BACKGROUND left ventricular hypertrophy(lVH) is a common manifestation of cardiovascular disease and a risk factor for cardiovascular morbidity and mortality, but available methods for its electrocardiographic(ECG) diagnosis have limited *** To investigate findings associated with lVH on ECG and developed an improved system for the diagnosis of *** A cohort study comparing ECG data acquired within 30 days of transthoracic echocardiography(TTE) was performed. Multivariate regression analysis identified ECG findings associated with increased lV mass and mass index. A scoring system was derived and performance compared to established criteria for *** Data from 5486 outpatients with TTEs and corresponding ECGs were included in the derivation cohort, 333(6.1%) of whom had lVH by TTE. In the primary regression analysis, findings associated with lVH were amplitudes of Q in V3, R in V6, S in V3, T in V6, P' in V1, P in V6, as well as R and T-axis discordance, R peak time in V6, QRS duration, weight, height, sex, and age. From this we derived a score consisting of 5 criteria, and validated it in an independent cohort of 910 patients. With a threshold of 1.5 points, sensitivity and specificity were67.9% and 81.4%, and 62.5% and 83.2% in the derivation and validation cohorts,respectively. Research by one of the authors suggested that the critical mass of constant-density neutron stars will be greater than eight solar masses when the majority of their neutrons group into bosons that form a Bose-Einstein condensate, provided the bosons interact with each other and have scattering lengths on the order of a picometer. That analysis was able to use Newtonian theory for the condensate with scattering lengths on this order, but general relativity provides a more fundamental analysis. In this paper, we determine the equilibrium states of a static, spherically-symmetric variable-density mixture of a degenerate gas of noninteracting neutrons and a Bose-Einstein condensate using general relativity. We use a Klein-Gordan lagrangian density with a Gross-Pitaevskii term for the condensate and an effective field for the neutrons. We show that a new class of compact stars can exist with masses above the Oppenheimer-Volkoff limit, provided the scattering length of the bosons is large enough. These stars have no internal singularities, obey causality, and demonstrate a quantum mechanism consistent with general relativity that could prevent collapsed stars from becoming black holes.

Effects of amendments of paper mill sludge and nutrients on soil surface CO2 flux in northern hardwood forests

Safe and economical disposal of paper mill sludge is a key consideration for forest products industry. A study was conducted to examine the effects of amendments of sludge and nutrients on soil surface CO2 flux （Rs） in northern hardwood forests and to quantify the relationship among Rs, soil temperature, and moisture in these stands. The experiment was a randomized complete block design that included sludge-amended, fertilized, and control treatments in sugar maple （Acer saccharum Marsh） dominated hardwood forests in the Upper Peninsula of Michigan, USA. Results showed that Rs was positively correlated to soil temperature （R^2 = 0.80, p 〈 0.001）, but was poorly correlated to soil moisture. Soil moisture positively affected the Rs only in the sludge-amended treatment. The Rs was significantly greater in the sludge-amended treatment than in the fertilized （p = 0.033） and the control （p = 0.048） treatments. The maximum Rs in the sludge-amended treatment was 8.8 μmol CO2 · m^ 2. s^-1, 91% and 126% greater than those in the fertilized （4.6 μmol CO2 · m^-2· s^-1） and control （3.9 μmol CO2· m^- 2· s^-1） treatments, respectively. The Rs did not differ significantly between the fertilized and control treatments. The difference in Rs between sludge-amended and the other treatments decreased with time following treatment.

Recently the American College of Cardiology and the American Heart Association instituted DTB of 90 minutes or less as a class I recommendation. Since 2006 the percentage of patient meeting this metric has substantially increased, although research has demonstrated discrepancies in whether or not this objective is associated with better patient outcome. Here, we reviewed seven studies in effort to investigate the validity of the 90 minute or less door to balloon time. Our findings suggest that patient outcome in the setting of acute myocardial infarction is multifactorial, and while the door to balloon time metric likely has played a role in better patient outcomes it is not the sole factor in improving mortality rates.

Introduction: Achieving the goal of “zero new human immunodeficiency virus (HIV) infections by 2030” requires the elimination of HIV transmission within serodiscordant couples. In Benin, the phenomenon of HIV serodiscordance remains poorly documented. Objective: the aim of this study was to determine the prevalence of HIV serodiscordance and its associated factors among infected couples in the city of Parakou. Study Framework and Method: This was a descriptive cross-sectional study with an analytical focusing on qualitative and quantitative plans. It was carried out on thirty approved sites in the city of Parakou between July and December 2022. We used a non-exhaustive convenience sample of people living with HIV (PlHIV) whose partner’s status was known. Data collection took place in two phases: an interview phase during which sociodemographic, economic, behavioral and marital/social data were collected. The second phase focused on clinical and therapeutic data which were collected from the patients’ personal records. Results: A total of 417 study subjects were recruited. The average age (SD) of the subjects was 38.17 (±10.25) years [37.19 - 39.16]. Females accounted for the largest proportion (79.62%). Most respondents (74.11%) were married. Monogamous unions were the most frequent, involving 67.87% of participants, most of whom (44.60%) had been infected for between 1 and 5 years. Couples in union for less than 10 years were the most numerous (218 people or 52.28%). The respondent was head of household among only 140 respondents (33.57%) of which 84 men (60.00%) and 56 women (40.00%). Female heads of household were mainly divorcees and widows. Around 75% of PlHIV had monthly incomes of less than 50,000F CFA (81.16 USD). Prevalence of HIV serodiscordance was 71.70%. Introduction: Neglected elbow dislocations are much more common in developing and underdeveloped countries. The main reason for the delay in diagnosis is that patients initially seek treatment from traditional healers. These will immobilize the elbow in extension, which will lead to the retraction of the triceps muscle and collateral ligaments. Patients and Method: We carried out a prospective study over five years (2014-2019). The study involved patients with unreduced elbow dislocation older than 21 days. All patients with a dislocated fracture and those whose x-ray showed joint fusion were not included in the study. We collected 26 patients, 20 men and 6 women with an average age of 27.5 years (17 and 45 years). The average consultation time was 8.9 months (3 - 13). The approach was posterior about 8 cm above the olecranon and 2 cm below the olecranon, the ulnar nerve was identified and isolated. The triceps tendon was cut in a Z from its myotendin junction to the beak of the olecranon, and then lengthened. Results: The patients were reviewed after a mean follow-up of 24 months (12 - 36). The Mayo Clinic’s average elbow performance index was 77.1 (60 - 85), with 19 good and 6 average results. The mean flexion was 114°(100°- 135°) and the mean extension deficit was 31.15°(10°- 55°). The average range of motion was 82.5°(60°- 120°). We did not find a correlation between the duration of the lesion and functional recovery (p > 0.05). The Wilcoxon test showed that surgery significantly improved the overall range of motion of the elbow (p = 0.001). Discussion: Based on this study, open reduction with tendon lengthening should remain a treatment option regardless of the age and chronicity of the dislocation.
