Obstructive jaundice due to hepatobiliary cystadenoma or cystadenocarcinoma

Hepatobiliary cystadenomas (HBC) and cystadenocarci- nomas are rare cystic lesions. most patients with these lesions are asymptomatic, but presentation with ob- structive jaundice may occur. The first patient presented with intermittent colicky pain and recurrent obstructive jaundice. Imaging studies revealed a polypoid lesion in the left hepatic duct. The second patient had recurrent jaundice and cholangitis. Endoscopic retrograde cholan- giopancreatography (ERCP) showed a cystic lesion at the confluence of the hepatic duct. In the third patient with intermittent jaundice and cholangitis, cholangioscopy re- vealed a papillomatous structure protruding into the left bile duct system. In the fourth patient with obstructive jaundice, CT-scan showed slight dilatation of the intrahe- patic bile ducts and dilatation of the common bile duct of 3 cm. ERCP showed filling of a cystic lesion. All patients underwent partial liver resection, revealing HBC in the specimen. In the fifth patient presenting with obstructive jaundice, ultrasound examination showed a hyperecho- genic cystic lesion centrally in the liver. The resection specimen revealed a hepatobiliary cystadenocarcinoma. HBC and cystadenocarcinoma may give rise to obstruc- tive jaundice. Evaluation with cross-sectional imaging techniques is useful. ERCP is a useful tool to differentiate extraductal from intraductal obstruction.

Amidst increasing concerns about plastic pollution’s impacts on ecology and health,nanoplastics are gaining global recognition as emerging environmental *** review aimed to examine the complex molecular consequences and underlying fundamental toxicity mechanisms reported from the exposure of diverse aquatic organisms to *** the comprehensive examination of transcriptomics,proteomics,and metabolomics studies,we explored the intricate toxicodynamics of nanoplastics in aquatic *** review raised essential questions about the consistency of findings across different omics approaches,the value of combining these omics tools to understand better and predict ecotoxicity,and the potential differences in molecular responses between *** amalgamating insights from 37 omics studies(transcriptome 22,proteome six,and metabolome nine)published from 2013 to 2023,the review uncovered both shared and distinct toxic effects and mechanisms in which nanoplastics can affect aquatic life,and recommendations were provided for advancing omics-based research on nanoplastic *** comprehensive review illuminates the nuanced connections between nanoplastic exposure and aquatic ecosystems,offering crucial insights into the complex mechanisms that may drive toxicity in aquatic environments.

A Coaching Program for Recently Licensed Young Drivers in the Netherlands： Which Drivers Are Attracted？

In line with European developments, a Dutch second phase coaching program, referred to as the DX- （Driver Xperience） program, was developed for young novice drivers to counteract their high accident risk. more specifically, the aim of the DX-program was to enable young drivers to make responsible decisions and develop positive attitudes regarding four levels of the driving task： combining life style and driving, planning and navigation, participating in different traffic situations and handling the vehicle. In this paper, the design principles of the program are described. The empirical study focused on the entry characteristics of the participating young drivers （n = 3,117） as compared to a reference group of young drivers （n = 345）. Results show that the DX-program attracted young drivers that, in some respects, showed a more risky profile than average young drivers in terms of speed violations, anger and the number of fines. In addition, four groups of participants with sharply differing driving styles could be distinguished. Implications for educational design and follow-up research are discussed within the theoretical framework of self-regulated learning.

Development and Evaluation of a Competence-BasedExam for Prospective Driving Instructors

According to changed views on driver training and driverintroduced in the Netherlands. The exam consists of two parts：（l）instructor preparation, a competence-based instructor exam wasmultimedia theory tests; （2） a performance lesson for drivinginstruction and coaching. An implicit idea behind the innovated exam is that it can have a positive backwash effect on the quality ofdriver instructor preparation programs. This study aims to evaluate the reliability, validity and fairness of the theoretical tests, whichappear in different versions for successive groups of PDIs （prospective driving instructors）. Data of 4,741 PDIs, enrolled during theperiod between January 2010 and October 2012, were used for analysis. The results of psychometric analyses show that the theory testsyielded reliable and fair decisions about instructor certification. The predictive validity of the theory tests for the final performanceassessment was low. Implications for the design and on-the-fly maintenance of exam item banks are discussed. Follow-up studies willfocus on the question, whether the improved instructor exam produces safer drivers in the end.

