Internal fixator vs external fixator in the management of unstable pelvic ring injuries:A prospective comparative cohort study

BACKGROUND Reconstruction of the pelvic ring anatomy in unstable anterior pelvic ring injuries is a significant step to reduce the mortality rate associated with these injuries *** is a debate on using either an anterior subcutaneous pelvis internal fixator(INfIX)or an anterior supra-acetabular external fixator(EXfIX)to manage an unstable anterior pelvic ring *** To compare the functional and radiological outcomes and complications of INfIX vs EXfIX in managing unstable pelvic ring *** A prospective cohort study included 54 patients with unstable pelvic ring *** patients were divided into two groups;the INfIX group,in which 30 cases were fixed by INfIX,and the EXfIX group,in which 24 patients were treated by *** average age in the EXfIX group was 31.17 years(16-57 years),while in the INfIX group,it was 34.5 years(17-53 years).The study included 20(66.7%)males and 10(33.3%)females in the INfIX group and 10(41.7%)males and 14(58.3%)females in the EXfIX *** radiological outcomes were evaluated using Matta and Tornetta's score,and the functional outcomes using the Majeed *** The results revealed a statistically significant difference between both groups(P=0.013)regarding radiological outcomes,according to Matta and Tornetta's score in favor of the INfIX ***,standing,and walking abilities were measured at a 3-mo follow-up visit using Majeed score *** was significantly better among the INfIX group than the EXfIX group in all three *** the final follow-up,both groups had no statistically significant difference according to the Majeed score;92.35 in the INfIX group and 90.99 in the EXfIX group(P=0.513).A lower surgical site infection rate was noticed in the INfIX group(P=0.007).CONCLUSION Anterior subcutaneous pelvis INfIX is associated with better radiological outcomes and a lower infection rate than anterior supra-acetabular EXfIX in managing patients with unstable anterior pelvic

BACKGROUND Helicobacter pylori(***)is a significant human pathogen that is responsible for a variety of illnesses,including mucosa-associated lymphoid tissue lymphoma,gastric cancer,peptic ulcers,and *** To investigate the frequency of *** infection and its resistance patterns among Egyptian patients and to determine the influence of *** virulence genetic determinants on the eradication success of 14-d triple therapy *** *** infections were investigated in 72 patients with gastroduodenal complications suggestive of *** *** cagA and vacA genotypes of cultured strains were studied using polymerase chain *** patients underwent 14 d of triple-therapy *** treatment response was examined using histology and a rapid urease test 6 wk after therapy *** The intention-to-treat eradication rate was 59.2%(95%CI:48.2%-70.3%).Rates of *** resistance to clarithromycin,amoxicillin,and metronidazole were 52.8%,81.9%,and 100%,*** eradication of *** was more significantly associated with vacA s1-positive strains[adjusted odds ratio(aOR)=0.507,95%CI:0.175-0.822].A significant association was found between failed eradication rate and *** strains resistant to clarithromycin(aOR=0.204,95%CI:-0.005 to 0.412)and amoxicillin(aOR=0.223,95%CI:0.026-0.537).CONCLUSION This study’s low *** eradication rate following 14-d triple therapy is concerning and *** pan-resistance to metronidazole followed by the high resistance to ciprofloxacin,amoxicillin,and clarithromycin in this research is challenging and of great concern.

Seminal plasma anti-Müllerian hormone level correlates with semen parameters but does not predict success of testicular sperm extraction （TESE）

