Seroprevalence of SARS-COV-2 Exposure among “High-Risk” Populations (Healthcare Workers, People Who Attend Markets, and School Children) in Zanzibar

In Zanzibar, from the start of the pandemic in March 2020 to the time of sampling in December 2020, SARS-CoV-2 seroprevalence data was limited. We conducted a seroprevalence study to evaluate the magnitude of SARS-CoV-2 exposure among healthcare workers, school children, and people who attended general markets in Zanzibar. The objectives of the study were to analyse the total antibodies from selected higher-risk population groups in order to determine magnitude in SARS CoV-2 exposure. Blood samples were collected from eligible and consented participants (adults and children), and their serum was analyzed for total antibodies against SARS-CoV-2 using ELISA. A questionnaire was used to collect participants’ demographic and clinical data. The overall SARS-CoV-2 seroprevalence across all age groups was 33%, and a higher seroprevalence was observed in the 40 - 49 years’ age group relative to other ages as well as in those who attended markets. A runny nose (18.8% of participants) was the most frequently reported SARS-CoV-2 infection-related symptom. Multivariable analysis showed significantly higher odds of infection in people living in urban districts. The findings provide insight into SARS-CoV-2 infection among school children, health workers, and people who attended markets in Zanzibar in the early stages of the pandemic. Exposure in these groups might have been influenced by infection and prevention strategies taken by the government, as well as shopping behavior, school overcrowding, and population density in urban settings. The study had methodological limitations, including cross-sectional design. Further, well-designed, longitudinal studies are recommended to understand exposure and transmission at a population level.

Bacteriological Profile of Infections Encountered in a Pneumology Service in a Moderate-Income Country [Pneumology Department of CHU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology Department of the University Hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by HIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

Seasonal variability in CDOM absorption and fluorescence properties in the Barataria Basin,Louisiana,USA

Absorption and fluorescence properties of chromophoric dissolved organic matter （CDOM） along a 124 km transect in the Barataria Basin, a large estuary located in Louisiana, USA, were investigated during high and low flow periods of the Mississippi River in the spring and winter of 2008-2009. Mean CDOM absorption at 355 nm from the marine to the freshwater end member stations ranged from （3.25 ± 0.56） to （20.76 ± 2.43） m^-1 for the three month high flow period whereas it varied from （1.48 ± 1.08） to （25.45 ± 7.03） m^-1 for the same stations during low flow period. Corresponding salinity values at these stations indicated the influence of river and shelf exchanges in the lower basin and precipitation and runoff in the upper basin. An inverse relationship of CDOM absorbance and fluorescence with salinity observed in the basin could he a useful indicator of salinity. CDOM fluorescence also varied over a large range showing an approximately 8 to 12-fold increase between the marine and freshwater end members for the two flow seasons. Excitation-emission matrix spectral plots indicated the presence of various fluorescence components with highest being the A-peak, lowest the T-peak, and the C and M-peaks showing similar trends along the transect. During low flow season the A/C ratio were well correlated with station locations indicating increased terrestrial influence towards the upper basin. CDOM absorption and fluorescence at 355 nm were highly correlated and independent of CDOM sources suggesting that fluorescence could be used to characterize CDOM in the basin.

Context: Antiretroviral therapies improve the prognosis of NeuroAIDS contrasting with a high frequency of Minor Neurocognitive Disorders (MND) even in aviremic subjects. Objective: The objective of this study is to present the neurological and cognitive profile of People Living with HIV (PLHIV) initiating antiretroviral treatment in Kinshasa in the era of Dolutegravir (DTG). Methods: This is a multicenter, cross-sectional study with a descriptive aim carried out in 16 HIV Outpatient Treatment Centers (OTC) in Kinshasa from October 4, 2021 to February 15, 2022. The International HIV Dementia Scale (IHDS) correlated with the Activities of Daily Living (IADL) scale facilitated the categorization of NeuroCognitive Disorders (NCD) of PLHIV evaluated after carrying out a summary neurological examination. Results: Of the 96 patients recruited, 56.3% were women with a sex ratio of 0.68. The average age was 40.1 ± 12.1 years. The secondary education level was the majority at 64.6%. Malaria (44.8%) and tuberculosis (32.3%) were more common as opportunistic infections. They were alcoholics in (30.2%). Their history was heart disease (15.6%), high blood pressure (18.8%);drug abuse (10.4%). The IHDS score was light in 55.2% of cases. The correlation between IHDS/IADL watches asymptomatic neurocognitive impairments (ANI) in 77.1%, almost all of subjects are found with normal overall functioning (94.8%) and a disturbed neurological examination in 53.1% of cases with a predominance of motor impairments in 79.1%. Conclusion: In view of these results, early and systematic screening of NCD and associated factors remains necessary in our context. .

