Low Socio-Economic Level: A Major Obstacle to Therapeutic Observation (Survey of 125 Diabetic Patients)

Introduction: Diabetic patients adhering to their treatment are less likely to face complication from their disease. For this reason, we studied the compliance of diabetics with their treatment. Materials and Method: This was a comprehensive cross-sectional study of adherence in diabetic patients. All patients meeting the inclusion criteria were enrolled in the study. This study took place at a private pharmacy called “pharmacy Elhadj Seydou Nourou Tall” located in the city of Pikine during the period from April 1 to September 30, 2019. Results: In total, the study population was composed of 125 diabetics;frequency of 8.47%. The average age was 62.5 years and there were more women (58.4%). Of the patients, 87% had a low socioeconomic level and 72% a low educational level. Only 65.6% of the patients knew their diabetes type. Even though 97.6% of the patients were confident in the treatment but due to financial reasons 20.8% did not adhere to the prescribed treatment and 16.8% to their diets. Moreover, 15.2% of the patients had changed the dosage of the prescribed medication, 44.4% had forgotten to take their medication regularly. 78.4% of them thought they were taking too much medication. No factors were found to be statistically associated with poor adherence, but the proportion of poor adherence patients was higher in patients over 60 years of age, with low socio-economic status and low education. Conclusion: Low socio-economic status is the main barrier to good adherence. Recommendations have been formulated to improve this adherence.

Minimum Penalized Hellinger Distance for Model Selection in Small Samples

In statistical modeling area, the Akaike information criterion AIC, is a widely known and extensively used tool for model choice. The φ-divergence test statistic is a recently developed tool for statistical model selection. The popularity of the divergence criterion is however tempered by their known lack of robustness in small sample. In this paper the penalized minimum Hellinger distance type statistics are considered and some properties are established. The limit laws of the estimates and test statistics are given under both the null and the alternative hypotheses, and approximations of the power functions are deduced. A model selection criterion relative to these divergence measures are developed for parametric inference. Our interest is in the problem to testing for choosing between two models using some informational type statistics, when independent sample are drawn from a discrete population. Here, we discuss the asymptotic properties and the performance of new procedure tests and investigate their small sample behavior.

Indications and Results of Repair of the Rhumatismal Valve in Children in Senegal concerning 63 Cases

In Africa, acute rheumatic fever is endemic. Cardiac involvement is one of the most common complications in the form of valvular heart disease representing all damages to the heart valves. It is in this perspective that we realized this study whose general objective was to evaluate the results of mitral repair surgery in children in Senegal and the specific objective was to state the indications for mitral repair surgery and assess the results in terms of morbidity and mortality. This is a retrospective and analytical monocentric study, in the thoracic and cardiovascular surgery department of FANN National University Hospital Center in Dakar. It took place over a period of 30 months. All the patients who underwent mitral surgery, aged less than 18 years were included. The total number of patients was 63, including 39 girls and 24 boys, a sex ratio of 0.62. The average age at the time of the surgery was 12 years old [5 - 17]. The functional symptomatology was dominated by the dyspnea found in all the patients. Cardiac ultrasound was diagnosed with mitral regurgitation in all patients. For all surgical procedures, the approach was a vertical midline sternotomy. The mitral valve was approached by left atriotomy in 40 patients (63.5%) and by transseptal way in 23 patients. All patients had undergone mitral valve repair associated or not with either aortic valve repair in 9 patients (14.2%), aortic valve replacement in 3 patients (4.8%), or one tricuspid valve repair in 31 patients (49%). The average duration of intubation was 6 hours [2 - 52]. The average length of stay in intensive care was 2 days. Postoperative complications have been observed. Surgery was performed in 10 patients with 3 mitral valve replacements, 2 aortic valve replacements, a double mitral and an aortic valve replacement associated with a tricuspid repair and in 4 cases a perfection of their mitral repair. Early and late surgical mortality was zero. The average follow-up time for our patients was 9

Sclerotherapy for Varicose Veins of the Lower Limbs

Summary: The diagnosis of venous insufficiency is increasing. Multiple techniques are available for its treatment. Chemical sclerotherapy has been performed for over 70 years. The technique improves lately. Objective: This study was conducted to assess early results of foam sclerotherapy in Senegal. Patients and Methods: This is a prospective and descriptive study conducted from January 1st, 2012 to December 31st, 2015 at cardiovascular center of Fann University hospital in Dakar, Senegal. Foam sclerotherapy was done in 76 patients. Polidocanol was used in concentration of 2%. Sclerotherapy was done several times in patients, every week, as much as needed to occlude varicose veins. We used venous compression in all patients. Results: Foam sclerotherapy sessions permitted to get globally good results. In fact, tight feeling and painful leg disappear in 97% of patients. Decreases of edema were gotten in 97% of patients. For leg ulcer, 64% of complete healing was obtained. The main ulcer healing time was 47 days (18;72). Some minor complications occurred. No major events, such as deep venous thrombosis or pulmonary embolism, were observed. Conclusion: Foam sclerotherapy is an efficient technique for the treatment of varicose veins of the limbs. Also, it opens good perspectives for vein care.

