Epithelioid malignant peripheral nerve sheath tumor of the bladder and concomitant urothelial carcinoma: A case report

BACKGROUND Epithelioid malignant peripheral nerve sheath tumor(EMPNST)of the bladder is a rare entity with devastating features.These tumors are thought to originate from malignant transformation of pre-existing schwannomas of pelvic autonomic nerve plexuses,and unlike the conventional malignant peripheral nerve sheath tumor(MPNST),are not associated with neurofibromatosis.The tumor has dis-tinctive morphological,immunohistochemical and molecular features.Addi-tionally,it tends to be more aggressive and have a higher mortality.This is the first case that presents with a synchronous urothelial carcinoma of the bladder and the epithelioid variant of MPNST in the literature.It’s also the second re-ported case of EMPNST originating from the bladder wall.CASE SUMMARY In this case report,we present the detailed clinical course of a 71-year-old patient with EMPNST of the bladder alongside a literature review.CONCLUSION During the management of EMPNST cases,offering aggressive treatment moda-lities to the patient,such as radical cystectomy,is appropriate for the best chance to contain the disease,regardless of the tumor stage and the extent of local disease at initial diagnosis.

Automated evaluation of parapapillary choroidal microvasculature in crowded optic discs:a controlled,optical coherence tomography angiography study

AIM:To compare superficial and deep vascular properties of optic discs between crowded discs and controls using optical coherence tomography angiography(OCT-A).METHODS:Thirty patients with crowded discs,and 47 control subjects were enrolled in the study.One eye of each individual was included and OCT-A scans of optic discs were obtained in a 4.5×4.5 mm^(2) rectangular area.Radial peripapillary capillary(RPC)density,peripapillary retinal nerve fiber layer(pRNFL)thickness,cup volume,rim area,disc area,cup-to-disc(c/d)area ratio,and vertical c/d ratio were obtained automatically using device software.Automated parapapillary choroidal microvasculature(PPCMv)density was calculated using MATLAB software.When the vertical c/d ratio of the optic disc was absent or small cup,it was considered as a crowded disc.RESULTS:The mean signal strength index of OCT-A images was similar between the crowded discs and control eyes(P=0.740).There was no difference in pRNFL between the two groups(P=0.102).There were no differences in RPC density in whole image(P=0.826)and peripapillary region(P=0.923),but inside disc RPC density was higher in crowded optic discs(P=0.003).The PPCMv density in the inner-hemisuperior region was also lower in crowded discs(P=0.026).The pRNFL thickness was positively correlated with peripapillary RPC density(r=0.498,P<0.001).The inside disc RPC density was negatively correlated with c/d area ratio(r=-0.341,P=0.002).CONCLUSION:The higher inside disc RPC density and lower inner-hemisuperior PPCMv density are found in eyes with crowded optic discs.

Hazardous waste stream needs to be managed so as not to exceed stock-and rate-limited properties of its recipient ecosystems.The co-pyrolysis of Chinese medicine residue(CMR)and textile dyeing sludge(TDS)and its bio-oil,biochar,and ash quality and quantity were characterized as a function of the immersion of K_(2)CO_(3),atmosphere type,blend ratio,and temperature.Compared to the mono-pyrolysis of TDS,its co-pyrolysis performance with CMR(the comprehensive performance index(CPI))significantly improved by 33.9%in the N_(2)atmosphere and 33.2%in the CO_(2)atmosphere.The impregnation catalyzed the co-pyrolysis at 370℃,reduced its activation energy by 77.3 kJ/mol in the N_(2)atmosphere and 134.6 kJ/mol in the CO_(2)atmosphere,and enriched the degree of coke gasification by 44.25%in the CO_(2)atmosphere.The impregnation increased the decomposition rate of the co-pyrolysis by weakening the bond energy of fatty side chains and bridge bonds,its catalytic and secondary products,and its bio-oil yield by 66.19%.Its bio-oils mainly contained olefins,aromatic structural substances,and alcohols.The immersion of K_(2)CO_(3)improved the aromaticity of the copyrolytic biochars and reduced the contact between K and Si which made it convenient for Mg to react with SiO_(2)to form magnesium-silicate.The co-pyrolytic biochar surfaces mainly included-OH,-CH_(2),C=C,and Si-O-Si.The main phases in the co-pyrolytic ash included Ca_(5)(PO_(4))_(3)(OH),Al_(2)O_(3),and magnesium-silicate.

研究在不同载荷(10~50 N)、滑动速度(160~220 mm/s)及滑动距离(250~1000 m)条件下AZ91合金的磨损行为。结果表明,在一定的滑动距离和滑动速度下,磨损体积损失随负载的增加而增加。当滑动速度为220 mm/s和滑动距离为1000 m时,载荷10、20、30、40及50 N下合金的体积损失分别为15.0、19.0、24.3、33.9及37.4 mm3。磨损表面显示,载荷为10 N时磨损表面存在磨损和氧化现象,载荷为50 N时发生分层现象。ANOVA结果显示,载荷、滑动距离和滑动速度的贡献率分别为12.99%、83.04%及3.97%。采用人工神经网络(ANN)、支持向量回归(SVR)和随机森林(RF)对AZ91合金的体积损失进行预测。SVR、RF及ANN的相关系数(R2)分别为0.9245、0.980及0.9845。因此,ANN模型能较好地预测AZ91合金的耐磨性能。

Dynamic area coverage with small unmanned aerial vehicle(UAV)systems is one of the major research topics due to limited payloads and the difficulty of decentralized decision-making process.Collaborative behavior of a group of UAVs in an unknown environment is another hard problem to be solved.In this paper,we propose a method for decentralized execution of multi-UAVs for dynamic area coverage problems.The proposed decentralized decision-making dynamic area coverage(DDMDAC)method utilizes reinforcement learning(RL)where each UAV is represented by an intelligent agent that learns policies to create collaborative behaviors in partially observable environment.Intelligent agents increase their global observations by gathering information about the environment by connecting with other agents.The connectivity provides a consensus for the decision-making process,while each agent takes decisions.At each step,agents acquire all reachable agents’states,determine the optimum location for maximal area coverage and receive reward using the covered rate on the target area,respectively.The method was tested in a multi-agent actor-critic simulation platform.In the study,it has been considered that each UAV has a certain communication distance as in real applications.The results show that UAVs with limited communication distance can act jointly in the target area and can successfully cover the area without guidance from the central command unit.

