Side effects and acceptability measures for thermal ablation as a treatment for cervical precancer in low-income and middle-income countries:a systematic review and meta-synthesis

Objective Understanding the side effects and acceptability of thermal ablation(TA)is necessary before large-scale application in screen-and treat programmes can be justified in low-income and middle-income countries(LMICs).Design Articles were selected for inclusion by two independent reviewers.Risk of bias was assessed using the Downs and Black’s criteria.Summary data were extracted,and authors contacted for data when necessary.Proportions of interest and 95%CIs were estimated using a random effects model.Subgroup analysis was performed based on place of treatment and timing of post-treatment follow-up.Heterogeneity was estimated using the I^(2).Eligibility criteria Studies that reported one or more side effects or patient acceptability measures after treatment of the cervix using TA in women living in LMICs who completed a cervical cancer screening test.Included articles were clinical trials or observational studies available in English and published before 18 December 2020.Information sources Ovid MEDLINE,EMBASE,CINAHL,CAB Global Health and WHO Global Index Medicus were searched for this systematic review and meta-synthesis.Results A total of 1590 abstracts were screened,84 full text papers reviewed and 15 papers selected for inclusion in the qualitative review,10 for meta-synthesis(N=2039).Significant heterogeneity was found in screening tests used to identify women eligible for TA and in methods to ascertain side effects.The most commonly reported side effect during treatment was pain(70%,95%CI 52%to 85%;I^(2)=98.01%)(8 studies;n=1454).No women discontinued treatment due to pain.At treatment follow-up,common side effects included vaginal discharge(72%,95%CI 18%to 100%;I^(2)=99.55%)(5 studies;n=771)and bleeding(38%,95%CI 15%to 64%;I^(2)=98.14%)(4 studies;n=856).Satisfaction with treatment was high in 99%(95%CI 98%to 100%;I^(2)=0.00%)of women(3 studies;n=679).Conclusions TA results in a number of common side effects,though acceptability remains high among women treated in LMICs.Standardised side effect and acc

Epidemiological, Clinical Progress Aspects of Congenital Heart Disease with Neonatal Revelation at the Mother-Child Hospital of Bingerville (HME) Concerning 98 Cases from January 2021 to December 2022 (Côte d’Ivoire)

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly imprecise and varies from one region to another due to the inequality of diagnostic facilities. The aim of this study was to determine the in-hospital prevalence of congenital heart disease in children at the Mother-Child hospital of Bingerville (HME) by specifying the diagnostic, therapeutic and evolutionary aspects. Materials and methods We conducted a retrospective, descriptive, cross-sectional study at HME of Bingerville from January 2021 to December 2022. All newborns with congenital heart disease confirmed by echocardiography were included in the study. Results Of 656 admissions to the neonatology department over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a mean chronological age of 5.18 days and extremes of 0 and 46 days. There were as many male patients (50%) as female (50%), i.e. a sex ratio of 1. These newborns were premature in 60.2% of cases, with a mean and median gestational age of 34 weeks’ amenorrhea. Most were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predominated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), common trunk artery (CTA) (3.1%) and open septal pulmonary atresia (OSPA) (1%) as the primary cyanogenic heart disease. Pulmonary arterial hypertension (PAH) (50%) was primary in 38.8% and secondary (61.2%). The mortality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatment was marked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively common at the Bingerville HME. Access to echocardiography should be facilitated in neonatology departments for rapid diagnosis and optimal management of congenital heart disease in newborns.

Early local intracoronary platelet activation after drug-eluting stent placement

Background Early local platelet activation after coronary intervention identifies patients at increased risk of acute stent thrombosis （AST）. However, early changes in platelet activation in coronary circulation following drug-eluting stent （DES） implantation have never been reported. Methods In a prospective study of 26 consecutive elective stable angina patients, platelet activation was analyzed by measuring soluble glycoprotein V （sGPV） and P-selectin （CD62P） before and after implantation of either DES or bare metal stent （BMS）. All patients were pretreated with clopidogrel （300 mg loading dose） and aspirin （75 mg orally） the day before the procedure. Blood samples were drawn from the coronary ostium and 10 - 20 mm distal to the lesion site. Results Consistent with the lower baseline clinical risk, the levels of CD62P and sGPV were within normal reference range, both in the coronary ostium and distal to the lesion before percutaneous coronary intervention （PCI） procedure. The levels of CD62P and sGPV did not change significantly （CD62P：（31.1 ± 9.86） ng/ml vs （29.5 ± 9.02） ng/ml, P=0.319 and sGPV：（52.4 ± 13.5） ng/ml vs （51.8 ± 11.7） ng/ml, P=0.674, respectively） after stent implantation when compared with baseline. Changes in these platelet activation markers did not differ between stent types. Conclusions Intracoronary local platelet activation does not occur in stable angina patients before and immediately followina DES implantation when dual anti-Dlatelet is administered.

