Comparative Study of the Complications of Pedicular Screw Fixation Techniques in the Thoracolumbar Spine: A Systematic Review

Introduction: The use of pedicle screws increases postoperative stability and consolidation of arthrodesis. Pedicle arthrodesis is currently the standard treatment for the thoracolumbar spine, presenting the best fusion and stiffness rates, and among its main indications is the possibility of better correction of spine deformities in the thoracic and lumbar region. However, due to different definitions and the lack of a control group, many of these studies have limited comparative analysis, resulting in the scarcity of comparative studies with standardized methodology. Objective: It was to analyze, through a systematic review, the safety and efficacy of instrumentation with pedicle screws in the spine which have been questioned, despite its wide use for stabilization of the spine, comparing the complications present in the insertion techniques of pedicle screws. Methods: The rules of the Systematic Review-PRISMA were followed. The literary search process was carried out from January to March 2023. A bibliographic search was carried out in MEDLINE, PubMed, and Scielo for articles produced between 2001 and 2023. The quality of the studies was based on the GRADE instrument, and the risk of bias was analyzed according to the Cochrane instrument. The Cohen test (Funnel Plot) and The Heterogeneity Test (Chi-Square Test – X2) were performed, with p Results and Conclusion: A total of 134 articles were found. A total of 67 articles were evaluated in full and 12 were selected to compose the results of this systematic review. According to the GRADE instrument, most studies (X2 = 90.2% > 50%) followed a controlled clinical study model and had a good methodological design, with p < 0.05. It was shown that poor positioning of pedicle screws is the most common cause of complications. Background: Colles’ fracture is a type of fracture of the distal third of the radius and its most common cause is falling from patient’s own height and, epidemiologically, it is more common in the elderly. Objectives: The main objective of this study is to analyze the epidemiology of Colles’ fracture retrospectively, in patients admitted to Fernandes Távora Hospital. Materials and Methods: This retrospective study conducted at the Fernandes Távora Hospital used 1030 radiographic images of patients with wrist fractures. Results: (After a careful analysis) It was found that this type of fracture occurred with a greater frequency in age groups of 50 to 69, 60 to 69, and 40 to 49 years old, which corresponds to 59.5% of patients and the most affected sex were females (51.1%). Conclusion: The study concluded that this type of bone injury is more common in people aged 40 to 69 years old, especially in women, because of their geometry and bone composition. It is evident that Colles’ fracture impacts the life of the affected person, therefore, an adequate diagnosis and treatment having utmost importance.

Analysis of the Tropism and Anti-Bacterial Potential of a Type of Clay

Natural clays have been used by man in infections of bacterial etiology, since the first historical registers. Our attention turned to a red-colored clay, known in the northeast of Brazil as “barro de louça” (dish clay). These clays and other natural earth materials seem interesting to us, as the blockage of the liberation of toxins or inactivation, may be related to the interruption of infection cycles in the skin and mucous membranes. The adsorptive and absorptive properties of the mineral clays are well documented in the cure process of skin and gastrointestinal diseases. Susceptibility and bacterial tropism tests were carried out. The results were analyzed and interpreted according to the conventional microbiological protocol. The bacterial strains, Staphylococcus aureus, Escherichia coli e Pseudomonas aeruginosa, did not present a susceptibility profile to an isotonic solution of clay, but there was an increase of the bacterial tropism as the concentration of the isotonic solution was increased, being the minimal observed concentration of 100 mg/mL. Our aim is to document a type of red clay from the northeast of Brazil with possible attraction properties (Tropism) to bacteria and their toxins.

Background. Primary Antiphospholipid Antibody Syndrome (PAAS) is characterized by detection of antiphospholipid antibodies associated with venous or arterial thrombosis and/or miscarriages by patients with no other associated disease such as systemic lupus erythematosus (SLE). Primary Antiphospholipid Antibody Syndrome has many clinical manifestations of which dermatological ones are probably the most common. The purpose of this study was to determine the frequency of each cutaneous lesion, describing clinical features essential for diagnosis, in patients with AntiphospholipidAntibody Syndrome (AAS) attendingWalter CantdioUniversityHospital. Methods. Sixty patients wit clinical findings suggestive of AAS were screened, and submitted for clinical and laboratory evaluations including lupus anticoagulant (KCT), anticardiolipin antibodies (IgG and IgM: ELISA), routine laboratory tests and screening tests for possible associated conditions. Results. Twenty-five cases of primary and 14 cases of secondary AAS were diagnosed by clinical and laboratory evidences. Persistent elevated antiphospholipid antibodieswithout history of thromboembolic events or miscarriages were demonstrated in 21 patients. Forty percent of the patients with AAS had a cutaneous feature as the major complaint. These were dermographism(15), acrocyanosis (13), urticaria (9), diffuse alopecia (9), livedo reticularis (seven), Raynaud’s phenomenon (three), purpura (two), ulcers and necrosis (four), nodules (four), pterygium ungueum (one) and subungual hemorrhage (one). Conclusions. Dermatological complaints are very frequent in patients with AAS and may be the first clue to the syndrome. Therefore a careful history and detailed physical examination are essential to diagnose AAS. All dermatologists should investigate the possibility of AAS when facing cutaneous findings related to venous or arterial thrombosis or microthrombosis.

The Schinus terebinthifolius Raddi is a native plant of South America popularly known in Brazil as aroeira. It is a medium-sized plant, which demonstrates a high adaptive potential in various environments, besides having various medicinal properties such as anti-inflammatory and antidiarrheal. Bacterial susceptibility tests were carried out and the minimal inhibitory concentration (MIC) was obtained. The results were interpreted based on the conventionally microbiological protocols and data from the CLSI. For the microbiological tests, microorganisms obtained from the American Type Culture Collection (ATCC), specifically, (Pseudomonas aeruginosa, Staphylococcus aureus and Encherichia coli) were used. Of the tested bacterial strains, only the Staphylococcus aureus 6538 presented susceptibility to the aroeira’s hydroalcoholic extract, forming zones of inhibition of 8.0 mm of diameter up to the MIC of 35.3 mg/mL, while the other tested strains showed to be resistant in all the concentrations of hydroalcoholic extract of the Schinus terebinthifolius Raddi bark. Our aim is to analyze the “in vitro” antibacterial potential of the 70% hydroalcoholic extract of the Schinus terebinthifolius Raddi.
