2-D Statistical Damage Detection of Concrete Structures Combining Smart Piezoelectric Materials and Scanning Laser Doppler vibrometry

In the present study a new structural health monitoring (SHM) technique isproposed as well as a new damage index based on 2-D error statistics. The proposedtechnique combines the electromechanical impedance technique (EMI) which is based onthe use of piezoelectric Lead Zirconate Titanate (PZT) patches and Scanning LaserDoppler vibrometry (SLDv) for damage detection purposes of concrete structures andearly age monitoring. Typically the EMI technique utilizes the direct and inversepiezoelectric effect of a PZT patch attached to a host structure via an impedance analyzerthat is used for both the actuation and sensing the response of the PZT-Host structuresystem. In the proposed technique the attached PZTs are actuated via a function generatorand the PZT-Host structure response is obtained by a Scanning Laser Dopplervibrometer. Spectrums of oscillation velocity of the surface of the attached PZTs verticalto the laser beam versus frequency are obtained and are evaluated for SHM *** damage detection approach also includes the use of a damage index denoted asECAR (Ellipse to Circle Area Ratio) based on 2-D error statistics and is compared to theRoot Mean Square Deviation (RMSD) damage index commonly used in SHMapplications. Experimental results include ascending uniaxial compressive load ofconcrete cubic specimens, ascending three point bending of reinforced concrete beamspecimens and early age monitoring of concrete. Results illustrate the efficiency of theproposed technique in damage detection as well as early age monitoring as, in the firstcase, both severity and location of damage can be determined by examining the values ofdamage indices for each damaged state and in the early age monitoring case damageindices follow the strength gain curve.

Interferon-free regimens in patients with hepatitis C infection and renal dysfunction or kidney transplantation

Treatment of patients with chronic kidney disease(CKD) and chronic hepatitis C(CHC) differs from that used in the general CHC population mostly when glomerular filtration rate(GFR) is below 30 m L/min, as sofosbuvir, the backbone of several current regimens, is officially contraindicated. Given that ribavirin free regimens are preferable in CKD, elbasvir/grazoprevir is offered in CHC patients with genotype 1 or 4 and ombitasvir/paritaprevir and dasabuvir in genotype 1b for 12 wk. Although regimens containing peginterferon with or without ribavirin are officially recommended for patients with CKD and genotype 2, 3, 5, 6, such regimens are rarely used because of their low efficacy and the poor safety and tolerance profile. In this setting, especially in the presence of advanced liver disease, sofosbuvirbased regimens are often used, despite sofosbuvir contraindication. It seems to have good overall safety with only 6% or 3.4% of CKD patients to discontinue therapy or develop serious adverse events without drug discontinuation. In addition, sustained virological response(SvR) rates with sofosbuvir based regimens in CKD patients appear to be comparable with SvR rates in patients with normal renal function. Treatment recommendations for kidney transplant recipients are the same with those for patients with CHC, taking into consideration potential drug-drug interactions and baseline GFR before treatment initiation. This review summarizes recent data on the current managementof CHC in CKD patients highlighting their strengths and weaknesses and determining their usefulness in clinical practice.

Clinical impact of microbiome in patients with decompensated cirrhosis

Cirrhosis is an increasing cause of morbidity and mortality. Recent studies are trying to clarify the role of microbiome in clinical exacerbation of patients with decompensated cirrhosis. Nowadays, it is accepted that patients with cirrhosis have altered salivary and enteric microbiome, characterized by the presence of dysbiosis. This altered microbiome along with small bowel bacterial overgrowth, through translocation across the gut, is associated with the development of decompensating complications. Studies have analyzed the correlation of certain bacterial families with the development of hepatic encephalopathy in cirrhotics. In general, stool and saliva dysbiosis with reduction of autochthonous bacteria in patients with cirrhosis incites changes in bacterial defenses and higher risk for bacterial infections, such as spontaneous bacterial peritonitis, and sepsis. Gut microbiome has even been associated with oncogenic pathways and under circumstances might promote the development of hepatocarcinogenesis. Lately, the existence of the oral-gutliver axis has been related with the development of decompensating events. This link between the liver and the oral cavity could be via the gut through impaired intestinal permeability that allows direct translocation of bacteria from the oral cavity to the systemic circulation. Overall, the contribution of the microbiome to pathogenesis becomes more pronounced with progressive disease and therefore may represent an important therapeutic target in the management of cirrhosis.

Upgraded role of autophagy in colorectal carcinomas

Autophagy is a basic catabolic process closely asso-ciated with degradation of cellular components. The role of autophagy in colorectal cancer(CRC) remains controversial. The mechanism of autophagy has been identified as protecting mechanism against tumorige-nesis by isolation of damaged organelles or as cytopro-tective provides energy in hypoxic regions of CRC tumors. Mutations in proto-oncogenes, such as RAS and BRAF, have been associated with autophagy initiation through signaling pathways of BRAF/MEK/ERK and PI3 K/AKT/m TOR. A combination therapy of chemotherapeutic agents and autophagy inhibitors such as hydroxychloroquine or immunotherapy might represent a major step that could be evaluated as a putative novel therapeutic strategy in CRC patients.