An analysis of quantitative PCR reliability through replicates using the C_(t)method

There is considerable interest in quantitatively measuring nucleic acids from single cells to small populations. The most commonly employed laboratory method is the real-time polymerase chain reaction (PCR) analyzed with the crossing point or crossing threshold (Ct) method. Utilizing a multiwell plate reader we have performed hundreds of replicate reactions each at a set of initial conditions whose initial number of copies span a concentration range of ten orders of magnitude. The resultant Ct value distributions are analyzed with standard and novel statistical techniques to assess the variability/reliability of the PCR process. Our analysis supports the following conclusions. Given sufficient replicates, the mean and/or median Ct values are statistically distinguishable and can be rank ordered across ten orders of magnitude in initial template concentration. As expected, the variances in the Ct distributions grow as the number of initial copies declines to 1. We demonstrate that these variances are large enough to confound quantitative classi?cation of the initial condition at low template concentrations. The data indicate that a misclassi?cation transition is centered around 3000 initial copies of template DNA and that the transition region correlates with independent data on the thermal wear of the TAQ polymerase enzyme. We provide data that indicate that an alternative endpoint detection strategy based on the theory of well mixing and plate ?lling statistics is accurate below the mis- classi?cation transition where the real time method becomes unreliable.

Right ventricular tear mimicking myocardial infarction following pericardiocentesis

A 72-year-old female was admitted to the CCU with a recent onset of progressive breathlessness for bedside pericardial drainage for chronic pericardial effusion. After an uncomplicated drainage procedure, initially a serous straw coloured fluid was aspired with subsequent hemorrhagic aspiration with haemoglobin value similar to the peripheral blood. The patient showed initially transient improvement followed by rapid deterioration into severe shock and death. Signs of infero-posterior myocardial infarction (mI) were seen on the ECG. Before death, further interventions were refused by her and her family but a permission was given for autopsy. At autopsy, right ventricular rupture was seen with a 0.6 cm tear with a large amount of 800 cc bloody fluid with clots. The result of histopathologic study of the tear was resembling three-days old mI. The drain was found to be properly localized in the pericardial space, was not blocked and caused no harm to the myocardium. Furthermore, histopathologic examination revealed pulmonary adenocarcinoma of the left upper lobe, pleuritis and lymphangitis carcinomatosa and enlarged mediastinal lymph nodes. A case of fatal complication is reported following bedside pericardial drainage. Postmortal, right ventricular tear mimicked myocardial infarction.

Results of percutaneous sclerotherapy and surgical treatment in patients with symptomatic simple liver cysts and polycystic liver disease

AIm: To evaluate the results of the treatment of simple liver cysts (solitary and multiple) and polycystic liver disease (PLD) using percutaneous sclerotherapy and/or surgical procedures in a single tertiary referral centre. mETHODS: Retrospective analysis of 54 patients referred for evaluation and possible treatment of simple liver cysts (solitary and multiple) and PLD, from January 1997 to July 2006. RESULTS: Simple liver cysts were treated in 41 pts (76/) with a mean size of 12.6 cm. The most common reason for referral was abdominal pain or discomfort (85/). Percutaneous sclerotherapy was performed as initial treatment in 30 pts, showing cyst recurrence in 6 pts (20/). Surgical treatment was initially performed in 11 pts with cyst recurrence in 3 pts (27/). PLD was treated in 13 pts (24/) with a mean size of the dominant cyst of 13 cm. Percutaneous sclerotherapy for PLD was performed in 9 pts with recurrence in 7 pts (77.8/). Surgical treatment for PLD was undertaken in 4 pts (30.8/) with recurrence in all. Eventually, 2 pts with PLD in the presence of polycystic kidney disease underwent liver-and kidney transplantation because of deterioration of liver and kidney function. CONCLUSION: The majority of patients with simple liver cysts and PLD are referred for progressive abdominal pain. As initial treatment, percutaneous sclerotherapy is appropriate. Surgical deroofing is indicated in caseof cyst recurrence after percutaneous sclerotherapy. However, the results of percutaneous sclerotherapy and surgical treatment for PLD are disappointing. Partial liver resection is indicated when there is suspicion of a pre-malignant lesion.
