Introduction: For several years, the fight against the human immunodeficiency virus (HIV) has been a major public health issue in Africa. Since 2012, Senegal has adopted WHO option B+, consisting of systematic triple therapy for HIV-positive pregnant women, combined with breastfeeding and antiretroviral (ARV) prophylaxis for exposed newborns. It is in this dynamic that we set ourselves the objectives of evaluating the rate of transmission of HIV from mother to child and taking stock of the monitoring of children born to HIV-positive mothers at the Pediatric Department of the CHN of Pikine located in the Dakar suburbs. Methodology: Thus, we conducted a descriptive and analytical cross-sectional cohort follow-up study from 11/25/2014 to 03/09/2022 including all children born to HIV-positive mothers followed at the Pikine CHN since the start of care. Results: We had collected 51 children exposed to HIV and followed in our structure. They were exposed to HIV1 in 92% of cases. The HIV status of the mothers was known before pregnancy in more than half of the cases. The couples were serodiscordant in 56% of cases. The mothers were in clinical stage 1 of the disease in 69.6% of cases and were already under treatment in 47% of cases. The most used treatment regimen was ATRIPLA with Tenofovir (TDF) + Emtracitabine (FTC) + Efavirenz (EFV) in 59% of cases. Compliance was good in the majority of cases. The CD4 count during pregnancy was low in 10.6% of cases. The pregnancy was well followed in only 36.2% of cases. The mothers had given birth in our structure in 91.4% of cases. The vaginal route was found in 72.5% of cases and delivery was carried out by a midwife in 69.4% of cases. The average birth weight was 2733 ± 564. The majority of newborns had received antiretroviral (ARV) prophylaxis after birth, half of them between 12 and 24 hours. The burden of maternal mortality (MM) and morbidity is especially high in ***,China has made significant progress in reducing MM over the past two decades,and hence maternal death rate has declined considerably in last decade,To analyze availability and quality of emergency obstetric care (EmOC) received by women at Tongji Hospital,Wuhan,China,this study retrospectively analyzed various pregnancy-related complications at the hospital from 2000 to *** baseline periods of equal length were used for the comparison of variables.A total of 11 223 obstetric complications leading to MM were identified on a total of 15 730 hospitalizations,either 71.35％ of all *** maternal death was *** age of women was 29.31 years with a wide range of 14-52 *** 96.26％ of women had higher levels of schooling,university degrees and above and received the education of secondary school or *** 3.74％ received primary education at period two (P2) from 2005 to 2009,which was significantly higher than that of period one (P1) from 2000 to 2004 (P＜0.05) (OR:0.586; 95％ CI:0.442 to 0.776).About 65.69％ were employed as skilled or professional workers at P2,which was significantly higher than that of P1 (P＜0.05).About 34.31％ were unskilled workers at P2,which was significantly higher than that of P1 (P＜0.05).Caesarean section was performed for 9,930 women (88.48％) and the percentage of the procedure increased significantly from 19.25％ at P1 to 69.23％ at P2 (P＜0.05).We were led to conclude that,despite the progress,significant gaps in the performance of maternal health services between rural and urban areas ***,MM reduction can be achieved in *** must include,but not limited to the following:secondary healthcare development,health policy and management,strengthening primary healthcare services.

