement in FP practices remains low with gaps to be filled. Direct or intermediate variables such as the social, cultural, economic and religious aspects are pitfalls. Indeed, the commitment of men and the consideration of their social attributes are variables to be integrated into the process of information and awareness on family planning;this is crucial in achieving Sustainable Development Goal (SDG) 3 (Good Health and Well-being), including its target 7 focused on sexual and proactive health: a second best move towards the demographic dividend.

Eosinophilic Gastroenteritis in Tropical Area: about 6 Cases from Senegal

The aetiologies of hypereosinophilia are dominated by digestive parasitosis and drug intake. In the tropics, because of the frequency of parasitosis, eosinophilic digestive pathologies of primary origin, which are rare, may be overlooked. We report 6 cases of eosinophilic gastroenteritis with polymorphic digestive manifestations testifying to the different locations of eosinophilic infiltration in the digestive parietal layer. Three patients had ascites rich in eosinophils, indicative of serous involvement, while the other two had a muscular form, and the last a mucosal involvement. The evolution was favourable in all cases with corticosteroid therapy, but a recurrence was noted in 4 cases when treatment was stopped.

Assessment of Radiofrequency in Otorhinolaryngology at the Military Hospital of Ouakam (MHO)

Introduction: Coblation® is a low temperature-controlled tissue ablation technique widely used in human surgery. Its development in otorhinolaryngology has allowed a spectacular advance in the management of diseases of the nasal and oral cavities. The objective of our work was to study the indications for radiofrequency in ENT, to describe the technical aspects and to evaluate the results of his practice at the HMO. Patients and Methods: This is a retrospective study carried out in the ENT and Head and Neck Surgery department of the Ouakam Military Hospital (MHO) over a period of 3 years (from January 2015 to the end of December 2017), involving 37 patients who received radiofrequency treatment in the ENT sphere. Results: The study was carried out on 37 patients. The sex ratio of 0.19. The mean age of the patients was 33.3 years. The indications were dominated by chronic caseous cryptic tonsillitis with or without halitosis with 24 cases (64.9%) followed by recurrent tonsillitis in 5 cases (13.5%) then we have obstructive hypertrophy of the palatal tonsils with syndrome sleep apnea (OSAS) in 4 cases (10.8%) followed by nasal obstruction (hypertrophy of the inferior turbinates) and hypertrophy of the soft palate and uvula in 2 cases (5.4%). The complication rate was 8.1% and dominated by transient dysgeusia in 1 case or 2.7%, uvula edema in 1 case or 2.7% and bleeding in 1 case or 2.7%. The postoperative follow-up was done one week, 1 month, 3 months, 6 months and 12 months after radiofrequency. Volumetric reduction begins to set in from 1 month (62.5%) and becomes maximum in 3 months (74.3%). Conclusion: Coblation® is a controlled tissue ablation technique at low temperature, patented by ArthroCare® Corporation in 1997. Very widespread in human surgery. Background: In disorders of sexual differentiation, sexual development may not conform to the chromosomal structure, thus forming different types of abnormalities. Among these abnormalities is syndrome 46, XX DSD where most patients are female phenotype with clitoral hypertrophy that can go to complete masculinization especially in the presence of the SRY gene. Objective: The goal of this work is to demonstrate a relationship between the genotype and the phenotype in five patients karyotype 46, XX with the presence of the SRY gene. Methodology: The study involves five patients referred to the laboratory under suspicion of sexual development anomalies. The diagnosis took place through hormonal and echography examinations, a classic cytogenetic study (Barr chromatin and karyotype) and an amplification of the SRY gene located on the Y chromosome. The resulting PCR products were sent for sequencing. Results: Based on the results of clinical and paraclinical tests carried out it was found clitoral hypertrophy, the presence of clitoris penis for some, presence of normal penis for others. In addition, echography revealed a lack of female internal genitalia (P2, P3), and a presence of testicles (P3, P4, P5). Genetic analysis (chromosomal and molecular) showed a karyotype 46, XX SRY (+) for all patients. New mutations were found c.246 T > A, p.82 Asn82Lys and c.171 G > C, p.57 Gln57His. Conclusion: In our study, we were able to correlate each DSD with karyotype 46, XX to a pathology such as 46, XX DSD testicular, 46, XX DSD with clitoral hypertrophy and ovotestis 46, XX. The next step will undoubtedly be the integration of other molecular techniques (genotyping, FISH, CGH or even the CGH array) to further genetic exploration.

Associated Factors with the Child’s Fully Immunized in the Tambacounda Health District (Senegal)

To improve vaccination coverage among children under one year of age in the Tambacounda health district, a household survey was carried out among mothers or babysitters. The objective was to study the factors related to child’s fully immunized in children aged 12 to 23 months. The cross-sectional, descriptive and analytical survey was carried out during the month of April 2019. A multistage cluster survey selected a sample of 657 mothers and babysitters. The data was collected using a questionnaire made from the World Health Organization reference guide. Data entry and analysis were done with Epi Info software and R. Among the women surveyed, biological mothers were the most representative (96.9%). In the series, 74.1% had a good knowledge of the age for initiating vaccination, 78.2% knew the number of contacts. The vaccination record of the children was available in 92.2%, and 71.0% of them had presented an adverse event. The proportion of children fully immunized was 41.0%. Complete childhood vaccination was positively associated with income-generating activity in women (0R = 2.4) and the short distance (<100 m) between home and place of vaccination (OR = 1.5). It was also improved by having a qualified health worker as a vaccinator (OR = 1.4) satisfaction in relation to visit (OR = 2.0), the advice given by the vaccinator (OR = 1.7) and the fixing of the date of the next vaccination appointment (OR = 2.5). The implementation of a good strategy for improving the quality of immunization services is an important element for strengthening immunization coverage in the Tambacounda health district.
