Anatomic and functional results of idiopathic macular epiretinal membrane surgery

AIM: to assess the impact of macular surgery on the functional and anatomic outcomes in patients with grade 2 epiretinal membrane(ERM), and the effect of internal limiting membrane(ILM) peeling on visual acuity and to analyze the long-term effect of pars plana vitrectomy(PPV) on intraocular pressure(IOP).MEtHODS: Pseudophakic eyes(62 eyes) diagnosed as idiopathic grade 2 ERM with at least 6 mo postoperative follow-up were included in this retrospective study. the fellow eye was nonvitrectomized. Patients were divided into two groups: group 1(29 eyes) treated with ERM and ILM peeling and group 2(33 eyes) with only ERM peeling. Preoperative and postoperative best corrected visual acuity(BCVA), slit-lamp, and a dilated fundus examination was performed. IOP was measured with Goldman applanation tonometer before, day 1 and first week and each visit after surgery. the incidence of significant IOP elevation was compared between vitrectomized eyes and nonvitrectomized fellow ***: Visual improvement was statistically significant and similar in both groups(P=0.008 in group 1, P=0.002 in group 2, P=0.09 inter-group). the amount of decrease in central macular thickness was statistically significant and similar in both groups(P=0.005 group 1, P=0.008 group 2, P=0.37 intergroup). At the final follow-up(14.1±9.6 mo) the incidence of significant IOP elevation was 4% in vitrectomized eyes(three eyes) and 3%(two eyes) in the nonvitrectomized fellow eyes(P=0.12). Four eyes(12.1%) had recurrent ERM after a mean follow-up of 8.6±1.1 mo in group 2, there was no recurrence in group 1(P=0.01).CONCLUSION: Recurrence of ERM may be decreased by ILM peeling during ERM surgery. However, it seems that ILM peeling do not affect the functional outcome and 23-gauge PPV alone do not have a significant effect on IOP.

Breast cancer seriously affects many *** breast cancer is detected at an early stage,it may be *** paper proposes a novel classification model based improved machine learning algorithms for diagnosis of breast cancer at its initial *** has been used by combining feature selection and Bayesian optimization approaches to build improved machine learning *** Vector Machine,K-Nearest Neighbor,Naive Bayes,Ensemble Learning and Decision tree approaches were used as machine learning *** experiments were tested on two different datasets,which are Wisconsin Breast Cancer Dataset(WBCD)and Mammographic Breast Cancer Dataset(MBCD).Experiments were implemented to obtain the best classification ***,Least Absolute Shrinkage and Selection Operator(LASSO)and Sequential Forward Selection were used to determine the most relevant features,*** machine learning models were optimized with the help of Bayesian optimization approach to obtain optimal hyperparameter *** results showed the unified feature selection-hyperparameter optimization method improved the classification performance in all machine learning *** the various experiments,LASSO-BO-SVM showed the highest accuracy,precision,recall and F1-score for two datasets(97.95%,98.28%,98.28%,98.28%for MBCD and 98.95%,97.17%,100%,98.56%for MBCD),yielding outperforming results compared to recent studies.

the goal of this study is to provide a stochastic method to investigate the eff ects of the randomness of soil properties due to their natural spatial variability on the response spectra spatial variation at sites with varying conditions. For this purpose, Monte Carlo Simulations are used to include the variability of both incident ground motion and soil parameters in the response spectra by mean of an appropriate coherency loss function and a site-dependent transfer function, respectively. the approach is built on the assumption of vertical propagation of SH type waves in soil strata with uncertain parameters. the response spectra are obtained by numerical integration of the governing equation of a single-degree-of-freedom (SDOF) system under non-stationary site-dependent and spatially varying ground motion accelerations simulated with non-uniform spectral densities and coherency loss functions. Numerical examples showed that randomness of soil properties signifi cantly aff ects the amplitudes of the response spectra, indicating that as the heterogeneity induced by the randomness of the parameters of the medium increases, the spectral ordinates attenuate.

