Primarily the production of wool, mohair, silk, goat coarse hair and cashmere fibers is made in turkey. However, there has been a significant level of decrease in the production of these fibers for many years. the factors mainly causing this decrease can be said to include rapid increase in the use of chemical fibers, changes in fashion which have negative impacts on the consumption these fibers and systematic problems which negatively affect the production of sheep, Angora goat, hair goat and silkworm. However although the production resources are continuing to decrease, turkey still has a significant level of animal fiber production potential. Attainment of a more effective utilization of this potential will significantly contribute to the establishment of a sustainable textile industry and rural development and the preservation and development of folkloric culture and native farm animals which produce fibers.

BACKGROUND Polymorphisms of human leukocyte antigen(HLA)genes are suggested to increase the risk of gastric cancer(GC).AIM to investigate the HLA allele frequencies of patients with GC relative to a control group in terms of CagA+multiple(≥2)EPIYA-C *** the patient group comprised 94 patients[44 GC and 50 duodenal ulcer(DU)patients],and the control group comprised 86 individuals[(50 non-ulcer dyspepsia patients and 36 people with asymptomatic Helicobacter pylori(***)].Polymerase chain reaction was performed for the amplification of the *** cagA gene and typing of EPIYA *** sequence-specific oligonucleotide(SSO)typing was performed using Lifecodes SSO typing kits(HLA-A,HLA-B HLA-C,HLA-DRB1,and HLA-DQA1-B1 kits).RESULtS the comparison of GC cases in terms of CagA+multiple(≥2)EPIYA-C repeats showed that only the HLA-DQB1*06 allele[odds ratio(OR):0.37,P=0.036]was significantly lower,but significance was lost after correction(Pc=0.1845).the HLA-DQA1*01 allele had a high ratio in GC cases with multiple EPIYA-C repeats,but this was not significant in the univariate *** compared allele frequencies in the DU cases alone and in GC and DU cases together using the same criterion,and none of the HLA alleles were significantly associated with GC or ***,none of the alleles were detected as independent risk factors after the multivariate *** the other hand,in a multivariate logistic regression with no discriminative criterion,HLA-DQA1*01(OR=1.848),HLA-DQB1*06(OR=1.821)and HLA-A*02(OR=1.579)alleles were detected as independent risk factors for GC and *** None of the HLA alleles were detected as independent risk factors in terms of CagA+multiple EPIYA-C ***,HLA-DQA1*01,HLA-DQB1*0601,and HLA-A*2 were independent risk factors with no criterion in the multivariate *** suggest that the association of these alleles with gastric malignancies is not specifically related to cagA and multiple EPIYA C repeats.

Purpose: to determine the relationship between the dental root indentation and maxillary sinusitis. Methods: We assessed the records of the patients who underwent paranasal sinus computed tomography imaging for suspected sinusitis. Results: We identified a total of 52 patients with a pre-diagnosis of maxillary sinusitis. Dental indentation was detected in 58 of 104 (55.7%) sinuses. Forty six of 58 sinuses (79.3%) with dental indentation had mucosal thickening higher than 2 mm. the difference between the groups was statistically significant (p = 0.007). Conclusion: Dental indentation should be kept in mind as a reason for chronic maxillary sinus inflammation, if an underlying cause cannot be identified.

