Prognostic factors for Diabetic foot at CNHU-HKM

Introduction: Predictions on the prevalence of diabetes mellitus, according to the International Diabetes federation, indicated 9.3% in 2019 and nearly 10.9% of the general population in 2045. In Benin, the increase in this prevalence, according to the World Health Organization (WHO), is constantly increasing. Diabetic foot is one of its most common complications. The aim of this work was to study the prognostic factors of diabetic foot in the Department of Endocrinology, Metabolism and Nutrition of the CNHU-HKM of Cotonou. Patients and method: This is a descriptive and analytical retrospective study of the prognostic factors of diabetic foot over a period of 3 years from January 2019 to December 2021 in patients who have been hospitalized or followed on an outpatient basis for diabetic foot in the Endocrinology, Metabolism and Nutrition Department of the CNHU-HKM of Cotonou. Results: A total of 112 patients were included in this study. The average age of the patients was 59.70 ± 2.10 years. A male predominance was noted with a sex ratio (M/f) of 1.7. Mixed gangrene and phlegmons were the most common lesions. According to the classification of diabetic feet according to the University of Texas, 59.1% of patients had a 100% risk of amputation. Ten patients died from sepsis (8.9%). The average blood glucose on admission was 2.74 ± 0.23 g/l, reflecting the glycemic imbalance in these patients. There is a statistically significant association between the duration of progression of diabetes, the type of lesion and amputation. Patients whose diabetes has lasted more than 30 years and patients who are not monitored have a greater risk of death. Conclusion: Diabetic patients most often consulted at a late stage, compromising conservative treatment. The duration of diabetes and the type of lesion on admission were the main factors leading to amputation, thus compromising the functional prognosis. Neonatal dermatoses (NND) are frequent, varied and of variable prognosis. The objective of this work was to study the epidemiological and clinical aspects of NND at the NTH-HKM. Methods: This was a cross-sectional descriptive and analytical study from December 1, 2014 to february 28, 2015. All newborns received in the Department of Pediatrics and Medical Genetics and with dermatosis were included. The diagnosis of dermatoses was clinical. Results: During the study period, 355 newborns had at least one NND on a total of 580 newborns received, a prevalence of 61.2%. The sex ratio was 1.54 and the average age was 3.11 days. Transient dermatoses were more frequent (80%), dominated by desquamation + xerosis (33.75%). Congenital melanocytic nevi (40.74%) and malformations and vascular tumors (18.52%) were the most noted pathological neonatal dermatoses. Only age was statistically associated with NND. Conclusion: DNH is common in newborns at the NTH-HKM. They were mostly transitory. Pathological conditions should be treated where appropriate.

Introduction: The dietary changes and the new lifestyles which occurred in low-income countries like Benin resulted in a nutritional transition with its burden of overweight and obesity among the child and adolescent population who is still suffering from undernutrition. This study aimed to identify the factors associated with overweight and obesity in the schools of Parakou in 2017. Patients and Methods: This research work is a cross-sectional descriptive and analytic study carried out from May to June 2017 in the state-owned and private high/secondary schools of Parakou. It focused on a representative sample of 730 high school students randomly selected according to the WHO two-stage cluster sampling technique. The weight and size of each of the students included in the study were measured;and then their body mass index (BMI) was calculated. The data related to respondents’ dietary pattern and lifestyle were collected. Those data were processed using the software Epi Info version 7.2.1. The prevalence ratio (PR) was used to measure associations with their confidence interval and the difference was considered significant when p findings: In total, 730 students were involved in the study, including 377 male i.e. a ratio of 1.1. Prevalence of overweight/obesity was estimated at 16.2% including 12.5% of overweight and 3.7% of obesity. The factors associated with overweight and obesity were female sex, excessive intake of sugar-sweetened beverages and drinks, low level of physical activity and family history of obesity. Conclusion: A behavior change regarding the different contributing factors is necessary to reverse the trends in overweight and obesity prevalence in the high/secondary schools in the District of Parakou.

Diabetic retinopathy (DR) is one of the microvascular complications of diabetes. The aim of this study was to analyze the risk factors associated with the occurrence of diabetic retinopathy (DR) at the National University Hospital Center-Hubert Koutoukou MAGA (CNHU-HKM). Patients and method: this was a descriptive and analytical cross-sectional study with prospective data collection. It was carried out over a three-month period from July 10 to October 10, 2019. It concerned all patients suffering from diabetes mellitus and who consulted in the Endocrinology department during the study period. Results: the frequency of diabetic retinopathy was 30.46% (53/174 patients). A female predominance was observed with a sex ratio (M/f) of 0.57. The main modifiable risk factors were represented by occupation and those that were not modifiable were represented by the presence of old nephropathy, glycated hemoglobin, age of discovery of diabetes and its length of service. Diabetic retinopathy is a serious condition that can ultimately lead to blindness. The frequency of DR remains high in our study.

As in other countries of West Africa, Benin has a low rate of MCM utilization. The factors which lead to this low rate of use vary from one country to another. Purpose: The purpose of this work was to determine the factors associated with the use of modern contraceptive method (MCM) by women of childbearing age (WCBA) in Parakou in 2017. Materials and Methods: It was a cross-sectional and descriptive study with an analytical purpose among WCBA selected in households of the municipality of Parakou after a 2-degree cluster survey. Results: The WCBA’s average age was 29 ± 5.6 years old with extremes of 15 and 46 years. The modern contraceptive prevalence was 14.74%. Among WCBA, 57.14% had an average level of knowledge. The media were the main source of information (75.84%). Among them 34.39% never discussed the matter with their spouse;the latter were not in favor of contraception. The educational level, the perception of woman, the spouse’s favorable opinion, the discussion with the spouse, the level of knowledge of MCM and the knowledge of the existence of fP service were significantly associated with the use of MCM with p = 0.0000 after multivariate analysis. Conclusion: The level of MCM use still remains low. Any effort to increase the modern contraceptive prevalence should take into account its associated factors in order to optimize the use of MCM in the municipality of Parakou in 2017.
