Cyanoacrylate spray as treatment in difficult-to-manage gastrointestinal bleeding

Gastrointestinal bleeding can be a life-treating event that is managed with standard endoscopic therapy in the majority of cases. However, up to 5%-10% of patients may have persistent bleeding that does not respond to conventional measures. Several endoscopic treatment techniques have been proposed as strategies to control such cases, such as epinephrine injection, hemoclips or argon plasma coagulation, but there are certain clinical scenarios where it is difficult to achieve hemostasis even though adequate use of the available resources is made. Reasons for these failures can be associated with the lesion features, such as extent or location. The use of long-standing techniques in nontraditional scenarios, such as with cyanoacrylate for gastric varices sclerosis, has been reported with favorable results. Although new products such as TC-325 or Ankaferd Blood Stopper hemosprays may be useful, their formulations are not available worldwide. Here we present two clinical cases with very different scenarios of gastrointestinal bleeding, where the use of cyanoac-rylate in spray had favorable results in uncommon indications. Cyanoacrylate used as a spray is a technique that can be used as an alternative method in emergent settings.

Translational approaches: from fatty liver to non-alcoholic steatohepatitis

Over the past few decades, non-alcoholic fatty liver disease(NAfLD) has become one, if not the most common,cause of chronic liver disease affecting both adults and children. The increasing number of cases at an early age is the most worrying aspect of this pathology, since it provides more time for its evolution. The spectrum of this disease ranges from liver steatosis to steatohepatitis, fibrosis and in some cases, hepatocellular carcinoma. NAfLD may not always be considered a benign disease and hepatologists must be cautious in the presence of fatty liver. This should prompt the use of the available experimental models to understand better the pathogenesis and to develop a rational treatment of a disease that is dangerously increasing. In spite of the growing efforts, the pathogenesis of NAfLD is still poorly understood. In the present article we review the most relevant hypotheses and evidence that account for the progression of NAfLD to non-alcoholic steatohepatitis(NASH) and fibrosis. The available in vitro and in vivo experimental models of NASH are discussed and revised in terms of their validity in translational studies. These studies must be aimed at the discovery of the still unknown triggers or mediators that induce the progression of hepatic inflammation, apoptosis and fibrosis.

Mixed Wastes Vitrification by Transferred Plasma

Thermal plasma technology provides a stable and long term treatment of mixed wastes through vitrification processes. In this work, a transferred plasma system was realized to vitrify mixed wastes, taking advantage of its high power density, enthalpy and chemical reactivity as well as its rapid quenching and high operation temperatures. To characterize the plasma discharge, a temperature diagnostic is realized by means of optical emission spectroscopy （OES）. To typify the morphological structure of the wastes samples~ scan- ning electron microscopy （SEM）, and X-ray diffraction （XRD） techniques were applied before and after the plasma treatment.

Assessment of Jordanian Radiologist Performance in the Detection of Breast Cancers

This study aims to monitor diagnostic accuracy amongst Jordanian mammography readers and identify parameters linked to higher levels of performance. In this study, we have used the Breast Screen Reader Assessment Strategy (BREAST) platform to facilitate 27 radiologists in reading a case set of 60 digital mammograms, 20 of which included cancers. Each case consisted of the four standard cranio-caudal (CC) and medio-lateral oblique (MLO) projections. All radiologists were registered to read mammograms at their workplace by the Jordanian Ministry of Health. Each reader was asked to locate any malignancies, provide a confidence rating using a scale of 1 - 5, and identify the type of appearance. All images were displayed using 8 MP monitor, supported by radiology workstations with full image manipulation facilities. Results were evaluated using Jackknife Alternative free-Response Receiver Operating Characteristic (JAfROC). Demographics obtained from each radiologist regarding their experience, qualifications and breast-reading activities were correlated against JAfROC scores using Spearman techniques. The results showed that the mean JAfROC score was 0. 52 (95% confidence interval (CI): 0.46, 0.58);location sensitivity score was 0. 41 (95% CI: 0.41, 0.56);specificity score was 0.73 (95% CI: 0.68, 0.83). Higher performance in term of JAfROC scores was directly related to number of years since professional qualification (r = 0.433;p = 0.024), number of years reading breast images (r = 0.62;r = 0.001) and number of mammography images read per year (r = 0.69;p = 0.001). On the other hand, higher performance was inversely linked to the frequency of reading other modalities per week (r = -0.48;p = 0.010). No other statistical differences were significant. finally, higher radiologists’ performance in cancer detection is correlated with increasing the number of mammograms reads per week.

Use of Platelet Rich Plasma (PRP) in the Treatment of Acute Post Intubation Tracheal Rupture: Report of Two Cases

The use of PRP involves a complex network of molecular events which accelerate tissue regenera-tion due to its richness in growth factors plus a variety of biologic mediators. Several communica-tions tell us about its usefulness on the dental implantology field, plastic surgery, orthopedics and peripheral nerve regeneration after trauma. We have presented our successful experience of its use in pneumology when treating severe hemoptysis, respiratory fistulae, spontaneous pneumothorax and one tracheal rupture. In this article we present two cases of post intubation tracheal rupture successfully treated with the local instillation of PRP on one of them and with the injection of PRP along the lips of the tracheal wound on the other. Tracheal rupture is a rare but life threatening complication of emergency intubation requiring an effective response in order to avoid the risk of patient death due to secondary mediastinitis and sepsis. Up to now there is no general consensus in the treatment of this condition and alternatives in use are not universally accepted. The use of local autologous PRP in our experience has demonstrated a favorable performance in such cases, turning it in a highly promising tool for the treatment of conditions such as this, in which a rapidly effective and minimally invasive handling is required.

Effect of Copper and Iron on Acidogenic Biomass in an Anaerobic Packed Bed Reactor

The aim of this study was to evaluate the effect of copper and iron on acidogenic biomass immobilized on clinoptilolite in an anaerobic packed bed reactor. Copper and iron were fed to the reactor at concentrations of 100 and 300 mg·L-1, respectively. Both metal ions had insignificant inhibitory effect over the metabolism of the biomass, specifically, on substrate consumption and production of volatile fatty acids (VfAs). The microstructural characterization of the biofilm by Scanning Electron Microscopy showed no effect on the morphology of the microorganisms after the metals treatment. Copper and iron removal was also measured in the reactor, achieving a breakthrough time of 3 days, during which removal efficiencies were higher than 90%. It was also observed that the biomass had a greater affinity for copper. The results indicate that acidogenic biomass can be used effectively as a sorbent agent. The pollution of river-streams with heavy metals—mainly copper and iron—was one of the most compelling motivations for conducting this investigation. The San Pedro River Basin, a trans-boundary river that originates near the mining town of Cananea, Sonora, México, and flows to Arizona, USA, exemplifies this environmental issue.

Ethanol consumption as inductor of pancreatitis

Alcohol abuse is a major cause of pancreatitis, a condition that can manifest as both acute necroinflammation and chronic damage (acinar atrophy and f ibrosis). Pancreatic acinar cells can metabolize ethanol via the oxidative pathway, which generates acetaldehyde and involves the enzymes alcohol dehydrogenase and possibly cytochrome P4502E1. Additionally, ethanol can be metabolized via a nonoxidative pathway involving fatty acid ethyl ester synthases. Metabolism of ethanol by acinar and other pancreatic cells and the consequent generation of toxic metabolites, are postulated to play an important role in the development of alcohol-related acute and chronic pancreatic injury. This current work will review some recent advances in the knowledge about ethanol actions on the exocrine pancreas and its relationship to inflammatory disease and cancer.
