BACKGROUND:A perennial challenge faced by clinicians and made even more relevant with the global obesity epidemic,difficult intravenous access(DIVA)adversely impacts patient outcomes by causing significant downstream delays with many aspects of diagnoses and *** most published DIVA strategies are limited to various point-of-care ultrasound techniques while other“tricks-of-the-trade”and pearls for overcoming DIVA are mostly relegated to informal nonpublished material,this article seeks to provide a narrative qualitative review of the iterature on DIVA and consolidate these strategies into a practical ***:We conducted a literature search on PubMed using the keywords“difficult intravenous access”,“peripheral vascular access”and“peripheral venous access”and searched emergency medicine and anaesthesiology resources for relevant *** strategies were then categorized and incorporated into a DIVA ***:We propose a Vortex approach to DIVA that is modelled after the Difficult Airway Vortex concept:starting off with standard peripheral intravenous cannulation(PIVC)techniques,progressing sequentially on to ultrasound-guided cannulation and central venous cannulation and finally escalating to the most invasive intraosseous access should the patient be in extremis or should best efforts with the other lifelines ***:DIVA is a perennial problem that healthcare providers across various disciplines will be increasingly challenged *** is crucial to have a systematic stepwise approach such as the DIVA Vortex when managing such patients and have at hand a wide repertoire of techniques to draw upon.

Value-relevance of accounting numbers for valuation

Religion is a fundamental factor in determining food avoidance, taboos and special regulation with respect to meat. A quarter of the world population is made up Muslims. Muslims have unique dietary requirements and are very keen to uphold the tenets of their religion. This group of consumers is scattered around all countries of the world and have a high purchasing power estimated at US$ 2.1 trillion. In order to tap into this lucrative market, the food industry must understand the Muslim requirements for halal food, particularly meat and meat products. Unlike other consumer groups whose purchasing decision is guided by aspects of lifestyle, culture, diet and health concerns, the purchasing decision of Muslims is normally guided by the halal status of the meat. Many Muslims may check for the halal logo of the product before even checking the expiry date. Although the halal status of meat is often believed to be equivalent to the application of halal slaughter, practically it is much more than that. The production of halal meat consists of many critical control points from farm to table in order to ensure that the meat is produced in the acceptable manner and there is no cross contamination with non-halal materials at all unit operations. Unfortunately, the food industry is often unaware of the requirements of the Muslim consumer or often indulges in adulteration for monetary benefits. Halal certifying authorities need robust analytical techniques in areas where fraud is most likely to occur. This paper sets out to highlight the requirements of the halal food chain as well as give an overview of the methods used to authenticate halal meat products.

Recent empirical work suggested that the ability of accounting numbers to explain the relationship between accounting numbers and stock prices has deteriorated over the past four decades. The findings of this study suggest that the accounting earnings and book value are capturing most of the information that is relevant to assess the values of firms. At the same time, it also suggests that earnings and book value as well as non-accounting beta are more valued during the financial crisis as compared to after the financial crisis. Overall, the accounting estimate of the value of the firm is not deviating from the markets＇ estimate, which suggests that accounting numbers play an important role in the valuation of firms in Malaysia.

The Viewpoint of Community Pharmacists about Their Professional Role in Dispensing Beauty Products: A Study from Western Region of Saudi Arabia

Background: Community pharmacists play a significant role in dispensing and counseling about medications and pharmaceuticals including beauty and cosmetic products. Aims and Objectives: The current study was conducted to assess the viewpoint of community pharmacists in the Western region of Saudi Arabia about their professional role in dispensing beauty products. Methods: A cross-sectional study was conducted between September and December 2019 using an online questionnaire. Expert professors from college of pharmacy, Taif University validated the questionnaire. Results: Five hundred seventy-seven community pharmacists completed the questionnaire. Most of the participants were male (99%) and aged 23 - 34 years old (71.1%). Slightly less than half (45.58%) of community pharmacists have 6 - 10 years of experience. Most pharmacists (96.9%) communicate directly with customers/ patients and most of them (85.3%) ask and counsel patients about their product. Pharmacists strongly agreed that pharmacists must keep in developing their communication skills with customers/patients. Conclusion: Most of the community pharmacists surveyed in this study reported a good perceived knowledge and attitudes and demonstrated an acceptable level of practices regarding beauty products. Saudi FDA is encouraged to adopt Good Pharmacy Practice standards and guidelines to improve current practice of community pharmacists concerning Beauty Products.

An image retrieval system was developed purposely to provide an efficient tool for a set of images from a collection of images in the large database that matches the user’s requirements in similarity evaluations such as image content similarity, edge, and colour similarity. Retrieving images based on the contents which are colour, texture, and shape is called content-based image retrieval (CBIR). This paper discusses and describes about the colour features technique for image retrieval systems. Several colour features technique and algorithms produced by the previous researcher are used to calculate the similarity between extracted features. This paper also describes about the specific technique about the colour basis features and combined features (hybrid techniques) between colour and shape features.