Aim： To assess seminal plasma anti-Müllerian hormone （AMH） level relationships in fertile and infertile males. Methods： Eighty-four male cases were studied and divided into four groups： fertile normozoospermia （n = 16）, oligoastheno- teratozoospermia （n = 15）, obstructive azoospermia （OA）（n = 13） and non-obstructive azoospermia （NOA）（n = 40）. Conventional semen analysis was done for all cases. Testicular biopsy was done with histopathology and fresh tissue examination for testicular sperm extraction （TESE） in NOA cases. NOA group was subdivided according to TESE results into unsuccessful TESE （n = 19） and successful TESE （n = 21）. Seminal plasma AMH was estimated by enzyme linked immunosorbent assay （ELISA） and serum follicular stimulating hormone （fSH） was estimated in NOA cases only by radioimmunoassay （RIA）. Results： Mean seminal AMH was significantly higher in fertile group than in oligoasthenoteratozoospermia with significance （41.5±10.9 pmol/L vs. 30.5±10.3 pmol/L, P 〈 0.05）. Seminal AMH was not detected in any OA patients. Seminal AMH wascorrelated positively with testicular volume （r = 0.329, P = 0.005）, sperm count （r = 0.483, P = 0.007）, sperm motility percent （r = 0.419, P = 0.021） and negatively with sperm abnormal forms percent （r = -0.413, p = 0.023）. Nonsignificant correlation was evident with age （r = -0.155, P = 0.414） and plasma fSH （ r = -0.014, P = 0.943）. In NOA cases, seminal AMH was detectable in 23/40 cases, 14 of them were successful TESE （57.5%） and was undetectable in 17/40 cases, 10 of them were unsuccessful TESE （58.2%）. Conclusion： Seminal plasma AMH is an absolute testicular marker being absent in all OA cases. However, seminal AMH has a poor predictability for successful testicular sperm retrieval in NOA cases.

The Effect of Using Wastewater from Stone Industry in Replacement of fresh Water on the Properties of Concrete

This research presents an attempt of using wastewater from stone slurry waste in production of concrete. Several concrete mixtures were prepared by using tap water and stone slurry wastewater at different w/c ratios and replacement ratios of wastewater in substitute of tap water. Testing of concrete samples included slump, compressive strength, flexural strength and absorption. Test results showed reduction of workability (slump) at all w/c ratios and replacement ratios. The maximum compressive strength didn’t change significantly at w/c = 0.7 and 28 days of curing compared with compressive strength at w/c = 0.5 and 0.6. from linear regression of the experimental results, the results showed that at 20% replacement ratio of tap water with wastewater, the reduction in compressive strength was insignificant (not greater than 10% to 15%). Test results showed varying reduction of absorption at different w/c and replacement ratios, up to 62% at w/c = 0.5.

Methylenetetrahydrofolate reductase(MTHfR)deficiency is the most common genetic cause of hyperhomocysteinemia,which has been implicated in the etiology of recurrent spontaneous abortion(RSA).This study was designed to investigate the association between two single nucleotide polymorphisms(SNP)(rs1801133[C677T]and rs1801131[A1298C])in the MTHfR gene and RSA,in *** two SNPs were selected as these polymorphisms have a different effect on the activity and stability of the enzyme,and significantly diverse effects have been reported in relation to the association with *** approval was acquired from the IRB at King Saud University(KKUH),Saudi Arabia,and written informed consent was obtained from each *** study group comprised of 100 Saudi women with unexplained RSA and 100 age-matched controls,both attending KKUH for a routine *** was drawn in EDTA tubes,and DNA was *** was conducted using TaqMan SNP genotyping assay *** frequency of the T allele of C677T was 0.165 in patients and 0.17 in *** frequencies for CC,CT and TT genotypes were 70%,27%and 3%,respectively in RSA,and 71%,24%and 5%,respectively,in the controls(p>0.05).for the A1298C polymorphism,the C allele frequencies were 0.345 in patients and 0.28 in controls,while genotype frequencies for AA,AC and CC genotypes were 44%,43%,and 13%,respectively,in patients,and 54%,36%,and 10%,respectively,in controls(p>0.05).The frequency of CC genotype and C allele of A1298C were higher in the patients with RSA,but not significantly,while C677T genotypes and allele frequencies did not differ between patients and *** results suggested that MTHfR gene polymorphisms are population-specific and may not associate with RSA in Saudi women.