Introduction: Cirrhosis is a public health problem and the causes are dominated by viral hepatitis and alcoholism. Deaths due to cirrhosis represented 2.4% of deaths worldwide in 2017. The aim of this work was to study the diagnostic and prognosis of cirrhosis at the department of internal medicine of the Abass Ndao hospital center. Patients and Methods: This is a retrospective, descriptive study, carried out on the basis of patient files followed from May 1, 1999 to February 28, 2018 (19 years) and included all patients for whom the diagnosis of cirrhosis was accepted. The diagnosis of cirrhosis was made on the basis of clinical, biological and ultrasound (portal hypertension, signs of hepatocellular insufficiency, a diffuse heterogeneous aspect, and irregularity of the liver contours). Results: 60 patient files were listed with an average age of 46.9 years, a sex ratio of 2.2 and alcoholism in 11.7% of the cases. The reasons for consultation were dominated by an increase in the size of the abdomen (55%), lose weight (50%). On physical examination, it was a hepatomegaly (43.3%) and jaundices (33.3%). Exploration of liver function showed a cholestasis (48%), a cytolysis (58%) and a hepatocellular insufficiency (46.2%). Anemia was found in (24%). The abdominal ultrasound found in all cases a dysmorphic, heterogeneous liver with irregular contours. Hepatic atrophy was found in 20%. Among the 34 patients who underwent upper gastrointestinal endoscopy, they were grade 2 to 3 esophageal varicose vein in 67.6%, an erosive gastropathy in 29.4%. The etiology was viral hepatitis B in 45%, alcoholism in 11.7% and undetermined in 43.3%. A hematemesis complicated the evolution in 16.7%. Carcinomatous degeneration was found in 5% and 16.7% of the patients had died. Conclusion: our experience with cirrhosis was marked by advanced forms at the stage of complications. Introduction: Cervicofacial cellulitis is a medical and surgical emergency, it is an extensive infection of the aponeurotic spaces of the face and neck. Although frequent in black Africa rather than in developed countries, they remain associated with a poor prognosis and few studies have been done on this subject. Their management must be early, probabilistic and then adapted to the bacteriological results of the pus taken. As Senegal is one of the many sub-Saharan African countries without recent data on cervicofacial cellulitis, we felt it was important to review the situation. The aim of our study was to describe the epidemiological and clinical aspects of cervicofacial cellulitis in the ENT department of Lamine Sine DIOP of the Fann National University Hospital of Dakar. Materials and Method: This was a retrospective study conducted over 5 years from January 2017 to December 2021 in the ENT-CCF LAMINE SINE DIOP Department of the CHNU FANN in Dakar. Our data sources were hospitalization registers and medical records of patients hospitalized for cervicofacial cellulitis. Our data were analyzed using the spss 12 for windows software. Results: During this 5-year period, we collected 297 cases of cervicofacial cellulitis, i.e. 19% of the hospitalizations. The average age was 35 years with a sex ratio of 2.23. The notion of taking NSAIDs was found in 90.57% of the cases, the immunocompromised background was present in 18 patients, i.e. 6.06% of the cases. The site of the swelling was sub-mento-maxillary in 49% of the cases and anterior cervical in 5% of cases. The portal of entry was dental in 189 patients, i.e. 64% of cases, and unknown in 3%. 4 deaths were recorded, i.e. 1.35% of patients hospitalized for cervicofacial cellulitis, and 1 case of mediastinitis with polyseritis was transferred to the Department of Thoracic and Cardiovascular Surgery, which had a favourable outcome. Conclusion: Cervicofacial cellulitis is a fairly common infection with a poor prognosis

Introduction: A newborn is preterm when it is born before 37 weeks of amenorrhoea. Preterm births account for 11.1% of live births worldwide, 60% of which are in South Asia and sub-Saharan Africa. Preterm birth is the leading cause of neonatal mortality and the second leading cause of mortality in children under 5 years of age. The aim of this study was to investigate prematurity in the neonatology unit of the Mali Hospital. Materials and Methods: This was a cross-sectional, descriptive study from January 2012 to December 2015 of premature newborns from 28 to 36 weeks of amenorrhea in the neonatology unit of Mali Hospital. Results: From January 2012 to December 2015, we recorded 1692 inpatient newborns, including 614 preterm newborns, a frequency of 36.29%. The sex ratio was 1.43 in favor of boys. The