＜正＞Objective The polysaccharides in Lycium barbarum,a well known traditional Chinese medicine,have been demonstrated to display multiple biological effects such as anti-aging,anti-tumor and *** the present study,we aimed to evaluate the effects of Lycium barbarum polysaccharides（LBP） on murine brain injury after induction of acute ischemic stroke by middle cerebral artery occlusion（MCAO）.Methods Mice were orally treated with either vehicle（PBS） or LBP（1 mg/kg or 10 mg/kg） for 1 week before induction of cerebral ischemia by *** 2 h of ischemia followed by 22 h of reperfusion,animals were evaluated for neurological *** after scoring of the neurological deficits,brains were isolated,cut into 6 coronal slices of 2 mm thickness and stained with 2%2,3,5-triphenyltetrazolium chloride（TTC） to detect lesion *** posterior surface of each brain slice was subsequently photographed and analyzed using a digital image analysis *** area,volume,and hemispheric brain swelling were calculated using an indirect method in which the effects of edema and brain swelling have been ***（1） Mice treated with either 1 mg/kg LBP or 10 mg/kg LBP（n=7 for each group） had less neurological deficits than the vehicle-treated mice（n=8）（vehicle=2.0+/-0.3,1 mg/kg LBP=1.4+/-0.2,10 mg/kg LBP=1.1+/-0.1）.（2） TTC staining revealed a significantly smaller infarct area in brain slice number 4 in the LBP-treated mice when compared with the vehicle-treated mice（vehicle=35.4+/- 4.2%,1 mg/kg LBP=17.5+/-5.8%,10 mg/kg LBP=17.6+/-4.9%）.（3） Consistent with the infarct area data,the overall infarct volumes were decreased in the LBP-treated groups（vehicle=25.8+/-3.1%,1 mg/kg LBP=215+/-2.5%,10mg/kg LBP=16.6+/-2.7%）. （4） Mice treated with LBP displayed less hemispheric brain swelling（vehicle=11.1+/-0.9%,1 mg/kg LBP=9.7+/-1.1%,10mg/kg LBP=7.9+/-0.6%）.Conclusions Taken together,these data indicate that treatment with LBP for 1 week could protect the mouse

Outcomes of emergency and interval hepatectomy for ruptured resectable hepatocellular carcinoma: a single tertiary referral centre experience

Aim: The short and long term outcomes of patients who underwent emergency and interval hepatectomy for ruptured and resectable hepatocellular carcinoma (HCC) were analysed. Methods: The data of patients with ruptured HCC presenting between April 2004 and October 2015 were analysed. Emergency hepatectomy was defined as hepatectomy within 48 h of the clinico-radiological diagnosis of HCC rupture. Results: Thirty patients underwent hepatectomy for ruptured HCC. Nine (30%) patients underwent emergency hepatectomy. The median age was 56 and 54 years (P = 0.13) with a similar gender distribution. The mean HCC size (10.5 vs. 8.3 cm, P = 0.17), total blood loss (3,000 vs. 850 mL, P = 0.002) and total units of red blood cell transfusion (1.9 vs. 0.5 units, P = 0.27) were greater in the emergency hepatectomy group. The complication rate was 44% and 38% (P = 0.53), with median length of hospital stay of 10 and 12 days (P = 0.07) in the emergency and interval hepatectomy groups, respectively, and no 30-day mortality in both groups. The median overall survival was 29 and 15.7 months (P = 0.25), with survival rates of 78%, 45%, 0% and 85%, 43% and 5% at 1, 3 and 5 years in the emergency and interval hepatectomy groups, respectively. Conclusion: Hepatectomy should be considered for ruptured HCC provided the patient could tolerate curative resection.