AIM: to investigate the indication, feasibility, safety, and clinical utility of endoscopic submucosal dissection (ESD) in the management of various gastrointestinal pathologies. MEtHODS: the medical records of 60 consecutive patients (34 female, 26 male) who underwent ESD at the gastroenterology department of Kocaeli University from 2006-2010 were examined. Patients selected for ESDhad premalignant lesions or non-invasive early cancers of the gastrointestinal tract and had endoscopic and histological diagnoses. Early cancers were considered to be confined to the submucosa, with no lymph node involvement by means of computed tomography and endosonography. RESULtS: Sixty ESD procedures were performed. the indications were epithelial lesions (n = 39) (33/39 adenoma with high grade dysplasia, 6/39 adenoma with low grade dysplasia), neuroendocrine tumor (n = 7), cancer (n = 7) (5/7 early colorectal cancer, 2/7 early gastric cancer), granular cell tumor (n = 3), gastrointestinal stromal tumor (n = 2), and leiomyoma (n = 2). En bloc and piecemeal resection rates were 91.6% (55/60) and 8.3% (5/60), respectively. Complete and incomplete resection rates were 96.6% (58/60) and 3.3% (2/60), respectively. Complications were major bleeding [n = 3 (5%)] and perforations [n = 5 (8.3%)] (4 colon, 1 stomach). two patients with colonic perforations and two patients with submucosal lymphatic and microvasculature invasion (1 gastric carcinoid tumor, 1 colonic adenocarcinoma) were referred to surgery. During a mean follow-up of 12 mo, 1 patient with adenoma with high grade dysplasia underwent a second ESD procedure to resect a local recurrence. CONCLUSION: ESD is a feasible and safe method for treatment of premalignant lesions and early malignant gastrointestinal epithelial and subepithelial lesions. Successful en bloc and complete resection of lesions yield high cure rates with low recurrence.

the present study aimed to investigate the correlation of MPV and splenomegaly as inflammation activity of FMF patients at the attacks free period. We retrospectively reviewed the medical records of 43 patients with FMF. this study was performed at the attack free period as clinical and laboratory. For this study, patients were divided into two groups. Patients with splenomegaly is called group 1 (n = 12) and patients with no splenomegaly is called group 2 (n = 31). Groups were compared respect to age, gender, platelet counts, acute phase reactants and MPV. the mean MPV (fl) were significantly higher in group 1 (8.9 ±0.8) than in group 2 (8.4 ±0.5, p <0.05). this study suggested that increased MPV and splenomegaly without amyloidosis could be a sign of chronic inflammation in children with FMF even in attack free period.

目的:通过光学相干断层扫描(optical coherence tomography,OCt;RtVue 100-2;V 5.1,Optovue,Fremont,CA,USA)测量比较非新生血管性与新生血管性年龄相关性黄斑变性(age-related macular degeneration,AMD)黄斑中心凹下脉络膜厚度(subfoveal choroidal thickness,SFCt),并分析脉络膜厚度(choroidal thickness,Ct)对AMD新血管形成的影响。方法:本研究为回顾性、横断面研究。以单侧眼患有新生血管性AMD且对侧眼患有非新生血管性AMD的24例患者(48眼)作为研究组,选取年龄与眼轴长度相匹配的40例健康眼作为对照组。非新生血管性AMD亚组研究对象是黄斑区有玻璃疣和/或色素变化眼,而新生血管性AMD亚组研究对象是由于脉络膜新生血管而致视网膜下或视网膜内有渗出液和/或脂性渗出眼。运用OCt垂直测量外侧高反射线(视网膜色素上皮层)到脉络膜巩膜交界面间的距离。选取7个不同点进行脉络膜厚度测量,测量区域为距黄斑中心凹颞侧和鼻侧1500μm范围,测量间距为500μm。测量结果在亚组间进行统计学对比研究。结果:研究组患者平均年龄为72.4±8.97(60～82)岁,对照组受试者平均年龄为71.2±8.8(58～81)岁。新生血管性AMD组中平均SFCt明显大于非新生血管性AMD组的厚度(Pt与平均鼻、颞侧脉络膜厚度比较,差异无统计学意义(P>0.05);而新生血管性AMD组中,平均SFCt与平均鼻、颞侧脉络膜厚度比较,差异有统计学意义(Pt对脉络膜厚度进行测量,有助于理解AMD的病理生理机制。然而,仍需要大型前瞻性研究来探求新生血管性AMD中SFCt增厚的原因。

