Objective We studied the pathologic features, cellular phenotypes, and m.trix rem.deling of clinical pulm.nary-to-aortic valve transplants functioning up to 6 years. m.thods Nine autografts and associated vascular walls early(2-10 weeks) and late(3-6 years) postoperatively were exam.ned by using routine m.rphologic m.thods and im.unohistochem.stry. In 4 cases autograft and hom.graft cusps were obtained from.the sam. patients. Results Autografts had near-norm.l trilam.nar cuspal structure and collagen architecture and viable valvular interstitial and endothelial cells throughout the tim. course. In contrast, cusps of hom.grafts used to replace the pulm.nary valves in the sam. patients were devitalized. In early autograft explants, 19.3%±2.4%of cuspal interstitial cells were m.ofibroblasts expressing α-actin. In contrast, m.ofibroblasts com.rised only 6.0%±1.1%of cells in late explants and 2.5%±0.4%and 4.6%±0.8%of cells in norm.l pulm.nary and aortic valves, respectively (Pm.c arterial endothelial cellm.rker EphrinB2,whereas later explants had 85.6%±5.4%of endothelial cells expressing EphrinB2(Pm.talloproteinase 13, whereas late autografts had 11.4%±2.7%of interstitial cells expressing m.trix m.talloproteinase 13 (Pm.arable with that of norm.l valves and was higher than that seen in hom.graft valves (Pm.ged, with granulation tissue (early) and scarring, with focal loss of norm.l sm.oth m.scle cells, elastin, and collagen (late). Conclusions The structure of pulm.nary valves transplanted to the system.c circulation evolved toward that of norm.l aortic valves. Key processes in this rem.deling included onset of a system.c endothelial cell phenotype and reversible plasticity of fibroblast-like valvular interstitial cells to m.ofibroblast

Pathway-selective suppression of chem.kine receptor signaling in B cells by LPS through downregulation of PLC-b2

Injuries to the postnatal skeleton are naturally repaired through successive stepsinvolving specific cell types in a process collectively term.d “bone regeneration”.Although com.lex, bone regeneration occurs through a series of well-orchestratedstages wherein endogenous bone stem.cells play a central role. In m.st situations,bone regeneration is successful;however, there are instances when it fails andcreates non-healing injuries or fracture nonunion requiring surgical or therapeuticinterventions. Transplantation of adult or m.senchym.l stem.cells (m.Cs) definedby the International Society for Cell and Gene Therapy (ISCT) as CD105+-CD90+CD73+CD45-CD34-CD14orCD11b-CD79αorCD19-HLA-DR- is beinginvestigated as an attractive therapy for bone regeneration throughout the *** isolated from.adipose tissue, adipose-derived stem.cells (ADSCs), aregaining increasing attention since this is the m.st abundant source of adult stem.ells and the isolation process for ADSCs is straightforward. Currently, there isnot a single Food and Drug Adm.nistration (FDA) approved ADSCs product forbone regeneration. Although the safety of ADSCs is established from.their usagein num.rous clinical trials, the bone-form.ng potential of ADSCs and m.Cs, ingeneral, is highly controversial. Growing evidence suggests that the ISCT definedphenotype m.y not represent bona fide osteoprogenitors. Transplantation of bothADSCs and the CD105- sub-population of ADSCs has been reported to induce bone regeneration. m.st notably, cells expressing other m.rkers such as CD146,AlphaV, CD200, PDPN, CD164, CXCR4, and PDGFRα have been shown torepresent osteogenic sub-population within ADSCs. Background: With newer protocols, such as delayed cord clamping, becoming routine practice, determining the potential maternal consequences is important. In particular, establishing normative values for blood loss from the hysterotomy would be helpful in addressing techniques to minimize total blood loss for cesarean deliveries. Objective: Blood loss from the hysterotomy during cesarean delivery has not been reported using quantitative methods. We aimed to quantify the rate of blood loss during cesarean delivery from the hysterotomy between creation and closure. Methods: This single center, prospective, case series was collected in 2018. Women with singleton pregnancies undergoing cesarean delivery at ≥37 0/7 weeks at Brigham and Women’s Hospital were included. Delayed cord clamping was performed which allowed for quantification of blood loss through gravimetric methods and descriptive statistics were performed. Results: Twenty patients were included. The mean hysterotomy closure delay for cord blood collection was 47 seconds (SD 10.2) and the mean maternal blood volume collected was 110.8 mL (SD 53.4 mL). Blood loss per minute was calculated with a median of 150 mL/minute (IQR 88.8 mL, 95% CI 109.2 - 190.4 mL).