Clustered regulatory interspaced short palindromic repeats(CRISPR)has changed biomedical research and provided entirely new models to analyze every aspect of biomedical sciences during the last decade.In the study of cancer,the CRISPR/CRISPR-associated protein(Cas)system opens new avenues into issues that were once unknown in our knowledge of the non-coding genome,tumor heterogeneity,and precision medicines.CRISPR/Cas-based geneediting technology now allows for the precise and permanent targeting of mutations and provides an opportunity to target small non-coding RNAs such as microRNAs(miRNAs).However,the development of effective and safe cancer gene editing therapy is highly dependent on proper design to be innocuous to normal cells and prevent introducing other abnormalities.This study aims to highlight the cutting-edge approaches in cancer-gene editing therapy based on the CRISPR/Cas technology to target miRNAs in cancer therapy.Furthermore,we highlight the potential challenges in CRISPR/Cas-mediated miRNA gene editing and offer advanced strategies to overcome them.

Background/Aims: Determining the levels of oral health and the quality of dental care are fundamental to building concepts of oral health. This study aims to assess toothbrushing techniques using a technical and physical model, clarifying how children and pre-adults learn to brush their teeth. Materials and Methods: Data were recorded from 23 participants, both male and female of various ages, using a proposed electronic toothbrush equipped with X-Y-Z axes pathways. The data, collected before and after training experiments, were processed with MATLAB to generate plots for the three axes. Results: The study revealed that most parameter values, such as Mean Difference Between Amplitudes (MAV, 6.00), Wilson Amplitude (WAMP, 179.419), and Average Amplitude Coupling (AAC, 1.270), decreased from before to after the experiments. Furthermore, the average overall epoch lengths (AVG) showed a 75% reduction in movement amplitude between the two experiments. Conclusion: Dentist observations indicated which brushing methods were acceptable or not. Analytical values suggest that individuals learn the toothbrushing technique effectively, and medical observations clearly demonstrate the success of the proposed method.

Plasma and aqueous humor levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy

AIM: To evaluate plasma and aqueous levels of adiponutrin and pannexin 1 in patients with and without diabetic retinopathy.METHODS: The study included three age and gendermatched groups of 20 cataract patients with no diabetes or additional disease(Group C), 20 cataract patients with diabetes and no retinopathy(Group DM+C), and 20 cataract patients with diabetic retinopathy(Group DR+C).All the patients were examined with respect to body mass index(BMI), fasting plasma glucose, hemoglobin A1c(HbA1c), and lipid profile.Phacoemulsification and intraocular lens(Phaco+IOL) implantation were performed to all patients in all the groups, and aqueous samples were taken during the operation.The plasma and aqueous adiponutrin and pannexin 1 levels were analyzed using enzyme-linked immunosorbent assays.RESULTS: A statistically significant difference was determined between the groups with respect to BMI, fasting plasma glucose, and HbA1c levels(P<0.05 for all parameters tested).The plasma adiponutrin levels of Group DR+C were statistically significantly lower than those of Group C and Group DM+C(P<0.001, P=0.004).No statistically significant difference was determined in the aqueous adiponutrin levels in three groups.The plasma pannexin 1 levels of Groups DM+C and DR+C were statistically significantly lower than those of Group C(both P=0.001).The aqueous pannexin 1 levels of Group DR+C were statistically significantly higher than those of Group C and Group DM+C(P=0.001, P<0.001).CONCLUSION: Adiponutrin and pannexin 1, which play an important role in the pathophysiology of diabetes and obesity, and have a regulatory role in hyperglycemia and insulin resistance.The measurement of adiponutrin and pannexin 1 levels may support clinicians in determining the risk of DR development.

Mechanical behavior and permeability properties of sustainable and high-performance anisotropic three-dimensional printable concrete

Three-dimensional printable concrete requires further development owing to the challenges encountered,including its brittle behavior,high cement requirement for the buildability of layers,and anisotropic behavior in different directions.The aim of this study is to overcome these challenges.First,three-dimensional printable concrete mixtures were prepared using silica fume,ground blast furnace slag,and metakaolin,instead of cement,to reduce the amount of cement.Subsequently,the rheological and mechanical behaviors of these concretes were investigated.Second,threedimensional printable concrete mixtures were prepared using 6-mm-long steel and synthetic fibers to eliminate brittleness and determine the effect of those fibers on the anisotropic behavior of the concrete.As a result of this study,it is understood that printable concretes with extremely low permeability and high buildability can be achieved using mineral additives.In addition,results showed that three-dimensional concrete samples containing short steel fibers achieve fracture energies up to 36 times greater than that of plain concrete.Meanwhile,its characteristic length values,as indicators of ductility,are 22 times higher than those of plain concrete.The weakest strength was recorded at the interfaces between layers.The bending and splitting tensile strengths of three-dimensional printed plain concrete samples were 15%and 19%lower than those of casted samples,respectively.However,the addition of fibers improved the mechanical strength of the interfaces significantly.