Incidence and predictors of upper gastrointestinal bleeding in patients receiving low-dose aspirin for secondary prevention of cardiovascular events in patients with coronary artery disease

AIM： The use of low-dose aspirin to prevent cardiovascular disease events is well established. However, the incidence and predictors of upper gastrointestinal bleeding （UGIB） with its use are unknown. We studied prospectively the incidence and outcome of peptic ulceration in low-dose aspirin users. METHODS： A total of 991 patients with coronary artery disease （CAD） on low-dose aspirin were prospectively followed-up for two years for the occurrence and clinical features of first hospitalized episode of UGIB. RESULTS： UGIB had a bimodal presentation with 45% occurring within four months of aspirin initiation and had an overall prevalence of 1.5% per year. There was no UGIB-related death. Hypertension （OR = 4.6, 95%CI 1.5 - 14.7, P = 0.009）, history of peptic ulceration （OR = 3.1, 95%CI 1.1 - 9.0, P = 0.039）, tertiary education （OR = 3.08, 95%CI 1.1 - 9.0, P = 0.039） and higher lean body mass （P = 0.016） were independent factors associated with UGIB. Use of nitrate did not reduce UGIB. CONCLUSION： The incidence of UGIB in patients with CAD on long-term low-dose aspirin is low, but is accompanied with significant morbidity. With prolonged use of aspirin, UGIB continues to be a problem for those with risk factors and especially in patients with a history of peptic ulcers, in which UGIB tends to occur early after aspirin therapy.

An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Background Early detection with screening mammography can potentially reduce breast cancer mortality rates. To achieve an efficient screening, a peer review system provides a compensatory double-check reviewing, will hopefully to prevent the omission of detectable lesions and reduce unnecessary recall. Methods In 2009, 4643 initial mammographic screenings reported by 74 screening radiologists had negative results with a recall rate of less than 5%. In the same year, 2538 initial positives screened by 18 screening radiologists had a recall rate higher than 15%. Those 7181 randomized screenings were evenly distributed for reassessment by 39 reviewing radiologists. The disagreement of assessments between the reviewers and screening radiologists was recorded. The differential rate was defined as the number of the disagreements divided by the number of audited films reviewed by a screening radiologist. The equality of the differential rates for each screening radiologists with negative and positive assessments was compared by a Chi-square test. The performance of the 39 auditors was measured by the Kendalrs tau statistic. P values less than 0.05 were considered statistically significant. Results The mean differential rate for screening radiologists of negative assessments was 6.7% （P=0.588）, while 35.0% for positive assessments were significant （P 〈0.001）. The result indicated that most of the initial negative assessments reported by the screening radiologists were generally accepted by the reviewers but not the positive assessments. With respect to the 39 reviewers, there was no significant evidence for the association of the difference rates between negative and positive assessments. Nine reviewers were found to have their differential rate for negative and positive assessments larger than the average of the population. 背景:在支气管肺发育不全中肺血管的发育往往发生紊乱。异常肺血管发育导致高阻力的肺血流可能对心脏的心室功能产生影响。目的:明确合并支气管肺发育不全的早产儿是否存在远期心脏功能的改变。方法:本研究共纳入47例儿童,其中组1为22例合并支气管肺发育不全的早产儿儿童(年龄为48.9±6.4月)、组2为13例未合并支气管肺发育不全的早产儿儿童(年龄为46.3±8.1月)和组3为12例健康足月儿童(年龄为53.4±12.2月)。通过二维斑点追踪超声心动图检测研究对象的左、右心室的心肌应变及应变率。结果:与组3相比,组1的右室整体收缩期应变明显下降(p=0.022),右室收缩期应变率(p=0.05)及舒张早期应变率(p=0.05地明显下降;同样,组1的左室纵向长轴收缩期应变率也有降低的趋势(p=0.06)。与组2相比,组1的右室整体收缩期应变(p=0.09)、舒张早期应变率(p=0.049)及左室纵向长轴收缩期应变率(p=0.08)也有降低趋势。从组1到组3,右室侧壁及室间隔的收缩期应变及应变率呈上升趋势(p<0.05)。多因素分析提示有创机械通气的时间是决定右心室收缩期应变机能的重要因素。结论:合并支气管肺发育不全的儿童右心功能往往较健康儿童受损,且与生后有创机械通气的时间呈负相关。
