目的:总结尼日利亚翼状胬肉的治疗现状。方法:选择1999-01/2002-12的4a期间的翼状胬肉手术患者,所有病例都来自于尼日利亚Enugu市的尼日利亚大学教学医院和Ebrans临床中心。收集这些患者的生理数据、临床表现、手术方法和术后并发症等资料,应用6.1版EPI-InFO方法进行数据处理。共选择3种不同的手术方法:60眼单纯施行巩膜暴露术,42眼施行联合5-氟尿嘧啶(5-FU)的巩膜暴露术,22眼施行联合丝裂霉素C的巩膜暴露术。结果:72例(124眼)施行翼状胬肉切除术的患者,术后至少随访1a,包括男41例(56.9%),女31例(43.1%),男女比例为1∶0.8。术后复发率最高的在病变第III期(55.6%),最低在病变第I期(22.2%)。3种手术方法比较,复发率分别为65.0%、52%和13.6%。结论:在像尼日利亚这样的发展中国家,联合丝裂霉素的早期手术切除术,是一种安全性强、治疗费用低、方法简单且创伤小的治疗翼状胬肉的有效手段,值得推广。

Ureas are widely used in drugs,materials and catalysts because of their diamide structure,which can form strong hydrogen ***,it is of great scientific significance to develop efficient and green methods for the synthesis of urea compounds,especially unsymmetrical ***,we have disclosed novel and highly efficient three-component coupling reactions of organic halides,sodium cyanate and amines enabled by nickel/photoredox dual catalysis for the preparation of unsymmetrical *** reaction features simple and safe operations,broad substrate scopes,and product *** allows the facile synthesis of n-aryl/vinyl ureas from readily available,user-friendly feedstocks under mild conditions(27 examples,36%—98%yields).In addition,this method is further derived to alcohols as nucleophiles to synthesize a series of carbamates(15 examples,40%—95%yields).The mechanism experiment shows that the isocyanate produced by the coupling of halide and sodium cyanate may be the key intermediate in this reaction.

Urea generation through electrochemical CO_(2) and nO_(3)~-co-reduction reaction(CO_(2)nO_(3)RR)is still limited by either the low selectivity or yield rate of ***,we report copper carbonate hydroxide(Cu_2(OH)_2CO_(3))as an efficient CO_(2)nO_(3)RR electrocatalyst with an impressive urea Faradaic efficiency of45.2%±2.1%and a high yield rate of 1564.5±145.2μg h~(-1)mg_(cat)~(-1).More importantly,H_(2) evolution is fully inhibited on this electrocatalyst over a wide potential range between-0.3 and-0.8 V versus reversible hydrogen *** thermodynamic simulation reveals that the first C-n coupling follows a unique pathway on Cu_2(OH)_2CO_(3) by combining the two intermediates,~*COOH and~**** work demonstrates that high selectivity and yield rate of urea can be simultaneously achieved on simple Cu-based electrocatalysts in CO_(2)nO_(3)RR,and provide guidance for rational design of more advanced catalysts.