A new generic open pit mine planning process with risk assessment ability

Conventionally, mining industry relies on a deterministic view, where a unique mine plan is determined based on a single resource model. A major shortfall of this approach is the inability to assess the risk caused by the well-known geological uncertainty, i.e. the in situ grade and tonnage variability of the mineral deposit. Despite some recent attempts in developing stochastic mine planning models which have demonstrated promising results, the industry still remains sceptical about this innovative idea. With respect to unbiased linear estimation, kriging is the most popular and reliable deterministic interpolation technique for resource estimation and it appears to remain its popularity in the near future. this paper presents a new systematic framework to quantify the risk of kriging-based mining projects due to the geological uncertainties. Firstly, conditional simulation is implemented to generate a series of equally-probable orebody realisations and these realisations are then compared with the kriged resource model to analyse its geological uncertainty. Secondly, a production schedule over the life of mine is determined based on the kriged resource model. Finally, risk profiles of that production schedule, namely ore and waste tonnage production, blending grade and Net Present Value （NPV）, are constructed using the orebody realisations. the proposed model was applied on a multi-element deposit and the result demonstrates that that the kriging-based mine plan is unlikely to meet the production targets. Especially, the kriging-based mine plan overestimated the expected NPV at a magnitude of 6.70% to 7.34% （135 M$ to 151 M$）. A new multivariate conditional simulation framework was also introduced in this paper to cope with the multivariate nature of the deposit. Although an iron ore deposit is used to prove the concepts, the method can easily be adapted to other kinds of mineral deposits, including surface coal mine.

Bone mass in axial spondyloarthritis:A literature review

AIM: to review the published literature reporting bone loss in patients with axial spondyloarthritis(SpA) particularly those studies using dual X-ray absorptiometry(DXA) methods. MEtHODS: this literature review examines the reported bone mass in patients with ax-SpA, particularly those using the DXA methods. the MEDLINE, Web of Science and Scopus databases were searched for relevant articles published between September 1992 and November 2013. Some of used search terms were ankylosing spondylitis(AS), SpA, spondyloarthropathy, bone loss, bone mass, osteopenia, bone mineraldensity, osteoporosis(OP), densitometry. Studies in which bone loss was investigated by using DXA in patients with Sp A were eligible. Each article was reviewed and the key elements were ***: there were 286 hits on MEDLINE, 200 on Web of Science and 476 on Scopus. After applying inclusion and exclusion criteria, we identified 55 articles in our systematic search. the sample size of the studies varied from 14 to 332 patients with SpA. the reported age range varied from 25 to 56 years in the reviewed studies. the symptom duration of patients with axS pA varied from 1.6 to 49 years. there were more males than females in these studies. Most of the recruited females were premenopausal women. Reported HLA-B27 positivity changed between 19% to 95%. the prevalence of OP and osteopenia in patients with Sp A varied from 3%-47% to 5%-88%, respectively, in the included studies. In particular, the prevalence of OP and osteopenia ranged from 2.0%-47.0% and 5.0%-78.3%, respectively, in patients with AS. there are conflicting results regarding the relationship among disease activity, acute phase response and bone mass. Mining truck scheduling with stochastic maintenance cost

Open pit mining operations utilize large scale and expensive equipment. For the mines implementing shovel and truck operation system, trucks constitute a large portion of these equipment and are used for hauling the mined materials. In order to have sustainable and viable operation, these equipment need to be utilized efficiently with minimum operating cost. Maintenance cost is a significant proportion of the overall operating costs. the maintenance cost of a truck changes non-linearly depending on the type, age and truck types. A new approach based on stochastic integer programming （SIP） techniques is used for annually scheduling a fixed fleet of mining trucks in a given operation, over the life of mine （multi-year time horizon） to minimize maintenance cost. the maintenance cost data in mining usually has uncertainty caused from the variability of the operational conditions at mines. to estimate the cost, usually historic data from different operations for new mines, and/or the historic data at the operating mines are used. However, maintenance cost varies depending on road conditions, age of equipment and many other local conditions at an operation. traditional models aim to estimate the maintenance cost as a deterministic single value and financial evaluations are based on the estimated value. However, it does not provide a confidence on the estimate. the proposed model in this study assumes the truck maintenance cost is a stochastic parameter due to the significant level of uncertainty in the data and schedules the available fleet to meet the annual production targets. the scheduling has been performed by applying both the proposed stochastic and deterministic approaches. the approach provides a distribution for the maintenance cost of the optimized equipment schedule minimizing the cost.
