A modified Rendezvous ERCP technique in duodenal diverticulum

AIM: To postoperative endoscopic retrograde cholangiopancreatography(ERCP) failure, we describe a modified Rendezvous technique for an ERCP in patients operated on for common bile duct stone(CBDS) having a T-tube with retained CBDSs. METHODS: Five cases operated on for CBDSs and having retained stones with a T-tube were referred from other hospitals located in or around Istanbul city to the ERCP unit at the Haydarpasa Numune Education and Research Hospital. Under sedation anesthesia, a sterile guide-wire was inserted via the T-tube into the common bile duct(CBD) then to the papilla. A guide-wire was held by a loop snare and removed through the mouth. The guide-wire was inserted into the sphincterotome via the duodenoscope from the tip to the handle. The duodenoscope was inserted down to the duodenum with a sphincterotome and a guidewire in the working channel. With the guidance of a guide-wire, the ERCP and sphincterotomy were suc-cessfully performed, the guide-wire was removed from the T-tube, the stones were removed and the CBD was reexamined for retained stones by contrast. RESULTS: An ERCP can be used either preoperatively or postoperatively. Although the success rate in an isolated ERCP treatment ranges from up to 87%-97%, 5%-10% of the patients require two or more ERCP treatments. If a secondary ERCP fails, the clinicians must be ready for a laparoscopic or open exploration. A duodenal diverticulum is one of the most common failures in an ERCP, especially in patients with an intradiverticular papilla. For this small group of patients, an antegrade cannulation via a T-tube can improve the success rate up to nearly 100%. CONCLUSION: The modified Rendezvous technique is a very easy method and increases the success of postoperative ERCP, especially in patients with large duodenal diverticula and with intradiverticular papilla.

Association of KRAS Gene and microRNA-124-3p in Sporadic Colorectal Tumours

Aim: To reveal the exonic and 3’UTR sequences of KRAS, TP53, APC, BRAF, PIK3CA genes in sporadic colorectal tumors and to investigate the clinical relevance of 3’UTR variations in miRNA profiles. Methods: In the study, the exonic and 3’UTR sequences of five genes in 12 sporadic colorectal tumors were extracted by next generation sequencing. In tumors with variation in the 3’UTR region, the changes caused by the variation in the miRNA binding profile were detected. The expression profile of these miRNAs in colorectal and other solid tumors compared to normal tissue was determined. Pathway analysis was performed to determine which signaling pathways miRNAs affect. Results: Case-10 in our study was wild type KRAS and received cetuximab treatment and developed drug resistance. In this case, it was concluded that the expression of KRAS increased and tumorigenesis progressed due to miRNAs that do not bind to this region due to variations in the 3’UTR region. Among these miRNAs, hsa-miR-124-3p was found to have decreased expression in colorectal tumors and to be associated with the ECM-receptor interaction pathway. Conclusion: Variations in the 3’UTR regions of genes critical in the process of carsinogenesis are associated with drug resistance and the process of tumorigenesis.

Singular and non-topological soliton solutions for nonlinear fractional differential equations

In this article, the fractional derivatives are described in the modified Riemann-Liouville sense. We propose a new approach, namely an ansatz method, for solving fractional differential equations（FDEs） based on a fractional complex transform and apply it to solve nonlinear space-time fractional equations. As a result, the non-topological as well as the singular soliton solutions are obtained. This method can be suitable and more powerful for solving other kinds of nonlinear fractional FDEs arising in mathematical physics.

The Causality and Relation of Corruption With Basic Macro-economic Tables （Charts）

Corruption which means the use of public power for private interests is one of the most important problems either in less developed and developing countries or developed countries. Especially in less developed and developing countries, corruption is a serious obstacle in front of economic development. Corruption is raised from deficiency in democracy, deficient judgement, unfair elections and dependent media sector includes bribery, embezzlement, forgery, fraud, favouritism, nepotism and illegal beneficial use of public authority. The aim of the paper is to determine the causal relationship between corruption and basic macro-economic indicators. For this purpose, in this study, the basic mic indications such as import, export, budget deficit, manufacturer price index are taken into account as variables to examine their relation with corruption. By getting use of the data obtained from CPI index, the relation between the variables is examined using the T-Test. Furthermore, to determine the causality among all the variables, correllation analysis is applied to present causal relations between the corruption and macro-economic variables. The findings concluded that there is a significant correlation among corruption, manufacturer price index and export-import. However, there is no significant correlation with budget deficit.

Enhancing the efficiency of cabin heaters in emergency shelters after earthquakes through an optimized fuzzy controller

In just one and half minutes,more than fifty thousand died due to the 7.7 and 7.6 magnitude earthquakes that struck Turkey’s southeast on February 6,2023;thousands of families who barely escaped struggled to survive in the freezing weather.A warm shelter was the most basic requirement of these *** buildings are a rapid and easy solution to this ***,there is a need for a more effective and safe heating option than a wood fire for these *** this study,cabin heaters,which allow truck drivers to warm up when they park their vehicles to sleep,are specially optimized for emergency shelters after an *** optimized fuzzy controller was developed to use in such buildings,which allows an air–fuel ratio in the combustion chamber of the cabin heater to be controlled adaptively based on system dynamics to get lower carbon emissions and fuel *** TRNSYS software was used to establish the transient simulation model of a cabin heater with a capacity of 4 kW for a typical 21 m^(2) shelter building in Turkey’s cold *** developed fuzzy controller carried out the heating process of this shelter from the 15th of November to the 15th of *** of using expert knowledge,the Gray Wolf Optimization(GWO)method was applied to optimize the fuzzy controller parameters developed for the cabin *** the optimized fuzzy controller,the fuel consumption at the end of the heating season was reduced by an average of 0.2 L/h,and the cabin heater’s efficiency increased by more than 13%.Our simulation results show that the intelligent controller we developed could improve diesel fuel combustion efficiency by up to 85%.

Exp-Function Method and Fractional Complex Transform for Space-Time Fractional KP-BBM Equation

In the present article, He＇s fractional derivative, the ansatz method, the （ C / G）-expansion method, and the exp-function method are used to construct the exact solutions of nonlinear space-time fractional Kadomtsev-Petviashvili- Benjamin-Bona Mahony （KP-BBM）. As a result, different types of exact solutions are obtained. Also we have examined the relation between the solutions obtained from the different methods. These methods are an efficient mathematical tool for solving fractional differential equations （FDEs） and it can be applied to other nonlinear FDEs.

Bright and dark soliton solutions for some nonlinear fractional differential equations

In this work, we propose a new approach, namely ansatz method, for solving fractional differential equations based on a fractional complex transform and apply it to the nonlinear partial space-time fractional modified Benjamin-Bona- Mahoney （mBBM） equation, the time fractional mKdV equation and the nonlinear fractional Zoomeron equation which gives rise to some new exact solutions. The physical parameters in the soliton solutions： amplitude, inverse width, free parameters and velocity are obtained as functions of the dependent model coefficients. This method is suitable and more powerful for solving other kinds of nonlinear fractional PDEs arising in mathematical physics. Since the fractional deriva- tives are described in the modified Riemann-Liouville sense.
