Return to sport after lower limb arthroplasty-why not for all?

BACKGROUND Total hip and knee replacements are being performed in increasing numbers in progressively younger patients with higher activity demands. Many such patients have expectations of returning to athletic activity post-operatively yet are not always able to do so and the reasons behind this have not been extensively examined. We hypothesise that any reasons for a failure to return to athletic activity post-operatively are *** To quantify the return to athletic activity following lower limb joint arthroplasty and understand qualitative reasons for altered activity *** A single centre, single surgeon retrospective questionnaire for hip and knee arthroplasty patients under age 60 years, minimum two years post-surgery with exclusion criteria of multiple degenerative joint involvement and multiple medical co-morbidities. Outcomes were validated joint-specific(Oxford hip and knee) and lifestyle questionnaires [short form 12(SF-12) and University of California, Los Angeles(UCLA)] and an activity questionnaire assessing ability participation in athletic activity post-operatively. Statistical analysis was performed on the validated outcome data, including comparison between hip and knee replacements. Frequency tables were produced to quantify the different athletic activities participated in by *** Responses were received from 64 patients(80% response rate). There was a statistically significant improvement in Oxford hip and knee scores following surgery. SF-12 scores also improved for all patients, but no statistically significant difference was seen between joints(P = 0.88). Mean UCLA scores pre-operatively were 7.67 and at two years post-operatively were 7.69, with no statistically significant change(P = 0.91). Predicting lower limb periprosthetic joint infections: A review of risk factors and their classification

AIM To undertook a systematic review to determine factors that increase a patient's risk of developing lower limb periprosthetic joint infections(PJI).METHODS This systematic review included full-text studies that reviewed risk factors of developing either a hip or knee PJI following a primary arthroplasty published from January 1998 to November 2016. A variety of keywords were used to identify studies through international databases referencing hip arthroplasty, knee arthroplasty, infection, and risk factors. Studies were only included if they included greater than 20 patients in their study cohort, and there was clear documentation of the statistical parameter used; specifically P-value, hazard ratio, relative risk, or/and odds ratio(OR). Furthermore a quality assessment criteria for the individual studies was undertaken to evaluate the presence of record and reporting bias. RESULTS Twenty-seven original studies reviewing risk factors relating to primary total hip and knee arthroplasty infections were included. Four studies(14.8%) reviewed PJI of the hip, 3(11.21%) of the knee, and 20(74.1%) reviewed both joints. Nineteen studies(70.4%) were retrospective and 8(29.6%) prospective. Record bias was identified in the majority of studies(66.7%). The definition of PJI varied amongst the studies but there was a general consensus to define infection by previously validated methods. The most significant risks were the use of preoperative high dose steroids(OR = 21.0, 95%CI: 3.5-127.2, P < 0.001), a BMI above 50(OR = 18.3, P < 0.001), tobacco use(OR = 12.76, 95%CI: 2.47-66.16, P= 0.017), body mass index below 20(OR = 6.00, 95%CI: 1.2-30.9, P = 0.033), diabetes(OR = 5.47, 95%CI: 1.77-16.97, P = 0.003), and coronary artery disease(OR = 5.10, 95%CI: 1.3-19.8, P = 0.017).CONCLUSION We have highlighted the need for the provider to optimise modifiable risk factors, and develop strategies to limit the impact of non-modifiable factors.

Randomised controlled trial of triclosan coated vs uncoated sutures in primary hip and knee arthroplasty