A batch enrichment technique was used to isolate atrazine-degrading Rhodococcus sp strain from an agricultural land with history of atrazine application in Bauchi state, Northeastern Nigeria. The strain was identified on the basis of physiological, biochemical and 16S r RNA gene sequencing. Growth studies and HPLC analysis showed that the strain has potential of atrazine degradation. An investigation into the catabolic genes Atz ABC, which transform atrazine to cyanuric acid, confirms the chromosomal DNA of strain to harbor BC genes, as compared with the positive control, Rhodococcus jostii RHA1. The strain does not possess the Atz A in all catabolic gene probe carried out. The isolation and characterization of the Rhodococcus sp strain showed that catabolic genes may have evolved from a single origin with widespread global distribution, with possible potential in atrazine bioremediation.

Objective: The present review aimed to determine the effect of palm oil versus other oils on changes in body weight or Body Mass Index (BMI). Design: A systematic review was conducted. Studies were identified by database searching (EMBASE, PUBMED, CENTRAL, SCOPUS, PROQUEST, Web of Science [ISI Web of Knowledge], Cumulative Index to Nursing and Allied Health Library (CINAHL Plus), LILAC and ***. Searching, selecting and reporting were done according to the PRISMA (Preferred Reporting Items for Systematic reviews and Meta-Analyses) Statement. Setting: Palm oil vs other oils intervention study on weight or BMI changes. Subjects: Individuals participating in palm oil vs other oil intervention study. Results: A total of 182 studies were screened for eligibility, five studies were finally included. Three studies compared the effect of palm oil vs sunflower oil on weight changes and BMI. One study examined the effect of hybrid palm oil vs extra virgin olive oil on weight changes. The last study examined the effect of palm oil vs olive oil vs lard on weight changes. Out of 292 participants in five studies, only one study (Iggman, 2014) had a low risk of bias. In this study, there was no significant difference between the group that received sunflower oil and palm oil (SMD: 0.04, 95% CI: -0.59 to 0.66). Other four studies had unclear risk of bias. Conclusions: Based on the review, there is insufficient evidence to suggest the impact of palm oil intake on weight changes or BMI.

GLP-1 receptor agonists (GLP-1 RAs) are among the most successful medications for treating people with type 2 diabetes mellitus (T2DM), giving reasonable glycemic control with a low risk of hypoglycemia in those who have failed to control their condition with other oral anti-diabetic drugs (OADs). However, GLP-1RAs are underutilized—as time patients remained on their last oral treatment regimen with inadequate glycemic control prior to GLP-1RA initiation is on average of 19 month—despite evidence supporting their effectiveness, safety, and possible CV outcome advantages. With the new advances in GLP-1 RAs, the first oral form for the semaglutide molecule was developed with proven efficacy, safety, and patient preferences that may help pave the road for more utilization of this class. Therefore, we, a Saudi task force, gathered to develop an explicit, evidence-based consensus on oral semaglutide use in Saudi patients with diabetes. The panel recommends a GLP-1RA in those T2DM patients with or without or at high risk for ASCVD, HF, and/or CKD when there is a need to minimize weight gain or promote weight loss, or when there is a need to minimize hypoglycemia. Ensure that people with T2DM and ASCVD, HF, or CKD are treated appropriately with an SGLT-2i or GLP-1 RA. This approach should be initiated independent of background therapy, glycaemic control, or individualized treatment goals. Healthcare professionals should do their best to prevent clinical inertia in T2DM to help people with T2DM achieve better glycemic control and prevent or delay diabetes-related complications. The availability of oral forms of GLP-1RA medications could help combat this problem of clinical inertia to start GLP-1RA at the right time, as patients prefer oral to injectable forms. The availability of oral GLP-1RA can help in starting this class early and encourage healthcare professionals in prescribing it at the right time. Emergency Department (ED) in Alnoor Hospital is considered the pulsating unit in the hospital by facing a daily challenge through a huge exposure to number of patients round between 500 - 700 per day in average. With this busy service in ED, our study emerges to measure the quality of provided services to patients in term of measuring the total length of stay time (LOS) in ED and its influencing factors. This is a prospective study aiming to estimate the average time patients spend in ED of Alnoor Hospital during the month of January (2013). In addition, it inspects factors influencing the LOS. The questionnaire which conducted and filled by emergency team over all patients was consisted of the following data: arrival time to ED, initial time of assessment by nurse, initial time of assessment by doctor, time of arrival to specific area, consultation time, arrival time of consulted specialty, time of laboratory investigation, time of radiological investigation, time of final disposition and time of physical disposition. For the 7604 patient visits analyzed, mean ED LOS was 3.02 hour (SD = 5.03 hour). About half of the patients spent less than 59 minutes (44%), 32.6% spent 1 to 3:59 hour, 15.2% spent 4 to 7:59 hour, and 8.2% of the patients spent more than 8 hours. A priceless such study will offer an opportunity to evaluate the recent ED performance and assist to adapt future optimization strategies to improve the quality of services provided to the patient.
