Clinical Judgment Development: Six Steps to Establish Professional Identity and Role-Specific Competencies in Caring for a Sexual Assault Survivor

Background: Clinical judgment is a specific role that establishes a professional identity. The purpose of this paper is to prepare nursing students to make better judgments in the clinical setting and realign learning and teaching. methods: We used six steps to arrive at a competent clinical judgment suggested by the National Council State Board of Nursing (NSCBN) as a clinical judgment model 1) recognizing cues, 2) analyzing cues, 3) prioritizing hypotheses, 4) generating solutions, 5) taking an action, and 6) evaluating outcomes during the head-to-toe examination of the patient. Results: The primary outcomes are stabilization of the hemodynamics of the patient and prevention of further blood loss. Fluids are being given to help keep the vascular volume from being depleted, but they cannot solve the underlying problem. Continued assessment, intervention, and monitoring of vital signs through the course of the hospital stay ending with the patient’s discharge. Discussion: Survivors of sexual assault are unique for a nurse to provide care. The nurse needs to assess, intervene, monitor, and pay attention to detail of the 6 steps to clinical judgment, resulting in positive outcomes for their patient. Conclusions: Forensic nursing is a field of nursing that focuses on sexual assault survivor care and works to make the aftermath of their tragic situation easier to cope with. Strengthening clinical judgment skills could remedy significant mistakes made by novice forensic nurses. Critical thinking and clinical ethical reasoning are the building blocks of clinical judgment.

Impinging jets are frequently adapted for cooling overheated parts. With the film cooling technique, this process improves thermal exchanges between walls and fluid. However if many works have concerned only the thermal aspect of this problem [1], its dynamic field has been rarely studied especially for multiple impingements. As the two phenomena cannot be totally dissociated, we have undertaken the aerodynamic and thermal study of jets impinging on a plane wall. Various techniques have been used as visualizations (spreading over method, LASER sheet visualizations), LDA measurements to propose a topology schema of the flow and infrared thermography.

Spatial coherence of electrically pumped random terahertz lasers

Light sources with high radiance and tailored coherence properties are highly desirable for imaging applications in the mid-infrared and terahertz(THz) spectral regions, which host a large variety of molecular absorptions and distinctive fingerprints to be exploited for sensing and tomography. Here, we characterize the spatial coherence of random multimode THz quantum cascade lasers(QCLs) emitting > m W optical power per mode and showing low divergence(10°–30°), performing a modified Young’s double-slit experiment. Partial spatial coherence values ranging between 0.16 and 0.34 are retrieved, depending on the specific degree of disorder. These values are significantly lower than those(0.82) of conventional Fabry–Perot THz QCLs exploiting an identical active region quantum design. We then incorporate the devised low spatial coherence random lasers into a confocal imaging system with micrometer spatial resolution and demonstrate notable imaging performances, at THz frequencies,against spatial cross talk and speckles.

AIm： To determine the prevalence of a new set ot anti-glycan and anti-outer membrane protein （anti- OmP） antibodies in a Hungarian cohort of adult Celiac disease （CD） patients. mETHODS： 190 consecutive CD patients [m/F： 71/119, age：39.9 （SD：14.1） years], 100 healthy, and 48 gastrointestinal controls were tested for glycan anti-Saccharomyces cerevisiae （gASCA）, anti-laminaribioside （ALCA）, anti-chitobioside, anti-mannobioside, anti-OmP antibodies and major NOD2/CARD15 mutations. Thirty out of 82 CD patients enrolled at the time of diagnosis were re-evaluated for the same antibodies after longstanding gluten-free diet （GFD）. RESULTS： 65.9% of the CD patients were positive for at least one of the tested antibodies at the time of the diagnosis. Except anti-OmP and ALCA, antimicrobial antibodies were exclusively seen in untreated CD; however, the overall sensitivity was low. Any glycan positivity （LR＋： 3.13; 95% CI： 2.08-4.73） was associated with an increased likelihood ratio for diagnosing CD. Significant correlation was found between the levels of anti-glycan and anti-endomysial or anti-transglutaminase antibodies. Anti-glycan positivity was lost after longstanding GFD. Anti-glycan antibody titers were associated with symptoms at presentation, but not the presence of NOD2/CARD15 mutations. Patients with severe malabsorption more frequently had multiple antibodies at diagnosis （P = 0.019）. CONCLUSION： The presence of anti-glycan antibodies in CD seems to be secondary to the impaired small bowel mucosa which can lead to increased antigen presentation. Furthermore, anti-glycan positivity may be considered an additional marker of CD and dietary adherence.