Unusual presentations of eosinophilic gastroenteritis:Case series and review of literature

Eosinophilic gastroenteritis (EG) is an uncommon disease characterized by focal or diffuse eosinophilic infiltration of the gastrointestinal tract, and is usually associated with dyspepsia, diarrhea and peripheral eosinophilia. Diffuse gastrointestinal tract and colonic involvement are uncommon. The endoscopic appearance may vary from normal to mucosal nodularity and ulceration. Gastrointestinal obstruction is unusual and is associated with predominantly muscular disease. We present five unusual cases of EG associated with gastric outlet and duodenal obstruction. Two cases presented with acute pancreatitis and one had a history of pancreatitis. Four cases responded well to medical therapy and one had recurrent gastric outlet obstruction that required surgery. Four out of the five cases had endoscopic and histological evidence of esophagitis and two had colitis. Two patients had ascites. These cases reaffirm that EG is a disorder with protean manifestations and may involve the entire gastrointestinal tract. Gastric outlet and/or small bowel obstruction is an important though uncommon presentation of EG. It may also present as esophagitis, gastritis with polypoid lesions, ulcers or erosions, colitis and pancreatitis and may mimic malignancy.

In this study,the pseudo-discontinuum modeling technique,called continuum Voronoi block model(CVBM),was applied to repre-sent the behavior of hard-rock pillars from underground mines subjected to high field *** CVBM’s ability to produce numerical results consistent with the observed behavior of pillars is demonstrated through the numerical analysis of a hypothetical case and a back analysis of the Creighton mine *** results show that the model can capture convergence displacements and explicitly show the formation of macro-fractures parallel to excavation walls,intact rock slabs,and V-shaped *** components are characteristics of brittle failure induced by highly stressed ground *** studies presented in this work confirm the CVBM as a convenient tool for the numerical modeling of intact rock pillars excavated in deep underground mines.

Effects of seed type and bruchid genotype on the performance and oviposition behavior of Zabrotes subfasciatus （Coleoptera： Bruchidae）

The effect of different bean varieties on the performance of the bruchid beetle Zabrotes subfasciatus Boheman （Coleoptera： Bruchidae）, was determined by using wild and cultivated seeds of the genus Phaseolus. Results showed that the quality of the host plant affected the performance and the oviposition behavior of female beetles. Overall, bruchid performance was higher on cultivated seeds than on wild seeds. It was also found that the oviposition behavior and the performance of their offspring differed between females that originated from wild versus cultivated seeds. We also demonstrated the importance of a genetic component in bruchid performance： longevity, fecundity, larval development, adult size, and the sex ratio of the progeny varied according to their matemal lineage （‘family＇）. For example, on the same host type, some females laid twice as many eggs as females from other‘families＇, and the sex ratio among the offspring correlated with that of their mother. Thus, the performance and behavior of Z. subfasciatus are not only affected by environmental factors such as the quality of the seeds on which they develop, but also have a genetic basis which can counterbalance a less suitable quality of the host plant. For a crop pest such as Z. subfasciatus, its ability to survive and adapt on host plants of differing quality may be an important attribute to consider for pest management.

Controllable Fabrication of Spider-Web-Like Structured Anaphe Panda Regenerated Silk Nanofibers / Nets via Electrospinning / Netting

An Anaphe panda silk nanofibers / nets( NFN) membrane with attractive structures consisting of common electrospun nanofibers and two-dimensional( 2D) spider-web-like structured nano-nets were successfully fabricated via electrospinning / netting technology. The unique structures of NFN membranes such as extremely small diameter( < 20 nm),high porosity,large specific surface area. and biocompatibility make this Anaphe panda silk NFN membrane a promising candidate for biomedical applications. In the present study, field emission scanning electron microscopy( FESEM) was used to investigate the influence of polymer solution and humidity on nano-nets coverage and morphology. The FE-SEM images revealed that nano-net coverage area increased with increase in concentration of solution while lowering the humidity increased the nano-nets formation. Moreover,the mechanical properties of the membrane were also tested and the result showed that the silk NFN membrane displayed a breaking stress of 3. 7 MPa and breaking strain of 13. 8%. For further structural elucidation, Fourier transform infrared spectroscopy( FT-IR) was used to analyze the structural conformation changes from random coil to β-sheet in the NFN membrane which was an important factor effecting the usability of membrane. Thus,the results above confirmed the feasibility of Anaphe panda NFN structures applicability in cell tissue culture and other biomedical applications.
