A 67-year-old woman underwent an orthotopic liver transplantation for end stage liver disease secondary to chronic autoimmune hepatitis. She developed sudden massive hematochezia on post-operative day 23 with hemodynamic compromise. The source of hemorrhage was found at colonoscopy after careful irrigation and inspection to be a dieulafoy lesion situated just proximal to the anorectal junction. Hemostasis was achieved with epinephrine injection and thermal coagulation.

糖尿病相关的足病是造成糖尿病患者经济负担和增加社会成本的主要原因之一。本指南以主要目标为导向,以循证医学为依据,且在全球得到合理实施。践行国际糖尿病相关的足病指南对于减轻患者经济负担和社会成本有着重要影响。国际糖尿病足工作组(IWGDF)从1999年起开始发布并随后更新国际指南。2023年更新版指南相关推荐均基于推荐、评估、发展和评价分级(GRADE)系统,包括制定相关临床问题和重要结局,完成系统的文献系统综述和荟萃分析,完成评判表汇总,并且给予具体、明确、操作性强的推荐以及理论依据。本文阐述了《国际糖尿病足工作组:糖尿病相关的足病预防和管理指南(2023版)》(以下简称为指南)的制定过程。指南由7个部分组成,包括预防、糖尿病相关的足溃疡分类、减压、周围动脉病变、感染、创面愈合干预以及活动性夏科神经骨关节病,分别由7个独立的国际专家工作组编写。IWGDF编辑委员会对7个章节进行了整合和简化,形成IWGDF实践指南。指南中的每条推荐均经过IWGDF编辑委员会成员和相关领域的独立的国际专家审阅通过。本文是IWGDF指南的制定和方法学。我们相信,医务人员、公共卫生机构和政策制定者采用和实施2023年IWGDF指南,将会改善糖尿病相关的足病的预防和管理水平,从而减少全球该疾病患者和减轻该疾病引起的全球社会负担。

Complex peptide natural products exhibit diverse biological functions and a wide range of physico-chemical *** a result,many peptides have entered the clinics for various *** main routes for the biosynthesis of complex peptides have evolved in nature:ribosomally synthesized and post-translationally modified peptide(RiPP)biosynthetic pathways and non-ribosomal peptide synthetases(NRPSs).Insights into both bioorthogonal peptide biosynthetic strategies led to the establishment of universal principles for each of the two *** universal rules can be leveraged for the targeted identification of novel peptide biosynthetic blueprints in genome sequences and used for the rational engineering of biosynthetic pathways to produce non-natural *** this review,we contrast the key principles of both biosynthetic routes and compare the different biochemical strategies to install the most frequently encountered peptide *** addition,the influence of the fundamentally different biosynthetic principles on past,current and future engineering ap-proaches is *** the different biosynthetic principles of both peptide biosynthetic routes,the arsenal of characterized peptide modifications encountered in RiPP and NRPS systems is largely *** continuous expansion of the biocatalytic toolbox of peptide modifying enzymes for both routes paves the way towards the production of complex tailor-made peptides and opens up the possibility to produce NRPS-derived peptides using the ribosomal route and vice versa.

Estrogens play important roles in the development and progression of multiple tumor *** evidence points to the significance of estrogen action not only in tumors of hormonally regulated tissues such as the breast,endometrium and ovary,but also in the development of colorectal cancer(CRC).The effects of estrogens in physiological and pathophysiological conditions are mediated by the nuclear estrogen receptorsαandβ,as well as the membranebound G protein-coupled estrogen receptor(GPER).The roles of GPER in CRC development and progression,however,remain poorly *** on the functions of GPER in the colon have shown that this estrogen receptor regulates colonic motility as well as immune responses in CRC-associated diseases,such as Crohn’s disease and ulcerative *** is also involved in cell cycle regulation,endoplasmic reticulum stress,proliferation,apoptosis,vascularization,cell migration,and the regulation of fatty acid and estrogen metabolism in CRC ***,multiple lines of evidence suggest that GPER may play an important role in colorectal *** this review,we present the current state of knowledge regarding the contribution of GPER to colon function and CRC.