MELD vs Child-Pugh and creatinine-modified Child-Pugh score for Dredicting survival in patients with decompensated cirrhosis

AIM: Model of End-stage Liver Disease (MELD) score has recently gained wide acceptance over the old Child-Pugh score in predicting survival in patients with decompensated cirrhosis, although it is more sophisticated. We compared the predictive values of MELD, Child-Pugh and creatinine modified Child-Pugh scores in decompensated cirrhosis. METHODS: A cohort of 102 patients with decompensated cirrhosis followed-up for a median of 6 mo was *** types of modified Child-Pugh scores estimated by adding 0-4 points to the original score using creatinine levels as a sixth categorical variable were ***: The areas under the receiver operating characteristic curves did not differ significantly among the four scores, but none had excellent diagnostic accuracy (areas:0.71-0.79). Child-Pugh score appeared to be the worst, while the accuracy of MELD was almost identical with that of modified Child-Pugh in predicting short-term and slightly better in predicting medium-term survival. In Cox regression analysis, all four scores were significantly associated with survival, while MELD and creatinine-modified Child-Pugh scores had better predictive values (c-statistics: 0.73 and 0.69-0.70) than Child-Pugh score (c-statistics: 0.65). Adjustment for gamma-glutamate transpeptidase levels increased the predictive values of all systems (c-statistics: 0.77-0.81). Analysis of the expected and observed survival curves in patients subgroups according to their prognosis showed that all models fit the data reasonably well with MELD probably discriminating better the subgroups with worse prognosis. CONCLUSION: MELD compared to the old Child-Pugh and particularly to creatinine-modified Child-Pugh scores does not appear to offer a clear advantage in predicting survival in patients with decompensated cirrhosis in daily clinical practice.

Study of the effect of switching speed of the a-SiC/c-Si(p)-based,thyristor-like,ultrahigh-speed switches,using two-dimensional simulation techniques

A parametric study for a series of technological and geometrical parameters affecting rise time of Al/aSiC/c-Si（p）/c-Si（n~＋）/Al thyristor-like switches,is presented here for the first time,using two-dimensional simulation *** varying anode current values in simulation procedure we achieved very good agreement between simulation and experimental results for the rising time characteristics of the switch.A series of factors affecting the rising time of the switches are studied *** factors among all others studied here,exerting most significant influence,of more than one order of magnitude on the rising time,are a-SiC and c-Si（p） region widths,validating our earlier presented model for device *** above widths can be easily varied on device manufacture *** also successfully simulated the rising time characteristics of our earlier presented simulated improved switch,with forward breakover voltage v（BF） = 11 v and forward voltage drop vF = 9.5 v at the ON state,exhibiting an ultra low rise time value of less than 10 ps,which in conjunction with its high anode current density values of 12 A/mm^2 and also cheap and easy fabrication techniques,makes this switch appropriate for ESD protection as well as RF MEMS and NEMS applications.

Review of the pharmacological management of hepatitis B viral infection before and after liver transplantation

The progress in treatment against hepatitis B virus(HBv)with the development of effective and well tolerated nucleotide analogues(NAs)has improved the outcome of patients with HBv decompensated cirrhosis and has prevented post-transplant HBv *** review summarizes updated issues related to the management of patients with HBv infection before and after liver transplantation(LT).A literature search using the PubMed/Medline databases and consensus documents was ***-transplant therapy has been initially based on lamivudine,but entecavir and tenofovir represent the currently recommended first-line NAs for the treatment of patients with HBv decompensated *** LT,the combination of HBv immunoglobulin(HBIG)and NA is considered as the standard of care for prophylaxis against HBv *** combination of HBIG and lamivudine is related to higher rates of HBv recurrence,compared to the HBIG and entecavir or tenofovir *** HBIG-free prophylactic regimens,entecavir and tenofovir should be the first-line *** choice of treatment for HBv recurrence depends on prior prophylactic therapy,but entecavir and tenofovir seem to be the most attractive ***,liver grafts from hepatitis B core antibody(anti-HBc)positive donors can be safely used in hepatitis B surface antigen negative,preferentially anti-HBc/anti-hepatitis B surface antibody positive recipients.

Objective:To determine the diagnostic value of ultrasound signs of urinary stones less than or equal to 10 mm and to determine clinico-radiological variants of ureteric ***:A total of 455 ultrasound investigations were performed in patients referring to emergency department with urolithiasis and symptoms suspected of ureteric colic between January 2021 and May *** addition to microscopic evaluation of urine sediment to detect different crystals and non-contrast spiral computed tomography to detect stones,B-mode and color Doppler sonography was performed to assess the presence of acoustic shadow(AS)and twinkle artifacts(TA)as possible signs of stone(s)in ***:While the sensitivity and specificity of AS and TA were higher than 90%in patients with stones greater than 5 mm;positive prognostic values of these parameters were found to be extremely low for stones with sizes of 1-3 mm with specificity and sensitivity values not exceeding 53%.The sensitivity and specificity of AS and TA in the upper and lower ureters were higher for stones greater than or equal to 5 than for compared to those less than 5 *** the same time,the diagnostic values of TA and AS for middle ureter stones were very *** most prevalent clinico-radiological variants of ureteric colic were types I,III,and v being observed in 39%,28% and 21% cases,***:Our results demonstrate that TA and AS parameters seem to have a very low sensitivity and specificity in the diagnosis of urinary stones less than 5 *** diagnostic value of TA and AS increase significantly in stones greater than or equal to 5 ***,clinicians need to be very careful for overestimating the diagnostic values of TA and AS for stones less than 5 mm and non-contrast spiral computed tomography must be the method of choice for patients presenting to emergency department with ureteric colic.
