Patient Quality-of-Life, Providers with Incentive Payments, Real-World Evidence, and Reducing Healthcare Costs through New Innovations

Background: Value-Based Care is a program that was created by Professor Michael E. PORTER and Elizabeth O. TEISBERG which has been modified for implementation over the last seventeen years. This new program is paid by Commercial Health Insurers (Insurance Companies) and Centers for Medicare and Medicaid Services (Medicare). This program incorporates the patients, doctors, Life Sciences Companies, Insurance Companies, Holistic practice, traditional Chinese medicine, etc. with alternative traditional treatments and therapies for Real-World Evidence based and value-based treatment customized to patients’ needs and the patients benefit. Methods: The program was proposed to achieve value for patients and an improved healthcare system which is especially needed to provide treatment for patients in Serious or Life-threatening conditions or diseases. The program will accurately measure the outcomes of patients and provide a wider choice of treatments that matter to patients, also to measure the cost that successful alternative treatments provide which may lower the cost to Medicare and Insurance Companies. Conclusion: The proposed and now implemented program is to achieve value for patients. Golden Sunrise Nutraceutical (Golden Sunrise) provides a New Medical Innovation and treatment alternative to patients suffering from Serious or Life-threatening illnesses. By using Golden Sunrise Innovation and with the incorporation of Value-based Care patients will become the drivers or it could be said the patient Ambassadors of their own health. By incorporating Golden Sunrise Innovation, the doctors will have more diversity of treatments available for their patients. Payment should reflect value not volume, and the New Medical Innovation Golden Sunrise offers will help in reducing the costs to Insurance Companies accelerating the adoption of Value-Based Care.

Lipopolysaccharide immune stimulation but not β-mannanase supplementation affects maintenance energy requirements in young weaned pigs

Background: Pathogen or diet-induced immune activation can partition energy and nutrients away from growth,but clear relationships between immune responses and the direction and magnitude of energy partitioning responses have yet to be elucidated. The objectives were to determine how β-mannanase supplementation and lipopolysaccharide(LPS) immune stimulation affect maintenance energy requirements(MEm) and to characterize immune parameters, digestibility, growth performance, and energy ***: In a randomized complete block design, 30 young weaned pigs were assigned to either the control treatment(CON; basal corn, soybean meal and soybean hul s diet), the enzyme treatment(ENZ; basal diet + 0.056% β-mannanase),or the immune system stimulation treatment(ISS; basal diet + 0.056% β-mannanase, chal enged with repeated increasing doses of Escherichia coli LPS). The experiment consisted of a 10-d adaptation period, 5-d digestibility and nitrogen balance measurement, 22 h of heat production(HP) measurements, and 12 h of fasting HP measurements in indirect calorimetry chambers. The immune chal enge consisted of 4 injections of either LPS(ISS) or sterile saline(CON and ENZ), one every48 h beginning on d 10. Blood was collected pre-and post-challenge for complete blood counts with differential,haptoglobin and mannan binding lectin, 12 cytokines, and glucose and insulin ***: Beta-mannanase supplementation did not affect immune status, nutrient digestibility, growth performance,energy balance, or MEm. During a terrorist attack on a supermarket,the use of emergency exits is essential for effective evacuation and saving ***,people tend to ignore emergency *** behavior can lengthen evacuation times,endanger individuals,and even prove *** this context,we conducted a series of experiments to explore the links between cognition and the dynamics of human capabilities in a complex and changing *** a series of behavioral experiments and computer simulations,we found that active guidance by green flashing lights at emergency exits impacts the behavior of individuals in an emergency evacuation situation in a supermarket;this tested our hypothesis that changing the environment in turn changes the evacuation behavior of *** also show that environmental modification can help in decision-making in an emergency ***,the results of computer simulations support a possible modeling of the influence of affordances on the evacuation behavior of agents in a complexvirtual environment.

Environmental chemical stressors as epigenome modifiers:a new horizon in assessment of toxicological effects

In eukaryotic cells, chromatin transformation from euchromatin into heterochromatin as a means of controlling gene expression and replication has been known as the ‘‘accessibility hypothesis' '. The interplay of epigenetic changes including histone modifications, DNA methylation, RNA interference(RNAi) and other func- tional epigenetic components are intricate. It is believed that these changes are well-programmed, inherited and can be modified by environmental contaminant stressors. Environmentally-driven epigenetic alterations during development, e.g. embryonic, foetal or neonatal stage, may influence disease susceptibility in adulthood. Therefore, understanding how epigenome modifications develop in response to environmental chemicals and, how epigenetic-xenobiotic interactions influence human health will shed new insights into gene-environment interactions in the epidemiology of several diseases including cancer. In this review, we consider studies of chemical modifiers including nutritional and xenobiotic effects on epigenetic components in vitro or in vivo. By examining the most-studied epigenome modifications and how their respective roles are interlinked, we highlight the central role of xenbiotic- modified epigenetic mechanisms. A major requirement will be to study and understand effects following environmentally-relevant exposures. We suggest that the study of epigenetic toxicology will open up new opportunities to devise strategies for the prevention or treatment of at-risk populations.