BACKGROUND Triclosan-coated vicryl plus suture(Ethicon, Inc.) was developed to reduce microbial colonisation during surgical procedures. However, its effect on wound healing and surgical site infections remain unclear after hip and knee arthroplasty *** To determine the effect of triclosan-coated sutures(TCS) vs non-coated sutures on wound healing, following primary hip and knee *** A single-centred, double-blind randomised controlled trial(RCT) was undertaken. We randomly allocated patients to receive either the triclosan-coated sutures(TCS vicryl plus) or non-coated sutures(NCS vicryl) during the closure of unilateral primary hip and knee arthroplasties. We utilised the ASEPSIS wound scoring system to evaluate wound healing for the first 6 weeks *** One hundred and fifty patients undergoing primary total hip or knee arthroplasty over a one-year period were included. Eighty-one were randomised to the TCS group and 69 to the NCS group. Despite no statistically significant difference in the ASEPSIS scores among the study groups(P = 0.75), sensitivity analysis using the Mann Whitney test(P = 0.036) as well as assessment of the wound complications at 6 weeks follow up, demonstrated significantly higher wound complication rates in the TCS group(8 vs 1, P = 0.03).CONCLUSION No clear advantage was demonstrated for using the TCS. However, larger multicentred RCTs are required to validate their use in hip and knee arthroplasty surgery.

Biphasic Insulin Aspart 30 Therapy in Insulin-Naïve and Insulin-Experienced Patients with Type 2 Diabetes: Results from the Jordanian Subgroup of the A<sub>1</sub>chieve Study

Objective: To analyse the safety and effectiveness of biphasic insulin aspart 30 (BIAsp 30) in a Jordanian subgroup of the 24-week, non-interventional A1chieve study. Methods: A total of 509 Jordanian patients with type 2 diabetes (392 insulin-naive and 117 insulin-experienced) starting BIAsp30, alone or in combination with oral glucose-lowering drugs, were included. Safety and effectiveness outcomes were analysed over 24 weeks. Results: Patients had a mean age of 55.8 years, body mass index of 28.8 kg/m2 and diabetes duration of 9.4 years at baseline. Two serious adverse drug reactions of hypoglycaemia were reported. The proportion of patients who reported major hypoglycaemic events decreased (2.4% at baseline vs. 0.2% at Week 24, p = 0.0039). The proportion of patients reporting overall hypoglycaemia increased marginally (6.3% at baseline vs. 9.9% at Week 24, p = 0.0378), primarily attributed to a rise in minor and nocturnal hypoglycaemia reported in insulin-naive patients. From baseline to Week 24, the mean ± SD glycated haemoglobin A1c level decreased from 9.8% ± 1.4% to 7.4% ± 0.9% (p < 0.001). Significant reductions after 24 weeks were also noted in the mean fasting plasma glucose, postprandial plasma glucose, lipids, systolic blood pressure and quality of life (all p < 0.001), while the mean body weight increased by 1.8 ± 6.5 kg (p < 0.001). Conclusion: Overall, BIAsp 30 therapy was well-tolerated and resulted in improved glycaemic control in this Jordanian subgroup over 24 weeks.

Introduction: Type 2 Diabetes Mellitus (T2DM) has witnessed a rise in its prevalence worldwide and in the Middle East region. The overall burden associated with the disease is well characterized, but little is known about patient satisfaction in the region. The purpose of the study is to evaluate the quality of life (QoL) and treatment satisfaction of patients T2DM. Methods: The SIMPLIFY study was an observational, cross-sectional, multicenter, regional study that used patient-reported outcomes of T2DM patients in Jordan and Lebanon. Results: Patients were more satisfied when they were treated exclusively with oral medications, mainly metformin alone or combined with either sulfonylurea or dipeptidyl peptidase-4 inhibitor. Targets for glycated hemoglobin (HbA1c) values were better reached in patients treated with oral medications. Occurrence of comorbidities did not seem to be affected by oral or injectable medications or to affect patients’ satisfaction. Data highlighted a suboptimal screening for albuminuria and showed that most patients were overweight or obese and around 30% suffered from hypoglycemia episodes. Conclusion: Data shed the light on the management of T2DM in Jordan and Lebanon and suggested the need for a more comprehensive approach to T2DM management and selection of medications that would support weight control and a lower hypoglycemia incidence.