随着新的工业革命模式代际转换,信息技术日益成为组织与塑造社会空间重要手段。而作为一种基于现代信息技术的重要阶段性成果,元宇宙技术现象的产生本身是科技革命发展到一定阶段的历史性产物。究其本质,元宇宙技术体系对于社会结构的影响作用本身是人类进入第四次工业革命之后,生产力水平发展的必然结果。因此,对于元宇宙技术体系特征可能对经济社会发展带来的影响研究,不能单纯以西方后现代主义思想为工具进行批判和否定,也不能简单否认技术体系在社会发展过程中可能起到的巨大作用。此时,以实践为核心的马克思主义唯物史观不应当也绝不应当缺席,其在理论和现实层面都是重要的解释武器,并能够对元宇宙技术的发展给予重要的引导与规范。

A modified Rendezvous ERCP technique in duodenal diverticulum

AIM: to postoperative endoscopic retrograde cholangiopancreatography(ERCP) failure, we describe a modified Rendezvous technique for an ERCP in patients operated on for common bile duct stone(CBDS) having a t-tube with retained CBDSs. MEtHODS: Five cases operated on for CBDSs and having retained stones with a t-tube were referred from other hospitals located in or around Istanbul city to the ERCP unit at the Haydarpasa Numune Education and Research Hospital. Under sedation anesthesia, a sterile guide-wire was inserted via the t-tube into the common bile duct(CBD) then to the papilla. A guide-wire was held by a loop snare and removed through the mouth. the guide-wire was inserted into the sphincterotome via the duodenoscope from the tip to the handle. the duodenoscope was inserted down to the duodenum with a sphincterotome and a guidewire in the working channel. With the guidance of a guide-wire, the ERCP and sphincterotomy were suc-cessfully performed, the guide-wire was removed from the t-tube, the stones were removed and the CBD was reexamined for retained stones by contrast. RESULtS: An ERCP can be used either preoperatively or postoperatively. Although the success rate in an isolated ERCP treatment ranges from up to 87%-97%, 5%-10% of the patients require two or more ERCP treatments. If a secondary ERCP fails, the clinicians must be ready for a laparoscopic or open exploration. A duodenal diverticulum is one of the most common failures in an ERCP, especially in patients with an intradiverticular papilla. For this small group of patients, an antegrade cannulation via a t-tube can improve the success rate up to nearly 100%. CONCLUSION: the modified Rendezvous technique is a very easy method and increases the success of postoperative ERCP, especially in patients with large duodenal diverticula and with intradiverticular papilla.

Plasma Concentrations of Some trace Element and Heavy Metals in Patients with Metastatic Colon Cancer

Purpose: It is well known that trace elements and heavy metals play a significant role in human health and disease. these elements play an important role in different metabolic pathways of cells and tissues. trace element deficiency or excess is implicated in the development or progression of some cancers. We present a study which investigates serum/ plasma levels of trace-heavy elements in metastatic colon cancer patients and healthy subjects. Methods: Blood samples were collected from 29 healthy subjects and 40 patients with metastatic colon cancer in Oncology Department of Medical School of Yuzuncu Yil University. In both groups, the serum levels of copper (Cu), magnesium (Mg), lead (Pb), chromium (Cr), zinc (Zn) selenium (Se), manganese (Mn), and cadmium (Cd) were determined. Results: Higher levels of Cu, Mg, Pb, Cr, Zn, Mn and Cd were seen in patients with metastatic colon cancer compared to healthy subjects and these findings were statistically significant (p 0.05). Se levels were noted to be lower in patients with colon cancer in comparison to healthy subjects and this finding was also statistically significant (p 0.05). Conclusions: According to our findings there was significant diffence in trace elements and heavy metals levels between healthy subjects and metastatic colon cancer patients. therefore heavy metal and trace elements may have prognostic significance in complex disorders including colon cancer. However, further comprehensive studies are needed in order to advance our understanding of the relationship between heavy metal and trace elements and their role in cancers.