The systemic processes involved in the manifestation of life・threatening COVID-19 and in disease recovery are still incompletely understood,despite investigations focusing on the dysregulation of immune responses after SARS-CoV-2 *** define hallmarks of severe COVID-19 in acute disease(n=58)and in disease recovery in con valesce nt patie nts(n=28)from Han nover Medical School,we used flow cytometry and proteomics data with unsupervised clustering *** our observational study,we combined analyses of immune cells and cytokine/chemokine networks with endothelial activation and *** patients displayed an altered immune signature with prolonged lymphopenia but the expansion of granulocytes and plasmablasts along with activated and terminally differentiated T and NK cells and high levels of SARS-CoV-2-specific *** core signature of seven plasma proteins revealed a highly inflammatory microenvironment in addition to endothelial injury in severe *** within this sign ature were associated with either disease progression or *** summary,our data suggest that besides a strong inflammatory response,severe COVID-19 is driven by endothelial activation and barrier disruption,whereby recovery depends on the regeneration of the endothelial integrity.

Natural organic matter(NOM)is known to play an important role in the transport and binding of trace metal elements in aquatic and soil *** is a pollutant for which the extent of the role played by NOM is poorly ***,this study investigates thallium(Ⅰ)and its complexation to a purified humic substance as proxy for *** were performed with the Donnan Membrane Technique to separate,for the first time,the free Tl^+ion from its complexed form in the bulk *** pH and concentrations were investigated at constant ionic strength and constant NOM proxy concentrations in *** results were described with NICA-Donnan *** complexation was compared to silver complexation using literature data and using the same NICA-Donnan *** for these two cations(Tl^+and Ag^+)are reported in this article,for the first *** display low thallium complexation to the NOM proxy while silver competes with divalent cations for the NOM binding *** speciation for dissolved thallium highlights the dominance of free thallium(Tl^+)in solution whereas Tl-NOM complexes contribute roughly 15%to total Tl(Ⅰ)species in river and lake type *** results are obtained for soil solutions,Tl-bound to NOMf total,from UK soils with different land use and geochemistry.

Huu S. TIEU Request for fDA to Establish Regenerating Human Cells as Law on December 13, 2016 President Barack H. Obama Signed the 21st Century CURES Act into fDA Regulation and Law

Background and Aims: On November 24, 2009, Huu S. TIEU and Golden Sunrise Pharmaceutical, Inc. (Golden Sunrise) applied for the Technology and Innovation to be reviewed and evaluated by the U.S. food and Drug Administration (fDA). In the review and evaluation, it was requested by Golden Sunrise designated the new indications for this application under Serious or Life-threatening conditions or diseases. Discussions followed with the fDA, Huu S. TIEU, and Golden Sunrise for fDA approval on new products and new indications on existing new Medical Technology and Innovation. It was agreed in Year-2015 that the fDA would take the request for new indications to the United States Congress to establish into fDA regulation and law. At that time the following was the fDA Guidance—“Emergency Use of a Test Article” is exempt from prior Institutional Review Board or Advisory Committee evaluation and approval, provided that such emergency use is reported to the Institutional Review Board within five working days after use. Expedited Institutional Review Board or Advisory Committee approval is not permitted in emergency use. There has been no funding to the authors for the writing or publication of this article. Methods: It was requested by Huu S. TIEU and Golden Sunrise in documents given to the fDA to have Serious or Life-threatening conditions or diseases indication be recognized by law. On August 08, 2015, the fDA responding to this request took the documentation produced by Golden Sunrise to the United States Congress on behalf of Golden Sunrise and Huu S. TIEU. This article encompasses the fDA regulatory method as well as the discussion and results of the establishment of the fDA and the 21st Century Cures Act. Results: On December 13, 2016, H.R.34—114th United States Congress (2015-2016) 21st Century CURES Act was signed into law by President Barack H. Obama which included the Serious or Life-threatening indication to be written into the CURE

Analysis of new bone,cartilage,and fibrosis tissue in healing murine allografts using whole slide imaging and a new automated histomorphometric algorithm

Histomorphometric analysis of histologic sections of normal and diseased bone samples,such as healing allografts and fractures,is widely used in bone ***,the utility of traditional semi-automated methods is limited because they are labor-intensive and can have high interobserver variability depending upon the parameters being assessed,and primary data cannot be re-analyzed *** histomorphometry has long been recognized as a solution for these issues,and recently has become more feasible with the development of digital whole slide imaging and computerized image analysis systems that can interact with digital ***,we describe the development and validation of an automated application（algorithm）using Visiopharm＇s image analysis system to quantify newly formed bone,cartilage,and fibrous tissue in healing murine femoral allografts in high-quality digital images of H＆E/alcian blue-stained decalcified histologic *** validate this algorithm,we compared the results obtained independently using OsteoMeasureTM and Visiopharm image analysis *** intraclass correlation coefficient between Visiopharm and OsteoMeasure was very close to one for all tissue elements tested,indicating nearly perfect reproducibility across *** new algorithm represents an accurate and labor-efficient method to quantify bone,cartilage,and fibrous tissue in healing mouse allografts.