Goals: The aim of this study was to determine the antibiotic resistance profile of serotypes of Streptococcus pneumoniae strains circulating in Bangui. Methodology: A prospective and analytical analysis was carried out at the national Laboratory of Clinical Biology and Public Health from 2017 to 2022. The strains came from our study on the contribution to the study of antibiotic sensitivity of Streptococcus pneumoniae strains. The multiplex PCR test was used for its cost-effectiveness in terms of amplifiers which can be purified in order to be sequenced. It also makes it possible to detect several germs as well as their serotypes. For a PCR reaction, several elements are involved in the reaction medium or Master Mix. These are the desoxyribonucleotides (dnTPs), the magnesium ions (MgCl2) and the primers. A set of 14 primers divided into 3 classes were used. Class 1 primers served as an internal control by targeting the cpsA gene. It is a highly conserved gene found in capsular loci characterized to date. The primers of the second class were used to target specific serotypes by specific reactions (out of six possibilities). The group reaction was carried out using the primers of the third class in order to carry out an initial screening of the samples and to classify the pneumococcal isolates. Related serotypes were grouped based on the amplification of common genes. Using the technique of electrophoresis on agarose gel and an ultraviolet radiation device, the migration bands are then visualized and analyzed. The data collected had been entered into Excel 2010 and analyzed with Epi info 7. The exact Fischer chi2 test at the 5% threshold, the relative risk and its 95% confidence interval were used to compare the proportions and determine the associations. Results: 187 antibiotic-resistant strains of Streptococcus pneumoniae were collected. Lawsonia inermis is a hairless plant growing in various regions of north Africa, the Indian subcontinent, and the Middle East. It possesses many medicinal attributes, including curative properties against infectious dermatoses. This study was carried out to evaluate the phytochemical profile of the crude ethanolic extract of the plant leaves and its fractions as well as their antimicrobial activities. The phytochemical profile was performed using high-performance thin-layer chromatography (HPTLC), gas chromatography-mass spectrometry (GC-MS), and high-performance liquid chromatography (HPLC). Additionally, the phenolic and flavonoid contents were determined using the Folin-Ciocalteu spectrophotometric and the aluminum trichloride methods. Antimicrobial activity was tested using disc diffusion and microdilution methods. The presence of flavonoids, tannins, sterols, and triterpenes was revealed. GC-MS detected twelve compounds main compounds consisting of saturated and unsaturated fatty acids and phenolic and terpenoid compounds among twenty-seven components. HPLC also detected high contents of phenolic acids and flavonoids. The most abundant triterpene and sterols were ursolic acid (around 43.14 g/100g DW, 13.9 g/100g dry weight (DW), and 0.68 g/100g DW) in the crude ethanolic extract of leaves (FeLi), hexane fraction (FHLi) and dichloromethane fraction (FDLi), respectively and, β-sitosterol in FeLi (56.7 mg/100g DW), FHLi (10.55 g/100g DW), FDLi (106.1 mg/100g DW) and butanol fraction (FBLi) (357.4 mg/100g DW). Among the flavonoids, rutin = 3.24 g/100g and quercetin = 0.63 g/100g in the ethanolic extract, rutin = 15.73 g/100g in the dichloromethane fraction, and rutin = 0.23 g/100g) in the aqueous fraction;and among phenolic compounds, caffeic acid (37.65 g/100g DW) and vanillic acid (22.70 g/100g DW) were the most important in the ethyl acetate fraction (FAeLi). Introduction: Acute rheumatic fever (AAR) is a non-suppurative complication of late infection by group A. Infections due to streptococci remain a public health problem in the Central African Republic. The present study aims to determine the incidence rate of AAR cases and its complications. Methodology: This was a retrospective and analytical study over a period of 4 years (from January 2015 to December 2018) at the national Laboratory of Clinical Biology and Public Health (LnBCSP) in Bangui. It focused on samples concerning the diagnosis of AAR and patient files seen in consultation in the capital’s reference health establishments. Laboratory registers and patient consultation files were used to collect data. They were entered into Excel 2010 to be analyzed with Epi Info 7. A univariate analysis by logistic regression, Ficher’s exact test, and chi2 at the 5% threshold (p Results: We analyzed 94 cases meeting Jones’s criteria. The ages of the patients ranged from 18 to 85 years (mean age 52 years and mode 45 years). The incidence rate of AAR for the female sex was higher than that of the male sex during the study period (p > 0.05). It went from 166.6 in 2015 to 200 in 2016 and 2018 cases of AAR per 1000 people per year. The average incidence rate was 296.18 cases of AAR per 1000 people per year. The average incidence rate was 223.5 cases of AAR per 1000 people per year for joint damage. Joint damage represented 80.85% (RR = 0.62;Chi2 = 4.88;95% CI [0.39-0.97];p Conclusion: The study allowed data on the impact rate and complications of the AAR in Bangui. Special attention to data management will help produce a complete result on the problem. Joint complications predominated. Sustaining an effective surveillance system and preventing infection would help reduce the risk of AAR occurrence.