Review of evolution of tunnel position in anterior cruciate ligament reconstruction

Anterior cruciate ligament(ACL) rupture is one of the commonest knee sport injuries. The annual incidence of the ACL injury is between 100000-200000 in the United States. Worldwide around 400000 ACL reconstructions are performed in a year. The goal of ACL reconstruction is to restore the normal knee anatomy and kinesiology. The tibial and femoral tunnel placements are of primordial importance in achieving this outcome. Otherfactors that influence successful reconstruction are types of grafts, surgical techniques and rehabilitation programmes. A comprehensive understanding of ACL anatomy has led to the development of newer techniques supplemented by more robust biological and mechanical concepts. In this review we are mainly focussing on the evolution of tunnel placement in ACL reconstruction, focusing on three main categories, i.e., anatomical, biological and clinical outcomes. The importance of tunnel placement in the success of ACL reconstruction is well researched. Definite clinical and functional data is lacking to establish the superiority of the single or double bundle reconstruction technique. While there is a trend towards the use of anteromedial portals for femoral tunnel placement, their clinical superiority over trans-tibial tunnels is yet to be established.

Flexible Traceable Generic Genetic Algorithm

This document elaborates on the generic implementation one of the main heuristics algorithms verified through its quick application to a biology problem requiring to find out an optimal sequences tree topology. In order to solve this problem, categorized as Non-Polynomial Hard (NP-Hard), “to minimize differences between given (leaf) and/or derived (parent) sequences”, many popular methods are used. “The higher the number of given sequences is, the more advisable and efficient it would be to go towards heuristics as they would provide a close-enough solution faster, as for instance genetic algorithms amongst others do. Thus, as part of a larger research in Heuristics and phylogenies, this paper aims to suggest a generic advanced flexible implementation of the Genetic Algorithm verified by a “general way to encode the problem into instances of different heuristic algorithms” as mentioned in our first reference below. The proposed algorithm will also present a chronology traceability feature for further analysis and potential improvements.

The Internet of Things (IoT) is a technological revolution that has changed everything we do and given us a new perspective on our daily lives, but despite the fact that numerous publications have focused on characterizing the many edges and technologies that make up an IoT system, the IoT ecosystem is still seen as too complex to be recognized as a stand-alone environment due to its significant diversity;hence, the objective of this research is to address such a complex environment in a way that highlights its components and distinguishes them both individually and in relation to their broader context. Therefore, the definition of IoT and its emergence were discussed and organized around the timeline of Internet development phases demonstrating that IoT has been a need that has accompanied the presence of the Internet since its early stages, and then its growth and impact were discussed and highlighted with estimates and numbers. On the technical side, each of the following groups, IoT components, protocols, and architectures, was defined, discussed, and grouped in such a way that their intergroup organization, as well as their placement and contribution to the overall ecosystem, was highlighted. This, in addition to the various examples mentioned throughout the discussion, will provide the reader with a better understanding of the Internet of Things and how deeply it has become entwined in our daily lives and routines as a result of its numerous applications.

Generic Simulated Annealing

One of the many problems that are considered to be NP-Hard is the Multiple Sequence Alignment one that initially requires, as for any other of its siblings, a specific encoding schema and design of the main functionalities of the heuristics algorithm being implemented and executed. This paper intends to discuss our proposed generic implementation of the Simulated Annealing, inspired for the procedure of cooling and shaping methods of metals. In our algorithm, we attempted to add some executions tracing functionalities in order to help later analysis for initial parameters tuning. On another hand, we also tried to get closer in our attempt to mimic the cooling of metals, but giving it an option to run under different cooling schedules. We proposed a few schedules that seemed to be studied and/or used in many algorithm implementations.

In this paper, a routing protocol for wireless sensor network, baptized energy based protocol (EBP) is proposed. Wireless sensor network presents many challenges and constraints, and one of the major constraints is the routing problem. Due to the limited energy of sensor nodes, routing in this type of network shall perform efficiently to maximize the network lifetime. One of the proposed algorithms is the directional source aware routing protocol (DSAP) which, after simulation, showed a lot of limitations and drawbacks. The modified directional source aware routing protocol (MDSAP) was proposed by the authors of this paper to address some of the DSAP’s limitations but remains limited to a fixed topology, fixed source and stationary nodes. So EBP is proposed and operated under different scenarios and showed, after its simulation using TinyOS, many advantages in terms of load balancing, free looping, minimizing packet error rate and maximizing network lifetime.
