Airway management is a skill necessary for healthcare providers who manage patients. Intubation is a skill that both anesthesiologists and intensivists use daily. We present a case of a neonate that developed significant worsening stridor after a successful intubation for a hernia repair. After 4 intubation attempts, with easy visualization of the vocal cords and unsuccessful placement of the endotracheal tube, the patient was emergently brought to the operating room for evaluation.

It has been shown before that liquids can slip at a solid boundary,which prompted the idea that parallel-surfaces bearings can be achieved just by alternating slip and non-slip regions in the direction of fluid *** amount of slip at the wall depends on the surface tension at the liquid-solid interface,which in turn depends on the chemical state of the surface and its *** the present study a heterogeneous surface was obtained by coating half of a circular glass disc with a coating repellant to glycerol.A rotating glass disc was placed at a known/calibrated distance and the gap was filled with *** the mobile surface moving from the direction of slip to non-slip region it can be theoretically shown that a pressure build up can be *** pressure gradient in the two regions is constant,similar to that in a Rayleigh step bearing,with the maximum pressure at the separation *** heterogeneous disc was placed on a holder supported by a load cell thus the force generated by this pressure increase can be measured *** were carried out at different sliding speeds and gaps and the load carried was measured and subsequently compared with theoretical *** allowed the slip coefficient to be evaluated.

Background: For migrating birds, stopover requires spending time and energy that otherwise could be allocated to flying. Thus, birds optimally refuel their subsequent migratory flight by reducing stopover duration or foraging activ-ity in food-rich environments. In coastal habitats, birds may forego refueling and take short stopovers irrespective of local food availability. Given the paucity of studies exploring how migrants adjust stopover behavior in response to temporal variation in food availability, especially in the Neotropics, we fixed radio tags to 51 Red-eyed Vireos(Vireo olivaceous) over two years at two sites on the coast of Mexico's Yucatan ***: We applied VHF radio tags during the fall of 2016 and 2017, and tracked birds using automatic and manual receiving units. We estimated stopover duration and activity levels(one site only) for between six and fifteen birds,depending on site and year. We measured fruit availability weekly along the net lanes where we captured birds. We used a generalized linear model to estimate the relationships between stopover duration/activity level and fruit den-sity, bird body mass and year. We interpreted relationships for the model with the lowest AICc ***: We found that approximately half of the birds departed on the same day they were captured. For the birds that stayed longer, we could not discern whether they did so because they were light, or fruit density was high. On the other hand, lighter birds were more active than heavier birds but only in one of the two ***: Given our results, it is unlikely that Red-eyed Vireos refuel along the Yucatan coast. However, they still likely need to recuperate from crossing the Gulf of Mexico, which may necessitate foraging more often if in poor body condition. If the birds then move inland then stopover should be thought of as a large-scale phenomenon, where habitats with different functions may be spread out over a broad landscape.

Human-induced habitat conversion and degradation,along with accelerating climatic change,have resulted in considerable global biodiversity ***,how local ecological assemblages respond to the interplay between climate and land-use change remains poorly ***,we examined the effects of climate and land-use interactions on butterfly diversity in different ecosystems of southwestern ***,we investigated variation in the alpha and beta diversities of butterflies in different landscapes along human-modified and climate *** found that increasing land-use intensity not only caused a dramatic decrease in butterfly alpha diversity but also significantly simplified butterfly species composition in tropical rainforest and savanna *** findings suggest that habitat modification by agricultural activities increases the importance of deterministic processes and leads to biotic *** land-use intensity model best explained species richness variation in the tropical rainforest,whereas the climate and land-use intensity interaction model best explained species richness variation in the *** results indicate that climate modulates the effects of land-use intensity on butterfly alpha diversity in the savanna *** also found that the response of species composition to climate varied between sites:specifically,species composition was strongly correlated with climatic distance in the tropical rainforest but not in the *** together,our long-term butterfly monitoring data reveal that interactions between human-modified habitat change and climate change have shaped butterfly diversity in tropical rainforest and *** findings also have important implications for biodiversity conservation under the current era of rapid human-induced habitat loss and climate change.

The last two decades have seen a rise of interest in the adoption and diffusion of agricultural technologies aimed at improving the sustainability of agricultural lands among smallholder farmers in developing *** papers set out to understand factors that influence the adoption of technologies that enhance soil carbon sequestration among smallholder farmers,using secondary data recorded in the World Overview of Conservation Approaches and Technologies(WOCAT)database from 45 to 50 smallholders'farmers in selected places in Kenya and Ethiopia respectively.A probit model was used to analyse whether socio-economic,institutional,off-farm income,technical know-how,farmers'percep-tions,and land use characteristics influences the adoption of technologies that enhance soil carbon *** results show that smallholder farmers that positively perceived net benefits of the soil carbon enhancing technologies were more likely to adopt such technologies that enhance soil carbon sequestration in both *** to off-farm income and land ownership with title deeds were also found to be positively associated with ***-farm income positively influences adoption among farmer with a moderate income(100-500US$per year)but not the rich(>500US$)*** to high level of skills and technical know-how required for implementing and maintaining a technology on the farm had a negative influence on *** shows that interventions,aimed at addressing specific factors such as inadequate skills and knowledge,change in perception among farmers,and off-farm income are likely to have the greatest impact in decisions relating to the adoption of the soil carbon enhancing practices among farmers in East Africa.