Cardiovascular complications during hyperthyroidism are dominated by arrhythmias, heart failure and coronary heart disease. Pericardial effusion which is a common complication of hypothyroidism is extremely rare in hyperthyroidism. We report the case of a 45-year-old woman admitted for management of pleuro-pericarditis associated with atrial fibrillation in the context of cardiothyreosis. The treatment consisted of a pericardial puncture associated with synthetic anti-thyroid drugs and anti-tuberculosis drugs with positive clinical and echocardiographic outcomes.

To date, few studies have investigated the impact of organizational factors such as organizational status or the rank of firefighters on the development of posttraumatic stress disorder (PTSD) following a terrorist attack. To fill this gap in the scientific literature, this field study aimed to investigate the consequences of terrorist attacks on firefighters’ psychological health in terms of PTSD. Data were collected in France following two terrorist attacks. PTSD was assessed with the PCL-S (DSM-IV) 3 to 6 months after the events. Confirmatory factor analyses (CFAs) with existing PTSD models were inconclusive, leading us to find a two-factor model via an exploratory factor analysis (EFA). A cluster analysis showed different symptom profiles that were influenced by the exposure level. Elements for a structural model explaining PTSD symptoms are proposed and suggest a central role of the exposure level. Firefighters I/II represented an at-risk sub-population, suggesting that PTSD was mainly experienced among those who performed tasks not common to their occupation.

Epidemiology of anorexia nervosa in a French community-based sample of 39,542 adolescents

Purpose: To assess the prevalence of DSM-IV anorexia nervosa criteria, anorexia nervosa and subthreshold subgroups, and their incidence between the ages of 12 and 17 years using Cole’s international thinness cut-offs, and to characterize these anorexic adolescents by parental socioeconomic status and whether or not they reported receiving treatment. Method: In all, a representative sample of 39,542 French adolescents (19,658 girls and 19,884 boys) was recruited in a cross-sectional study in 2008. Anorexia nervosa DSM-IV diagnosis was determined by a self-administered questionnaire. Results: Among females, 0.5% (n = 105) met criteria for anorexia nervosa between the ages of 12 and 17 years, whereas among males, the prevalence was 0.03% (n = 6). In females, the prevalence of sub-threshold anorexia nervosa was found to be between 1.2% (n = 216) and 3.3% (n = 618);more than 75% were of the restrictive subtype. The highest incidence of anorexia nervosa was at 16 years. There was also a greater prevalence of sub-threshold anorexia nervosa subgroups among subjects with high parental socioeconomic status. More than half of the female adolescents who met the anorexia nervosa criteria reported receiving treatment for their disorder, versus 23% to 40% of the adolescents in the sub-threshold subgroups (P sample of adolescents. Using Cole’s international thinness cut-off could improve international comparability among studies. Adolescents from the higher socioeconomic categories were more likely to be anorexic.

Assessment of the Physicochemical and Microbiological Parameters of a Teaching Hospital’s Wastewaters in Abidjan in Côte d’Ivoire

This work deals with the physicochemical and microbiological characterization of a hospital wastewater that is directly discharged in water bodies without treatment. Our focus was paid on the teaching hospital of Treichville (Cote d’Ivoire). For the purpose, various physicochemical parameters such as temperature, pH, dissolved oxygen, total dissolved solid, conductivity, nitrate, phosphate, chloride, chemical oxygen demand (COD), biological oxygen demand for five days (BOD5), salinity, and total suspended solids have been assessed. For the microbiological investigations, the parameters consisting in Pseudomonas aeruginosa, Salmonella and total coliforms have been assessed. From the analysis, it has been found that the wastewaters of the teaching hospital of Treichville are highly loaded in organic pollutants and in pathogens bacteria. The values of nitrate, dissolved oxygen demand, COD, BOD5 and biological parameters do not respect the international (WHO) values recommended for the water to be discharged in the environment. The ratio COD/BOD5 has been determined to vary between 1.25 and 2.80. The results showed that the studied wastewater is a domestic type wastewater composed either by mostly biodegradable pollutants or a mixture of biodegradable and non-biodegradable organic pollutants. These wastewaters constitute therefore a risk for the populations since they are discharged in water bodies without any treatment and used by communities.