The Arabidopsis heterotrimeric G-protein controls defense responses to necrotrophic and vascular fungi. The agbl mutant impaired in the Gβ subunit displays enhanced susceptibility to these pathogens. Gβ/AGB1 forms an obligate dimer with either one of the Arabidopsis Gγsubunits （γ1/AGG1 and γ2/AGG2）. Accordingly, we now demonstrate that the aggl agg2 double mutant is as susceptible as agbl plants to the necrotrophic fungus Plectosphaerella cucumerina. To elucidate the molecular basis of heterotrimeric G-protein-mediated resistance, we performed a comparative transcriptomic analysis of agbl-1 mutant and wild-type plants upon inoculation with P cucumerina. This analysis, together with metab- olomic studies, demonstrated that G-protein-mediated resistance was independent of defensive pathways required for resistance to necrotrophic fungi, such as the salicylic acid, jasmonic acid, ethylene, abscisic acid, and tryptophan-derived metabolites signaling, as these pathways were not impaired in agbl and aggl agg2 mutants. Notably, many mis-reguiated genes in agbl plants were related with cell wall functions, which was also the case in aggl agg2 mutant. Biochemical analyses and Fourier Transform InfraRed （FTIR） spectroscopy of cell walls from G-protein mutants revealed that the xylose content was lower in agbl and aggl agg2 mutants than in wild-type plants, and that mutant walls had similar FTIR spec-tratypes, which differed from that of wild-type plants. The data presented here suggest a canonical functionality of the Gβ and Gγ1/γ2 subunits in the control of Arabidopsis immune responses and the regulation of cell wall composition.

Imetit Dihydrobromide and Thioperamide medication in Cough Hypersensitivity model—The Role of H₃ Receptors

Chronic cough is a troublesome problem and it is frequently associated with diseases such as gastroesophageal reflux, asthma and upper airway diseases—so called diagnostic triade. The magnitude and severity of cough is strongly associated with the ongoing nasal inflammation in subjects with rhinosinusitis and treatment of nasal inflammation leads to the down regulation of pathologically up-regulated cough. Histamine plays a key role in the inflammation of the upper airways of different aetiologies;therefore histamine receptors seem to be promising targets. The aim of our study was to ascertain the effect of H3R agonist imetit and H3R antagonist thioperamide on cough and symptoms of allergic rhinitis (AR) in an animal model of upper airway cough syndrome in ovalbumin sensitized guinea pigs. OVA sensitized guinea pigs (n = 10) were repeatedly challenged with i.n. allergen-OVA to induce allergic rhinitis and to enhance cough reflex according to the validated model of experimental allergic rhinitis. Animals were pre-treated by i.p. administration of imetit (1 mg/kg and 2 mg/kg of body weight) and thioperamide 30 min. prior i.n. OVA administration. Rhinitis evaluation was based on the occurrence of typical symptoms. The effect on cough was assessed from the response to inhalation of citric acid (0.4 m, 10 min), final cough count and cough latency were analysed from the airflow traces, cough motor pattern and the cough sound. AR up-regulated the cough response from 9 ± 2 to 16 ± 1 cough per provocation, med ± IQR, p metit (1 mg/kg) suppressed nasal symptoms and decreased number of cough from 16 ± 1 to 12 ± 1;however the data did not reach significance. Imetit (2 mg/kg) significantly suppressed the nasal symptoms, and number of coughs from 16 ± 1 to 6 ± 2, med ± IQR, p mide (5 mg/kg of body weight) did not have expected effects on tested parameters.