AIM To provide a "patient-normalized" parameter in the proximal forearm. METHODS Sixty-three cadaveric upper extremities from thirty-five cadavers were studied. A muscle splitting approach was utilized to locate the posterior interosseous nerve(pIN) at the point where it emerges from beneath the supinator. The supinator was carefully incised to expose the midpoint length of the nerve as it passes into the forearm while preserving the associated fascial connections, thereby preserving the relationship of the nerve with the muscle. We measured the transepicondylar distance(TED), pIN distance in the forearm's neutral rotation position, pronation position, supination position, and the nerve width. Two individuals performed measurements using a digital caliper with inter-observer and intraobserver blinding. The results were analyzed with the Wilcoxon-Mann-Whitney test for paired samples. RESULTS In pronation, the pIN was within two confidence intervals of 1.0 TED in 95% of cases(range 0.7-1.3 TED); in neutral, within two confidence intervals of 0.84 TED in 95% of cases(range 0.5-1.1 TED); in supination,within two confidence intervals of 0.72 TED in 95% of cases(range 0.5-0.9 TED). The mean pIN distance from the lateral epicondyle was 100% of TED in a pronated forearm, 84% in neutral, and 72% in supination. predictive accuracy was highest in supination; in all cases the majority of specimens(90.47%-95.23%) are within 2 cm of the forearm position-specific percentage of TED. When comparing right to left sides for TEDs with the signed Wilcoxon-Mann-Whitney test for paired samples as well as a significance test(with normal distribution), the p-value was 0.0357(significance-0.05) indicating a significant difference between the two *** This "patient normalized" parameter localizes the pIN crossing a line drawn between the lateral epicondyle and the radial styloid. Accurate pIN localization will aid in diagnosis, injections, and surgical approaches.

Adrenocortical carcinoma(ACC)is a rare malignancy with generally poor outcomes and limited treatment *** surgical resection can be curative for early local disease,most patients present with advanced ACC owing to nonspecific *** those patients,treatment options include systemic chemotherapy and locoregional therapies including radiofrequency ablation and transarterial *** present the first reported case of utilizing yttrium-90 microsphere selective internal radiation therapy(SIRT)in combination with first line EDp-M(Etoposide,Doxorubicin,Cisplatin,Mitotane)chemotherapy and debulking surgical primary tumor resection for treatment of metastatic *** complete radiologic response has been maintained after twelve months with resolution of clinical *** findings prompt the need for further consideration and studies to elucidate the role of SIRT in combination with systemic and surgical treatment for metastatic ACC.

Objective: to identify the correlation between N-terminal pro-brainnatriuretic peptide （NT-proBNp） with diastolic function in asymptomaticpatients at risk of developing heart failure, and the role of NT-proBNp inidentifying diastolic dysfunction. Methods: eighty patients （age45–70years） were selected based onhistory of hypertension, diabetes mellitus, and coronary artery *** with at least one risk factor for HF were selected in this *** were enrolled in this study from January to July2011in FirstAffiliated Hospital of Chongqing Medical University. Left ventricular （LV）diastolic function was measured by conventional Doppler and TissueDoppler imaging （TDI） echocardiography of mitral annulus. plasmaNT-proBNp concentration was measured by immunoassay. Results: There were36men and44women in this study. The mean age ofpatients was58.8years old. Hypertension was present in78/80patients（97.5%）;diabetes in32/80patients （40%）;and coronary artery disease in 25/80（31.3%）. In transmitral pulsed-wave Doppler velocity measurement,there was no significant difference of NT-proBNp between normal andimpaired relaxation （mild diastolic dysfunction）. patients withpseudonormal filling （262.2±55.63pg/ml） and restrictive like pattern（781.5±152pg/ml） had significantly higher NT-proBNp level （p＜0.0005）.In TDI measurement, patients were grouped into3groups based on E/E’ratio ＜8（group1）, E/E’ ratio8-15（group2）, E/E’ ratio＞15（group3）. TheNT-proBNp level in group1was47.54±34.08pg/ml, group2was94.98±78.88pg/ml, and group3was781.5±152pg/ml. There was goodcorrelation between NT-proBNp and E/E’lateral （r=063;p＜0.0005）. Conclusion: NT-proBNp level increases as left ventricular diastolicfunction decreases, and there was good correlation between NT-proBNpwith E/E’ ratio. patient with moderate-severe diastolic dysfunction hadhigher NT-proBNp level. NT-proBNp cannot be used for identification ofmild diastolic dysfunction in patient at risk of developing heart failure.

Using spin-assisted ab initio random structure searches,we explore an exhaustive quantum phase diagram of archetypal interfaced Mott insulators,***-iron and lanthanum-titanium *** particular,we report that the charge transfer induced by the interfacial electronic reconstruction stabilises a high-spin ferrous Fe2+*** provide a pathway to control the strength of correlation in this electronic state by tuning the epitaxial strain,yielding a manifold of quantum electronic phases,***-Hubbard,charge transfer and Slater insulating ***,we report that the electronic correlations are closely related to the structural oxygen octahedral rotations,whose control is able to stabilise the low-spin state of Fe2+at low pressure previously observed only under the extreme high pressure conditions in the Earth’s lower ***,we provide avenues for magnetic switching via THz radiations which have crucial implications for next generation of spintronics technologies.