Diffusive gradients in thin films(DGT)measurements have been shown to outperform other phosphorus(P)tests in soils with strong P sorption,but this has not been confirmed for moderately weathered European *** compared the performance of DGT in predicting wheat grain yield in Swedish long-term fertility experiments with those of standard intensity(water-extractable P(P-H2O))and quantity(ammonium lactate-extractable P(P-AL))tests.A Mitscherlich-type model was used to fit wheat yield response to P application rates(0,15,30 or 35,and 45 kg P ha-1 year-1)in each individual trial replicate to estimate the maximum *** trials with clear plateau-type yield responses and the goodness of fit(R2)>0.75,relative yields(RYs)were calculated for each P treatment and plotted against the soil P test results(n=143).The goodness of the Mitscherlich-type fits decreased in the following order:DGT-measured P(P-DGT)(R2=0.35)>P-H2O(R2=0.18)>P-AL(R2=0.13).When excluding soils with P-AL:P-DGT≥0.1 L g-1,R2 was considerably improved to 0.55 for P-AL,0.46 for P-H2O,and 0.65 for P-DGT(n=61).At 95%of maximum yield,the upper limit of P deficiency for P-DGT was 44.8(the soils with P-AL:P-DGT<0.1 L g-1)and 61.9μg L-1(all soils),falling within the range reported for other European and Australian soils(6.0–142μg L-1).We show that in the investigated Swedish soils,DGT performed better than the quantity and intensity tests,which is attributed to its ability to capture P diffusion and resupply from the soil solid phase,similar to plant roots in the rhizosphere.

Despite their positive effect in reducing pest populations, exotic generalist predators sometimes become invasive and contribute to the displacement of indigenous species in the same trophic level. Although laboratory experiments have linked intraguild predation （IGP） to these interactions, field evidence and quantification of IGP are still lacking for most systems. The recent establishment of the exotic Harmonia axyridis （Pallas）（Coleoptera： Coccinellidae） in Italy raises concern about the detrimental effect that the ladybird could have on native coccinellids. Here we assessed, under laboratory conditions, the acceptability and suitability of eggs of 2 native ladybirds, Adalia bipunctata L. and Oenopia conglobata （L.）, as prey items for H. axyridis larvae. Then we developed primers for molecular gut-content analysis to detect predation by H. axyridis on the 2 ladybirds and on the aphid Eucallipterus tiliae L. Species-specific 16S primers were developed for the 3 species and laboratory feeding trials were conducted to quantify the rate of prey DNA breakdown in the gut of//. axyridis. Moreover, to field evaluate primers, H. axyridis 4th instars （n = 132） were systematically collected from linden trees in northern Italy and screened for the presence of prey DNA. Seventy-three percent and 7% of field collected H. axyridis were positive for aphid and coccinellid DNA, respectively. Predation upon aphid and A. bipunctata was lower than predicted if density dependent consumption was expected, while predation upon O. conglobata was significantly higher. Here, we provided the first evidence of IGP among feral populations ofH. axyridis and indigenous ladybird beetles, occurring in Italy.

Responses of early-successional songbirds to a two-stage shelterwood harvest for oak forest regeneration

Background: The early stage of forest succession following disturbance is characterized by a shift in songbird composition as well as increased avian richness due to increased herbaceous growth in the forest understory. However, regeneration of woody species eventually outcompetes the herbaceous understory, subsequently shifting vegetation communities and decreasing availability of vital foraging and nesting cover for disturbance-dependent birds, ultimately resulting in their displacement. These early stages following forest disturbance, which are declining throughout the eastern United States, are ephemeral in nature and birds depend on such disturbances for nesting and other purposes throughout their ***: We investigated the use of a two-stage shelterwood method to manage long-term persistence of seven early successional songbirds over a 13-year period in an upland hardwood forest within the southern end of the midCumberland Plateau in the eastern United ***: Canopy and midstory gaps created after initial harvest were quickly exploited by tree growth and canopy cover returned to these areas, accelerating the displacement of early-successional species. Woody stem densities increased substantially following stage two harvest as advanced tree regeneration combined with the re-opening of the overstory layer increased resource competition for early-successional plants in the understory. Carolina Wren(Thryothorus ludovicianus), Eastern Towhee(Pipilo erythrophthalmus), Indigo Bunting(Passerina cyanea), and Yellowbreasted Chat(Icteria virens) were characterized by immediate increases following initial harvest in 2001; while the American Goldfinch(Spinus tristis), Prairie Warbler(Setophaga discolor), and White-eyed Vireo(Vireo griseus) did not show an immediate response.