作为主要通信的里程碑,互联网和移动无线技术正在迅速发展,改善本地和广域无线网络的通信环境。由于漫游对无线网络的要求,未来的无线网络的发展趋势是无线异构网络,需要融合无线局域网和蜂窝技术,利用各个不通方法中不同的技术,不长相互之间的优缺点,这就是其与单个网络的优势。这些无线技术,通过支持多种互联网应用,如web浏览,电子邮件,视频和音频流具有不同的QoS要求,在这种不同的情况下,对于时延和丢报率的要求各不相同。比如垂直切换,被切换的两个网络之间要通过良好设计,使得两种网络之间可以平滑切换。合适的垂直切换授权机制在使HWn保证网络QoS的条件下,还有很长的研究过程。由于现存的授权控制机制的引入,保证网络对于多服务类型,多优先级的QoS进行了不同的服务。服务提供者运用不同的优先级对不同的用户进行区分,在这种情况下,有些移动终端通过支付更高的费用被赋予了更高的优先级,例如,在过拥塞的网络中,一些漫游的移动终端由于具有更高的优先级,可以在很短的时间内进行切换,保证通信质量,而其他移动终端也许就不能进行连接。但由于用户都趋向于廉价的服务,所以上述并没有从本质上得到改善。例如在无线局域网的情况下,授权系统给普通用户提供稳定的授权和服务机制,但是对于过拥塞的时间段,多个移动终端从不同的蜂窝网络中来来会会,授权和切换频频发生,在应用QoS保障机制的情况下,这会从很大程度上恶化网络,增加掉线概率。本论文中提供一种动态自适应的增强型的授权机制,融合无线局域网,2G网络,3G网络,还有蜂窝网,对移动终端的切换机制进行了改进。我们对每个地区的高于一个门限的切换次数进行统计和测量,并使用稳定授权机制来处理多切换中,减小切换门限和降低掉线率的方法。我们使用MATLAB和OPnET对进行仿真,仿真结果显示,当引入增强机制,移动终端可以从一个地区平滑切换到另外一个地区,并且切换时延很小,可以保证实时的服务得到很好的保障。

Blood transfusion saves lives and reduces morbidity and mortality for a large number of diseases and clinical conditions, but it is not without danger. The aim of this study was to determine the seroprevalence of HIV and hepatitis B in blood donors received at the regional Blood Transfusion Centre of n’Zérékoré (Guinea). This was a 5-year retrospective analytical study. We included records of blood donors aged 18 to 60 years admitted to the n’Zérékoré Regional Blood Transfusion Centre for blood donation from January 2016 to December 2020. We performed a descriptive analysis followed by Chi-2 or Fish-er-exact tests and the Student or Wilcoxon test, followed by multivariate logistic regression. In this study, donor age ranged from 18 - 60 years, with a pre-dominance of donors aged 25 - 34 (44.2%). Male donors were the most represented in our study (79.0% versus 21.0% female). More than half of the donors were blood group O (55.6%). We observed a seroprevalence of 3.6% for HIV, 13.4% for HBsAg and 0.2% for co-infection. In our series, age 25 - 34 (OR = 1.89 and P = 0.001) and 35 - 44 for HIV (OR = 2.01 and P = 0.001), HBsAgserostatus (OR = 3.04 and P = 0.001) and blood donation history (OR of 3.04 and P = 0.001) were factors associated with HIV positivity (P n our study, HIV serostatus (OR = 3.04 and P = 0.001) and blood donation history (OR = 0.01 and P = 0.001) were factors associated with HBsAgseropositivity. We reported a high prevalence of HIV and HBsAg. Sex, serological status and blood donation history were associated factors.