Management and Maternal-Fetal Prognosis of Placental Abruption: A Retrospective Study of 130 Cases

Objective: To describe the epidemiological, clinical and prognostic factors and assess treatment of placental abruption in the obstetrics gynecology department of the Dakar Principal Hospital. Patients and Method: We carried out a retrospective observational study of 130 successive cases of placental abruption, which occurred from January 2015 to December 2017 at the Level 3 Maternity Unit of the Dakar Principal Hospital. Data were collected from non-computerized obstetric records and analyzed using Excel and Epi info software. Results: There were 130 cases of placental abruption, that is a prevalence of 1.5%. The average age of onset of placental abruption was 30 years. The history of hypertension concerned 32.3% of patients, the average gestational age of 32.5 weeks at the time of diagnosis, grade 3 of Sher was found in 48.5% of cases. The outcome of the pregnancy was a caesarean section in 79.2% of cases, the average weight of newborns was 2058 g. The management of the complications required a blood transfusion and intensive care. A haemostasis hysterectomy was performed in 6.2% of cases. Stillbirth rate was 53.7% and maternal mortality was zero. Discussion and Conclusion: Placental abruption, a severe complication of pregnancy, is associated with high perinatal morbidity and mortality linked to the severity of the clinical picture, despite an improved maternal prognosis.

Dirichlet-to-Neumann Map for a Hyperbolic Equation

In this paper, we provide an explicit expression for the full Dirichlet-to-Neumann map corresponding to a radial potential for a hyperbolic differential equation in 3-dimensional. We show that the Dirichlet-Neumann operators corresponding to a potential radial have the same properties for hyperbolic differential equations as for elliptic differential equations. We numerically implement the coefficients of the explicit formulas. Moreover, a Lipschitz type stability is established near the edge of the domain by an estimation constant. That is necessary for the reconstruction of the potential from Dirichlet-to-Neumann map in the inverse problem for a hyperbolic differential equation.

Domestic Accidents in Under-Fives: Epidemiology, Diagnosis, Management, and Outcomes

Introduction: Domestic accidents are a public health problem, and under-fives are particularly affected. We report these accidents’ epidemiology, diagnosis, management, and outcomes through this study. Patients and Methods: We conducted a prospective descriptive study for four months in the Albert Royer National Children’s Hospital Center pediatric surgery department in Dakar, Senegal. Of the 149 cases of domestic accidents, 109 were included in this study. Results: Infants (59.6%) were the most affected, with a slight male predominance (50.5%). Accidents occurred on Tuesday in 22.9% of cases and the afternoon in 38.5% of cases. Playing (59.6%) was the main activity, and falling (44%) was the most found mechanism. 78.9% of accidents occurred in the house, with the bedroom (27.5%) being the most represented place. In most cases (67.9%), the consultation was done within the first 24 hours. The upper limbs (48.6%) were the most affected, and fractures (31.2%) were the most frequent injuries. The outcomes were unremarkable in 94.5% of cases, as complications occurred in five patients with three who had an infection, one a persistent limping, and the last, a dental avulsion. No mortality was recorded. Conclusion: Domestic accidents among under-fives are frequent in our environment. Compared to those in older children, they occur similarly in both sexes, more often in the bedroom, and have a better prognosis.

New Approach of Structural Setting of Gold Deposits in Birimian Volcanic Belt in West African Craton: The Example of the Sabodala Gold Deposit, SE Senegal

The Sabodala gold deposit in the Mako volcanic Belt (SE-Senegal, West African Craton) occurs in mafic and ultramafic rocks overlain by clastic sedimentary rocks and felsic volcanic-volcaniclastic sequences. It is a structurally controlled mesothermal vein gold deposit developed in deformed rocks units near the Falombo pluton. The Sabodala Goldfield comprises several faults systems striking N-S to NE-SW. New structural evolution consisting of both thrusting and transcurrent events has been defined for the Sabodala Goldfield. The majority of the gold at Sabodala occurs on the western flank of a large, doubling plunging dome (the Sabodala Antiform) and is also associated with sinistral wrenching that overprints early structures within the Sabodala Mine. The gold bearing veins are hosted by flat lying and NW-trending shear zones. The NW-trending faults acted as transfers syn-gold mineralization, although only discrete segments of these faults were active during the main stage of gold mineralization. Late exhumation resulted in extension along fault zones was responsible for the juxtaposition of medium and low grade metamorphic rocks. The granitic suite of the Falombo pluton is linked to exhumation. Final dextral transpression postdating exhumation produced shearing and folding. Gold lodes developed during brittle deformation are associated with high fluid pressures in a northwest-southeast shortening regime. These are termed mainly the Sabodala deposit, and secondary the Niakafiri and Masato deposits.

The Geodynamic Context of the Cenozoic Volcanism of the Cap-Vert Peninsula (Senegal)

The Cenozoic alkaline volcanism of the Senegalo-Mauritania sedimentary basin presents an episodic long-lasting volcanic activity from the Eocene-Oligocene boundary up to the Quaternary. Two volcanic episodes are usually distinguished on stratigraphical grounds: a Miocene one and a Quaternary one separated by a period of quiescence of several million years corresponding to the main phase of lateritic weathering. The Tertiary lavas are highly silica-undersaturated alkaline rocks ranging from nephelinites to basanites. They contain nepheline and fassaite-type clinopyroxene in their mineralogy. The Quaternary lavas are more evolved with Hy-normative in comparison to the Tertiary ones;they are composed of basanites and medium to coarser-grained dolerites. The distribution of the REE and other incompatible elements are typical of alkaline lavas with generally strong LREE enrichment without significant Eu anomaly. The Cenozoic Cap-Vert lavas have OIB (oceanic islands basalts)-affinities as shown by their relatively radiogenic Nd and unradiogenic Sr characters closed to the HIMU-OIB. This suggests a HIMU-type end member in the magma sources which is fairly comparable to those erupted in the Canaries and Cape Verde archipelagoes during the same period.