AIM:To share the results of a national screening program for amblyopia in school children in the north of ***:This is a prospective national screening study for *** program rolls first and second-grade children(6 to 7 years old) in the north of *** eye examination included:best-corrected visual acuity,cover-uncover test,and cycloplegic *** visual acuity was tested using an ETDRS visual acuity chart without ***,children were tested with full cycloplegic refraction when the test criteria were *** amblyopia was defined as a best-corrected visual acuity difference of 2 or more *** comparison,bilateral amblyopia was defined as a best-corrected visual acuity of 20/40 or worse in the best ***:The prevalence of amblyopia for the total sample tested(n=17 203) was 2.78%(n=479).The most common cause of amblyopia was hypermetropia(64.45%),followed by previous ocular surgeries(15.1%),myopia(10.43%),strabismus(9.39%),and congenital cataract(0.63%).CONCLUSION:This is the first and only study,identifing modifiable risk factors in Jordanian children with *** their first couple of years of elementary education,many Jordanian children are affected by amblyopia and pass unnoticed.A more governmental effort is needed into screening programs to improve vision in the Jordanian population.

Comparative Study on five Sudanese Date (<i>Phoenix dactylifera</i>L.) fruit Cultivars

The date palm (Phoenix dactylifera L.) is one of mankind’s oldest cultivated plants. A date palm fruit is an important component of the diet in most of the hot arid and semi arid regions of the world. The present study aimed at investigating the physical and chemical properties as well as microbiological characteristics of five date palm cultivars cultivated in Sudan. The results showed that the physical characteristics like fruit weight, length, flesh thickness, seed weight differed significantly between the various cultivars. Although most of the cultivars had similar chemical components, there were some few differences. However, the microbiological analyses proved that no significant differences were found among the cultivars.

The α-target semimicroscopic single folding potentials have been derived by folding a composite (repul-sive and attractive) effective α-α interaction with the α-cluster distribution density in the target nuclei. The obtained potentials are considered as the real part of the nuclear optical model potentials, while the imaginary parts are phe-nomenologicaly expressed using the Woods-Saxon form. Nine sets of measured experimental data of the 4He+12C and 4 He+16O elastic rainbow scattering over the energy range 80-240 MeV are analyzed using the obtained potentials. The data are successfully reproduced using the extracted potentials. The resulted reaction cross sections are also investigated and compared with the available corresponding data.

Brain CT findings in first Episode Depression in Older Adults

Introduction:?Different studies reported the strong relationship between cerebrovascular changes (CVC) and depression. Moreover, many researchers tried to correlate between the pathogenesis and location of the CVC and severity of depression. Methods: In this study we looked for the possibility of presence of CVC in patients with an age ranging from 60 - 75 years, presented to psychiatric clinics and hospitals in Hail, Saudi Arabia and diagnosed for the first time with depression. All patients have been diagnosed neither with cerebral attacks (CA) nor with depression. A cross sectional study was done on 149 patients. Hamilton rating scale (HRS) for depression was used to evaluate the severity of depression before staring any treatment. National institutes of health stroke scale (NIHSS) was applied for all the patients to exclude any possibility of CA. Brain CT was done to all patients to detect an evidence of CVC. Results: According to the CT findings, the patients were divided into four groups: Group 1 with normal CT findings;Group 2 with ischemic changes;Group 3 with single lacunar infraction;and Group 4 with multiple lacunar infarctions. After analyzing the results and correlating the severity of depression and CT findings in the four different groups we found significance in the correlation between severity of depression and the presence of minor strokes. Conclusions: A first episode of depression in older adult patients may indicate the presence of minor or silent stroke or CA.

Therapeutic Modalities in Diabetic Nephropathy: <i>future Approaches</i>

Diabetes mellitus is the leading cause of end stage renal disease and is responsible for more than 40% of all cases in the United States. Several therapeutic interventions for the treatment of diabetic nephropathy have been developed and implemented over the past few decades with some degree of success. However, the renal protection provided by these therapeutic modalities is incomplete. More effective approaches are therefore urgently needed. Recently, several novel therapeutic strategies have been explored in treating DN patients including Islet cell transplant, Aldose reductase inhibitors, Sulodexide (GAC), Protein Kinase C (PKC) inhibitors, Connective tissue growth factor (CTGf) inhibitors, Transforming growth factor-beta (TGf-β) inhibitors and bardoxolone. The benefits and risks of these agents are still under investigation. This review aims to summarize the utility of these novel therapeutic approaches.