average age of the mothers was 23.9 years with a minimum of 15 and a maximum of 47 years. The majority were housewives (92.7%). The pregnancy was poorly monitored in 51.8% of the mothers. The average gestational age was 32.9 days with extremes of 25 and 36 days. The most frequent mode of delivery was vaginal delivery (93.2%), with late prematurity in 51.5%, moderate prematurity (24.4%), extreme prematurity (18.7%) and very premature (5.4%). The mean birth weight was 1464 g with a standard deviation of 485 g and 2751 g. Prematurity was associated with neonatal infection in 49.4% of newborns, perinatal anoxia (13.9%) and hypotrophy (2.6%). The most frequent maternal risk factors were fever (83.4%), twinship (38.9%) and high blood pressure (8.6%). The average length of hospitalization was 9.75 days with extremes of 0 and 68 days. Introduction: SARS-COV2 infection is a major public health problem and diabetes mellitus is a serious factor. The objective was to determine the characteristics of COVID-19 in diabetics at the Epidemic Treatment Center (ETC) of the Abass Ndao Hospital Center. Observation: This was a retrospective, descriptive and analytical study carried out at Abass Ndao Hospital from March 01, 2021 to September 31, 2021. We evaluated the epidemiological, clinical and evolutionary characteristics. Results: Among 333 patients infected with COVID-19, 203 were diabetic (60.96%). The sex ratio was 0.70 and the mean age 60.18 years. Diabetes was type 2 (84.2%) and inaugural (15.8%). Arterial hypertension was the main comorbidity (86.96% of cases). The RT-PCR test was positive in 86.21% of cases. The main clinical signs were: asthenia (90.14%), dyspnea (78.81%), cough (52.21%) and body aches (52.21%). Oxygen saturation was alanced in 71.43% with an average blood sugar of 2.02 ± 4 g/l. At the paraclinical, the D dimer was ≥1000 ng/l (71.52%) and the C-reactive protein ≥100 mg/l (32.01%). Chest CT scan found mild to moderate (55.17%), severe (21.67%) and critical (17.24%) lung lesions. Pulmonary embolism was found in 4 diabetics. The treatment of diabetes was represented by insulin therapy (85.82%) and oral antidiabetics (24.11%). Anti-COVID-19 treatment was dominated by azithromycin (89.11%), corticosteroid therapy (92.11%), anticoagulants (99%), hydroxychloroquine (21.29%). Mortality was found in 14.36% (n = 29) and significantly correlated with age ≥ 60 years (OR = 3.013 [1.266 - 7.173], P = 0.005), short duration of hospitalization (OR = 3.154 [1.149 - 8.663], P = 0.009), C-reactive protein > 100 mg/l (OR = 6.370 [2.704 - 15.006], P < 0.001), lung saturation < 90% (OR = 6.331 [2.633 - 15.222], P [3.367 - 18.322], P Conclusion: Diabetes is one of the most important comorbidities linked to the severity of SARS-CoV-2 infection. Profile of Female Hyperandrogenism at the Medical Clinic II: About 19 Cases

Introduction: Female hyperandrogenism is essentially manifested by hirsutism. It is present in Senegal but not yet elucidated for lack of data. This is why we proposed to study the profile of hyperandrogenism in our context. Patients and Methods: This was an observational, cross-sectional cohort study of 19 patients of reproductive age followed for hyperandrogenism at the Medical Clinic II of Abass Ndao Hospital, from November 1st, 2019 to August 31st, 2021. Results: Our cohort consisted of women with an average age of 25.3 years, single (73.7%), with a low socioeconomic level (42.1%). A family history of hirsutism was found in 31.6% of cases. The main reasons for consultation were a menstrual cycle disorder in 94.7% of cases, and hirsutism in 78.9% of cases. The latter was post-pubertal (66.7%), with a slow or progressive evolution. The physical examination revealed: hirsutism (100%) with an average modified Ferriman Gallwey score (mFG) of 8.9 ± 5.8, acne (36.8%), hyperseborrhea (57.9%), major signs of virilization (10.5%), acanthosis nigricans (47.4%) and galactorrhea (5.3%). Hormonal explorations revealed an elevation of: testosterone (31.6%), 17-hydroxyprogesterone (5.3%), dihydrotestosterone (31.6%), and prolactin (10.5%). The ovarian morphology was micropolycystic (84.2%). The etiological profile corresponded to polycystic ovarian syndrome or PCOS (68.4%), ovarian hyperthecosis (10.5%), hyperprolactinemia (10.5%), congenital adrenal hyperplasia or CAH (5.3%). Idiopathic hirsutism was found in 5.3% of cases. Conclusion: Female hyperandrogenism is a less frequent reason for consultation in endocrinology. It deserves to be further evaluated in a large-scale study focused on epidemiological, clinico-biological and etiological investigation, in order to assess its prevalence and better define its profile in our context.
